APPLICATION TYPE: ‘ISSUE’ is the record associated with the initial license application. ‘RENEW’ is a subsequent renewal record. All renewal records are created with a term start date and term expiration date. ‘C_LOC’ is a change of location record. It means the business moved. ‘C_CAPA’ is a change of capacity record. Only a few license types may file this type of application. ‘C_EXPA’ only applies to businesses that have liquor licenses. It means the business location expanded. 'C_SBA' is a change of business activity record. It means that a new business activity was added or an existing business activity was marked as expired.
LICENSE STATUS: ‘AAI’ means the license was issued. ‘AAC’ means the license was cancelled during its term. ‘REV’ means the license was revoked. 'REA' means the license revocation has been appealed.
LICENSE STATUS CHANGE DATE: This date corresponds to the date a license was cancelled (AAC), revoked (REV) or appealed (REA).

The California Department of Public Health (CDPH), Center for Health Care Quality, Licensing and Certification (L&C) Program licenses more than 30 types of healthcare facilities. The Electronic Licensing Management System (ELMS) is a California Department of Public Health data system created to manage state licensing-related data. ELMS records healthcare service provider applications, issues licenses, generates license renewal notices, determines license fees, issues and tracks State enforcement actions, and generates management reports. This file lists the services that are associated with California healthcare facilities that are operational and have a current license issued by the CDPH and/or a current U.S. Department of Health and Human Services’ Centers for Medicare and Medicaid Services (CMS) certification. This file can be linked by FACID to the Healthcare Facility Locations (Detailed) Open Data file for facility-related attributes, including geo-coding. The L&C Open Data facility services file is updated monthly. To link the CDPH facility IDs with those from other Departments, like HCAI, please reference the "Licensed Facility Cross-Walk" Open Data table at https://data.chhs.ca.gov/dataset/licensed-facility-crosswalk. A list of healthcare facilities with addresses can be found at: https://data.chhs.ca.gov/dataset/healthcare-facility-locations. I have taken this dataset from the NYC Open Data Website: https://data.cityofnewyork.us
I wanted to use the cleaned version of this dataset and I thought people might like to use this version. The original dataset was last updated on 10th September 2018.
Description: All dog owners residing in NYC are required by law to license their dogs. The data is sourced from the DOHMH Dog Licensing System (https://a816-healthpsi.nyc.gov/DogLicense), where owners can apply for and renew dog licenses. Each record represents a unique dog license that was active during the year, but not necessarily a unique record per dog, since a license that is renewed during the year results in a separate record of an active license period. Each record stands as a unique license period for the dog over the course of the yearlong time frame.
The original dataset contained 122K rows and 15 columns. After cleaning the data, the count has reduced to 121862 rows.
Thank you to the city of new york for collecting and providing this data! As well as the NYC Department of Health who acquired this data from owners who registered their dogs for the dog license.
