The L&C District Office contact information is geocoded. The L&C Program is mandated to conduct various state re-licensing surveys to ensure that a provider is in compliance with all state laws and regulations by conducting on-site inspections (surveys) and investigating complaints and facility-reported events. The L&C Program is also contracted by the United States Department of Health and Human Services Centers for Medicare and Medicaid Services (CMS) to ensure that facilities accepting Medicare and Medi-Cal payments meet federal requirements. L&C is the largest program within CDPH, consisting of about 1,200 managers and staff located at headquarters in Sacramento and 14 district offices throughout the state. In addition, the Program contracts with the Los Angeles County (LA) to provide federal and state survey and certification services throughout that area; LA is divided into 4 District Offices. This dataset provides a California healthcare facilities interdepartmental crosswalk using the Department of Health Care Access and Information (HCAI) assigned licensed facility identification numbers linked with matched California Department of Public Health (CDPH), Licensing and Certification facility lists based on license number housed in the Electronic Licensing Management System (ELMS) and/or the CMS Certification Number (CCN) from the Federal Automated Survey Process Environment (ASPEN) databases. This is not a comprehensive matched list, facility identification numbers that did not match are also included from both the HCAI and CDPH lists. Facility Status or Facility Level designations may explain some HCAI non-matches, for additional information contact HCAI directly. Please contact CDPH directly for more information regarding un-matched facility identifiers that do not have corresponding OSHPD identifiers.

The “Health Facilities’ State Enforcement Actions” dataset includes summary information for state enforcement actions (state citations or administrative penalties) issued to California healthcare facilities. This file, a sub-set of the ELMS system data, includes state enforcement actions that have been issued from July 1, 1997 through June 30, 2024. Data are presented for each citation/penalty, and include information about the type of enforcement action, violation category, penalty amount, violation date, appeal status, and facility.
The “LTC Citation Narrative” dataset contains the full text of citations that were issued to long-term care (LTC) facilities between January 1, 2012 – December 31, 2017. DO NOT DOWNLOAD in Excel as this file has large blocks of text which may truncate. For example, Excel 2007 and later display, and allow up to, 32,767 characters in each cell, whereas earlier versions of Excel allow 32,767 characters, but only display the first 1,024 characters. This list includes detail information about every Point of Investigation (POI) water rights record in the State Water Resources Control Board's "Electronic Water Rights Information Management System" (EWRIMS) database. A Point of Investigation is the geographic area associated to a water right investigation. POI is a location of interest during an investigation and there may be several POIs in an investigation. Generally, POIs can be permit/license/statement PODs, places of use, point of alleged unauthorized diversion, place of alleged unauthorized use. Each row in the dataset corresponds with a unique Point of Investigation ID and its associated data. The list include basic summary information about the Water Right Investigation record, such as the type and status, the location of the Point of Investigation, the inspections and summary data associated with the electronic Water Right record. This file is in flat file format and may not include all information associated to a water right such all uses and seasons or the amounts reported used for every month. That information may be available in the associated flat files for each category.
