    Description

The Adult Social Care Outcomes Framework (ASCOF) measures how well care and support services achieve the outcomes that matter most to people. The ASCOF is used both locally and nationally to set priorities for care and support, measure progress and strengthen transparency and accountability. This report focuses on the main findings for each measure in the ASCOF in 2023-24. Data are provided at council, regional and national level for each outcome. The 2023-24 ASCOF report gives a summary of the measures in the framework. The report is complemented by an interactive Power BI report, which allows users to explore the data further. The raw data used in the report are available in the accompanying csv file.

    Description

The Department of Health Care Services (DHCS) Long-Term Services and Supports (LTSS) Data Dashboard is an initiative of the Home and Community Based Services Spending Plan. The initiative's primary goal is to create a public-facing LTSS data dashboard to track demographic, utilization, quality, and cost data related to LTSS services. This dashboard will link statewide long-term care and home and community-based services (HCBS) data with the goal of increased transparency to make it possible for regulators, policymakers, and the public to be informed while the state continues to expand, enhance, and improve the quality of LTSS in all home, community, and congregate settings.

The first iteration of the LTSS Dashboard was released in December 2022 as an Open Data Portal file with 40 measures pertaining to LTSS beneficiaries, which includes ten different demographics, plan-related dimensions, and dual stratification. The December 2023 Data Release includes 16 new measures on the Medi-Cal LTSS Dashboard and Open Data Portal (Select “View Underlying Data”); and additional measures and dimensions, including dual stratification, will be added to the Open Data Portal in 2024.

Note: The LTSS Dashboard measures are based on certified eligible beneficiaries who were enrolled in Medi-Cal for one or more months during the reporting interval. Most of the DHCS LTSS dashboard measures report the annual number of certified eligible Medi-Cal beneficiaries who have used LTSS services within a year. Other departments may report on these programs differently. For example, the Department of Social Services (CDSS) reports monthly IHSS recipient/consumer counts. The California Department of Aging (CDA) reports monthly CBAS Medi-Cal participants. DHCS’ annual utilization / enrollment counts of IHSS and CBAS beneficiaries are larger than CDSS/CDA's monthly counts because of data source differences and new enrollment or program attrition over time. Monthly snap-shot measures (average monthly utilization) for IHSS and CBAS have been added to the LTSS Dashboard to align with CDSS and CDA monthly reporting.

Refer to the LTSS-Dashboard (ca.gov) program page for: 1) a Fact Sheet with highlights from the initial data release including changes over time in use of Home and Community-Based Services as well as select demographic information; 2) the Measure Specifications document – that describes business rules and inclusion/exclusion criteria related to age groups, plan types, aid code, geographic, or other important program/waiver-specific eligibility criteria; and 3) User guide – that shows how to navigate the Open Data Portal data file with specific examples.

    Description

This collection consists of data collated and used in a digital dashboard as part of the 'Valuing local provenance and place-based economies in natural capital and low carbon markets and industries' project under the CSIRO Valuing Sustainability Future Science Platform, 2021-2025. Datasets include: Information about the location, carbon abatement methodologies used, expected co-benefits and government investments made in Land Restoration Fund projects and Australian Carbon Credit Unit (ACCU) Scheme projects in Queensland, Australia (as at 2023); Quantitative data describing populations, businesses, and socio-economic conditions across SA2 and SA3 areas of Queensland, Australia (data from 2021 Census and business registries); Relative frequency that regional plan documents (written by local governments, natural resource management organisations, and Indigenous organisations) refer to drivers of change and benefits they are aspiring to achieve in their future and summaries of these drivers of change and benefit aspirations. The visualisation of these datasets in digital dashboard, co-designed with the Queensland Government staff administering the Land Restoration Fund, supported the identification of key socio-economic characteristics and data sets that could be used to inform investment decisions (alongside environmental datasets) where the objective is to invest in: (1) regions with high-capability, minimising delivery risk of investment; or (2) regions that offer opportunities to build capability through investment.

    Description

The system of social indicators for the Federal Republic of Germany - developed in its original version as part of the SPES project under the direction of Wolfgang Zapf - provides quantitative information on levels, distributions and changes in quality of life, social progress and social change in Germany from 1950 to 2013, i.e. over a period of more than sixty years. With the approximately 400 objective and subjective indicators that the indicator system comprises in total, it claims to measure welfare and quality of life in Germany in a differentiated way across various areas of life and to observe them over time. In addition to the indicators for 13 areas of life, including income, education and health, a selection of cross-cutting global welfare measures were also included in the dashboard, i.e. general welfare indicators such as life satisfaction, social isolation or the Human Development Index. Based on available data from official statistics and survey data, time series were compiled for all indicators, ideally with annual values from 1950 to 2013. Around 90 of the indicators were marked as "key indicators" in order to highlight central dimensions of welfare and quality of life across the various areas of life. The further development and expansion, regular maintenance and updating as well as the provision of the data of the system of social indicators for the Federal Republic of Germany have been among the tasks of the Center for Social Indicator Research, which is based at GESIS, since 1987. For a detailed description of the system of social indicators for the Federal Republic of Germany, see the study description under "Other documents".

    Description

As of 1/19/2022, this dataset is no longer being updated. For more data on COVID-19 in Connecticut, visit data.ct.gov/coronavirus.

This tables shows the percent of people who have received at least one dose of COVID-19 vaccine who live in a Priority SVI Zip Code. About a third of people in CT live in a Priority SVI zip code.

SVI refers to the CDC's Social Vulnerability Index - a measure that combines 15 demographic variables to identify communities most vulnerable to negative health impacts from disasters and public health crises. Measures of social vulnerability include socioeconomic status, household composition, disability, race, ethnicity, language, and transportation limitations - among others. SVI scores were calculated for each zip code in CT. The zip codes in the top 20% were designated as Priority SVI zip codes. Percentages are based on 2018 zip code population data supplied by ESRI corporation.

All data in this report are preliminary; data for previous dates will be updated as new reports are received and data errors are corrected.

The data are presented cumulatively and by week of first dose of vaccine. Percentages are reported for all providers combined and for pharmacies, FQHCs (Federally Qualified Health Centers), local public health departments / districts and hospitals. The table excludes people with a missing or out-of-state zip code and doses administered by the Federal government (including Department of Defense, Department of Correction, Department of Veteran’s Affairs, Indian Health Service) or out-of-state providers.

    Description

This publication provides statistics on the number of unique NHS numbers with an associated national data opt-out. The national data opt-out was introduced on 25 May 2018. It was introduced following recommendations from the National Data Guardian. It indicates that a patient does not want their confidential patient information to be shared for purposes beyond their individual care across the health and care system in England. The service allows individuals to set a national data opt-out or reverse a previously set opt-out. It replaced the previous type 2 opt-outs which patients registered via their GP Practice. Previous type 2 opt-outs have been converted to national data opt-outs each month, until November 2018. This is why the monthly increase in opt-outs decreases from December 2018 onward. This publication includes the number of people who have a national data opt-out, broken down by age, gender and a variety of geographical breakdowns. From June 2020 the methodology for reporting NDOP changed, representing a break in time series. Therefore, caution should be used when comparing data to publications prior to June 2020. The number of deceased people with an active NDOP has been captured and reported for the first time in June 2020. Please note that this publication is no longer released monthly. It is released annually or when the national opt-out rate changes by more than 0.1 per cent. Prior to September 2020 there is a slight inflation of less than 0.05 percent in the number of National Data Opt-outs. This is due to an issue with the data processing, which has been resolved and does not affect data after September 2020. This issue does not disproportionately affect any single breakdown, including geographies. Please take this into consideration when using the data. Management information describes aggregate information collated and used in the normal course of business to inform operational delivery, policy development or the management of organisational performance. It is usually based on administrative data but can also be a product of survey data. We publish these management information to ensure equality of access and provide wider public value. Please note, there are instances of legacy CCG codes in the Resident Geography CSV file due to outdated reference tables which will be corrected in future publications.

    Description

Our Corporate Performance Report contains Key Performance Indicators (KPIs) focussing on exceptions; indicators which clearly have a significant change in performance. The report includes the top key performance indicators (Super Key Performance Indicators (SKPIs)) which have been chosen due to their direct impact upon residents of Calderdale.

Our performance dashboard is interactive allowing you to view current performance (15 super key measures, 5 for each priority, plus a range of measures) but also how we compare our progress over time and against others.

For the definitions of the indicators and the different comparator groups used, see Indicator definitions and comparator groups.

    Description

BackgroundThe Veterans Health Administration (VA) is the largest integrated healthcare organization in the US and cares for the largest cohort of individuals with hepatitis C (HCV). A national HCV population management dashboard enabled rapid identification and treatment uptake with direct acting antiviral agents across VA hospitals. We describe the HCV dashboard (HCVDB) and evaluate its use and user experience.MethodsA user-centered design approach created the HCVDB to include reports based on the HCV care continuum: 1) 1945–1965 birth cohort high-risk screening, 2) linkage to care and treatment of chronic HCV, 3) treatment monitoring, 4) post-treatment to confirm cure (i.e., sustained virologic response), and 5) special populations of unstably housed Veterans. We evaluated frequency of usage and user experience with the System Usability Score (SUS) and Unified Theory of Acceptance and Use of Technology 2 (UTAUT2) instruments.ResultsBetween November 2016 and July 2021, 1302 unique users accessed the HCVDB a total of 163,836 times. The linkage report was used most frequently (71%), followed by screening (13%), sustained virologic response (11%), on-treatment (4%), and special populations (
