    The California Department of Public Health (CDPH), Center for Health Care Quality, Licensing and Certification (L&C) Program licenses more than 30 types of healthcare facilities. The Electronic Licensing Management System (ELMS) is a California Department of Public Health data system created to manage state licensing-related data. This file lists the bed types and bed type capacities that are associated with California healthcare facilities that are operational and have a current license issued by the CDPH and/or a current U.S. Department of Health and Human Services’ Centers for Medicare and Medicaid Services (CMS) certification. This file can be linked by FACID to the Healthcare Facility Locations (Detailed) Open Data file for facility-related attributes, including geo-coding. The L&C Open Data facility beds file is updated monthly.     These data are part of NACJD's Fast Track Release and are distributed as they were received from the data depositor. The files have been zipped by NACJD for release, but not checked or processed except for the removal of direct identifiers. Users should refer to the accompanying readme file for a brief description of the files available with this collection and consult the investigator(s) if further information is needed.The purpose of the study was to: Examine resident abuse from staff report by direct care workers in a nationally representative sample of assisted living settings;Expand the scope of our understanding of resident abuse by including medication abuse in assisted living settings; and Use the data collected to examine: a. rates of abuse; b. the association of external organizational, and internal factors with resident abuse; c. direct care worker characteristics associated with abuse; d. resident characteristics associated with abuse; e. leadership characteristics associated with abuse; f. the inter-correlations of different types of abuse; and, g. resident-to-resident abuse.For the purposes of this research, an assisted living facility is defined as a long term care setting that typically provides residents for activities of daily living and instrumental activities of daily living. Using state listings from websites, researchers identified approximately 21,000 of assisting living facilities. A random sample of approximately seven percent (n=1,500) of these eligible assisted living facilities were selected from all 50 states. Administrators of assisted living facilities were asked if they were willing to complete the questionnaire (n=1,376). Administrators were also asked if they would be willing to distribute the questionnaire to direct care workers (n=12,555). Researchers requested that they distribute the survey to all full-time and part-time direct care workers on all shifts. Follow up reminder mailings and emails were used for the administrators.

    Nursing Homes A nursing home is commonly referred to as a skilled nursing facility, long term care (LTC) facility, or rest home, and may have a different standardized name throughout the United States, but is most commonly referred to as a nursing home. A nursing home traditionally offers 24-hour (skilled) nursing to the elderly or to disabled patients having a variety of medical conditions who require personal care services above that of an assisted living but do not require hospitalization. The personal care services provided may or may not include, but are not limited to: skilled nursing, long term inpatient care, room and board, meals, laundry, and assistance with: dressing, grooming, getting in and out of bed, medications, bathing, and toileting. For purposes of this dataset, an assisted living facility is defined as a facility where the elderly, who are not related to the operator, reside and receive care, treatment, or services. Although not at the level of a nursing home, the services are above the level of an independent living community. They may include several hours per week of supportive care, personal care, or nursing care per resident. Generally, an assisted living facility offers help in daily living (laundry, cooking, cleaning, etc.) and personal assistance (bathing, eating, clothing, etc.). Many assisted living facilities offer assistance with medication and a lesser level of nursing care than what is offered at a nursing home. Assisted living facilities may be regulated by size restrictions depending on which type of assisted living facility it is considered to be in the state in which it exists. For example, Adult Family Homes in Wisconsin have between 3-4 elderly residents while Community Based Residential Facilities have 5 or more. Almost every state has different terminology to describe their version of the assisted living facility system. The structures in which assisted living facilities exist are varied as well. Depending on the type, an assisted living facility may operate out of a personal residence or a nursing home style structure, and it may be set up as apartment style living or as a campus setting in a continuing care retirement community. Multiple assisted living facilities may exist at one location or may be co-located with nursing homes and/or other similar health care facilities. If a facility is licensed by a state and holds multiple licenses, it is represented once in this dataset for each license, even if the licenses are for the same location. This dataset does not include retirement communities, adult daycare facilities, or rehabilitation facilities. Nursing Homes that are operated by and co-located with a hospital are also excluded because the locations are included in the hospital dataset. Records with "-DOD" appended to the end of the [NAME] value are located on a military base, as defined by the Defense Installation Spatial Data Infrastructure (DISDI) military installations and military range boundaries. "#" and "*" characters were automatically removed from standard fields populated by TechniGraphics. Double spaces were replaced by single spaces in these same fields. Text fields in this dataset have been set to all upper case to facilitate consistent database engine search results. All diacritics (e.g., the German umlaut or the Spanish tilde) have been replaced with their closest equivalent English character to facilitate use with database systems that may not support diacritics. The currentness of this dataset is indicated by the [CONTDATE] field. Based on this field, the oldest record dates from 09/29/2009 and the newest record dates from 01/08/2010.

    This data set includes data reported to VDH on outbreaks in Nursing Homes, Assisted Living Facilities and Multicare Facilities. The data included is the name of the facility, the date VDH was notified and the number of cases and deaths as well as the status of the outbreak. This data set was first published on July 03, 2020. The data set increases in size daily and as a result, the dataset may take longer to update; however, it is expected to be available by 12:00 noon weekly. When you download the data set, the dates will be sorted in ascending order, meaning that the earliest date will be at the top. To see data for the most recent date, please scroll down to the bottom of the data set. Starting November 02, 2020, the data set update schedule has changed from daily to weekly.

    The American Community Survey (ACS) is an ongoing survey that provides vital information on a yearly basis about our nation and its people by contacting over 3.5 million households across the country. The resulting data provides incredibly detailed demographic information across the US aggregated at various geographic levels which helps determine how more than $675 billion in federal and state funding are distributed each year.     This table shows selected demographic information for Cambridge residents living in skilled nursing or assisted living facilities who are classified as confirmed, probable, or suspect cases (see “Case Count by Classification” section for definitions). Demographic information includes gender, age range, and race/ethnicity.

    About the COVID-19 Rapid Testing Program: On April 9, the Broad Institute, in partnership with the City of Cambridge and Pro EMS, launched a surveillance testing pilot program in Cambridge skilled nursing and assisted living facilities. The goal of the program is to gain an accurate picture of the true infection rate in these facilities by testing all residents and workers regardless of whether they have symptoms or feel ill. Positive cases among facility residents reflect three rounds of testing in April and May of all residents at the seven skilled nursing and assisted living facilities in Cambridge, as well as other testing ordered by medical providers.

    Of note:

    The case count includes those who have recovered, are currently sick with COVID-19, and who have died from complications of the disease. Any category with a case count less than five is omitted to protect individual privacy. The Cambridge case count reflects current data received from the Massachusetts Department of Public Health.

    It is important to note that race and ethnicity data are collected and reported by multiple entities and may or may not reflect self-reporting by the individual case. The Cambridge Public Health Department (CPHD) is actively reaching out to cases to collect this information. Due to these efforts, race and ethnicity information have been confirmed for over 80% of Cambridge cases, as of June 2020. Race/Ethnicity Category Definitions: “White” indicates “White, not of Hispanic origin.” “Black” indicates “Black, not of Hispanic origin.” “Hispanic” refers to a person having Hispanic origin. A person having Hispanic origin may be of any race. “Asian” indicates “Asian, not of Hispanic origin.” "Unknown" indicates that the originating reporter or reporting system did not capture race and ethnicity information or the individual refused to provide the information. "Other" indicates multiple races, another race that is not listed above, and cases who have reported nationality in lieu of a race category recognized by the US Census. Population data are from the U.S. Census Bureau’s 2014–2018 American Community Survey estimates and may differ from actual population counts. "Other" also includes a small number of people who identify as Native American or Native Hawaiian/Pacific islander. Because the count for Native Americans or Native Hawaiian/Pacific Islanders is currently < 5 people, these categories have been combined with “Other” to protect individual privacy.

    The table is updated daily at 4 p.m.

    Description

    This dataset is derived from reports to Public Health England (PHE) of infectious disease outbreaks in care homes. Care homes in this dataset refers to all supported living facilities such as residential homes, nursing homes, rehabilitation units and assisted living units.

    The tables in this publication provide the latest management information on suspected or confirmed outbreaks of COVID-19 for upper tier local authorities, lower tier local authorities, government office regions and PHE centres.

    Any individual care home will only be included in the dataset once. If a care home has reported more than one outbreak, only the first is included in this dataset.

    As the details of an outbreak are investigated data will be subject to revision and the numbers in this dataset may change in future publications.

    This dataset contains no indication of whether the reported outbreaks are still active.

    Each weekly total refers to reports in the period Monday to the following Sunday.

    As the COVID-19 situation in England continues to evolve, the previous report providing management information on care home outbreaks is no longer appropriate. Therefore, this publication ceased on 23 July 2020.

    PHE continues to share all relevant case and outbreak data with local authorities and other stakeholders regularly and is developing additional integrated tools to support their ongoing need for intelligence. The COVID-19 surveillance report is published weekly.

Article 28 facilities are hospitals, nursing homes, and diagnostic treatment centers. Article 36 facilities are certified home health care agencies and long term home health care programs. Article 40 facilities are hospices. The dataset currently only contains the locations of hospitals and hospital extension clinics. The data for the remaining facility types will be added in the future. While the NYS Department of Health makes every effort to post accurate and reliable information, it does not guarantee or warrant that the information in this dataset is complete, accurate or up-to-date.
