    Over the past 30 years, the birth rate in the United States has been steadily declining, and in 2023, there were 10.7 births per 1,000 of the population. In 1990, this figure stood at 16.7 births per 1,000 of the population. Demographics have an impact The average birth rate in the U.S. may be falling, but when broken down along ethnic and economic lines, a different picture is painted: Native Hawaiian and other Pacific Islander women saw the highest birth rate in 2022 among all ethnicities, and Asian women and white women both saw the lowest birth rate. Additionally, the higher the family income, the lower the birth rate; families making between 15,000 and 24,999 U.S. dollars annually had the highest birth rate of any income bracket in the States. Life expectancy at birth In addition to the declining birth rate in the U.S., the total life expectancy at birth has also reached its lowest value recently. Studies have shown that the life expectancy of both men and women in the United States has been declining over the last few years. Declines in life expectancy, like declines in birth rates, may indicate that there are social and economic factors negatively influencing the overall population health and well-being of the country.

    BackgroundWe examined over a million California birth records for 2010 through 2021 to investigate whether disparities in preterm birth (PTB) by nativity and race support the widely held but hitherto unsubstantiated belief that genetic differences explain the persistent Black-White disparity in PTB.MethodsWe examined PTB rates and risk ratios among African-, Caribbean-, and U.S.-born Black women compared to U.S.-born White women. Multivariate analyses adjusted for maternal age, education, number of live births, delivery payer, trimester of prenatal care initiation, pre-pregnancy BMI, smoking, and prevalence of poverty in a woman’s residence census tract; and for paternal education.ResultsIn adjusted analyses, African-born Black women’s PTB rates were no different from those of U.S.-born White women.DiscussionThe results add to prior evidence making a genetic etiology for the racial disparity in PTB unlikely. If genetic differences tied to “race” explained the Black-White disparity in PTB among U.S.-born women, the African immigrants in this study would have had higher rates of PTB, not the lower rates observed. Multiple explanations for the observed patterns and their implications are discussed. Failure to distinguish causes of PTB from causes of the racial disparity in PTB have likely contributed to erroneous attribution of the racial disparity to genetic differences. Based on the literature, unmeasured experiences of racism, including racism-related stress and adverse environmental exposures, are plausible explanations for the PTB disparity between Black and White U.S.-born women. The favorable birth outcomes of African-born Black immigrants may reflect less exposure to racism during sensitive life periods, e.g., childhood, when they were in African countries, where Black people are in the racial majority.

    Striking racial/ethnic disparities exist in pregnancy outcomes among various racial/ethnic To determine the incidence and risk factors associated with stillbirth in Asian-American women. We conducted this retrospective cohort study using the United States Birth and Fetal Death data files 2014–2017. We used the fetuses‐at‐risk approach to generate stillbirth trends by gestational age among Non-Hispanic (NH)-White and Asian-American births during the study period. We calculated the adjusted risk of stillbirth for Asian-Americans, overall, and for each Asian-American subgroup: Asian Indians, Koreans, Chinese, Vietnamese, Japanese and Filipinos, with NH-Whites as the referent category. Of the 715,297 births that occurred among Asian-Americans during the study period, stillbirth incidence rate was 3.86 per 1000 births. From the gestational age of 20 weeks through 41 weeks, the stillbirth rates were consistently lower among Asian-Americans compared to NH-Whites. Stillbirth incidence ranged from a low rate of 2.6 per 1000 births in Koreans to as high as 5.3 per 1000 births in Filipinos. After adjusting for potentially confounding characteristics, Asian-Americans were about half as likely to experience stillbirth compared to NH-White mothers [adjusted hazards ratio (AHR) = 0.57, 95% confidence interval (CI) = 0.51–0.64]. This intrauterine survival advantage was evident in all Asian-American subgroups. The risk of stillbirth is twofold lower in Asian-Americans than in NH-Whites. It will be an important research agenda to determine reasons for the improved intrauterine survival among Asian-Americans in order to uncover clues for reducing the burden of stillbirth among other racial/ethnic minority women in the United States.

    The total fertility rate of the world has dropped from around 5 children per woman in 1950, to 2.2 children per woman in 2025, which means that women today are having fewer than half the number of children that women did 75 years ago. Replacement level fertility This change has come as a result of the global demographic transition, and is influenced by factors such as the significant reduction in infant and child mortality, reduced number of child marriages, increased educational and vocational opportunities for women, and the increased efficacy and availability of contraception. While this change has become synonymous with societal progress, it does have wide-reaching demographic impact - if the global average falls below replacement level (roughly 2.1 children per woman), as is expected to happen in the 2050s, then this will lead to long-term population decline on a global scale. Regional variations When broken down by continent, Africa is the only region with a fertility rate above the global average, and, alongside Oceania, it is the only region with a fertility rate above replacement level. Until the 1980s, the average woman in Africa could expect to have 6-7 children over the course of their lifetime, and there are still several countries in Africa where women can still expect to have 5 or more children in 2025. Historically, Europe has had the lowest fertility rates in the world over the past century, falling below replacement level in 1975. Europe's population has grown through a combination of migration and increasing life expectancy, however even high immigration rates could not prevent its population from going into decline in 2021.

    The fertility rate of a country is the average number of children that women from that country will have throughout their reproductive years. In the United States in 1800, the average woman of childbearing age would have seven children over the course of their lifetime. As factors such as technology, hygiene, medicine and education improved, women were having fewer children than before, reaching just two children per woman in 1940. This changed quite dramatically in the aftermath of the Second World War, rising sharply to over 3.5 children per woman in 1960 (children born between 1946 and 1964 are nowadays known as the 'Baby Boomer' generation, and they make up roughly twenty percent of todays US population). Due to the end of the baby boom and increased access to contraception, fertility reached it's lowest point in the US in 1980, where it was just 1.77. It did however rise to over two children per woman between 1995 and 2010, although it is expected to drop again by 2020, to just 1.78.

    Description

    In an increasingly diverse United States (US) population, racial disparities in preterm birth outcomes continue to widen. In this study, we examined temporal trends and risk of preterm birth among Asian American women over a quarter century (1992–2018). This is a retrospective cohort study using the 1992–2018 Natality data files. We conducted joinpoint regression analyses to examine trends in preterm birth among Asian Americans and non-Hispanic (NH) Whites. Bivariate and multivariable analyses were used to identify risk factors associated with preterm birth among Asian Americans and their ethnic sub-groups as compared to NH-Whites. There were a total of 251,278 preterm births among Asian American women, corresponding to a rate of 10.0%, which was relatively stable over time. The incidence of extremely, very and moderate-to-late preterm birth among Asian Americans was 0.4%, 0.9% and 8.7% respectively. Overall, Asian American women exhibited lower adjusted odds (OR = 0.92; 95% CI: 0.88–0.97) of preterm birth than their NH-White counterparts. Comparing Asian American subgroups to NH-Whites, Filipinas and Vietnamese mothers had increased adjusted odds, whereas Chinese, Korean, Japanese and Asian Indian women showed decreased adjusted odds for preterm birth. The risk of preterm birth varied among the ethnic subgroups of Asian Americans in the United States. Future studies should explore the socio-cultural and environmental nuances that might explain these differences.

    BackgroundLife expectancy at birth in the United States will likely surpass 80 years in the coming decade. Yet recent studies suggest that longevity gains are unevenly shared across age and socioeconomic groups. First, mortality in midlife has risen among non-Hispanic whites. Second, low-educated whites have suffered stalls (men) or declines (women) in adult life expectancy, which is significantly lower than among their college-educated counterparts. Estimating the number of life years lost or gained by age and cause of death, broken down by educational attainment, is crucial in identifying vulnerable populations.Methods and FindingsUsing U.S. vital statistics data from 1990 to 2010, this study decomposes the change in life expectancy at age 25 by age and cause of death across educational attainment groups, broken down by race and gender. The findings reveal that mortality in midlife increased for white women (and to a lesser extent men) with 12 or fewer years of schooling, accounting for most of the stalls or declines in adult life expectancy observed in those groups. Among blacks, mortality declined in nearly all age and educational attainment groups. Although an educational gradient was found across multiple causes of death, between 60 and 80 percent of the gap in adult life expectancy was explained by cardiovascular diseases, smoking-related diseases, and external causes of death. Furthermore, the number of life years lost to smoking-related, external, and other causes of death increased among low- and high school-educated whites, explaining recent stalls or declines in longevity.ConclusionsLarge segments of the American population—particularly low- and high school-educated whites under age 55—are diverging from their college-educated counterparts and losing additional years of life to smoking-related diseases and external causes of death. If this trend continues, old-age mortality may also increase for these birth cohorts in the coming decades.

    BackgroundMigrant and ethnic minority groups are often assumed to have poor health relative to the majority population. Few countries have the capacity to study a key indicator, mortality, by ethnicity and country of birth. We hypothesized at least 10% differences in mortality by ethnic group in Scotland that would not be wholly attenuated by adjustment for socio-economic factors or country of birth.Methods and findingsWe linked the Scottish 2001 Census to mortality data (2001–2013) in 4.62 million people (91% of estimated population), calculating age-adjusted mortality rate ratios (RRs; multiplied by 100 as percentages) with 95% confidence intervals (CIs) for 13 ethnic groups, with the White Scottish group as reference (ethnic group classification follows the Scottish 2001 Census). The Scottish Index of Multiple Deprivation, education status, and household tenure were socio-economic status (SES) confounding variables and born in the UK or Republic of Ireland (UK/RoI) an interacting and confounding variable. Smoking and diabetes data were from a primary care sub-sample (about 53,000 people). Males and females in most minority groups had lower age-adjusted mortality RRs than the White Scottish group. The 95% CIs provided good evidence that the RR was more than 10% lower in the following ethnic groups: Other White British (72.3 [95% CI 64.2, 81.3] in males and 75.2 [68.0, 83.2] in females); Other White (80.8 [72.8, 89.8] in males and 76.2 [68.6, 84.7] in females); Indian (62.6 [51.6, 76.0] in males and 60.7 [50.4, 73.1] in females); Pakistani (66.1 [57.4, 76.2] in males and 73.8 [63.7, 85.5] in females); Bangladeshi males (50.7 [32.5, 79.1]); Caribbean females (57.5 [38.5, 85.9]); and Chinese (52.2 [43.7, 62.5] in males and 65.8 [55.3, 78.2] in females). The differences were diminished but not eliminated after adjusting for UK/RoI birth and SES variables. A mortality advantage was evident in all 12 minority groups for those born abroad, but in only 6/12 male groups and 5/12 female groups of those born in the UK/RoI. In the primary care sub-sample, after adjustment for age, UK/RoI born, SES, smoking, and diabetes, the RR was not lower in Indian males (114.7 [95% CI 78.3, 167.9]) and Pakistani females (103.9 [73.9, 145.9]) than in White Scottish males and females, respectively. The main limitations were the inability to include deaths abroad and the small number of deaths in some ethnic minority groups, especially for people born in the UK/RoI.ConclusionsThere was relatively low mortality for many ethnic minority groups compared to the White Scottish majority. The mortality advantage was less clear in UK/RoI-born minority group offspring than in immigrants. These differences need explaining, and health-related behaviours seem important. Similar analyses are required internationally to fulfil agreed goals for monitoring, understanding, and improving health in ethnically diverse societies and to apply to health policy, especially on health inequalities and inequities.

The Asian population has the smallest poverty rate in the United States, with about 8.1 percent living in poverty.

    The median annual family income in the United States in 2020 earned by Black families was about 57,476 U.S. dollars, while the average family income earned by the Asian population was about 109,448 U.S. dollars. This is more than 25,000 U.S. dollars higher than the U.S. average family income, which was 84,008 U.S. dollars.

    In 1991, the birth rate for girls aged 10 to 14 years in the United States stood at 1.4 births per every thousand girls. Since 1991, this rate has consistently decreased, dropping to .2 in the year 2023. This statistic depicts the number of births per thousand U.S. females aged 10 to 14 years between 1991 and 2023. Teenage pregnancy and contraception Over the years, the rate of teenage pregnancy and birth has declined in the United States, most likely due to lower rates of sexual activity in this age group as well as increased use of birth control methods. However, the use and accessibility of contraceptives remains a problem in many parts of the United States. For example, in 2021, only 21 percent of sexually active high school students reported using the birth control pill to prevent pregnancy before their last sexual intercourse. This rate was highest among white high students and lowest among Black students, with only 11 percent reporting use of the birth control pill before their last intercourse. Condom use is more prevalent among high school students than use of the pill, but still only just over half of high school students reported using a condom the last time they had sex as of 2021. Disparities in teenage pregnancy Although rates have decreased over the past decades, teenage pregnancy and birth rates in the U.S. are still higher than in other Western countries. Geographic, racial, and ethnic disparities in teen birth rates are still prevalent within the country. In 2023, teenage birth rates were highest among Native Hawaiian and Pacific Islanders. Other contributing factors to high teen birth rates also include poor socioeconomic conditions, low education, and low-income status.

Native Hawaiian and Pacific Islander women had the highest fertility rate of any ethnicity in the United States in 2022, with about 2,237.5 births per 1,000 women. The fertility rate for all ethnicities in the U.S. was 1,656.5 births per 1,000 women. What is the total fertility rate? The total fertility rate is an estimation of the number of children who would theoretically be born per 1,000 women through their childbearing years (generally considered to be between the ages of 15 and 44) according to age-specific fertility rates. The fertility rate is different from the birth rate, in that the birth rate is the number of births in relation to the population over a specific period of time. Fertility rates around the world Fertility rates around the world differ on a country-by-country basis, and more industrialized countries tend to see lower fertility rates. For example, Niger topped the list of the countries with the highest fertility rates, and Taiwan had the lowest fertility rate.
