The Provider of Services File (POS) - Internet Quality Improvement and Evaluation System (iQIES) - Home Health Agency (HHA), Ambulatory Surgical Center (ASC), and Hospice Providers data provides information on provider demographic and associated certification information. In this file you will find provider number (CMS Certification Number), name, address, and other characteristics of the participating institution providers.

The Provider of Services (POS) Clinical Laboratories (CLIA) data provides information on CLIA demographics and types of testing services the facility provides. In this file you will find provider number, name, address and characteristics of the participating institution providers.

The POS file contains data on characteristics of hospitals and other types of healthcare facilities, including the name and address of the facility and the type of Medicare services the facility provides, among other information. The data are collected through the Centers for Medicare and Medicaid Services (CMS) Regional Offices. The file contains an individual record for each Medicare-approved provider and is updated quarterly. The data is an invaluable resource to a variety of stakeholders, including researchers and application developers.

The Medicare Fee-For-Service Public Provider Enrollment dataset includes information on providers who are actively approved to bill Medicare or have completed the 855O at the time the data was pulled from the Provider Enrollment, Chain, and Ownership System (PECOS). The release of this provider enrollment data is not related to other provider information releases such as Physician Compare or Data Transparency. Note: This full dataset contains more records than most spreadsheet programs can handle, which will result in an incomplete load of data. Use of a database or statistical software is required.

Context

The CMS National Plan and Provider Enumeration System (NPPES) was developed as part of the Administrative Simplification provisions in the original HIPAA act. The primary purpose of NPPES was to develop a unique identifier for each physician that billed medicare and medicaid. This identifier is now known as the National Provider Identifier Standard (NPI) which is a required 10 digit number that is unique to an individual provider at the national level.

Once an NPI record is assigned to a healthcare provider, parts of the NPI record that have public relevance, including the provider’s name, speciality, and practice address are published in a searchable website as well as downloadable file of zipped data containing all of the FOIA disclosable health care provider data in NPPES and a separate PDF file of code values which documents and lists the descriptions for all of the codes found in the data file.

The dataset contains the latest NPI downloadable file in an easy to query BigQuery table, npi_raw. In addition, there is a second table, npi_optimized which harnesses the power of Big Query’s next-generation columnar storage format to provide an analytical view of the NPI data containing description fields for the codes based on the mappings in Data Dissemination Public File - Code Values documentation as well as external lookups to the healthcare provider taxonomy codes . While this generates hundreds of columns, BigQuery makes it possible to process all this data effectively and have a convenient single lookup table for all provider information.

Verify the accuracy of SSNs of all individual Medicare providers, owners, managing/directing employees, authorized representatives, ambulance service medical directors, ambulance crew members, technicians, chain organization administrators, Independent Diagnostic Test Facility (IDTF), supervising/directing physicians, and IDTF interpretation service providers. Also included in this Agreement are individual health care providers who apply for a National Provider Identification Number (NPI).

The Medicare Post-Acute Care (PAC) & Hospice by Provider and Service dataset contains information on services provided to Medicare beneficiaries by home health agencies, hospices, skilled nursing facilities (SNFs), inpatient rehabilitation facilities (IRFs), and long-term care hospitals. It includes information on demographic and clinical characteristics of beneficiaries served, professional and paraprofessional service utilization, submitted charges, and payments at the provider, state, and national levels. There are three additional reports which include information at the Home Health Resource Group, Resource Utilization Group, and Case-Mix Group levels for home health agencies, SNFs and IRFs. Please note: The data included may not be representative of a physician’s entire practice, as the file only includes information on Medicare fee-for-service beneficiaries. The data are also not intended to indicate the quality of care provided and are not risk-adjusted to account for differences in underlying severity of disease of patient populations. For the 2019 Medicare Post-Acute Care & Hospice by Provider and Service datasets only, we have modified the data for SNFs presented. Specifically, SNF data are reported in a separate file and only cover the period between January 1, 2019 and September 30, 2019. The dataset contains information on ‘Provider Utilization and Payment Data Physician and Other Supplier Public Use File (PUF)’ prepared by the Centers for Medicare & Medicaid Services (CMS) and organized by National Provider Identifier (NPI), Healthcare Common Procedure Coding System code, and place of service. This PUF is based on information from CMS’s National Claims History (NCH) Standard Analytic Files (SAFs).

The Public Provider Enrollment data for Medicare fee-for-service includes providers who are actively approved to bill Medicare. This dataset contains an enrollment ID crosswalk between those individual providers reassigning their benefits and those providers receiving the reassignment of benefits.

NCHS has linked data from various surveys with Medicare program enrollment and health care utilization and expenditure data from the Centers for Medicare & Medicaid Services (CMS). Linkage of the NCHS survey participants with the CMS Medicare data provides the opportunity to study changes in health status, health care utilization and costs, and prescription drug use among Medicare enrollees. The Hospice Utilization and Payment Public Use File provides information on services provided to Medicare beneficiaries by hospice providers. The Hospice PUF contains information on utilization, payment (Medicare payment and standard payment), submitted charges, primary diagnoses, sites of service, and hospice beneficiary demographics organized by CMS Certification Number 6-digit provider identification number, and state. This PUF is based on information from CMSs Chronic Conditions Data Warehouse (CCW) data files. The data in the Hospice PUF covers calendar year 2014 and contains 100 percent final-action i.e., all claim adjustments have been resolved, hospice claims for the Medicare population including beneficiaries enrolled in a Medicare Advantage plan.
Although the Hospice PUF has a wealth of payment and utilization information about hospice services, the data set also has a number of limitations. The information presented in this file does not indicate the quality of care provided by individual hospice providers. The file only contains cost and utilization information. Additionally, the data are not risk adjusted and thus do not account for differences in patient populations. For additional limitations, please review the methodology document available below.

This dataset tracks the updates made on the dataset "NCHS Survey Data Linked to Centers for Medicare & Medicaid Services (CMS) Medicare Data Files" as a repository for previous versions of the data and metadata.

CMS has released a series of publicly available data files that summarize the utilization and payments for procedures, services, and prescription drugs provided to Medicare beneficiaries by specific inpatient and outpatient hospitals, physicians, and other suppliers. These Medicare Provider Utilization and Payment Data files include information for common inpatient and outpatient services, all physician and other supplier procedures and services, and all Part D prescriptions. Providers determine what they will charge for items, services, and procedures provided to patients and these charges are the amount that providers bill for an item, service, or procedure.
