Medicaid coverage gap Uninsured individuals who live in states that have chosen not to expand Medicaid under the Affordable Care Act (ACA) are referred to as being in the Medicaid coverage gap. As of January 2021, 12 states have not adopted the Medicaid expansion provision under the Affordable Care Act (ACA). More than two million uninsured adults fall into this coverage gap, and among them, more than 60 percent are people of color.

Medicaid is basically a joint federal and state health program that provides medical coverage to low-income individuals and families. Currently, Medicaid is responsible for ** percent of the nation’s health care bill, making it the third-largest payer behind private insurances and Medicare. From the beginning to ObamacareMedicaid was implemented in 1965 and since then has become the largest source of medical services for Americans with low income and limited resources. The program has become particularly prominent since the introduction of President Obama’s health reform – the Patient Protection and Affordable Care Act - in 2010. Medicaid was largely impacted by this reform, for states now had the opportunity to expand Medicaid eligibility to larger parts of the uninsured population. Thus, the percentage of uninsured in the United States decreased from over ** percent in 2010 to *** percent in 2022. Who is enrolled in Medicaid?Medicaid enrollment is divided mainly into four groups of beneficiaries: children, adults under 65 years of age, seniors aged 65 years or older, and disabled people. Children are the largest group, with a share of approximately ** percent of enrollees. However, their share of Medicaid expenditures is relatively small, with around ** percent. Compared to that, disabled people, accounting for **** percent of total enrollment, were responsible for **** percent of total expenditures. Around half of total Medicaid spending goes to managed care and health plans.

State Medicaid coverage Initially, the ACA mandated that all state Medicaid programs would have to be extended to provide medical coverage to nearly all low-income groups. However, the Supreme Court rejected that part of the act in 2012, leaving the door open for states to make their own decision on whether they expand their plans. The 1985 National Nursing Home Survey was designed to gather a variety of data on all types of nursing homes providing nursing care in the United States. In this collection data are available on nursing and related care facilities, services provided by the facilities, residents of the nursing homes, and discharges. Nursing home care is examined from the perspectives of both the recipients and the providers of services. Information about patients, both current and discharged, includes basic demographic characteristics, marital status, place of residence prior to admission, health status, services received, and, for discharges, the outcomes of care. A family member of both current and discharged patients was contacted by telephone to obtain data on socioeconomic status and prior episodes of health care. Facility-level data include basic characteristics such as size, ownership, Medicare/Medicaid certification, occupancy rate, and days of care provided.

Abstract (en): Sponsored by the Robert Wood Johnson Foundation, this survey is one component of the Community Tracking Study (CTS), a national study designed to track changes in the health care system and the effects of the changes on care delivery and on individuals. Central to the design of the CTS is its community focus. Sixty sites (51 metropolitan areas and 9 nonmetropolitan areas) were randomly selected to form the core of the CTS and to be representative of the nation as a whole. The Physician Survey was administered to physicians in the 60 CTS sites and to a supplemental national sample of physicians. Information gathered by the survey instrument includes physician supply and specialty distribution, practice arrangements and physician ownership of practices, sources of practice revenue, level and determinants of physician compensation, effects of care management strategies, and physicians' allocation of time, provision of charity care, career satisfaction, and perceptions of their ability to deliver care. For primary care physicians, the survey instrument also provided vignettes of various clinical presentations for which there was no prescribed method of treatment. These physicians were asked to indicate the percentage of patients for whom they would recommend the course of action specified in each particular vignette. Part 3, the Site and County Crosswalk Data File, describes which counties constitute each site. Part 4, the Physician Survey Summary File, contains site-level averages and percentages and standard errors of these estimates for selected attributes, e.g., the percentage of physicians who were foreign medical school graduates, average age of physicians, average percentage of patient care practice revenue from Medicaid, etc. Physicians who were practicing in the contiguous United States, were providing direct patient care for at least 20 hours per week, and were not federal employees. Residents and fellows, as well as physicians in selected specialties, were excluded. The CTS sites were selected using stratified sampling with probability proportional to population size. The supplemental sample, selected with stratified random sampling, was included in the survey to increase the precision of national estimates. The sample frame was developed by combining lists of physicians from the American Medical Association and the American Osteopathic Association. 2011-11-17 ICPSR added Stata setups to this collection.2001-12-21 (1) Data and documentation for the Restricted-Use Version of the Main Data File (Part 2) have been updated.
