Abstract (en): The purpose of the Health Interview Survey is to obtain information about the amount and distribution of illness, its effects in terms of disability and chronic impairments, and the kinds of health services people receive. There are five types of records in this core survey, each in a separate data file. The variables in the Household File (Part 1) in this collection include type of living quarters, size of family, and geographic region. The Person File (Part 2) variables include sex, age, race, marital status, veteran status, education, income, occupation, and limits on activity. The Condition File (Part 3) contains variables on the incidence of illness or injury within the past year. The Hospital Episode File (Part 4) contains variables on the incidence of hospitalizations and presence of chronic conditions. The Doctor Visit File (Part 5) includes variables regarding frequency of doctor visits, type of doctor seen, and reasons for each visit. A sixth and seventh file have been provided. The Family Medical Expenses File (Part 6) provides variables including sex, age, race, marital status, veteran status, education, income, industry and occupation codes, and limits on activity. Other variables include amounts paid for personal, family, and outside family dental bills, doctor bills, hospital bills, optical bills, prescription medicine, health insurance, and other medical expenses. Other questions include total personal, family, and outside family medical expenses, including and excluding health insurance, and the sex and race of the family head. The Immunization File (Part 7) includes basic demographic variables, hospital stay length, doctor visit periods, types of immunizations received, and when they were obtained. These data contain multiple weight variables for each part. Users should refer to the User Guide for further information regarding the weights and their derivation. Additionally, users may need to weight the data prior to analysis. ICPSR data undergo a confidentiality review and are altered when necessary to limit the risk of disclosure. ICPSR also routinely creates ready-to-go data files along with setups in the major statistical software formats as well as standard codebooks to accompany the data. In addition to these procedures, ICPSR performed the following processing steps for this data collection: Created online analysis version with question text.. The Health Statistics and Health Research Database is Estonian largest set of health-related statistics and survey results administrated by National Institute for Health Development. Use of the database is free of charge.
The database consists of eight main areas divided into sub-areas. The data tables included in the sub-areas are assigned unique codes. The data tables presented in the database can be both viewed in the Internet environment, and downloaded using different file formats (.px, .xlsx, .csv, .json). The National Health Interview Survey (NHIS) is the principal source of information on the health of the civilian noninstitutionalized population of the United States and is one of the major data collection programs of the National Center for Health Statistics (NCHS) which is part of the Centers for Disease Control and Prevention (CDC). The National Health Survey Act of 1956 provided for a continuing survey and special studies to secure accurate and current statistical information on the amount, distribution, and effects of illness and disability in the United States and the services rendered for or because of such conditions. The survey referred to in the Act, now called the National Health Interview Survey, was initiated in July 1957. Since 1960, the survey has been conducted by NCHS, which was formed when the National Health Survey and the National Vital Statistics Division were combined. NHIS data are used widely throughout the Department of Health and Human Services (DHHS) to monitor trends in illness and disability and to track progress toward achieving national health objectives. The data are also used by the public health research community for epidemiologic and policy analysis of such timely issues as characterizing those with various health problems, determining barriers to accessing and using appropriate health care, and evaluating Federal health programs. The NHIS also has a central role in the ongoing integration of household surveys in DHHS. The designs of two major DHHS national household surveys have been or are linked to the NHIS. The National Survey of Family Growth used the NHIS sampling frame in its first five cycles and the Medical Expenditure Panel Survey currently uses half of the NHIS sampling frame. Other linkage includes linking NHIS data to death certificates in the National Death Index (NDI). While the NHIS has been conducted continuously since 1957, the content of the survey has been updated about every 10-15 years. In 1996, a substantially revised NHIS questionnaire began field testing. This revised questionnaire, described in detail below, was implemented in 1997 and has improved the ability of the NHIS to provide important health information.
The JPFHS is part of the worldwide Demographic and Health Surveys (DHS) program, which is designed to collect data on fertility, family planning, and maternal and child health.
The 1990 Jordan Population and Family Health Survey (JPFHS) was carried out as part of the Demographic and Health Survey (DHS) program. The Demographic and Health Surveys is assisting governments and private agencies in the implementation of household surveys in developing countries.
The JPFIS was designed to provide information on levels and trends of fertility, infant and child mortality, and family planning. The survey also gathered information on breastfeeding, matemal and child health cam, the nutritional status of children under five, as well as the characteristics of households and household members.
The main objectives of the project include: a) Providing decision makers with a data base and analyses useful for informed policy choices, b) Expanding the international population and health data base, c) Advancing survey methodology, and d) Developing skills and resources necessary to conduct high quality demographic and health surveys in the participating countries.
National
Sample survey data
The sample for the JPFHS survey was selected to be representative of the major geographical regions, as well as the nation as a whole. The survey adopted a stratified, multi-stage sampling design. In each governorate, localities were classified into 9 strata according to the estimated population size in 1989. The sampling design also allowed for the survey results to be presented according to major cities (Amman, Irbid and Zarqa), other urban localities, and the rural areas. Localities with fewer than 5,000 people were considered rural.
For this survey, 349 sample units were drawn, containing 10,708 housing units for the individual interview. Since the survey used a separate household questionnaire, the Department of Statistics doubled the household sample size and added a few questions on labor force, while keeping the original individual sample intact. This yielded 21,172 housing units. During fieldwork for the household interview, it was found that 4,359 household units were ineligible either because the dwelling was vacant or destroyed, the household was absent during the team visit, or some other reason. There were 16,296 completed household interviews out of 16,813 eligible households, producing a response rate of 96.9 percent.
The completed household interviews yielded 7,246 women eligible for the individual interview, of which 6,461 were successfully interviewed, producing a response rate of 89.2 percent.

The 1990 JPFIS utilized two questionnaires, one for the household interview and the other for individual women. Both questionnaires were developed first in English and then translated into Arabic. The household questionnaire was used to list all members of the sample households, including usual residents as well as visitors. For each member of the household, basic demographic and socioeconomic characteristics were recorded and women eligible for the individual interview were identified. To be eligible for individual interview, a woman had to be a usual member of the household (part of the de jure population), ever-married, and between 15 and 49 years of age. The household questionnaire was expanded from the standard DHS-II model questionnaire to facilitate the estimation of adult mortality using the orphanhood and widowhood techniques. In addition, the questionnaire obtained information on polygamy, economic activity of persons 15 years of age and over, family type, type of insurance covering the household members, country of work in the summer of 1990 which coincided with the Gulf crisis, and basic data for the calculation of the crude birth rate and the crude death rate. Additional questions were asked about deceased women if they were ever-married and age 15-49, in order to obtain information for the calculation of materoal mortality indices.
The individual questionnaire is a modified version of the standard DHS-II model "A" questionnaire. Experience gained from previous surveys, in particular the 1983 Jordan Fertility and Family Health Survey, and the questionnaire developed by the Pan Arab Project for Child Development (PAPCHILD), were useful in the discussions on the content of the JPFHS questionnaire. A major change from the DHS-II model questionnaire was the rearrangement of the sections so that the marriage section came before reproduction; this allowed the interview to flow more smoothly. Questions on children's cause of death based on verbal autopsy were added to the section on health, which, due to its size, was split into two parts. The first part focused on antenatal care and breastfeeding; the second part examined measures for prevention of childhood diseases and information on the morbidity and mortality of children loom since January 1985. As questions on sexual relations were considered too sensitive, they were replaced by questions about the husband's presence in the household during the specified time period; this served as a proxy for recent sexual activity.
The JPFHS individual questionnaire consists of nine sections: - Respondent's background and household characteristics - Marriage - Reproduction - Contraception - Breastfeeding and health - Immunization, morbidity, and child mortality - Fertility preferences - Husband's background, residence, and woman's work - Height and weight of children
For the individual interview, the number of eligible women found in the selected households and the number of women successfully interviewed are presented. The data indicate a high response rate for the household interview (96.9 percent), and a lower rate for the individual interview (89.2 percent). Women in large cities have a slightly lower response rate (88.6 percent) than those in other areas. Most of the non-response for the individual interview was due to the absence of respondents and the postponement of interviews which were incomplete.

The results from sample surveys are affected by two types of errors, non-sampling error and sampling error. Nonsampling error is due to mistakes made in carrying out field activities, such as failure to locate and interview the correct household, errors in the way the questions are asked, misunderstanding on the part of either the interviewer or the respondent, data entry errors, etc. Although efforts were made during the design and implementation of the JPFHS to minimize this type of error, non-sampling errors are impossible to avoid and difficult to evaluate statistically
Sampling errors, on the other hand, can be measured statistically. The sample of women selected in the JPFHS is only one of many samples that could have been selected from the same population, using the same design and expected size. Each one would have yielded results that differed somewhat from the actual sample selected. The sampling error is a measure of the variability between all possible samples; although it is not known exactly, it can be estimated from the survey results.
Sampling error is usually measured in terms of standard error of a particular statistic (mean, percentage, etc.), which is the square root of the variance. The standard error can be used to calculate confidence intervals within which one can reasonably assured that, apart from nonsampling errors, the true value of the variable for the whole population falls. For example, for any given statistic calculated from a sample survey, the value of that same statistic as measured in 95 percent of all possible samples with the same design (and expected size) will fall within a range of plus or minus two times the standard error of that statistic.
If the sample of women had been selected as a simple random sample, it would have been possible to use straightforward formulas for calculating sampling errors. However, the JPFI-IS sample design depended on stratification, stages and clusters. Consequently, it was necessary to utilize more complex formulas. The computer package CLUSTERS, developed by the International Statistical Institute for the World Fertility Survey, was used to assist in computing the sampling errors with the proper statistical methodology.

The purpose of the NHIS is to obtain information about the amount and distribution of illness, its effects in terms of disability and chronic impairments, and the kinds of health services people receive across the United States population through the collection and analysis of data on a broad range of health topics. The redesigned NHIS questionnaire introduced in 1997 (see National Health Interview Survey, 1997 [ICPSR 2954]) consists of a core that remains largely unchanged from year to year, plus an assortment of supplements varying from year to year. The 2010 NHIS Core consists of three modules: Family, Sample Adult, and Sample Child. The datasets derived from these modules include Household Level, Family Level, Person Level, Injury/Poison Episode Level, Injury/Poison Verbatim Level, Sample Adult Level, and Sample Child level. The 2010 NHIS supplements consist of stand alone datasets for Cancer Level and Quality of Life data derived from the Sample Adult core and Disability Questions Tests 2010 Level derived from the Family core questionnaire. Additional supplementary questions can be found in the Sample Child dataset on the topics of cancer, immunization, mental health, and mental health services and in the Sample Adult dataset on the topics of epilepsy, immunization, and occupational health. Part 1, Household Level, contains data on type of living quarters, number of families in the household responding and not responding, and the month and year of the interview for each sampling unit. Parts 2-5 are based on the Family Core questionnaire. Part 2, Family Level, provides information on all family members with respect to family size, family structure, health status, limitation of daily activities, cognitive impairment, health conditions, doctor visits, hospital stays, health care access and utilization, employment, income, participation in government assistance programs, and basic demographic information. Part 3, Person Level, includes information on sex, age, race, marital status, education, family income, major activities, health status, health care costs, activity limits, and employment status. Parts 4 and 5, Injury/Poisoning Episode Level and Injury/Poisoning Verbatim Level, consist of questions about injuries and poisonings that resulted in medical consultations for any family members and contains information about the external cause and nature of the injury or poisoning episode and what the person was doing at the time of the injury or poisoning episode, in addition to the date and place of occurrence. A randomly-selected adult in each family was interviewed for Part 6, Sample Adult Level, regarding specific health issues, the relation between employment and health, health status, health care and doctor visits, limitation of daily activities, immunizations, and behaviors such as smoking, alcohol consumption, and physical activity. Demographic information, including occupation and industry, also was collected. The respondents to Part 6 also completed Part 7, Cancer Level, which consists of a set of supplemental questions about diet and nutrition, physical activity, tobacco, cancer screening, genetic testing, family history, and survivorship. Part 8, Sample Child Level, provides information from an adult in the household on medical conditions of one child in the household, such as developmental or intellectual disabilities, respiratory problems, seizures, allergies, and use of special equipment like hearing aids, braces, or wheelchairs. The Ghana Demographic and Health Survey (GDHS) is a national sample survey designed to provide information on fertility, family planning and health in Ghana. The survey, which was conducted by the Statistical Service of Ghana, is part of a worldwide programme coordinated by the Institute for Resource Development/Macro Systems, Inc., in more than 40 countries in Africa, Asia and Latin America.
The short-term objectives of the Ghana Demographic and Health Survey (GDHS) are to provide policymakers and those implementing policy with current data on fertility levels, knowledge and use of contraception, reproductive intentions of women 15-49, and health indicators. The information will also serve as the basis for monitoring and evaluating programmes initiated by the government such as the extended programme on immunization, child nutrition, and the family planning programme. The long-term objectives are to enhance the country's ability to undertake surveys of excellent technical quality that seek to measure changes in fertility levels, health status (particularly of children), and the extent of contraceptive knowledge and use. Finally, the results of the survey will form part of an international data base for researchers investigating topics related to the above issues.
National
Sample survey data
The 150 clusters from which a representative sample of women aged 15-49 was selected from a subsample of the 200 clusters used for the Ghana Living Standards Survey (GLSS). All census Enumeration Areas (EAs) were first stratified by ecological zones into 3 strata, namely Coastal Savanna, Forest, and Northern Savanna. These were further stratified into urban, semi-urban, and rural EAs. The EAs (in some cases, segments of EAs) were then selected with probability proportional to the number of households. All households in the selected EAs were subsequently listed.

Face-to-face
Three different types of questionnaires were used for the GDHS. These were the household, individual and the husband questionnaires. The household and the individual questionnaires were adapted from the Model "B" Questionnaire for the DHS program. The GDHS is one of the few surveys in which special effort was made to collect information from husbands of interviewed women on such topics as fertility preferences, knowledge and use of contraception, and environmental and health related issues.
All usual members and visitors in the selected households were listed on the household questionnaire. Recorded in the household questionnaire were data on the age and sex of all listed persons in addition to information on fostering for children aged 0-14. Eligible women and eligible husbands were also identified in the household questionnaire.
The individual questionnaire was used to collect data on eligible women. Eligible women were definedas those aged 15-49 years who spent the night prior to the household interview in the selected household, irrespective of whether they were usual members of the household or not. Items of information collected in this questionnaire are as follows: 1) Respondent's Background 2) Reproductive Behavior 3) Knowledge and Use of Contraception 4) Health and Breastfeeding 5) Marriage 6) Fertility Preferences 7) Husband's Background and Women's Work 8) Weight and Height of Children Aged 3-36 Months.
In half of the selected clusters a husband's questionnaire was used to collect data on eligible husbands. Eligible husbands were defined as those who were co-resident with their wives and whose wives had been successfully interviewed. Data on the husband's background, contraceptive knowledge and use, as well as fertility preferences were collected.
All three questionnaires were translated into seven local languages, namely, Twi, Fante, Nzema, Ga, Ewe, Hausa and Dagbani. All the GDHS interviewers were able to conduct interviews in English and at least one local language. The questionnaires were pretested from mid-October to early November 1987. Five teams were used for the pretest fieldwork. These included 19 persons who were trained for 11 days.
Completed questionnaires were collected weekly from the regions by the field coordinators. Coding, data entry and machine editing went on concurrently at the Ghana Statistical Service in Accra as the fieldwork progressed. Coding and data entry were started in March 1988 and were completed by the end of June 1988. Preliminary tabulations were produced by mid-July 1988, and by August 1988 preliminary results of the survey were published.
Of the 4966 households selected, 4406 were successfully interviewed. Excluding 9 percent of households that were vacant, absent, etc., the household response rate is 98 percent.
Out of 4574 eligible women in the household schedule, 4488 were interviewed successfully. The response rate at the individual level is 98 percent. Of the 997 eligible husbands, 943 were successfully interviewed, representing a response rate of 95 percent.

Each sample would have yielded slightly different results from the sample actually selected. The variability observed among all possible samples constitutes sampling error, which can be estimated from survey results (though not measured exactly).

The National Health Interview Survey (NHIS) is conducted annually by the National Center for Health Statistics (NCHS), Centers for Disease Control and Prevention (CDC). The main objective of the NHIS is to monitor the health of the United States population through the collection and analysis of data on a broad range of health topics. The NHIS contains many similar questions every year. The repeated items are called "core questions." Beginning with a new NHIS design in 1997, these core questions were divided into three components -- Family, Sample Adult, and Sample Child. The 2012 NHIS data release consists of six core data files, a paradata file, the three Disability Questions Tests files, a Functioning and Disability file, and two Complementary and Alternative Medicine files. Users may see the Survey Description document for more details. The 2012 NHIS contains the core questions, as well as enhanced questions on health care access and utilization. Supplemental topics are covered in the following questionnaires: the Family questionnaire covers subjects of food security; the Sample Adult questionnaire covers subjects of immunization, complementary and alternative medicine, non-cigarette tobacco use, voice, speech, and language; and the Sample Child questionnaire covers subjects of mental health, mental health services, immunization, complementary and alternative medicine, balance, voice, speech, and language. Along with the 2012 NHIS core data files are the Disability Questions Tests 2012 files which contain person-level data collected via a field test of six disability questions. These supplemental questions appeared on the NHIS, at the end of the Family, Sample Adult, and Sample Child Cores. The Disability Questions Tests 2012 files are released as three separate files. A fourth disability supplement was also fielded in 2012 as part of the Sample Adult Core and is called "Adult Functioning and Disability Level." The Adult and Child Alternative Health Supplement files were intended to expand on knowledge of alternative medical services. Questions focus on how often various types of alternative therapies are used, the associated costs, and the reasons they are used. Lastly, the Paradata Level file contains information about the survey and data collection processes; included are data on response rates, keystrokes, interview times, and number of contact attempts.
The surveys is designed to collect, analyze and disseminate demographic and health data pertaining to the Palestinian population living in the Palestinian Territory, with a focus on demography, fertility, family planning and maternal and child health. The 2000 survey also includes new sections and elements, such as basic health and socio-economic information on different groups within the population, and children less than five years, children aged 5-17 years.

The survey covered all the Palestinian households who are a usual residence in the Palestinian Territory.

Sample frame and sample design: The list of all Palestinian households has been constructed from the updated frame in 1997. The master sample was drawn to be used for different surveys.
The sample type was a stratified cluster random sample: First stage: 481 EAs were selected from all Palestinian Territory. Second stage: A systematic random sample of 120 households was selected from each enumeration Area (EA) in the West Bank and Gaza Strip.
Sample size: The number of the households in the sample was 6,349 households: 4,295 in the West Bank and 2,054 in Gaza Strip.
Moreover, the standard errors of survey estimates must be calculated so that the user may identify the accuracy of the estimates and reliability of the survey. The total error of the survey can be categorized into two types: Statistical errors and non-statistical errors. Non-statistical errors are related to the procedures of statistical operation at the different stages such as failure to interpret the questions of the questionnaire, not wanting or failure to give the correct answer, and poor statistical coverage, etc. These errors depend on the type of work, training and supervision, efficiency of design, implementation and related activities. The work team made best efforts to reduce non-statistical errors during all stages. Statistical errors, however, can be assessed and often measured by the standard deviation, which was calculated using CENVAR software package using the Ultimate Cluster method to calculate variation

The survey sample consists of about 6,349 cv households of which 6,204 households completed the interview; whereas 4,164 households from the West Bank and 2,040 households in Gaza Strip. Weights were modified to account for non-response rate. The response rate in the West Bank reached 96.9% while in the Gaza Strip it reached 99.3%. The response rate in the Palestinian Territory reached 97.7%.

The 2006 NHIS contains the Household, Family, Person, Sample Adult, and Sample Child files from the basic module. Each record in Part 1, Household Level, contains data on type of living quarters, number of families in the household responding and not responding, and the month and year of the interview for each sampling unit. Part 2, Family Level, is made up of reconstructed variables from the person-level data of the basic module and includes information on sex, age, race, marital status, Hispanic origin, education, veteran status, family income, family size, major activities, health status, activity limits, and employment status, along with industry and occupation. As part of the basic module, Part 3, Personl Level, provides information on all family members with respect to health status, limitation of daily activities, cognitive impairment, and health conditions. Also included are data on years at current residence, region variables, height, weight, bed days, doctor visits, hospital stays, and health care access and utilization. A randomly-selected adult in each family was interviewed for Part 4, Sample Adult, regarding respiratory conditions, use of nasal spray, renal conditions, AIDS, joint symptoms, health status, limitation of daily activities, and behaviors such as smoking, alcohol consumption, and physical activity in addition to questions regarding stroke, diabetes, arthritis, and weight control. Part 5, Sample Child, provides information from an adult in the household on medical conditions of one child in the household, such as respiratory problems, seizures, allergies, and use of special equipment like hearing aids, braces, or wheelchairs. Also included are variables regarding child behavior, the use of mental health services, and Attention Deficit Hyperactivity Disorder (ADHD), as well as responses to the SDQ, the Strengths and Difficulties questionnaire on child mental health. The 2006 data contain the Child Mental Health Brief (CMB), Child Mental Health Services (CMS) and Child Influenza Immunization (CFI) sections. Part 6, Injury/Poison Episode, is an episode-based file that contains information about the external cause and nature of the injury or poisoning episode and what the person was doing at the time of the injury or poisoning episode, in addition to the date and place of occurrence. Part 7, Injury/Poison Episode Verbatim, contains edited narrative text descriptions of the injury or poisoning, provided by the respondent. Imputed income files for 2006 are now available through the NCHS Web site.
The National Health Examination Surveys, Cycle I (NHES I), conducted during the period 1959-1962, were designed to secure statistics on the health status of the population of the United States. More specifically, their purpose was to determine the prevalence of certain chronic diseases, the status of dental health, and the distributions of auditory and visual acuity and certain anthropometric measurements. This collection contains demographic, household, and personal information for each sample person including age, race, sex, income, region, size of residence, usual activity, and sampling weight. Also included are responses to 12 items from a medical history questionnaire that were selected as indicators of psychological distress. The National Health and Nutrition Examination Surveys (NHANES) is a program of studies designed to assess the health and nutritional status of adults and children in the United States. The NHANES combines personal interviews and physical examinations, which focus on different population groups or health topics. These surveys have been conducted by the National Center for Health Statistics (NCHS) on a periodic basis from 1971 to 1994. In 1999, the NHANES became a continuous program with a changing focus on a variety of health and nutrition measurements which were designed to meet current and emerging concerns. The sample for the survey is selected to represent the U.S. population of all ages. Many of the NHANES 2007-2008 questions also were asked in NHANES II 1976-1980, Hispanic NHANES 1982-1984, NHANES III 1988-1994, and NHANES 1999-2006. New questions were added to the survey based on recommendations from survey collaborators, NCHS staff, and other interagency work groups. Estimates for previously undiagnosed conditions, as well as those known to and reported by survey respondents, are produced through the survey. In the 2003-2004 wave, the NHANES includes more than 100 datasets. Most have been combined into three datasets for convenience. Each starts with the Demographic dataset and includes datasets of a specific type. 1. The 2015-16 Armenia Demographic and Health Survey (2015-16 ADHS) is the fourth in a series of nationally representative sample surveys designed to provide information on population and health issues. It is conducted in Armenia under the worldwide Demographic and Health Surveys program. Specifically, the objective of the 2015-16 ADHS is to provide current and reliable information on fertility and abortion levels, marriage, sexual activity, fertility preferences, awareness and use of family planning methods, breastfeeding practices, nutritional status of young children, childhood mortality, maternal and child health, domestic violence against women, child discipline, awareness and behavior regarding AIDS and other sexually transmitted infections (STIs), and other health-related issues such as smoking, tuberculosis, and anemia. The survey obtained detailed information on these issues from women of reproductive age and, for certain topics, from men as well.
The 2015-16 ADHS results are intended to provide information needed to evaluate existing social programs and to design new strategies to improve the health of and health services for the people of Armenia. Data are presented by region (marz) wherever sample size permits. The information collected in the 2015-16 ADHS will provide updated estimates of basic demographic and health indicators covered in the 2000, 2005, and 2010 surveys.
The long-term objective of the survey includes strengthening the technical capacity of major government institutions, including the NSS. The 2015-16 ADHS also provides comparable data for longterm trend analysis because the 2000, 2005, 2010, and 2015-16 surveys were implemented by the same organization and used similar data collection procedures. It also adds to the international database of demographic and health–related information for research purposes.
National coverage
The survey covered all de jure household members (usual residents), children age 0-4 years, women age 15-49 years and men age 15-49 years resident in the household.
Sample survey data [ssd]
The sample was designed to produce representative estimates of key indicators at the national level, for Yerevan, and for total urban and total rural areas separately. Many indicators can also be estimated at the regional (marz) level.
The sampling frame used for the 2015-16 ADHS is the Armenia Population and Housing Census, which was conducted in Armenia in 2011 (APHC 2011). The sampling frame is a complete list of enumeration areas (EAs) covering the whole country, a total number of 11,571 EAs, provided by the National Statistical Service (NSS) of Armenia, the implementing agency for the 2015-16 ADHS. This EA frame was created from the census data base by summarizing the households down to EA level. A representative probability sample of 8,749 households was selected for the 2015-16 ADHS sample. The sample was selected in two stages. In the first stage, 313 clusters (192 in urban areas and 121 in rural areas) were selected from a list of EAs in the sampling frame. In the second stage, a complete listing of households was carried out in each selected cluster. Households were then systematically selected for participation in the survey. Appendix A provides additional information on the sample design of the 2015-16 Armenia DHS. Because of the approximately equal sample size in each marz, the sample is not self-weighting at the national level, and weighting factors have been calculated, added to the data file, and applied so that results are representative at the national level.

Five questionnaires were used for the 2015-16 ADHS: the Household Questionnaire, the Woman’s Questionnaire, the Man’s Questionnaire, the Biomarker Questionnaire, and the Fieldworker Questionnaire. These questionnaires, based on The DHS Program’s standard Demographic and Health Survey questionnaires, were adapted to reflect the population and health issues relevant to Armenia. Input was solicited from various stakeholders representing government ministries and agencies, nongovernmental organizations, and international donors. After all questionnaires were finalized in English, they were translated into Armenian. They were pretested in September-October 2015.
The processing of the 2015-16 ADHS data began shortly after fieldwork commenced. All completed questionnaires were edited immediately by field editors while still in the field and checked by the supervisors before being dispatched to the data processing center at the NSS central office in Yerevan. These completed questionnaires were edited and entered by 15 data processing personnel specially trained for this task. All data were entered twice for 100 percent verification. Data were entered using the CSPro computer package. The concurrent processing of the data was an advantage because the senior ADHS technical staff were able to advise field teams of problems detected during the data entry. In particular, tables were generated to check various data quality parameters. Moreover, the double entry of data enabled easy comparison and identification of errors and inconsistencies. As a result, specific feedback was given to the teams to improve performance. The data entry and editing phase of the survey was completed in June 2016.
A total of 8,749 households were selected in the sample, of which 8,205 were occupied at the time of the fieldwork. The main reason for the difference is that some of the dwelling units that were occupied during the household listing operation were either vacant or the household was away for an extended period at the time of interviewing. The number of occupied households successfully interviewed was 7,893, yielding a household response rate of 96 percent. The household response rate in urban areas (96 percent) was nearly the same as in rural areas (97 percent).
In these households, a total of 6,251 eligible women were identified; interviews were completed with 6,116 of these women, yielding a response rate of 98 percent. In one-half of the households, a total of 2,856 eligible men were identified, and interviews were completed with 2,755 of these men, yielding a response rate of 97 percent. Among men, response rates are slightly lower in urban areas (96 percent) than in rural areas (97 percent), whereas rates for women are the same in urban and in rural areas (98 percent).
The 2015-16 ADHS achieved a slightly higher response rate for households than the 2010 ADHS (NSS 2012). The increase is only notable for urban households (96 percent in 2015-16 compared with 94 percent in 2010). Response rates in all other categories are very close to what they were in 2010.
SAS computer software were used to calculate sampling errors for the 2015-16 ADHS. The programs used the Taylor linearization method of variance estimation for means or proportions and the Jackknife repeated replication method for variance estimation of more complex statistics such as fertility and mortality rates.

The aim of the survey is to collect data on the health and accessibility to health services of the Palestinian Population in the Palestinian Territory. The main objectives of the survey was to study the availability of health facilities in the Palestinian Territory, to study the satisfaction of beneficiaries and to provide a database on accessibility to health institutions.

Data Entry In this stage data were entered into the computer, using a data entry template Microsoft Access. The data entry program was prepared in order to satisfy the following requirements:
·Duplication of the questionnaire on the computer screen. ·Logical and consistency checks of data entered. ·Possibility for internal editing of questionnaire answers. ·Maintaining a minimum of errors in digital data entry and fieldwork. ·User- friendly handling.
The response rate for the survey is 98.2%.
Since the data reported here are based on a sample survey and not on complete enumeration, they are subjected to two main types of errors: sampling errors and non-Sampling errors.
Sampling errors are random outcomes of the sample design, and are, therefore, easily measurable.
Non-sampling errors can occur at the various stages of the survey implementation in data collection and data processing, and are generally difficult to be evaluated statistically. They cover a wide range of errors, including errors resulting from non-response, sample frame coverage, data processing and response (both respondent and interviewer-related). The use of effective training and supervisions and the careful design of questions as measures have direct bearing on the magnitude of non-sampling errors, and hence the quality of the resulting data are of high quality.

The basic purpose of the Health Interview Survey is to obtain information about the amount and distribution of illness, its effects in terms of disability and chronic impairments, and the kind of health services people receive. Household variables in this data collection include type of living quarters, size of family, number of families in household, and geographic region. Person variables include sex, age, race, marital status, veteran status, education, income, industry and occupation codes, and limits on activity. The 2023 Jordan Population and Family Health Survey (JPFHS) is the eighth Population and Family Health Survey conducted in Jordan, following those conducted in 1990, 1997, 2002, 2007, 2009, 2012, and 2017–18. It was implemented by the Department of Statistics (DoS) at the request of the Ministry of Health (MoH).
The primary objective of the 2023 JPFHS is to provide up-to-date estimates of key demographic and health indicators. Specifically, the 2023 JPFHS: • Collected data at the national level that allowed calculation of key demographic indicators • Explored the direct and indirect factors that determine levels of and trends in fertility and childhood mortality • Measured contraceptive knowledge and practice • Collected data on key aspects of family health, including immunisation coverage among children, prevalence and treatment of diarrhoea and other diseases among children under age 5, and maternity care indicators such as antenatal visits and assistance at delivery • Obtained data on child feeding practices, including breastfeeding, and conducted anthropometric measurements to assess the nutritional status of children under age 5 and women age 15–49 • Conducted haemoglobin testing with eligible children age 6–59 months and women age 15–49 to gather information on the prevalence of anaemia • Collected data on women’s and men’s knowledge and attitudes regarding sexually transmitted infections and HIV/AIDS • Obtained data on women’s experience of emotional, physical, and sexual violence • Gathered data on disability among household members
The information collected through the 2023 JPFHS is intended to assist policymakers and programme managers in evaluating and designing programmes and strategies for improving the health of the country’s population. The survey also provides indicators relevant to the Sustainable Development Goals (SDGs) for Jordan.
National coverage
The survey covered all de jure household members (usual residents), all women aged 15-49, men aged 15-59, and all children aged 0-4 resident in the household.
Sample survey data [ssd]
The sampling frame used for the 2023 JPFHS was the 2015 Jordan Population and Housing Census (JPHC) frame. The survey was designed to produce representative results for the country as a whole, for urban and rural areas separately, for each of the country’s 12 governorates, and for four nationality domains: the Jordanian population, the Syrian population living in refugee camps, the Syrian population living outside of camps, and the population of other nationalities. Each of the 12 governorates is subdivided into districts, each district into subdistricts, each subdistrict into localities, and each locality into areas and subareas. In addition to these administrative units, during the 2015 JPHC each subarea was divided into convenient area units called census blocks. An electronic file of a complete list of all of the census blocks is available from DoS. The list contains census information on households, populations, geographical locations, and socioeconomic characteristics of each block. Based on this list, census blocks were regrouped to form a general statistical unit of moderate size, called a cluster, which is widely used in various surveys as the primary sampling unit (PSU). The sample clusters for the 2023 JPFHS were selected from the frame of cluster units provided by the DoS.
The sample for the 2023 JPFHS was a stratified sample selected in two stages from the 2015 census frame. Stratification was achieved by separating each governorate into urban and rural areas. In addition, the Syrian refugee camps in Zarqa and Mafraq each formed a special sampling stratum. In total, 26 sampling strata were constructed. Samples were selected independently in each sampling stratum, through a twostage selection process, according to the sample allocation. Before the sample selection, the sampling frame was sorted by district and subdistrict within each sampling stratum. By using a probability proportional to size selection at the first stage of sampling, an implicit stratification and proportional allocation were achieved at each of the lower administrative levels.

Computer Assisted Personal Interview [capi]
Five questionnaires were used for the 2023 JPFHS: (1) the Household Questionnaire, (2) the Woman’s Questionnaire, (3) the Man’s Questionnaire, (4) the Biomarker Questionnaire, and (5) the Fieldworker Questionnaire. The questionnaires, based on The DHS Program’s model questionnaires, were adapted to reflect the population and health issues relevant to Jordan. Input was solicited from various stakeholders representing government ministries and agencies, nongovernmental organisations, and international donors. After all questionnaires were finalised in English, they were translated into Arabic.
All electronic data files for the 2023 JPFHS were transferred via SynCloud to the DoS central office in Amman, where they were stored on a password-protected computer. The data processing operation included secondary editing, which required resolution of computer-identified inconsistencies and coding of open-ended questions. Data editing was accomplished using CSPro software. During the duration of fieldwork, tables were generated to check various data quality parameters, and specific feedback was given to the teams to improve performance. Secondary editing and data processing were initiated in July and completed in September 2023.
A total of 20,054 households were selected for the sample, of which 19,809 were occupied. Of the occupied households, 19,475 were successfully interviewed, yielding a response rate of 98%.
In the interviewed households, 13,020 eligible women age 15–49 were identified for individual interviews; interviews were completed with 12,595 women, yielding a response rate of 97%. In the subsample of households selected for the male survey, 6,506 men age 15–59 were identified as eligible for individual interviews and 5,873 were successfully interviewed, yielding a response rate of 90%.
The estimates from a sample survey are affected by two types of errors: nonsampling errors and sampling errors. Nonsampling errors are the results of mistakes made in implementing data collection and in data processing, such as failure to locate and interview the correct household, misunderstanding of the questions on the part of either the interviewer or the respondent, and data entry errors. Although numerous efforts were made during the implementation of the 2023 Jordan Population and Family Health Survey (2023 JPFHS) to minimise this type of error, nonsampling errors are impossible to avoid and difficult to evaluate statistically.
Sampling errors, on the other hand, can be evaluated statistically. The sample of respondents selected in the 2023 JPFHS is only one of many samples that could have been selected from the same population, using the same design and sample size. Each of these samples would yield results that differ somewhat from the results of the actual sample selected. Sampling errors are a measure of the variability among all possible samples. Although the degree of variability is not known exactly, it can be estimated from the survey results.
Sampling error is usually measured in terms of the standard error for a particular statistic (mean, percentage, etc.), which is the square root of the variance. The standard error can be used to calculate confidence intervals within which the true value for the population can reasonably be assumed to fall. For example, for any given statistic calculated from a sample survey, the value of that statistic will fall within a range of plus or minus two times the standard error of that statistic in 95% of all possible samples of identical size and design.
If the sample of respondents had been selected by simple random sampling, it would have been possible to use straightforward formulas for calculating sampling errors. However, the 2023 JPFHS sample was the result of a multistage stratified design, and, consequently, it was necessary to use more complex formulas. Sampling errors are computed using SAS programs developed by ICF. These programs use the Taylor linearisation method to estimate variances for survey estimates that are means, proportions, or ratios. The Jackknife repeated replication method is used for variance estimation of more complex statistics such as fertility and mortality rates.

The Associated Press is sharing data from the COVID Impact Survey, which provides statistics about physical health, mental health, economic security and social dynamics related to the coronavirus pandemic in the United States.
Conducted by NORC at the University of Chicago for the Data Foundation, the probability-based survey provides estimates for the United States as a whole, as well as in 10 states (California, Colorado, Florida, Louisiana, Minnesota, Missouri, Montana, New York, Oregon and Texas) and eight metropolitan areas (Atlanta, Baltimore, Birmingham, Chicago, Cleveland, Columbus, Phoenix and Pittsburgh).
The survey is designed to allow for an ongoing gauge of public perception, health and economic status to see what is shifting during the pandemic. When multiple sets of data are available, it will allow for the tracking of how issues ranging from COVID-19 symptoms to economic status change over time.

Each set of survey data will be numbered and have the date the embargo lifts in front of it in the format of: 01_April_30_covid_impact_survey. The survey has been organized by the Data Foundation, a non-profit non-partisan think tank, and is sponsored by the Federal Reserve Bank of Minneapolis and the Packard Foundation. It is conducted by NORC at the University of Chicago, a non-partisan research organization. (NORC is not an abbreviation, it part of the organization's formal name.)
Data for the national estimates are collected using the AmeriSpeak Panel, NORC’s probability-based panel designed to be representative of the U.S. household population. Interviews are conducted with adults age 18 and over representing the 50 states and the District of Columbia. Panel members are randomly drawn from AmeriSpeak with a target of achieving 2,000 interviews in each survey. Invited panel members may complete the survey online or by telephone with an NORC telephone interviewer.
Once all the study data have been made final, an iterative raking process is used to adjust for any survey nonresponse as well as any noncoverage or under and oversampling resulting from the study specific sample design. Raking variables include age, gender, census division, race/ethnicity, education, and county groupings based on county level counts of the number of COVID-19 deaths. Demographic weighting variables were obtained from the 2020 Current Population Survey. The count of COVID-19 deaths by county was obtained from USA Facts. The weighted data reflect the U.S. population of adults age 18 and over.
Data for the regional estimates are collected using a multi-mode address-based (ABS) approach that allows residents of each area to complete the interview via web or with an NORC telephone interviewer. All sampled households are mailed a postcard inviting them to complete the survey either online using a unique PIN or via telephone by calling a toll-free number. Interviews are conducted with adults age 18 and over with a target of achieving 400 interviews in each region in each survey.Additional details on the survey methodology and the survey questionnaire are attached below or can be found at https://www.covid-impact.org.

The main objective of a demographic household survey (DHS) is to provide estimates of a number of basic demographic and health variables. This is done through interviews with a scientifically selected probability sample that is chosen from a well-defined population.
The 2007 Nauru Demographic and Health Survey (2007 NDHS) was one of four pilot demographic and health surveys conducted in the Pacific under an Asian Development Bank ADB/ Secretariat of the Pacific Community (SPC) Regional DHS Pilot Project. The primary objective of this survey was to provide up-to-date information for policy-makers, planners, researchers and programme managers, for use in planning, implementing, monitoring and evaluating population and health programmes within the country. The survey was intended to provide key estimates of Nauru's demographics and health situation. The findings of the 2007 NDHS are very important in measuring the achievements of family planning and other health programmes. To ensure better understanding and use of these data, the results of this survey should be widely disseminated at different planning levels. Different dissemination techniques will be used to reach different segments of society.
The primary purpose of the 2007 NDHS was to furnish policy-makers and planners with detailed information on fertility, family planning, infant and child mortality, maternal and child health, nutrition, and knowledge of HIV and AIDS and other sexually transmitted infections.
NOTE: The only dissemination used was wide distribution of the report. A planned data use workshop was not undertaken. Hence there is some misconceptions and lack of awareness on the results obtained from the survey. The report is provided on the NBOS website free for download.

The Papua New Guinea (PNG) questionnaire was proposed for Nauru to adapt as in comparison to the existing DHS model, this is not as lengthy and time-consuming. The PNG questionnaire also dealt with high incidence of alcohol and tobacco in Nauru. Questions on HIV/AIDS and STI knowledge were included in the men's questionnaire where it was not included in the PNG questionnaire.

The primary objective of the Cambodia National Health Survey is to provide the Ministry of Health with reliable, population-based, nationally representative data or infant/child mortality, fertility, and related health service indicators.
A secondary objective was to provide the ADB-financed Basic Helath Services Project (BHSP) and the World Bank finaced Cambodia Disease Control and Health Development Project (CDCP) with baseline information about their respective Project areas, against which project impact could later be assessed.

Sample Design and Selection The NHS sample was designed to provide estimates of kwy health indicators including infant/ child mortality rates and fertility rates for the country as a whole, for urban and rural residence, and for the two project catchment areas (the Basic Health Services Project and the Cambodia Disease Control and Health Development Project). In addition, the design allows for estimates of most key variables (but not for the vaccination coverage of children, fertility rates, or mortality rates) for 14 Provinces. In the other Provinces, the sample size is not sufficiently large to allow for province-level estimates. In order to provide sufficient cases to meet the survey objectives, the number of households selected in the NHS sample from each Province was disproportional to the size of the population in the Province. The above arrangements imply stratification into 40 strata, with 40 different sampling fractions. These strata are 20 Provinces, each divided into an urban and a rural sector. As a result, the NHS sample is self-weighting within strata; weights are only necessary when making estimates across more than one stratum.

The NHS involved two types of questionnaires: a household questionnaire and an individual questionnaire. The household questionnaire was administered to all selected households; the individual questionnaire was administered to all women aged 15-49 identified in the household questionnaire as either usual residents of the household or visitors who stayed there on the night before the day of interview. These questionnaires were developed to measure the desired indicators identified by the MOH and Technical Steering Committee. Wording and structure of the questionnaires, where applicable, was based on the model survey instruments Macro International has used in similar surveys worldwide.
The household questionnaire consisted of three parts: 1) a household schedule giving demographic details of all usual household members and overnight visitors; 2) a series of questions relating to the utilization of health services for any household members who had been ill or injured in the past 30 days; and 3) questions about wall and roof materials of the home and household possessions, which in turn were used to compose a measure of overall household socio-economic status.
The individual questionnaire administered to women aged 15-49 gathered detailed information about the woman's reproductive history, and maternal and child health related knowledge and practices. Questions specific to child health practices were limited to children born after January 1993. (i.e., children under age 5)
The questionnaire was developed in English, translated into Khmer, then back translated and corrected. Following this, a three day pretest covering 100 households was conducted in Phnom Penh and rural Kandal Province by twenty interviewers after initial two week training. The questionnaires were finalized following the pretest.
Data Processing was conducted by NIPH with technical assistance form Macro International. The NIPH central office collected questionnaires form supervisors as soon as a cluster was completed. Office editors reviewed questionnaires for consistency and completeness. The data from the questionnaires were then entered and edited on microcomputers using the Integrated System for Survey Analysis (ISSA), a software package developed especially for such surveys by Macro International. During the machine entry, all questionnaires were reentered for verification. Entry and editing of data began one week after the fieldwork started and was completed by the beginning of August 1998. To provide feedback for the field teams, quality tables were produced every two weeks during the fieldwork. These tables were designed to identify major systematic errors in data collection (e.g. age displacement). The fieldwork coordinators reviewed these tables and, if they found a problem, notified and advised all teams of the steps to be taken to avoid this problem in the future.
A total of 7,654 women were identified as eligible to be interviewed. Questionnaires were completed for 7,630 of those women, a response rate of 99.7 precent. There is a little difference between the household and individual response rates in urban and rural areas. The same is true for the two project areas.
The estimate from a sample survey is affected by two types of errors: 1) nonsampling errors, and 2) sampling errors. Nonsampling errors are the results of mistake made in implementing data collection and data processing, such as failure to locate and interview the correct household, misunderstanding of the questions on the part of either the interviewer or the respondent, and data entry errors. Although numerous efforts were made during the implementation of the National Health Survey (NHS) to minimize this type of error, nonsampling errors are impossible to avoid and difficult to evaluate statistically.
Sampling error, on the other hand, can be evaluated statistically. The sample of respondents selected in the NHS is only one of many samples that could have been selected from the same population, using the same design and expected size. Each of these samples would yield results that differ somewhat from the results of the actual sample selected. Sampling errors are a measure of the variability between all possible samples. Although the degree of variability is nor known exactly, it can be estimated from the survey results.
A sampling error is usually measured in terms of the standard error for a particular statistic (mean, percentage, etc.), which is the square root of the variance. The standard error can be reasonably be assumed to fall. For example, for any given statistic calculated from a sample survey, the value of that statistics will fall within a range of plus or minus two times the standard error of that statistic in 95 percent of all possible sample of identical size and design.
If the sample of respondents had been selected as a simple random sample, it would have been possible to use straightforward formulas for calculating sampling errors. However, the NHS sample is the result of a multi-stage stratified design and consequently, it was necessary to use more complex formulae. The computer software used to calculate sampling errors for the NHS is the ISSA Sampling Error Module. This module used the Taylor linearization method of variance estimation for survey estimates that are means of proportions. The Jackknife repeated replication method is used for variance estimation of more complex statistics such as fertility and mortality rates.

Abstract (en): The purpose of the Health Interview Survey is to obtain information about the amount and distribution of illness, its effects in terms of disability and chronic impairments, and the kinds of health services people receive. There are five types of records in this core survey, each in a separate data file. The variables in the Household File (Part 1) in this collection include type of living quarters, size of family, and geographic region. The Person File (Part 2) variables include sex, age, race, marital status, veteran status, education, income, occupation, and limits on activity. The Condition File (Part 3) contains variables on the incidence of illness or injury within the past year. The Hospital Episode File (Part 4) contains variables on the incidence of hospitalizations and presence of chronic conditions. The Doctor Visit File (Part 5) includes variables regarding frequency of doctor visits, type of doctor seen, and reasons for each visit. A sixth and seventh file have been provided. The Family Medical Expenses File (Part 6) provides variables including sex, age, race, marital status, veteran status, education, income, industry and occupation codes, and limits on activity. Other variables include amounts paid for personal, family, and outside family dental bills, doctor bills, hospital bills, optical bills, prescription medicine, health insurance, and other medical expenses. Other questions include total personal, family, and outside family medical expenses, including and excluding health insurance, and the sex and race of the family head. The Immunization File (Part 7) includes basic demographic variables, hospital stay length, doctor visit periods, types of immunizations received, and when they were obtained. These data contain multiple weight variables for each part. Users should refer to the User Guide for further information regarding the weights and their derivation. Additionally, users may need to weight the data prior to analysis. ICPSR data undergo a confidentiality review and are altered when necessary to limit the risk of disclosure. ICPSR also routinely creates ready-to-go data files along with setups in the major statistical software formats as well as standard codebooks to accompany the data.
