COVID-19 vaccinations administered to Chicago residents based on the home ZIP Code of the person vaccinated, as provided by the medical provider in the Illinois Comprehensive Automated Immunization Registry Exchange (I-CARE). The ZIP Code where a person lives is not necessarily the same ZIP Code where the vaccine was administered.

Definitions: ·People with at least one vaccine dose: Number of people who have received at least one dose of any COVID-19 vaccine, including the single-dose Johnson & Johnson COVID-19 vaccine.

·People with a completed vaccine series: Number of people who have completed a primary COVID-19 vaccine series. Requirements vary depending on age and type of primary vaccine series received.

·People with a bivalent dose: Number of people who received a bivalent (updated) dose of vaccine. Updated, bivalent doses became available in Fall 2022 and were created with the original strain of COVID-19 and newer Omicron variant strains.

·Total doses administered: Number of all COVID-19 vaccine doses administered.

Data Notes: Daily counts are shown for the total number of doses administered, number of people with at least one vaccine dose, number of people who have a completed vaccine series, and number of people who have received a bivalent dose. Cumulative totals for each measure as of that date are also provided. Vaccinations are counted based on the day the vaccine was administered.

Coverage percentages are calculated based on cumulative number of people who have received at least one vaccine dose, cumulative number of people who have a completed vaccine series, and cumulative number of people who have received a bivalent dose in each ZIP Code.

Population counts are from the U.S. Census Bureau American Community Survey 2015-2019 5-year estimates and can be seen in the ZIP Code, 2019 rows of the Chicago Population Counts dataset (https://data.cityofchicago.org/d/85cm-7uqa).

Actual counts may exceed population estimates and lead to >100% coverage, especially in areas with small population sizes. Additionally, the medical provider may report a work address or incorrect home address for the person receiving the vaccination which may lead to over or under estimates of vaccination coverage by geography.

Chicago residents who are up to date with COVID-19 vaccines by ZIP Code, based on the reported home address and age group of the person vaccinated, as provided by the medical provider in the Illinois Comprehensive Automated Immunization Registry Exchange (I-CARE). “Up to date” refers to individuals who meet the CDC’s updated COVID-19 vaccination criteria based on their age and prior vaccination history. For surveillance purposes, up to date is defined based on the following criteria: People ages 5 years and older: · Are up to date when they receive 1+ doses of a COVID-19 vaccine during the current season. Children ages 6 months to 4 years: · Children who have received at least two prior COVID-19 vaccine doses are up to date when they receive one additional dose of COVID-19 vaccine during the current season, regardless of vaccine product. · Children who have received only one prior COVID-19 vaccine dose are up to date when they receive one additional dose of the current season's Moderna COVID-19 vaccine or two additional doses of the current season's Pfizer-BioNTech COVID-19 vaccine. · Children who have never received a COVID-19 vaccination are up to date when they receive either two doses of the current season's Moderna vaccine or three doses of the current season's Pfizer-BioNTech vaccine. Note: In these datasets, a person is defined as up to date if they have received at least one dose of an updated COVID-19 vaccine. The Centers for Disease Control and Prevention (CDC) recommends that certain groups, including adults ages 65 years and older, receive additional doses.

Starting on July 13, 2022, the denominator for calculating vaccine coverage has been changed from age 5+ to all ages to reflect new vaccine eligibility criteria. Previously the denominator was changed from age 16+ to age 12+ on May 18, 2021, then changed from age 12+ to age 5+ on November 10, 2021, to reflect previous changes in vaccine eligibility criteria. The previous datasets based on age 12+ and age 5+ denominators have been uploaded as archived tables.

Starting June 30, 2021, the dataset has been reconfigured so that all updates are appended to one dataset to make it easier for API and other interfaces. In addition, historical data has been extended back to January 5, 2021.

This dataset shows full, partial, and at least 1 dose coverage rates by zip code tabulation area (ZCTA) for the state of California. Data sources include the California Immunization Registry and the American Community Survey’s 2015-2019 5-Year data.

This is the data table for the LHJ Vaccine Equity Performance dashboard. However, this data table also includes ZTCAs that do not have a VEM score.

This dataset also includes Vaccine Equity Metric score quartiles (when applicable), which combine the Public Health Alliance of Southern California’s Healthy Places Index (HPI) measure with CDPH-derived scores to estimate factors that impact health, like income, education, and access to health care. ZTCAs range from less healthy community conditions in Quartile 1 to more healthy community conditions in Quartile 4.

The Vaccine Equity Metric is for weekly vaccination allocation and reporting purposes only. CDPH-derived quartiles should not be considered as indicative of the HPI score for these zip codes. CDPH-derived quartiles were assigned to zip codes excluded from the HPI score produced by the Public Health Alliance of Southern California due to concerns with statistical reliability and validity in populations smaller than 1,500 or where more than 50% of the population resides in a group setting.

These data do not include doses administered by the following federal agencies who received vaccine allocated directly from CDC: Indian Health Service, Veterans Health Administration, Department of Defense, and the Federal Bureau of Prisons.

For some ZTCAs, vaccination coverage may exceed 100%. This may be a result of many people from outside the county coming to that ZTCA to get their vaccine and providers reporting the county of administration as the county of residence, and/or the DOF estimates of the population in that ZTCA are too low. Please note that population numbers provided by DOF are projections and so may not be accurate, especially given unprecedented shifts in population as a result of the pandemic.

The COVID-19 Vaccination Rate for Total Population: Zip Codes dashboard shows the history of the rate of vaccinations by first dose and second dose/fully vaccinated per 100,000 people for the total population in each zip code. The percent of population is out of the total population for that zip code.

The vaccine dose data categories include:

• 1st dose of a 2-dose vaccine series (i.e., the start of a vaccine series)
• 1st dose of a 1-dose vaccine series and 2nd dose of a 2-dose vaccine series (i.e., the completion of a vaccine series, or fully vaccinated)

The Maricopa County Department of Public Health (MCDPH) releases the COVID-19 vaccination rate per 100,000 of the vaccine eligible population and COVID-19 vaccination rate per 100,000 of the total population for each zip code and city in Maricopa County at ~12:00 PM weekly on Wednesdays via the Maricopa County GIS Open Data website (https://data-maricopa.opendata.arcgis.com/). The dashboard’s values are refreshed at 4:50 PM weekly on Wednesdays. The most recent date included on the dashboard is available by hovering over the last point on the right-hand side of each chart. A person who has received at least one dose of any COVID-19 vaccine is considered to have initiated vaccination. A person is considered fully vaccinated if they have completed a primary vaccine series by receiving 2 doses of the Pfizer, Novavax or Moderna vaccines or 1 dose of the Johnson & Johnson vaccine. The fully vaccinated are a subset of the number who have received at least one dose.

A person who completed a Pfizer, Moderna, Novavax or Johnson & Johnson primary series (as defined above) and then had an additional dose of COVID-19 vaccine is considered to have had additional dose 1. The additional monovalent dose may be Pfizer, Moderna, Novavax or Johnson & Johnson and may be a different type from the primary series. For people who had a primary Pfizer or Moderna series, additional dose 1 was counted starting August 18th, 2021. For people with a Johnson & Johnson primary series additional dose 1 was counted starting October 22nd, 2021. For most people, additional dose 1 is a booster. However, additional dose 1 may represent a supplement to the primary series for a people who is moderately or severely immunosuppressed. Bivalent booster administrations are not included in the additional dose 1 calculations.

SVI refers to the CDC's Social Vulnerability Index - a measure that combines 15 demographic variables to identify communities most vulnerable to negative health impacts from disasters and public health crises. Measures of social vulnerability include socioeconomic status, household composition, disability, race, ethnicity, language, and transportation limitations - among others. SVI scores were calculated for each zip code in CT. The zip codes in the top 20% were designated as SVI Priority Zip Codes. Percentages are based on 2018 zip code population data supplied by ESRI corporation.

The percent with at least one dose many be over-estimated and the percent fully vaccinated and with additional dose 1 may be under-estimated because of vaccine administration records for individuals that cannot be linked because of differences in how names or date of birth are reported.

Connecticut COVID-19 Vaccine Program providers are required to report information on all COVID-19 vaccine doses administered to CT WiZ, the Connecticut Immunization Information System. Data on doses administered to CT residents out-of-state are being added to CT WiZ jurisdiction-by-jurisdiction. Doses administered by some Federal entities (including Department of Defense, Department of Correction, Department of Veteran’s Affairs, Indian Health Service) are not yet reported to CT WiZ.  Data reported here reflect the vaccination records currently reported to CT WiZ.

Note: As part of continuous data quality improvement efforts, duplicate records were removed from the COVID-19 vaccination data during the weeks of 4/19/2021 and 4/26/2021.

Measures of social vulnerability include socioeconomic status, household composition, disability, race, ethnicity, language, and transportation limitations - among others. SVI scores were calculated for each zip code in CT. The zip codes in the top 20% were designated as Priority SVI zip codes. Percentages are based on 2018 zip code population data supplied by ESRI corporation.

All data in this report are preliminary; data for previous dates will be updated as new reports are received and data errors are corrected.

The data are presented cumulatively and by week of first dose of vaccine. Percentages are reported for all providers combined and for pharmacies, FQHCs (Federally Qualified Health Centers), local public health departments / districts and hospitals. The table excludes people with a missing or out-of-state zip code and doses administered by the Federal government (including Department of Defense, Department of Correction, Department of Veteran’s Affairs, Indian Health Service) or out-of-state providers.

This dataset tracks the updates made on the dataset "COVID-19 Vaccine Progress Dashboard Data by ZIP Code" as a repository for previous versions of the data and metadata.

These data show the percent of NYC residents who received at least one dose of COVID-19 vaccine, and the percent who are fully vaccinated by, Modified ZIP Code Tabulation Areas (MODZCTAs). MODZCTAs can provide better estimates of population size than ZIP codes because they combine census blocks that have smaller populations.

Note: Starting on July 13, 2022, the denominator for calculating vaccine coverage has been changed from age 5+ to all ages to reflect new vaccine eligibility criteria. Previously the denominator was changed from age 16+ to age 12+ on May 18, 2021, then changed from age 12+ to age 5+ on November 10, 2021, to reflect previous changes in vaccine eligibility criteria. The previous datasets based on age 16+ and age 5+ denominators have been uploaded as archived tables.

Note: Starting on May 29, 2021 the methodology for calculating on-hand inventory in the shipped/delivered/on-hand dataset has changed. Please see the accompanying data dictionary for details. In addition, this dataset is now down to the ZIP code level.

This data is from the same source as the Vaccine Progress Dashboard at https://covid19.ca.gov/vaccination-progress-data/ which summarizes vaccination data at the county level by county of residence. Where county of residence was not reported in a vaccination record, the county of provider that vaccinated the resident is included. This applies to less than 1% of vaccination records. The sum of county-level vaccinations does not equal statewide total vaccinations due to out-of-state residents vaccinated in California.

This dataset also includes Vaccine Equity Metric score quartiles, which combine the Public Health Alliance of Southern California’s Healthy Places Index (HPI) measure with CDPH-derived scores to estimate factors that impact health, like income, education, and access to health care. Zip codes range from less healthy community conditions in Quartile 1 to more healthy community conditions in Quartile 4.

The Vaccine Equity Metric is for weekly vaccination allocation and reporting purposes only. CDPH-derived quartiles should not be considered as indicative of the HPI score for these zip codes. CDPH-derived quartiles were assigned to zip codes excluded from the HPI score produced by the Public Health Alliance of Southern California due to concerns with statistical reliability and validity in populations smaller than 1,500 or where more than 50% of the population resides in a group setting.

These data do not include doses administered by the following federal agencies who received vaccine allocated directly from CDC: Indian Health Service, Veterans Health Administration, Department of Defense, and the Federal Bureau of Prisons.

Note: Totals for the Vaccine Progress Dashboard and this dataset may not match, as the Dashboard totals doses by Report Date and this dataset totals doses by Administration Date. Dose numbers may also change for a particular Administration Date as data is updated.

In the data shown here, a person who has received at least one dose of COVID-19 vaccine is considered to have initiated vaccination. A person is considered fully vaccinated if he/she has completed a primary vaccination series by receiving 2 doses of the Pfizer, Novavax or Moderna vaccines or 1 dose of the Johnson & Johnson vaccine. The fully vaccinated are a subset of the people who have received at least one dose. A person who completed a Pfizer, Moderna, Novavax or Johnson & Johnson primary series (as defined above) and then had an additional monovalent dose of COVID-19 vaccine is considered to have had additional dose 1. The additional dose may be Pfizer, Moderna, Novavax or Johnson & Johnson and may be a different type from the primary series. For people who had a primary Pfizer or Moderna series, additional dose 1 was counted starting August 18th, 2021. For people with a Johnson & Johnson primary series additional dose 1 was counted starting October 22nd, 2021. For most people, additional dose 1 is a booster.
