Objective: Our objective is to evaluate the efficacy of ChatGPT 4 in accurately and effectively delivering genetic information, building on previous findings with ChatGPT 3.5. We focus on assessing the utility, limitations, and ethical implications of using ChatGPT in medical settings. Materials and Methods: A structured questionnaire, including the Brief User Survey (BUS-15) and custom questions, was developed to assess ChatGPT 4's clinical value. An expert panel of genetic counselors and clinical geneticists independently evaluated ChatGPT 4's responses to these questions. We also involved comparative analysis with ChatGPT 3.5, utilizing descriptive statistics and using R for data analysis. Results: ChatGPT 4 demonstrated improvements over 3.5 in context recognition, relevance, and informativeness. However, performance variability and concerns about the naturalness of the output were noted. No significant difference in accuracy was found between ChatGPT 3.5 and 4.0. Notably, the efficacy of ChatGPT 4 varied significantly across different genetic conditions, with specific differences identified between responses related to BRCA1 and HFE. Discussion and Conclusion: This study highlights ChatGPT 4's potential in genomics, noting significant advancements over its predecessor. Despite these improvements, challenges remain, including the risk of outdated information and the necessity of ongoing refinement. The variability in performance across different genetic conditions underscores the need for expert oversight and continuous AI training. ChatGPT 4, while showing promise, emphasizes the importance of balancing technological innovation with ethical responsibility in healthcare information delivery. Relevance of information: The chatbot provides relevant and appropriate information/answer to people at each stage to make them closer to their goal. 3. Maxim of quantity: The chatbot responds in an informative way without adding too much information. 4. Resilience to failure: Chatbot seems able to find ways to respond appropriately even when it encounters situations or arguments it is not equipped to handle. 5. Understandability and politeness: The chatbot seems able to understand input and convey correct statements and answers without ambiguity and with acceptable manners. 6. Perceived conversational credibility: The chatbot responds in a credible and informative way without adding too much information. 7. Meet the neurodiverse needs: Chatbot seems able to meet needs and be used by users independently form their health conditions, well-being, age, etc. Expert Panel and Data Collection A panel of experts (two genetic counselors and two clinical geneticists) was provided with a link to the survey containing the questions. They independently evaluated the responses from ChatGPT 4 without discussing the questions or answers among themselves until after the survey submission. This approach ensured unbiased evaluation.

Background: Several chatbots that utilize large language models now exist. As a particularly well-known example, ChatGPT employs an autoregressive modeling process to generate responses, predicting the next word based on previously derived words. Consequently, instead of deducing a correct answer, it arranges the most frequently appearing words in the learned data in order. Optimized for interactivity and content generation, it presents a smooth and plausible context, regardless of whether the content it presents is true. This report aimed to examine the reliability of ChatGPT, an artificial intelligence (AI) chatbot, in diagnosing diseases and treating patients, how to interpret its responses, and directions for future development.Current Concepts: Ten published case reports from Korea were analyzed to evaluate the efficacy of ChatGPT, which was asked to describe the correct diagnosis and treatment. ChatGPT answered 3 cases correctly after being provided with the patient’s symptoms, findings, and medical history. The accuracy rate increased to 7 out of 10 after adding laboratory, pathological, and radiological results. In one case, ChatGPT did not provide appropriate information about suitable treatment, and its response contained inappropriate content in 4 cases. In contrast, ChatGPT recommended appropriate measures in 4 cases.Discussion and Conclusion: ChatGPT’s responses to the 10 case reports could have been better. To utilize ChatGPT efficiently and appropriately, users should possess sufficient knowledge and skills to determine the validity of its responses. AI chatbots based on large language models will progress significantly, but physicians must be vigilant in using these tools in practice.

The employment of ChatGPT has ignited extensive conversations, encompassing both challenges and opportunities within the nursing profession. One of the highlighted opportunities is the potential utilization of ChatGPT to effectively aid frontline nurses, particularly in managing time-intensive responsibilities within care settings. Additionally, it presents advantages for addressing prevalent issues in healthcare systems, such as the scarcity of health professionals, clinician burnout (Kleebayoon and Wiwanitkit, 2023; Scerri and Morin, 2023; Sun and Hoelscher, 2023). However, there are challenges to consider. These include the ChatGPT' potential biases, ethical considerations, privacy concerns, cybersecurity vulnerabilities, and environmental impacts (Abdulai and Hung, 2023; Alkhaqani, 2023). The field of nursing is constantly changing, requiring nurses to stay up-to-date with the latest advancements and adjust to them while maintaining their dedication to delivering high-quality healthcare (Buchanan et al., 2020; Booth et al., 2021). Gaining consciousness regarding the potential negative impacts in conjunction with the advantages offered by ChatGPT across diverse domains, ranging from nursing education to its tangible application and utilization in nursing care, holds the utmost significance. Innovative technologies, shifts in healthcare delivery approaches, and the increasing emphasis on collaborative and patient-centered care. In order to thrive in this evolving landscape, nurses must stay informed about these developments and be prepared to adapt their skills and practices while upholding their commitment to providing excellent patient care . Nurses should be aware of both the usage areas and benefits of ChatGPT on one hand, and the potential negative impacts on the other hand. Therefore, staying updated on the current literature comprehensively is essential. In this relatively new field, relying on the literature will be beneficial for guiding studies and initiatives (Ahmed, 2023; Sun and Hoelscher, 2023; Teixeira da Silva, 2023). There are numerous questions awaiting responses regarding the utilization of ChatGPT and its potential impact on the trajectory of future nursing care.
