    Description

    Note: Reporting of new COVID-19 Case Surveillance data will be discontinued July 1, 2024, to align with the process of removing SARS-CoV-2 infections (COVID-19 cases) from the list of nationally notifiable diseases. Although these data will continue to be publicly available, the dataset will no longer be updated.

    Authorizations to collect certain public health data expired at the end of the U.S. public health emergency declaration on May 11, 2023. The following jurisdictions discontinued COVID-19 case notifications to CDC: Iowa (11/8/21), Kansas (5/12/23), Kentucky (1/1/24), Louisiana (10/31/23), New Hampshire (5/23/23), and Oklahoma (5/2/23). Please note that these jurisdictions will not routinely send new case data after the dates indicated. As of 7/13/23, case notifications from Oregon will only include pediatric cases resulting in death.

    This case surveillance public use dataset has 12 elements for all COVID-19 cases shared with CDC and includes demographics, any exposure history, disease severity indicators and outcomes, presence of any underlying medical conditions and risk behaviors, and no geographic data.

    Overview

    The COVID-19 case surveillance database includes individual-level data reported to U.S. states and autonomous reporting entities, including New York City and the District of Columbia (D.C.), as well as U.S. territories and affiliates. On April 5, 2020, COVID-19 was added to the Nationally Notifiable Condition List and classified as “immediately notifiable, urgent (within 24 hours)” by a Council of State and Territorial Epidemiologists (CSTE) Interim Position Statement (Interim-20-ID-01). CSTE updated the position statement on August 5, 2020, to clarify the interpretation of antigen detection tests and serologic test results within the case classification (Interim-20-ID-02). The statement also recommended that all states and territories enact laws to make COVID-19 reportable in their jurisdiction, and that jurisdictions conducting surveillance should submit case notifications to CDC. COVID-19 case surveillance data are collected by jurisdictions and reported voluntarily to CDC.

    The deidentified data in the “COVID-19 Case Surveillance Public Use Data” include demographic characteristics, any exposure history, disease severity indicators and outcomes, clinical data, laboratory diagnostic test results, and presence of any underlying medical conditions and risk behaviors. All data elements can be found on the COVID-19 case report form located at www.cdc.gov/coronavirus/2019-ncov/downloads/pui-form.pdf.

    COVID-19 Case Reports

    COVID-19 case reports have been routinely submitted using nationally standardized case reporting forms. On April 5, 2020, CSTE released an Interim Position Statement with national surveillance case definitions for COVID-19 included. Current versions of these case definitions are available here: https://ndc.services.cdc.gov/case-definitions/coronavirus-disease-2019-2021/.

    All cases reported on or after were requested to be shared by public health departments to CDC using the standardized case definitions for laboratory-confirmed or probable cases. On May 5, 2020, the standardized case reporting form was revised. Case reporting using this new form is ongoing among U.S. states and territories.

    Data are Considered Provisional

    • The COVID-19 case surveillance data are dynamic; case reports can be modified at any time by the jurisdictions sharing COVID-19 data with CDC. CDC may update prior cases shared with CDC based on any updated information from jurisdictions. For instance, as new information is gathered about previously reported cases, health departments provide updated data to CDC. As more information and data become available, analyses might find changes in surveillance data and trends during a previously reported time window. Data may also be shared late with CDC due to the volume of COVID-19 cases.
    • Annual finalized data: To create the final NNDSS data used in the annual tables, CDC works carefully with the reporting jurisdictions to reconcile the data received during the year until each state or territorial epidemiologist confirms that the data from their area are correct.

    Data Quality Assurance Procedures

    CDC’s Case Surveillance Section routinely performs data quality assurance procedures (i.e., ongoing corrections and logic checks to address data errors). To date, the following data cleaning steps have been implemented:

    • Questions that have been left unanswered (blank) on the case report form are reclassified to a Missing value, if applicable to the question. For example, in the question “Was the individual hospitalized?” where the possible answer choices include “Yes,” “No,” or “Unknown,” the blank value is recoded to Missing because the case report form did not include a response to the question.
    • Logic checks are performed for date data. If an illogical date has been provided, CDC reviews the data with the reporting jurisdiction. For example, if a symptom onset date in the future is reported to CDC, this value is set to null until the reporting jurisdiction updates the date appropriately.
    • Additional data quality processing to recode free text data is ongoing. Data on symptoms, race and ethnicity, and healthcare worker status have been prioritized.

    Data Suppression

    To prevent release of data that could be used to identify people, data cells are suppressed for low frequency (<5) records and indirect identifiers (e.g., date of first positive specimen). Suppression includes rare combinations of demographic characteristics (sex, age group, race/ethnicity). Suppressed values are re-coded to the NA answer option; records with data suppression are never removed.

    Description

    This dataset contains model-based census tract estimates. PLACES covers the entire United States—50 states and the District of Columbia—at county, place, census tract, and ZIP Code Tabulation Area levels. It provides information uniformly on this large scale for local areas at four geographic levels. Estimates were provided by the Centers for Disease Control and Prevention (CDC), Division of Population Health, Epidemiology and Surveillance Branch. PLACES was funded by the Robert Wood Johnson Foundation in conjunction with the CDC Foundation. The dataset includes estimates for 36 measures: 13 for health outcomes, 9 for preventive services use, 4 for chronic disease-related health risk behaviors, 7 for disabilities, and 3 for health status. These estimates can be used to identify emerging health problems and to help develop and carry out effective, targeted public health prevention activities. Because the small area model cannot detect effects due to local interventions, users are cautioned against using these estimates for program or policy evaluations. Data sources used to generate these model-based estimates are Behavioral Risk Factor Surveillance System (BRFSS) 2021 or 2020 data, Census Bureau 2010 population data, and American Community Survey 2015–2019 estimates. The 2023 release uses 2021 BRFSS data for 29 measures and 2020 BRFSS data for seven measures (all teeth lost, dental visits, mammograms, cervical cancer screening, colorectal cancer screening, core preventive services among older adults, and sleeping less than 7 hours) that the survey collects data on every other year.     State and territorial executive orders, administrative orders, resolutions, proclamations, and other official publicly available government communications are collected from government websites and cataloged and coded using Microsoft Excel by one or more coders with one or more additional coders conducting quality assurance.

    Data were collected to determine when individuals in states and territories were subject to executive orders, administrative orders, resolutions, proclamations, and other official publicly available government communications related to COVID-19 banning gatherings of various sizes either (1) generally, or specified that the gathering limit applied only when social distancing was not possible, or (2) even if participants practiced social distancing.

    These data are derived from on the publicly available state and territorial executive orders, administrative orders, resolutions, and proclamations (“orders”) for COVID-19 that expressly ban gatherings found by the CDC, COVID-19 Community Intervention and Critical Populations Task Force, Monitoring and Evaluation Team & CDC, Center for State, Tribal, Local, and Territorial Support, Public Health Law Program from March 11, 2020 through August 15, 2021. These data will be updated as new orders are collected. Any orders not available through publicly accessible websites are not included in these data. Only official copies of the documents or, where official copies were unavailable, official press releases from government websites describing requirements were coded, as well as official government communications such as announcements that counties have progressed through new phases of reopening pursuant to an executive order, directive, or other executive branch action, and posted to government websites; news media reports on restrictions were excluded. Recommendations and guidance documents not included or adopted by reference in an order are not included in these data. These data do not include mandatory business closures, curfews, or requirements/recommendations for people to stay in their homes. Due to limitations of the National Environmental Public Health Tracking Network Data Explorer, these data do not include tribes or cities, nor was a distinction made between county orders that applied county-wide versus those that were limited to unincorporated areas of the county. Effective and expiration dates were coded using only the date provided; no distinction was made based on the specific time of the day the order became effective or expired. These data do not necessarily represent an official position of the Centers for Disease Control and Prevention.

    The Centers for Disease Control and Prevention (CDC) has released deidentified line-listed datasets based on COVID-19 cases reported to CDC. Data suppression is performed on low frequency records (<5), indirect identifiers, and uncommon combinations of demographic characteristics.

    The public-use dataset includes 12 data elements assessing demographic characteristics, testing and reporting dates, health status, comorbidities, and disease outcomes. The restricted-access dataset contains an additional 20 elements (32 in total), including state and county of residence information, details on the delivery of care, and symptoms experienced.

    Datasets were previously updated on a monthly basis. Although the data remain publicly available, reporting of new data was discontinued as of July 1, 2024.

    This dataset contains historical trends in vaccinations and cases by age group, at the US national level. Data is stratified by at least one dose and fully vaccinated. Data also represents all vaccine partners including jurisdictional partner clinics, retail pharmacies, long-term care facilities, dialysis centers, Federal Emergency Management Agency and Health Resources and Services Administration partner sites, and federal entity facilities.

    This dataset contains model-based Census tract level estimates for the PLACES project by the Centers for Disease Control and Prevention (CDC), Division of Population Health, Epidemiology and Surveillance Branch. It represents a first-of-its kind effort to release information uniformly on this large scale. Data sources used to generate these model-based estimates include Behavioral Risk Factor Surveillance System (BRFSS) 2019 or 2018 data, Census Bureau 2010 population estimates, and American Community Survey (ACS) 2015–2019 or 2014–2018 estimates. The 2021 release uses 2019 BRFSS data for 22 measures and 2018 BRFSS data for 7 measures (all teeth lost, dental visits, mammograms, cervical cancer screening, colorectal cancer screening, core preventive services among older adults, and sleeping less than 7 hours a night). Seven measures are based on the 2018 BRFSS data because the relevant questions are only asked every other year in the BRFSS. This data only covers the health of adults (people 18 and over) in East Baton Rouge Parish. All estimates lie within a 95% confidence interval.

    Description

    This dataset contains model-based place (incorporated and census designated places) level estimates for the PLACES project 2020 release. The PLACES project is the expansion of the original 500 Cities project and covers the entire United States—50 states and the District of Columbia (DC)—at county, place, census tract, and ZIP Code tabulation Areas (ZCTA) levels. It represents a first-of-its kind effort to release information uniformly on this large scale for local areas at 4 geographic levels. Estimates were provided by the Centers for Disease Control and Prevention (CDC), Division of Population Health, Epidemiology and Surveillance Branch. The project was funded by the Robert Wood Johnson Foundation (RWJF) in conjunction with the CDC Foundation. The dataset includes estimates for 27 measures: 5 chronic disease-related unhealthy behaviors, 13 health outcomes, and 9 on use of preventive services. These estimates can be used to identify emerging health problems and to inform development and implementation of effective, targeted public health prevention activities. Because the small area model cannot detect effects due to local interventions, users are cautioned against using these estimates for program or policy evaluations. Data sources used to generate these model-based estimates include Behavioral Risk Factor Surveillance System (BRFSS) 2018 or 2017 data, Census Bureau 2010 population data, and American Community Survey (ACS) 2014-2018 or 2013-2017 estimates. The 2020 release uses 2018 BRFSS data for 23 measures and 2017 BRFSS data for 4 measures (high blood pressure, taking high blood pressure medication, high cholesterol, and cholesterol screening). Four measures are based on the 2017 BRFSS because the relevant questions are only asked every other year in the BRFSS. More information about the methodology can be found at www.cdc.gov/places.

    Description

    After over two years of public reporting, the Community Profile Report will no longer be produced and distributed after February 2023. The final release will be on February 23, 2023. We want to thank everyone who contributed to the design, production, and review of this report and we hope that it provided insight into the data trends throughout the COVID-19 pandemic. Data about COVID-19 will continue to be updated at CDC’s COVID Data Tracker.

    The Community Profile Report (CPR) is generated by the Data Strategy and Execution Workgroup in the Joint Coordination Cell, under the White House COVID-19 Team. It is managed by an interagency team with representatives from multiple agencies and offices (including the United States Department of Health and Human Services, the Centers for Disease Control and Prevention, the Assistant Secretary for Preparedness and Response, and the Indian Health Service). The CPR provides easily interpretable information on key indicators for all regions, states, core-based statistical areas (CBSAs), and counties across the United States. It is a snapshot in time that:

  19. Focuses on recent COVID-19 outcomes in the last seven days and changes relative to the week prior
  20. Provides additional contextual information at the county, CBSA, state and regional levels
  21. Supports rapid visual interpretation of results with color thresholds*

    Data in this report may differ from data on state and local websites. This may be due to differences in how data were reported (e.g., date specimen obtained, or date reported for cases) or how the metrics are calculated. Historical data may be updated over time due to delayed reporting. Data presented here use standard metrics across all geographic levels in the United States. It facilitates the understanding of COVID-19 pandemic trends across the United States by using standardized data. The footnotes describe each data source and the methods used for calculating the metrics. For additional data for any particular locality, visit the relevant health department website. Additional data and features are forthcoming.

    *Color thresholds for each category are defined on the color thresholds tab

    Effective April 30, 2021, the Community Profile Report will be distributed on Monday through Friday. There will be no impact to the data represented in these reports due to this change.

    Effective June 22, 2021, the Community Profile Report will only be updated twice a week, on Tuesdays and Fridays.

    Effective August 2, 2021, the Community Profile Report will return to being updated Monday through Friday.

    Effective June 22, 2022, the Community Profile Report will only be updated twice a week, on Wednesdays and Fridays.

    The Population online databases contain data from the US Census Bureau. The Census Estimates online database contains contains county-level population counts for years 1970 - 2000. The data comprise the April 1st Census counts for years 1970, 1980, 1990 and 2000, the July 1st intercensal estimates for years 1971-1979 and 1981-1989, and the July 1st postcensal estimates for years 1991-1999. The Census Projections online database contains population projections for years 2004-2030 by year, state, age, race and sex, prodyced by teh Cenus Bureau in 2005. The data are produced by the United States Department of Commerce, U.S. Census Bureau, Population Division.
