    Description

    Background: Strengthening health systems to improve access to maternity services remains challenging for Nigeria due partly to weak and irregular in-service training and deficient data management. This paper reports the implementation of digital health tools for video training (VTR) of health workers and digitization of health data at scale, supported by satellite communications (SatCom) technology and existing 3G mobile networks.Objective: To understand whether, and under what circumstances using digital interventions to extend maternal, newborn and child health (MNCH) services to remote areas of Nigeria improved standards of healthcare delivery.Methods: From March 2017 to March 2019, VTR and data digitization interventions were delivered in 126 facilities across three states of Nigeria. Data collection combined documents review with 294 semi-structured interviews of stakeholders across four phases (baseline, midline, endline, and 12-months post-project closedown) to assess acceptability and impacts of digital interventions. Data was analyzed using a framework approach, drawing on a modified Technology Acceptance Model to identify factors that shaped technology adoption and use.Results: Analysis of documents and interview transcripts revealed that a supportive policy environment, and track record of private-public partnerships facilitated adoption of technology. The determinants of technology acceptance among health workers included ease of use, perceived usefulness, and prior familiarity with technology. Perceptions of impact suggested that at the micro (individual) level, repeated engagement with clinical videos increased staff knowledge, motivation and confidence to perform healthcare roles. At meso (organizational) level, better-trained staff felt supported and empowered to provide respectful healthcare and improved management of obstetric complications, triggering increased use of MNCH services. The macro level saw greater use of reliable and accurate data for policymaking.Conclusions: Simultaneous and sustained implementation of VTR and data digitization at scale enabled through SatCom and 3G mobile networks are feasible approaches for supporting improvements in staff confidence and motivation and reported MNCH practices. By identifying mechanisms of impact of digital interventions on micro, meso, and macro levels of the health system, the study extends the evidence base for effectiveness of digital health and theoretical underpinnings to guide further technology use for improving MNCH services in low resource settings.Trial Registration: ISRCTN32105372.

    Background: To ensure countries can effectively benefit from digital health investments, digital adaptation kits (DAKs) are designed to facilitate the accurate reflection of clinical, public health and data use guidelines within the digital systems countries are adopting. DAKs are operational, software-neutral, standardized documentation that distill clinical, public health and data use guidance into a format that can be transparently incorporated into digital systems. Objectives: This DAK provides operational requirements for implementing Smart Discharges recommendations in digital systems. With a focus on discharge care, this DAK aims to provide a common language across various audiences – discharge and other programme managers, software developers, and implementers of digital systems – to ensure a common understanding of the appropriate health information content within a defined health programme area, as a mechanism to catalyse the effective use of these digital systems. The key objectives of this DAK are: to ensure adherence to clinical, public health and data use guidelines, and facilitate consistency of the health content that is used to inform the development of a patient-centred digital tracking and decision-support (DTDS) system; to enable both health programme leads and digital health teams (including software developers) to have a joint understanding of the health content within the digital system, with a transparent mechanism to review the validity and accuracy of the health content; and to provide a starting point of the core data elements and decision-support logic that should be included within DTDS systems for Smart Discharges.     About this change

    The information standard provides reference data about the Organisations and Sites that comprise the health and social care services, including non-direct-care Organisations, primarily in England but also in the other UK-constituent countries. The data is distributed and uploaded to health IT systems. It supports user security, access control, messaging and is used as reference data for both operations and reporting.

    All health and care bodies must consider the impact of updates to the Organisation Reference Data information standard and make timely, appropriate changes to systems and processes.

    The Introduction of Integrated Care Boards

    The latest release formally introduced Integrated Care Boards (ICBs) into this information standard, following the publication of the Health and Care Act 2022.

    Integrated Care Boards replaced the functions of Clinical Commissioning Groups in July 2022. However, although ICB boundaries are established and known there is not enough clarity around other requirements and so the update to this standard is taking place in 2 phases; version 1.2 of this standard covers the transition phase only:

    • Retain and rename current, active STP codes and associate new Non-Primary Role to reflect Integrated Care Boards
    • Retain and rename current, active CCG codes and associate new Non-Primary Role to reflect the Sub ICB Location / Geographical area.

    ODS Product Modernisations

    The withdrawal of the .csv format Organisation Reference Data files, introduced in SCCI0090 Amd 24/2015, was postponed, following feedback from data consumers regarding the impact of migrating to XML data files or API feeds.

    In recognition of the broad range of consumers of ODS data, the service has invested in the development of new tools that bring the benefit of a single data source (improving reliability and consistency of data), but also provide the consumer with a range of data formats and download frequencies to suit their individual needs.

    ODS Data Search and Export is in public beta until the end of August 2024. This tool will replace the ODS Portal, ODS DataPoint and .csv files published on the ODS website and via TRUD (Terminology Reference Data Update). The tool allows users to view and download data in excel or .csv format and contains a range of predefined reports that replicate legacy .csv content.

    ODS Data Search and Export is fed by a new Organisation Data Terminology FHIR R4 API. This API is also in public beta until the end of August 2024.

    Publishing ODS data via a FHIR compliant API is a modernisation of the service, allows for future uplift in line with the FHIR standard and supports interoperability across the health and social care system.

    The Organisation Data Terminology FHIR API will replace the STU3 ODS API, and following engagement with users, will potentially replace the current ORD API. It should be noted that the ORD API, although an existing product, is now fed by the same data source as the FHIR R4 API, maintaining the ‘single source of truth’ for ODS data.

    The ODS website will be continually updated as these new tools progress to live and legacy tools are deprecated. Updates to the 0090 standard are also planned in line with this service modernisation.

    BackgroundNigeria has one of the worst health and development profiles globally. A weak health system, poor infrastructure, and varied socio-cultural factors are cited as inhibitors to optimal health system performance and improved maternal and child health status. eHealth has become a major solution to closing these gaps in health care delivery in low- and middle-income countries (LMICs). This research reports the use of satellite communication (SatCom) technology and the existing 3G mobile network for providing video training (VTR) for health workers and improving the digitization of healthcare data.ObjectiveTo evaluate whether the expected project outcomes that were achieved at the end-line evaluation of 2019 were sustained 12 months after the project ended.MethodsFrom March 2017 to March 2019, digital innovations including VTR and data digitization interventions were delivered in 62 healthcare facilities in Ondo State, southwest Nigeria, most of which lacked access to a 3G mobile network. Data collection for the evaluation combined documents' review with quantitative data extracted from health facility registers, and 24 of the most significant change stories to assess the longevity of the outcomes and impacts of digital innovation in the four domains of healthcare: use of eHealth technology for data management, utilization of health facilities by patients, the standard of care, and staff attitude. Stories of the most significant changes were audio-recorded, transcribed for analysis, and categorized by the above domains to identify the most significant changes 12 months after the project closedown.ResultsFindings showed that four project outcomes which were achieved at end-line evaluation were sustained 12 months after project closedown namely: staff motivation and satisfaction; increased staff confidence to perform healthcare roles; improved standard of healthcare delivery; and increased adoption of eHealth innovations beyond the health sector. Conversely, an outcome that was reversed following the discontinuation of SatCom from health facilities is the availability of accurate and reliable data for decision-making.ConclusionDigital technology can have lasting impacts on health workers, patients, and the health system, through improving data management for decision-making, the standard of maternity service delivery, boosting attendance at health facilities, and utilization of services. Locally driven investment is essential for ensuring the long-term survival of eHealth projects to achieve sustainable development goals (SDGs) in LMICs.

    Description

    About this change

    The information standard provides reference data about the Organisations and Sites that comprise the health and social care services, including non-direct-care Organisations, primarily in England but also in the other UK-constituent countries. The data is distributed and uploaded to health IT systems. It supports user security, access control, messaging and is used as reference data for both operations and reporting.

    All health and care bodies must consider the impact of updates to the Organisation Reference Data information standard and make timely, appropriate changes to systems and processes.

    The Introduction of Integrated Care Boards

    The latest release formally introduced Integrated Care Boards (ICBs) into this information standard, following the publication of the Health and Care Act 2022.

    Integrated Care Boards replaced the functions of Clinical Commissioning Groups in July 2022. However, although ICB boundaries are established and known there is not enough clarity around other requirements and so the update to this standard is taking place in 2 phases; version 1.2 of this standard covers the transition phase only:

    • Retain and rename current, active STP codes and associate new Non-Primary Role to reflect Integrated Care Boards
    • Retain and rename current, active CCG codes and associate new Non-Primary Role to reflect the Sub ICB Location / Geographical area.

    Phase 2 will be introduced at a later stage – the exact date and form of these changes is yet to be confirmed because it is dependent on the requirements of the system, however, it is likely to include closure of the CCG/Sub ICB Location codes and introduction of further new codes to identify ICB Place.

    ODS Product Modernisations

    The withdrawal of the .csv format Organisation Reference Data files, introduced in SCCI0090 Amd 24/2015, was postponed, following feedback from data consumers regarding the impact of migrating to XML data files or API feeds.

    In recognition of the broad range of consumers of ODS data, the service has invested in the development of new tools that bring the benefit of a single data source (improving reliability and consistency of data), but also provide the consumer with a range of data formats and download frequencies to suit their individual needs.

    ODS Data Search and Export is in public beta until the end of August 2024. This tool will replace the ODS Portal, ODS DataPoint and .csv files published on the ODS website and via TRUD (Terminology Reference Data Update). The tool allows users to view and download data in excel or .csv format and contains a range of predefined reports that replicate legacy .csv content.

    ODS Data Search and Export is fed by a new Organisation Data Terminology FHIR R4 API. This API is also in public beta until the end of August 2024.

    Publishing ODS data via a FHIR compliant API is a modernisation of the service, allows for future uplift in line with the FHIR standard and supports interoperability across the health and social care system.

    The Organisation Data Terminology FHIR API will replace the STU3 ODS API, and following engagement with users, will potentially replace the current ORD API. It should be noted that the ORD API, although an existing product, is now fed by the same data source as the FHIR R4 API, maintaining the ‘single source of truth’ for ODS data.

    The ODS website will be continually updated as these new tools progress to live and legacy tools are deprecated. Updates to the 0090 standard are also planned in line with this service modernisation.

Background: Strengthening health systems to improve access to maternity services remains challenging for Nigeria due partly to weak and irregular in-service training and deficient data management. This paper reports the implementation of digital health tools for video training (VTR) of health workers and digitization of health data at scale, supported by satellite communications (SatCom) technology and existing 3G mobile networks.Objective: To understand whether, and under what circumstances using digital interventions to extend maternal, newborn and child health (MNCH) services to remote areas of Nigeria improved standards of healthcare delivery.Methods: From March 2017 to March 2019, VTR and data digitization interventions were delivered in 126 facilities across three states of Nigeria. Data collection combined documents review with 294 semi-structured interviews of stakeholders across four phases (baseline, midline, endline, and 12-months post-project closedown) to assess acceptability and impacts of digital interventions. Data was analyzed using a framework approach, drawing on a modified Technology Acceptance Model to identify factors that shaped technology adoption and use.Results: Analysis of documents and interview transcripts revealed that a supportive policy environment, and track record of private-public partnerships facilitated adoption of technology. The determinants of technology acceptance among health workers included ease of use, perceived usefulness, and prior familiarity with technology. Perceptions of impact suggested that at the micro (individual) level, repeated engagement with clinical videos increased staff knowledge, motivation and confidence to perform healthcare roles. At meso (organizational) level, better-trained staff felt supported and empowered to provide respectful healthcare and improved management of obstetric complications, triggering increased use of MNCH services. The macro level saw greater use of reliable and accurate data for policymaking.Conclusions: Simultaneous and sustained implementation of VTR and data digitization at scale enabled through SatCom and 3G mobile networks are feasible approaches for supporting improvements in staff confidence and motivation and reported MNCH practices. By identifying mechanisms of impact of digital interventions on micro, meso, and macro levels of the health system, the study extends the evidence base for effectiveness of digital health and theoretical underpinnings to guide further technology use for improving MNCH services in low resource settings.Trial Registration: ISRCTN32105372.
