The 2019-20 Gambia Demographic and Health Survey (2019-20 GDHS) is a nationwide survey with a nationally representative sample of residential households. The survey was implemented by The Gambia Bureau of Statistics (GBoS) in collaboration with the Ministry of Health (MoH).
The primary objective of the 2019-20 GDHS is to provide up-to-date estimates of basic demographic and health indicators. Specifically, the 2019-20 GDHS: ▪ collected data on fertility levels and preferences; contraceptive use; maternal and child health; infant, child, and neonatal mortality levels; maternal mortality; gender; nutrition; awareness about HIV/AIDS; self-reported sexually transmitted infections (STIs); and other health issues relevant to the achievement of the Sustainable Development Goals (SDGs) ▪ obtained information on the availability of, access to, and use of mosquito nets as part of the National Malaria Control Programme ▪ gathered information on other health issues such as injections, tobacco use, hypertension, diabetes, and health insurance ▪ collected data on women’s empowerment, domestic violence, fistula, and female genital mutilation/cutting ▪ tested household salt for the presence of iodine ▪ obtained data on child feeding practices, including breastfeeding, and conducted anthropometric measurements to assess the nutritional status of children under age 5 and women age 15-49 ▪ conducted anaemia testing of women age 15-49 and children age 6-59 months ▪ conducted malaria testing of children age 6-59 months

The survey covered all de jure household members (usual residents), all women aged 15-49, all men age 15-59, and all children aged 0-5 resident in the household.
Sample survey data [ssd]
The sampling frame used for the 2019-20 GDHS was based on an updated version of the 2013 Gambia Population and Housing Census (2013 GPHC) conducted by GBoS. The census counts were updated in 2015-16 based on district-level projected counts from the 2015-16 Integrated Household Survey (IHS). Administratively, The Gambia is divided into eight Local Government Areas (LGAs). Each LGA is subdivided into districts and each district is subdivided into settlements. A settlement, a group of small settlements, or a part of a large settlement can form an enumeration area (EA). These units allow the country to be easily separated into small geographical area units, each with an urban or rural designation. There are 48 districts, 120 wards, and 4,098 EAs in The Gambia; the EAs have an average size of 68 households.
The sample for the 2019-20 GDHS was a stratified sample selected in two stages. In the first stage, EAs were selected with a probability proportional to their size within each sampling stratum. A total of 281 EAs were selected.
In the second stage, the households were systematically sampled. A household listing operation was undertaken in all of the selected clusters. The resulting lists of households served as the sampling frame from which a fixed number of 25 households were systematically selected per cluster, resulting in a total sample size of 7,025 selected households. Results from this sample are representative at the national, urban, and rural levels and at the LGA levels.

Computer Assisted Personal Interview [capi]
Five questionnaires were used for the 2019-20 GDHS: the Household Questionnaire, the Woman’s Questionnaire, the Man’s Questionnaire, the Biomarker Questionnaire, and the Fieldworker Questionnaire. These questionnaires, based on The DHS Program’s standard questionnaires, were adapted to reflect the population and health issues relevant to The Gambia. Suggestions were solicited from various stakeholders representing government ministries, departments, and agencies; nongovernmental organisations; and international donors. All questionnaires were written in English, and interviewers translated the questions into the appropriate local language to carry out the interview.
All electronic data files were transferred via the Internet File Streaming System (IFSS) to the GBoS central office. The IFSS automatically encrypts the data and sends the data to a server, and the server in turn downloads the data to the data processing supervisor’s password-protected computer in the central office. The data processing operation included secondary editing, which required resolution of computeridentified inconsistencies and coding of open-ended questions. The data were processed by two IT specialists and three secondary editors who took part in the main fieldwork training; they were supervised remotely by staff from The DHS Program. Data editing was accomplished using CSPro software. During the fieldwork, field-check tables were generated to check various data quality parameters, and specific feedback was given to the teams to improve performance. Secondary editing and data processing were initiated in November 2019 and completed in May 2020.
All 6,985 households in the selected housing units were eligible for the survey, of which 6,736 were occupied. Of the occupied households, 6,549 were successfully interviewed, yielding a response rate of 97%. Among the households successfully interviewed, 1,948 interviews were completed in 2019 and 4,601 in 2020.
In the interviewed households, 12,481 women age 15-49 were identified for individual interviews; interviews were completed with 11,865 women, yielding a response rate of 95%, a 4 percentage point increase from the 2013 GDHS. Among men, 5,337 were eligible for individual interviews, and 4,636 completed an interview; this yielded a response rate of 87%, a 5 percentage point increase from the previous survey.
The estimates from a sample survey are affected by two types of errors: nonsampling errors and sampling errors. Nonsampling errors are the results of mistakes made in implementing data collection and data processing, such as failure to locate and interview the correct household, misunderstanding of the questions on the part of either the interviewer or the respondent, and data entry errors. Although numerous efforts were made during the implementation of the 2019-20 Gambia Demographic and Health Survey (GDHS) to minimise this type of error, nonsampling errors are impossible to avoid and difficult to evaluate statistically.
Sampling errors, on the other hand, can be evaluated statistically. The sample of respondents selected in the 2019-20 GDHS is only one of many samples that could have been selected from the same population, using the same design and expected size. Each of these samples would yield results that differ somewhat from the results of the actual sample selected. Sampling errors are a measure of the variability among all possible samples. Although the degree of variability is not known exactly, it can be estimated from the survey results.
Sampling error is usually measured in terms of the standard error for a particular statistic (mean, percentage, etc.), which is the square root of the variance. The standard error can be used to calculate confidence intervals within which the true value for the population can reasonably be assumed to fall. For example, for any given statistic calculated from a sample survey, the value of that statistic will fall within a range of plus or minus two times the standard error of that statistic in 95% of all possible samples of identical size and design.
If the sample of respondents had been selected as a simple random sample, it would have been possible to use straightforward formulas for calculating sampling errors. However, the 2019-20 GDHS sample is the result of a multi-stage stratified design, and, consequently, it was necessary to use more complex formulas. Sampling errors are computed in SAS, using programs developed by ICF. These programs use the Taylor linearisation method to estimate variances for survey estimates that are means, proportions, or ratios. The Jackknife repeated replication method is used for variance estimation of more complex statistics such as fertility and mortality rates.

The Indonesia Demographic and Health Survey (IDHS), which is part of the Demographic and Health Surveys (DHS) Project, is one of prominent national surveys in the field of population, family planning, and health. The survey is not only important nationally for planning and evaluating population, family planning, and health developments, but is also important internationally since IDHS has been designed so uniquely that it can be compared with similar surveys in other developing countries.
The 1997 Indonesia Demographic and Health Survey (IDHS) is a follow-on project to the 1987 National Indonesia Contraceptive Prevalence Survey (NICPS), the 1991 IDHS, and the 1994 IDHS. The 1997 IDHS was expanded from the 1994 survey to include a module on family welfare; however, unlike the 1994 survey, the 1997 survey no longer investigated the availability of family planning and health services. The 1997 IDHS also included as part of the household schedule a household expenditure module that provided a means of identifying the household's economic status.
The 1997 IDHS was specifically designed to meet the following objectives: - Provide data concerning fertility, family planning, maternal and child health, maternal mortality, and awareness of AIDS that can be used by program managers, policymakers, and researchers to evaluate and improve existing programs - Provide data about availability of family planning and health services, thereby offering an opportunity for linking women's fertility, family planning, and child care behavior with the availability of services - Provide household expenditure data that which can be used to identify the household's economic status - Provide data that can be used to analyze trends over time by examining many of the same fertility, mortality, and health issues that were addressed in the earlier surveys (1987 NICPS, 1991 IDHS and 1994 IDHS) - Measure changes in fertility and contraceptive prevalence rates and at the same time study factors that affect the changes, such as marriage patterns, urban/rural residence, education, breastfeeding habits, and the availability of contraception - Measure the development and achievements of programs related to health policy, particularly those concerning the maternal and child health development program implemented through public health clinics in Indonesia - Provide indicators for classifying families according to their welfare status.
National
Sample survey data
Indonesia is divided into 27 provinces. For the implementation of its family planning program, the National Family Planning Coordinating Board (NFPCB) has divided these provinces into three regions as follows:
The 1990 Population Census of Indonesia shows that Java-Bali accounts for 62 percent of the national population, Outer Java-Bali I accounts for 27 percent, and Outer Java-Bali II accounts for 11 percent. The sample for the 1997 IDHS was designed to produce reliable estimates of fertility, contraceptive prevalence and other important variables for each of the provinces and urban and rural areas of the three regions.
In order to meet this objective, between 1,650 and 2,050 households were selected in each of the provinces in Java-Bali, 1,250 to 1,500 households in the ten provinces in Outer Java-Bali I, and 1,000 to 1,250 households in each of the provinces in Outer Java-Bali II, for a total of 35,500 households. With an average of O.8 ever-married women 15-49 per household, the sample was expected to yield approximately 28,000 women eligible for the individual interview.

The 1997 IDHS used three questionnaires: the household questionnaire, the questionnaire on family welfare, and the individual questionnaire for ever-married women 15-49 years old. The general household and individual questionnaires were based on the DHS Model "A" Questionnaire, which is designed for use in countries with high contraceptive prevalence. Additions and modifications to the model questionnaire were made in order to provide detailed information specific to Indonesia. The questionnaires were developed mainly in English and were translated into Indonesian. One deviation from the standard DHS practice is the exclusion of the anthropometric measurement of young children and their mothers. A separate survey carried out by MOH provides this information.
The household questionnaire includes an expenditure schedule adapted from the core Susenas questionnaire model. Susenas is a national household survey carried out annually by CBS to collect data on various demographic and socioeconomic indicators of the population. The family welfare questionnaire was aimed at collecting indicators developed by the NFPCB to classify families according to their welfare status. Families were identified from the list of household members in the household questionnaire. The expenditure module and the family welfare questionnaire were developed in Indonesian.
The first stage of data editing was carried out by the field editors who checked the completed questionnaires for thoroughness and accuracy. Field supervisors then further examined the questionnaires. In many instances, the teams sent the questionnaires to CBS through the regency/municipality statistics offices. In these cases, no checking was done by the PSO. In other cases, Technical Coordinators are responsible for reviewing the completeness of the forms. At CBS, the questionnaires underwent another round of editing, primarily for completeness and coding of responses to open-ended questions. The data were processed using microcomputers and the DHS computer program, ISSA (Integrated System for Survey Analysis). Data entry and office editing were initiated immediately after fieldwork began. Simple range and skip errors were corrected at the data entry stage. Data processing was completed by February 1998, and the preliminary report of the survey was published in April 1998.
A total of 35,362 households were selected for the survey, of which 34,656 were found. Of the encountered households, 34,255 (99 percent) were successfully interviewed. In these households, 29,317 eligible women were identified, and complete interviews were obtained from 28,810 women, or 98 percent of all eligible women. The generally high response rates for both household and individual interviews were due mainly to the strict enforcement of the rule to revisit the originally selected household if no one was at home initially. No substitution for the originally selected households was allowed. Interviewers were instructed to make at least three visits in an effort to contact the household or eligible woman.

The 2015-16 Myanmar Demographic and Health Survey (2015-16 MDHS) is the first DHS survey to be conducted in Myanmar. A nationally representative sample of about 13,260 households was selected. All women age 15-49 who were usual residents of the selected households or who slept in the households the night before the survey were eligible for the survey. Apart from the women’s survey, a men’s survey was also conducted at the same time in a subsample consisting of one household in every second household selected for the female survey. All men age 15-49 who were usual residents of the selected households or who slept in the households the night before the survey were eligible for the male survey.
In all of the selected households, parents or guardians of children age 6-59 months asked permission to collect a blood sample through a finger prick, also used to test for anemia. These children were also weighed and measured to obtain anthropometric indicators. Anemia testing and anthropometric measurements were also obtained for women age 15-49 in the sample households.
The primary objective of the 2015-16 MDHS was to provide up-to-date estimates of basic demographic and health indicators. Specifically, the survey collected information on fertility levels, marital status, fertility preferences, awareness and use of family planning methods, breastfeeding practices, nutrition, mother and child mortality and health, HIV/AIDS and other sexually transmitted infections (STIs), and other health-related issues, such as smoking and knowledge of tuberculosis.
The information collected through the 2015-16 MDHS is intended to assist policy makers and program managers in evaluating and designing programs and strategies for improving the health of the country’s population. Moreover, this survey has come at a beneficial time for Myanmar, as the results will be used to develop the next 5-year National Health Plan (2017-2021) and to update the national comprehensive development plan.

Sample survey data [ssd]
The sample was based on the 2014 census frame, which is used to coordinate household-based surveys conducted in Myanmar, including the current 2015-16 MDHS. The master sample is a large, nationally representative sample consisting of 4,000 PSUs drawn from the entire census frame; these can be used for sub-selecting multi-stage household-based survey samples.
The 2015-16 MDHS followed a stratified two-stage sample design and was intended to allow estimates of key indicators at the national level, in urban and rural areas, and for each of the seven States and eight Regions of Myanmar. The first stage involved selecting sample points (clusters) consisting of EAs or ward/village tracts. A total of 442 clusters (123 urban and 319 rural) were selected from the master sample.
At the second stage, a fixed number of 30 households was selected from each of the selected clusters (a total of 13,260 households), using equal probability systematic sampling. For the clusters, which were completely enumerated during the population census, the census household listings were taken as the base and updated in the field by the household listing teams. These updated lists were used for selecting the sample households. For the clusters that were not enumerated or partially enumerated during the census, an independent household listing operation was carried out. Because of the non-proportional sample allocation, the sample was not a self-weighting sample. Weighting factors had to be calculated, added to the data file, and applied so that results are representative at the national as well as regional level.
All women age 15-49 who were either permanent residents of the selected households or visitors who stayed in the households the night before the survey were eligible to be interviewed. In half of the selected households (every second household), all men age 15-49 who were either residents or visitors who stayed in the household the night before the survey were eligible to be interviewed.

During the course of the fieldwork, 4 clusters were identified as insecure and were replaced with other clusters in the vicinity. In addition, 1 urban cluster had to be dropped due to worsening security.

Three sets of questionnaires were used in the 2015-16 MDHS: a Household Questionnaire, a Woman’s Questionnaire, and a Man’s Questionnaire. These questionnaires, developed for the worldwide DHS program, were revised to accord with Myanmar culture as well as to reflect some country-specific health issues.
The 2015-16 MDHS used computer-assisted field editing (CAFE) procedures with tablet computers. Thus, data processing began simultaneously with the fieldwork. All completed questionnaires were entered into the tablets while in the field by the field editors after they edited them on paper. Entries were checked by the supervisors before the questionnaires were dispatched to the data processing center at the MoHS central office in Nay Pyi Taw. These completed questionnaires were reviewed and re-entered by 13 data processing personnel specially trained for this task. All data were thus entered twice (100 percent verification), once in the field by the field editors and then again in the data processing center in Nay Pyi Taw. Data were entered using the CSPro computer package. The operation included secondary editing, using CSPro software, to resolve computer-identified inconsistencies and to code open-ended questions. The concurrent processing of the data offered a distinct advantage, because it maximized the likelihood of the data being error-free and accurate. Moreover, the double entry of data enabled easy comparison and identification of errors and inconsistencies. Inconsistencies were resolved by tallying with the paper questionnaire entries.
The secondary editing was implemented by four editors and was completed in the second week of July 2016. The final cleaning of the data set was carried out by the DHS Program data processing specialist by the end of July 2016.
The total number of households selected was 13,238, of which 12,780 households were occupied. Of those occupied, 12,500 households were interviewed, yielding a 98% response rate.
In the interviewed households, 13,454 women were identified as eligible for the individual Woman’s Questionnaire. Interviews were successfully completed with 12,885 women, yielding a 96% response rate. In the subsample of one-half of the households, 5,218 men were identified as eligible for individual interview. Interviews were completed for 4,737 men, with a 91% response rate.

The Demographic and Health Survey is mainly concerned with the determination of fertility, infant mortality rates and closely related issues. Questions surrounding respondent’s background, reproduction, contraception, health and breastfeeding, marriage, fertility preferences, and husband’s background and woman’s work were asked. This study consists of two datasets, one household dataset and the other an individual dataset, the respondent being a female of reproductive age that has already given birth or who is married or exposed to pregnancy. Females qualifying for the individual interview schedule were chosen from the responses to household (cover) questionnaire.

Specifically, the ZDHS collected information on: - Fertility levels and preferences; contraceptive use; maternal and child health; infant, child, and neonatal mortality levels; maternal mortality; and gender, nutrition, and awareness regarding HIV/AIDS and other health issues relevant to the achievement of the Sustainable Development Goals (SDGs) - Ownership and use of mosquito nets as part of the national malaria eradication programmes - Health-related matters such as breastfeeding, maternal and childcare (antenatal, delivery, and postnatal), children’s immunisations, and childhood diseases - Anaemia prevalence among women age 15-49 and children age 6-59 months - Nutritional status of children under age 5 (via weight and height measurements) - HIV prevalence among men age 15-59 and women age 15-49 and behavioural risk factors related to HIV - Assessment of situation regarding violence against women

Sample survey data [ssd]
The sampling frame used for the 2018 ZDHS is the Census of Population and Housing (CPH) of the Republic of Zambia, conducted in 2010 by ZamStats. Zambia is divided into 10 provinces. Each province is subdivided into districts, each district into constituencies, and each constituency into wards. In addition to these administrative units, during the 2010 CPH each ward was divided into convenient areas called census supervisory areas (CSAs), and in turn each CSA was divided into enumeration areas (EAs). An enumeration area is a geographical area assigned to an enumerator for the purpose of conducting a census count; according to the Zambian census frame, each EA consists of an average of 110 households.
The current version of the EA frame for the 2010 CPH was updated to accommodate some changes in districts and constituencies that occurred between 2010 and 2017. The list of EAs incorporates census information on households and population counts. Each EA has a cartographic map delineating its boundaries, with identification information and a measure of size, which is the number of residential households enumerated in the 2010 CPH. This list of EAs was used as the sampling frame for the 2018 ZDHS.
The 2018 ZDHS followed a stratified two-stage sample design. The first stage involved selecting sample points (clusters) consisting of EAs. EAs were selected with a probability proportional to their size within each sampling stratum. A total of 545 clusters were selected.
The second stage involved systematic sampling of households. A household listing operation was undertaken in all of the selected clusters. During the listing, an average of 133 households were found in each cluster, from which a fixed number of 25 households were selected through an equal probability systematic selection process, to obtain a total sample size of 13,625 households. Results from this sample are representative at the national, urban and rural, and provincial levels.

Four questionnaires were used in the 2018 ZDHS: the Household Questionnaire, the Woman’s Questionnaire, the Man’s Questionnaire, and the Biomarker Questionnaire. In addition, information about the fieldworkers for the survey was collected through a self-administered Fieldworker Questionnaire.
All electronic data files were transferred via a secure internet file streaming system to the ZamStats central office in Lusaka, where they were stored on a password-protected computer. The data processing operation included secondary editing, which required resolution of computer-identified inconsistencies and coding of open-ended questions. The data were processed by two IT specialists and one secondary editor who took part in the main fieldwork training; they were supervised remotely by staff from The DHS Program. Data editing was accomplished using CSPro software. During the fieldwork, field-check tables were generated to check various data quality parameters, and specific feedback was given to the teams to improve performance. Secondary editing and data processing were initiated in July 2018 and completed in March 2019.
Of the 13,595 households in the sample, 12,943 were occupied. Of these occupied households, 12,831 were successfully interviewed, yielding a response rate of 99%.
In the interviewed households, 14,189 women age 15-49 were identified as eligible for individual interviews; 13,683 women were interviewed, yielding a response rate of 96% (the same rate achieved in the 2013-14 survey). A total of 13,251 men were eligible for individual interviews; 12,132 of these men were interviewed, producing a response rate of 92% (a 1 percentage point increase from the previous survey).
Of the households successfully interviewed, 12,505 were interviewed in 2018 and 326 in 2019. As the large majority of households were interviewed in 2018 and the year for reference indicators is 2018.
The estimates from a sample survey are affected by two types of errors: nonsampling errors and sampling errors. Nonsampling errors are the results of mistakes made in implementing data collection and data processing, such as failure to locate and interview the correct household, misunderstanding of the questions on the part of either the interviewer or the respondent, and data entry errors. Although numerous efforts were made during the implementation of the 2018 Zambia Demographic and Health Survey (ZDHS) to minimise this type of error, nonsampling errors are impossible to avoid and difficult to evaluate statistically.
Sampling errors, on the other hand, can be evaluated statistically. The sample of respondents selected in the 2018 ZDHS is only one of many samples that could have been selected from the same population, using the same design and expected size. Each of these samples would yield results that differ somewhat from the results of the actual sample selected. Sampling errors are a measure of the variability among all possible samples. Although the degree of variability is not known exactly, it can be estimated from the survey results.
Sampling error is usually measured in terms of the standard error for a particular statistic (mean, percentage, etc.), which is the square root of the variance. The standard error can be used to calculate confidence intervals within which the true value for the population can reasonably be assumed to fall. For example, for any given statistic calculated from a sample survey, the value of that statistic will fall within a range of plus or minus two times the standard error of that statistic in 95% of all possible samples of identical size and design.
If the sample of respondents had been selected as a simple random sample, it would have been possible to use straightforward formulas for calculating sampling errors. However, the 2018 ZDHS sample is the result of a multi-stage stratified design, and, consequently, it was necessary to use more complex formulas. Sampling errors are computed in SAS, using programs developed by ICF. These programs use the Taylor linearisation method to estimate variances for survey estimates that are means, proportions, or ratios. The Jackknife repeated replication method is used for variance estimation of more complex statistics such as fertility and mortality rates.

The 2019-20 Liberia Demographic and Health Survey (2019-20 LDHS) is a nationwide survey with a nationally representative sample of residential households. All women age 15-49 who are usual members of the selected households or who spent the night before the survey in the selected households were eligible for individual interviews. The primary objective of the 2019-20 LDHS is to provide up-to-date estimates of key demographic and health indicators necessary for program managers, policymakers, and implementers to monitor and evaluate the impact of existing policies and programs and to design new initiatives for health policies in Liberia. This survey is considered a key resource for the new sixth National Health Strategic Plan (NHSP) 2017-2021.
Specifically, the main objectives of the survey are: - To collect high-quality data on fertility levels and preferences; contraceptive use; maternal and child health; neonatal, infant, and child mortality levels; maternal mortality; and other health issues relevant to the achievement of the Sustainable Development Goals (SDGs) (e.g., gender, nutrition, awareness regarding HIV/AIDS) - To provide information on availability of, access to, and use of mosquito nets as part of national malaria control programs - To assess protection of children from violence and exploitation - To provide information on other health issues, such as tobacco use, tuberculosis, and health insurance - To obtain data on women’s empowerment, domestic violence, and female genital cutting - To test household salt for the presence of iodine - To obtain data on child feeding practices, including breastfeeding, and collect anthropometric measures to assess the nutritional status of children under age 5 and women age 15-49 - To conduct anemia testing of women age 15-49 and children age 6-59 months - To measure HIV prevalence levels among men age 15-59 and women age 15-49 - To measure hepatitis B and C prevalence levels among men age 15-59 and women age 15-49 - To measure the seroprevalence of Ebola virus disease (EVD) antibodies among men age 15-59 and women age 15-49 and collect data on risk factors related to Ebola

The survey covered all de jure household members (usual residents), all women aged 15-49, all men age 15-59, and all children aged 0-5 resident in the household.
Sample survey data [ssd]
The sampling frame used for the 2019-20 LDHS is based on the 2008 National Population and Housing Census (NPHC), conducted by the LISGIS. Liberia is divided into 15 counties grouped to form five geographical regions, with each region consisting of three counties. Each county is divided into districts and each district into clans. In the 2008 NPHC, each clan was subdivided into enumeration areas (EAs). An enumeration area is a geographical area assigned to an enumerator for the purpose of conducting a census count; according to the Liberian census frame, each EA consists of an average of 100 households.
The 2019-20 LDHS followed a stratified two-stage cluster design. The first stage involved selecting sample points (clusters) consisting of EAs. EAs were drawn with a probability proportional to their size within each sampling stratum. A total of 325 clusters were selected.
The second stage involved systematic sampling of households. A household listing operation was undertaken in all of the selected clusters. During the listing, an average of 129 households were found in each cluster, from which a fixed number of 30 households were selected with an equal probability systematic selection process; the total sample size was 9,745 households. Results from this sample will be representative at the national, urban (Greater Monrovia and all other urban areas), and rural levels, including each of the five regions. The survey will also produce separate representative results for most key indicators of the 15 counties.

Computer Assisted Personal Interview [capi]
Seven questionnaires were used for the 2019-20 LDHS: the Household Questionnaire, the Woman’s Questionnaire, the Man’s Questionnaire, the Biomarker Questionnaire Part A, the Biomarker Questionnaire Part B, the Biomarker Revisit Questionnaire, and the Fieldworker Questionnaire. These questionnaires, based on The DHS Program’s standard questionnaires, were adapted to reflect the population and health issues relevant to Liberia. Suggestions were solicited from various stakeholders representing government ministries and agencies, nongovernmental organizations, and international donors. After all questionnaires were finalized in English, they were translated into a form of simple English commonly understood in Liberia.
Data processing for the 2019-20 LDHS began a few days after fieldwork started. As data collection was completed for each cluster, team supervisors transferred all electronic data files to the LISGIS central office in Monrovia via the Internet File Streaming System (IFSS), where they were stored on a password-protected computer. IFSS automatically encrypts the data and sends the data to a server, which in turn downloads the data to the data processing supervisor’s password-protected computer in the central office. These data files were registered and checked for inconsistencies, incompleteness, and outliers. Field supervisors were alerted of and resolved any errors any issues found.
The LISGIS data processing operation also included secondary editing, which required resolution of computeridentified inconsistencies and coding of open-ended questions. The data were processed by the LISGIS data processing manager and two secondary editors who took part in the pretest and main fieldwork training; they were supervised remotely by staff from The DHS Program. Data editing was accomplished using Censuses and Survey Processing (CSPro) software.
Biomarker paper questionnaires were compared with electronic data files to check for any inconsistencies in data entry. Daily generation of check reports in addition to weekly generation of field-check tables allowed for effective monitoring. Specific feedback was given to the teams to improve their performance. Secondary editing and data processing were initiated in October 2019 and completed in March 2020.
All 9,745 households in the selected housing units were eligible for the survey, and 9,207 of these households were occupied. Of the occupied households, 9,068 were successfully interviewed, yielding a response rate of 99%. Of the successful household interviews, 5,192 were completed in 2019 and 3,876 in 2020.
In the interviewed households, 8,364 women age 15-49 were identified for individual interviews; 8,065 women were interviewed, yielding a response rate of 96%. A total of 4,527 men were eligible for individual interviews; 4,249 of these men were interviewed, producing a response rate of 94%.

The 2022 Nepal Demographic and Health Survey (NDHS) is the sixth survey of its kind implemented in the country as part of the worldwide Demographic and Health Surveys (DHS) Program. It was implemented by New ERA under the aegis of the Ministry of Health and Population (MoHP) of the Government of Nepal with the objective of providing reliable, accurate, and up-to-date data for the country.
The primary objective of the 2022 NDHS is to provide up-to-date estimates of basic demographic and health indicators. Specifically, the 2022 NDHS collected information on fertility, marriage, family planning, breastfeeding practices, nutrition, food insecurity, maternal and child health, childhood mortality, awareness and behavior regarding HIV/AIDS and other sexually transmitted infections (STIs), women’s empowerment, domestic violence, fistula, mental health, accident and injury, disability, and other healthrelated issues such as smoking, knowledge of tuberculosis, and prevalence of hypertension.
The information collected through the 2022 NDHS is intended to assist policymakers and program managers in evaluating and designing programs and strategies for improving the health of Nepal’s population. The survey also provides indicators relevant to the Sustainable Development Goals (SDGs) for Nepal.

Sample survey data [ssd]
The sampling frame used for the 2022 NDHS is an updated version of the frame from the 2011 Nepal Population and Housing Census (NPHC) provided by the National Statistical Office. The 2022 NDHS considered wards from the 2011 census as sub-wards, the smallest administrative unit for the survey. The census frame includes a complete list of Nepal’s 36,020 sub-wards. Each sub-ward has a residence type (urban or rural), and the measure of size is the number of households.
In September 2015, Nepal’s Constituent Assembly declared changes in the administrative units and reclassified urban and rural areas in the country. Nepal is divided into seven provinces: Koshi Province, Madhesh Province, Bagmati Province, Gandaki Province, Lumbini Province, Karnali Province, and Sudurpashchim Province. Provinces are divided into districts, districts into municipalities, and municipalities into wards. Nepal has 77 districts comprising a total of 753 (local-level) municipalities. Of the municipalities, 293 are urban and 460 are rural.
Originally, the 2011 NPHC included 58 urban municipalities. This number increased to 217 as of 2015. On March 10, 2017, structural changes were made in the classification system for urban (Nagarpalika) and rural (Gaonpalika) locations. Nepal currently has 293 Nagarpalika, with 65% of the population living in these urban areas. The 2022 NDHS used this updated urban-rural classification system. The survey sample is a stratified sample selected in two stages. Stratification was achieved by dividing each of the seven provinces into urban and rural areas that together formed the sampling stratum for that province. A total of 14 sampling strata were created in this way. Implicit stratification with proportional allocation was achieved at each of the lower administrative levels by sorting the sampling frame within each sampling stratum before sample selection, according to administrative units at the different levels, and by using a probability-proportional-to-size selection at the first stage of sampling. In the first stage of sampling, 476 primary sampling units (PSUs) were selected with probability proportional to PSU size and with independent selection in each sampling stratum within the sample allocation. Among the 476 PSUs, 248 were from urban areas and 228 from rural areas. A household listing operation was carried out in all of the selected PSUs before the main survey. The resulting list of households served as the sampling frame for the selection of sample households in the second stage. Thirty households were selected from each cluster, for a total sample size of 14,280 households. Of these households, 7,440 were in urban areas and 6,840 were in rural areas. Some of the selected sub-wards were found to be overly large during the household listing operation. Selected sub-wards with an estimated number of households greater than 300 were segmented. Only one segment was selected for the survey with probability proportional to segment size.

Computer Assisted Personal Interview [capi]
Four questionnaires were used in the 2022 NDHS: the Household Questionnaire, the Woman’s Questionnaire, the Man’s Questionnaire, and the Biomarker Questionnaire. The questionnaires, based on The DHS Program’s model questionnaires, were adapted to reflect the population and health issues relevant to Nepal. In addition, a self-administered Fieldworker Questionnaire collected information about the survey’s fieldworkers.
Input was solicited from various stakeholders representing government ministries and agencies, nongovernmental organizations, and international donors. After all questionnaires were finalized in English, they were translated into Nepali, Maithili, and Bhojpuri. The Household, Woman’s, and Man’s Questionnaires were programmed into tablet computers to facilitate computer-assisted personal interviewing (CAPI) for data collection purposes, with the capability to choose any of the three languages for each questionnaire. The Biomarker Questionnaire was completed on paper during data collection and then entered in the CAPI system.
Data capture for the 2022 NDHS was carried out with Microsoft Surface Go 2 tablets running Windows 10.1. Software was prepared for the survey using CSPro. The processing of the 2022 NDHS data began shortly after the fieldwork started. When data collection was completed in each cluster, the electronic data files were transferred via the Internet File Streaming System (IFSS) to the New ERA central office in Kathmandu. The data files were registered and checked for inconsistencies, incompleteness, and outliers. Errors and inconsistencies were immediately communicated to the field teams for review so that problems would be mitigated going forward. Secondary editing, carried out in the central office at New ERA, involved resolving inconsistencies and coding the open-ended questions. The New ERA senior data processor coordinated the exercise at the central office. The NDHS core team members assisted with the secondary editing. The paper Biomarker Questionnaires were compared with the electronic data file to check for any inconsistencies in data entry. The pictures of vaccination cards that were captured during data collection were verified with the data entered. Data processing and editing were carried out using the CSPro software package. The concurrent data collection and processing offered a distinct advantage because it maximized the likelihood of the data being error-free and accurate. Timely generation of field check tables allowed for effective monitoring. The secondary editing of the data was completed by July 2022, and the final cleaning of the data set was completed by the end of August.
A total of 14,243 households were selected for the sample, of which 13,833 were found to be occupied. Of the occupied households, 13,786 were successfully interviewed, yielding a response rate of more than 99%. In the interviewed households, 15,238 women age 15-49 were identified as eligible for individual interviews. Interviews were completed with 14,845 women, yielding a response rate of 97%. In the subsample of households selected for the men’s survey, 5,185 men age 15-49 were identified as eligible for individual interviews and 4,913 were successfully interviewed, yielding a response rate of 95%.

The major objective of this survey was to provide up-to-date and accurate information on fertility, contraception, child mortality, child nutrition and health status of children.
This sample survey is further intended to serve as a source of demographic data for comparison with earlier surveys such as Sri Lanka Demographic and Health Survey 1987 (DHS87) and Sri Lanka Contraceptive Prevalence Survey 1982 (CPS82). Such comparisons help to understand the demographic changes over a period of time.
Two types of questionnaires were used in the survey. ie (1) Household and (2) Individual.

The country has been stratified into nine zones on the basis of socio economic and ecological criteria for DHS87. The same zones were used without major changes. Although there are nine zones the survey was confined to seven excluding Northern and Eastern provinces; the few areas covered in Amparai district in the Eastern Province during DHS87 had to be excluded due to security reasons of the country.

The survey interviews were designed to obtain responses from all usual residents and any visitors who slept in the household the night before the interview. An eligible respondent was defined as an ever married woman aged 15 - 49 years who slept in the household the night before the interview.

Selection process : The sample is a multi-stage stratified probability sample representative of the entire country excluding Northern and Eastern Provinces. The country has been stratified into nine zones on the basis of socio-economic and ecological criteria for DHS87. The same zones were used without major changes. Although there are nine zones the survey was confined to seven, excluding Northern and Eastern Provinces. The seven zones are:
Zone 1 - Colombo Metro consisting some urban areas in Colombo and Gampaha District Zone 2 - Colombo feeder areas Zone 3 - South Western coastal low lands Zone 4 - Lower South Central hill country excluding Districts with a concentration of estates Zone 5 - South Central hill country with a concentration of estates Zone 6 - Irrigated dry zone with major or minor irrigation schemes Zone 7 - Rain-fed Dry zone
Each zone was further stratified into three strata - urban, rural and estate sectors. The number of stages of the design and the Primary Sampling Units (PSU) vary according to the sector.
In urban areas PSU is the ward and generally two census blocks have been selected per ward as the second stage unit. The selections were carried out with probability proportional to size(PPS). The number of housing units was taken as the measure of size.
The PSU's were mostly selected from a specially organized frame consisting of wards and Grama Niladhari divisions organized by zone, sector and within sector geographically. The organization provided a better basis for stratification as it is arranged on a geographical basis.
The census blocks were selected from the only frame available from 1981 Census of Population and Housing. The ever married women aged 15-49 found in the selected housing units were interviewed.
In rural areas, Grama Niladhari (GN) division was taken as PSU and generally a single village has been selected per sample GN division with PPS. As such in rural areas villages form effective PSU's. However special steps were taken to merge and divide the villages to deal with areas which are too small or too large.
Unlike the GN divisions and wards, the selection in the estate sector has to take into account the fact that many estates are very small in size to form proper units for first stage of selection. To avoid the need to group estates in the whole frame special procedure was applied to select estates depending on the relative size of the estate compared to the nearby estates.
The target sample size was 6500 ever married women in the age group 15-49. This includes an over-sampling of around 500 women in five less developed areas in zones 6 and 7. The latter addition to the sample is needed to provide Policy relevant information and permit comparative analysis of these areas. In order to get that target sample, a total of 9007 housing units were selected for the survey.

Household Questionnaire - listed all usual residents any visitors who slept in the household the night before the interview and some basic information was collected on the characteristics of each person listed such as age, sex, marital status, relationship to head of household. The household questionnaire was used to identify women who were eligible for the individual questionnaire.
Individual questionnaire - Administered to each eligible woman who was defined as one who is an ever married female aged between 15 - 49 who slept in the household the night before the interview. This questionnaire had eight sections such as Respondent's background, Reproduction, Contraception, Health of children, Marriage, Fertility, Husband's background, length and weight of infants.

Manual editing covered basic investigations such as checking of identification details, completeness of the questionnaire, coding, age and birth history, checking of certain internal consistencies, checking the information recorded in filter questions and coding of few items.

The sample of women had been selected as a simple sample, it would have been possible to use straightforward formulas for calculating sampling errors. However the sample design for this survey depended on stratification, stages and clusters. The computer package CLUSTERS developed by the International Statistical Institute for the World Fertility Survey was used to assist in computing the sampling errors with the proper statistical methodology.
In general, the sampling errors are small, which implies that the results are reliable.

The primary objective of the 2015-16 Tanzania Demographic and Health Survey and Malaria Indicator Survey (TDHS-MIS) is to provide up-to-date estimates of basic demographic and health indicators. This survey collected information on fertility levels, marriage, sexual activity, fertility preferences, awareness and use of family planning methods, breastfeeding practices, nutrition, childhood and maternal mortality, maternal and child health, malaria, and other health-related issues. In addition, the 2015-16 TDHS-MIS provided estimates of anaemia prevalence among children age 6-59 months and women age 15-49 years, estimates of malaria prevalence among children age 6-59 months, and estimates of iodine concentration in household salt and women’s urine.
The information collected through the 2015-16 TDHS-MIS is intended to assist policy makers and programme managers in evaluating and designing programmes and strategies to improve the health of the country’s population.

Sample survey data [ssd]
Sample Design The sample design for the 2015-16 TDHS-MIS was done in two stages and was intended to provide estimates for the entire country, for urban and rural areas in Tanzania Mainland, and for Zanzibar. For specific indicators such as contraceptive use, the sample design allowed the estimation of indicators for each of the 30 regions (25 regions from Tanzania Mainland and 5 regions from Zanzibar). The first stage involved selecting sample points (clusters), consisting of enumeration areas (EAs) delineated for the 2012 Tanzania Population and Housing Census. A total of 608 clusters were selected.
In the second stage, a systematic selection of households was involved. A complete households listing was carried out for all 608 selected clusters prior to the fieldwork. From the list, 22 households were then systematically selected from each cluster, yielding a representative probability sample of 13,376 households for the 2015-16 TDHS-MIS. To estimate geographic differentials for certain demographic indicators, Tanzania was divided into nine geographic zones. Although these zones are not official administrative areas, this classification system is also used by the Reproductive and Child Health Section of the MoHCDGEC. Grouping the regions into zones allowed a relatively large number of people in the denominator and a reduced sampling error. Note that the zones, defined below, differ slightly from the zones used in previous DHS surveys. Therefore, comparisons across the zones and from survey to survey should be made with caution. All women age 15-49 who were either usual residents or visitors in the household on the night before the survey were included in the 2015-16 TDHS-MIS and were eligible to be interviewed. In a subsample of one-third of all the households selected for the survey, all men age 15-49 were eligible to be interviewed if they were either usual residents or visitors in the household on the night before the survey. In all households, with the parent's or guardian's consent, children age 6-59 months were tested for anaemia and malaria. All interviewed women were tested for anaemia. In the households selected for interviews with men, interviewed women were asked to provide a urine sample and a sample of household salt for laboratory testing to detect the presence of iodine.

Four questionnaires were used for the 2015-16 TDHS-MIS: the Household Questionnaire, the Woman’s Questionnaire, the Man’s Questionnaire, and the Biomarker Questionnaire. These questionnaires were based on the DHS Program’s standard Demographic and Health Survey (DHS) questionnaires. They were adapted to reflect the population and health issues relevant to Tanzania. Inputs were solicited from various stakeholders representing government ministries, departments, and agencies; non-governmental organizations; and development partners. After the preparation of the definitive questionnaires in English, the questionnaires were translated into Kiswahili.
In the 2015-16 TDHS-MIS the first data entry was done concurrently with data collection in the field. After the paper questionnaires were completed, edited, and checked by both the field editor and the supervisor, the data was entered into a tablet equipped with a data entry programme. This was done by the editor. Completed questionnaires were then sent to NBS headquarters, where they were entered for the second time and edited by data processing personnel who were given special training for this task. ICF International provided technical assistance during the entire data processing period.
Processing the data concurrently with data collection allowed for regular monitoring of team performance and data quality. Field check tables were generated regularly during data processing to check various data quality parameters. As a result, feedback was given on a regular basis, encouraging teams to continue in areas of good performance and to correct areas in need of improvement. Feedback was individually tailored to each team. Data entry, which included 100% double entry to minimise keying errors, and data editing, were completed on March 21, 2016. Data cleaning and finalization were completed on April 22, 2016.
A total of 13,360 households were selected for the survey, of which 12,767 were occupied. Of the occupied households, 12,563 were successfully interviewed, yielding a response rate of 98%.
In the interviewed households, 13,634 eligible women were identified for individual interviews; interviews were completed with 13,266 women, yielding a response rate of 97%. In the subsample of households selected for the male survey, 3,822 eligible men were identified and 3,514 were successfully interviewed, yielding a response rate of 92%. There is little variation in household response rates between rural and urban residences.
The estimates from a sample survey are affected by two types of errors: (1) nonsampling errors, and (2) sampling errors. Nonsampling errors are the results of mistakes made in implementing data collection and data processing, such as failure to locate and interview the correct household, misunderstanding the questions on the part of either the interviewer or the respondent, and data entry errors. Although numerous efforts were made during the implementation of the 2015 Tanzania Demographic and Health Survey (TDHS) to minimize this type of error, nonsampling errors are impossible to avoid and difficult to evaluate statistically.
Sampling errors, on the other hand, can be evaluated statistically. The sample of respondents selected in the 2015 TDHS is only one of many samples that could have been selected from the same population, using the same design and identical size. Each of these samples would yield results that differ somewhat from the results of the actual sample selected. Sampling errors are a measure of the variability between all possible samples. Although the degree of variability is not known exactly, it can be estimated from the survey results.
A sampling error is usually measured in terms of the standard error for a particular statistic (mean, percentage, etc.), which is the square root of the variance. The standard error can be used to calculate confidence intervals within which the true value for the population can reasonably be assumed to fall. For example, for any given statistic calculated from a sample survey, the value of that statistic will fall within a range of plus or minus two times the standard error of that statistic in 95 percent of all possible samples of identical size and design.
If the sample of respondents had been selected as a simple random sample, it would have been possible to use straightforward formulas for calculating sampling errors. However, the 2015 TDHS sample is the result of a multi-stage stratified design, and, consequently, it was necessary to use more complex formulas. The computer software used to calculate sampling errors for the 2015 TDHS is a SAS program. This program used the Taylor linearization method for variance estimation for survey estimates that are means or proportions. The Jackknife repeated replication method was used for variance estimation of more complex statistics such as fertility and mortality rates.
The Taylor linearization method treats any percentage or average as a ratio estimate, r = y/x, where y represents the total sample value for variable y, and x represents the total number of cases in the group or subgroup under consideration.

Stastistics South Africa (Stats SA), in partnership with the South African Medical Research Council (SAMRC), conducted the South Africa Demographic and Health Survey 2016 (SADHS 2016) at the request of the National Department of Health (NDoH). Technical assistance was provided through The DHS Program. Timely information about the health of the nation is essential for monitoring and evaluation. Survey data collection took place from 27 June 2016 to 4 November 2016.
The primary objective of the SADHS 2016 is to provide up-to-date estimates of basic demographic and health indicators. Specifically, the SADHS 2016 collected information on fertility levels; marriage; sexual activity; fertility preferences; awareness and use of contraceptives; breastfeeding practices; nutrition; childhood and maternal mortality; maternal health, including antenatal and postnatal care; key aspects of child health, including immunisation coverage and prevalence and treatment of acute respiratory infection (ARI), fever, and diarrhoea; potential exposure to the risk of HIV infection; coverage of HIV counselling and testing (HCT); and physical and sexual violence against women. Another critical objective of the SADHS 2016 is to provide estimates of health and behaviour indicators for adults age 15 and older, including use of tobacco, alcohol, and codeine-containing medications. In addition, the SADHS 2016 provides estimates of the prevalence of anaemia among children age 6-59 months and adults age 15 and older, and the prevalence of hypertension, anaemia, high HbA1c levels (an indicator of diabetes), and HIV among adults age 15 and older.
The information collected through the SADHS 2016 is intended to assist policymakers and programme managers in evaluating and designing programmes and strategies for improving the health of the country’s population.

Sample survey data [ssd]
The sampling frame used for the SADHS 2016 is the Statistics South Africa Master Sample Frame (MSF), which was created using Census 2011 enumeration areas (EAs). In the MSF, EAs of manageable size were treated as primary sampling units (PSUs), whereas small neighbouring EAs were pooled together to form new PSUs, and large EAs were split into conceptual PSUs. The frame contains information about the geographic type (urban, traditional, or farm) and the estimated number of residential dwelling units (DUs) in each PSU. The sampling convention used by Stats SA is DUs. One or more households may be located in any given DU; recent surveys have found 1.03 households per DU on average.
Administratively, South Africa is divided into nine provinces. The sample for the SADHS 2016 was designed to provide estimates of key indicators for the country as a whole, for urban and non-urban areas separately, and for each of the nine provinces in South Africa. To ensure that the survey precision is comparable across provinces, PSUs were allocated by a power allocation rather than a proportional allocation. Each province was stratified into urban, farm, and traditional areas, yielding 26 sampling strata.
The SADHS 2016 followed a stratified two-stage sample design with a probability proportional to size sampling of PSUs at the first stage and systematic sampling of DUs at the second stage. The Census 2011 DU count was used as the PSU measure of size. A total of 750 PSUs were selected from the 26 sampling strata, yielding 468 selected PSUs in urban areas, 224 PSUs in traditional areas, and 58 PSUs in farm areas.
A listing operation was carried out in all selected PSUs from January to March 2016, and the updated lists of DUs served as a sampling frame for the selection of DUs in the second stage. In the second stage of selection, a fixed number of 20 DUs per cluster were selected with systematic selection from the created listing. All households in a selected DU were eligible for interviews.
Some of the selected PSUs were informal, unstructured settlements with no clear identifications of DUs. To ensure listing coverage within each informal, unstructured PSU selected, segmentation was carried out, with the PSU divided into multiple segments of about 20 DUs each. Only one segment was selected at random for the survey; in segments with 20 DUs or fewer, all DUs in the segment were eligible for the survey. In segments with more than 20 DUs, 20 DUs were randomly selected and were eligible for the survey. A cluster in the SADHS 2016 is therefore either a PSU or a segment of a PSU.
In half of selected DUs, all households were eligible for interviews with the Household Questionnaire, and all women age 15-49 who were either permanent residents of the selected households or visitors who stayed in the household the night before the survey were eligible for interviews with a standard individual questionnaire. Within this subsample, households in every other DU were eligible to have their salt tested for the presence of iodine.
In the remaining half of DUs, all households were eligible for interviews with the Household Questionnaire, and all women and men age 15 and older who were either permanent residents of the selected households or visitors who stayed in the household the night before the survey were eligible for individual interviews and for biomarker collection. Women age 15-49 and men age 15-59 were eligible for the standard individual questionnaire, as well as a South Africa-specific module on adult health; women age 50 and older and men age 60 and older were eligible for a few sections of the individual questionnaire and the adult health module. In addition, children age 0-59 months were eligible for biomarker collection.
Finally, in all households in selected DUs, one woman age 18 and older was selected for a module on domestic violence. In addition, for each child age 0-5 whose biological mother did not live in the household, a guardian was eligible to complete the Caregiver’s Questionnaire.
Computer Assisted Personal Interview [capi]
Five questionnaires were used in the SADHS 2016: the Household Questionnaire, the individual Woman’s Questionnaire, the individual Man’s Questionnaire, the Caregiver’s Questionnaire, and the Biomarker Questionnaire. These questionnaires, based on The DHS Program’s standard Demographic and Health Survey questionnaires, were adapted to reflect the population and health issues relevant to South Africa. Input was solicited from various stakeholders representing government ministries and agencies, nongovernmental organisations, and international donors. After the preparation of the questionnaires in English, the questionnaires were translated into South Africa’s 10 other official languages. In addition, information about the fieldworkers for the survey was collected through a self-administered Fieldworker Questionnaire.
All electronic data files for the SADHS 2016 were transferred via the IFSS to the Stats SA head office in Pretoria, where they were stored on a password-protected computer. The data processing operation included secondary editing, which required resolution of computer-identified inconsistencies and coding of open-ended questions. The data were processed by a core group of four people; secondary editing was completed by 11 people. All persons involved in data processing took part in the main fieldwork training, and they were supervised by senior staff from Stats SA with support from ICF. Data editing was accomplished using CSPro software. Secondary editing was initiated in October 2016 and completed in February 2017. Checking inconsistencies in dates of immunisations was aided by the digital images of the immunisation page of the Road-to-Health booklet that had been collected on the tablet by fieldworkers at the time of the interview for that purpose.
Appropriate analysis weights were calculated, taking the design probabilities and the response rate into account. Standard methods of analysis (Rutstein and Rojas 2006) were applied involving conversion of all dates to century month codes to facilitate calculation of ages at the time of different life events.
A total of 15,292 households were selected for the sample, of which 13,288 were occupied. Of the occupied households, 11,083 were successfully interviewed, yielding a response rate of 83%.
In the interviewed households, 9,878 eligible women age 15-49 were identified for individual interviews; interviews were completed with 8,514 women, yielding a response rate of 86%. In the subsample of households selected for the male survey, 4,952 eligible men age 15-59 were identified and 3,618 were successfully interviewed, yielding a response rate of 73%. In this same subsample, 12,717 eligible adults age 15 and older were identified and 10,336 were successfully interviewed with the adult health module, yielding a response rate of 81%. Response rates were consistently lower in urban areas than in nonurban areas.
The 2019 Sierra Leone Demographic and Health Survey (2019 SLDHS) is a nationwide survey with a nationally representative sample of approximately 13,872 selected households. All women age 15-49 who are usual household members or who spent the night before the survey in the selected households were eligible for individual interviews.
The primary objective of the 2019 SLDHS is to provide up-to-date estimates of basic demographic and health indicators. Specifically, the survey collected information on fertility, awareness and use of family planning methods, breastfeeding practices, nutritional status of women and children, maternal and child health, adult and childhood mortality, women’s empowerment, domestic violence, female genital cutting, prevalence and awareness and behaviour regarding HIV/AIDS and other sexually transmitted infections (STIs), and other health-related issues such as smoking.
The information collected through the 2019 SLDHS is intended to assist policymakers and programme managers in evaluating and designing programmes and strategies for improving the health of the country’s population.
National coverage
The survey covered all de jure household members (usual residents), all women aged 15-49, all men age 15-59, and all children aged 0-5 resident in the household.
Sample survey data [ssd]
The sampling frame used for the 2019 SLDHS is the Population and Housing Census of the Republic of Sierra Leone, which was conducted in 2015 by Statistics Sierra Leone. Administratively, Sierra Leone is divided into provinces. Each province is subdivided into districts, each district is further divided into chiefdoms/census wards, and each chiefdom/census ward is divided into sections. During the 2015 Population and Housing Census, each locality was subdivided into convenient areas called census enumeration areas (EAs). The primary sampling unit (PSU), referred to as a cluster for the 2019 SLDHS, is defined based on EAs from the 2015 EA census frame. The 2015 Population and Housing Census provided the list of EAs that served as a foundation to estimate the number of households and distinguish EAs as urban or rural for the survey sample frame.
The sample for the 2019 SLDHS was a stratified sample selected in two stages. Stratification was achieved by separating each district into urban and rural areas. In total, 31 sampling strata were created. Samples were selected independently in every stratum via a two-stage selection process. Implicit stratifications were achieved at each of the lower administrative levels by sorting the sampling frame before sample selection according to administrative order and by using probability-proportional-to-size selection during the first sampling stage.
In the first stage, 578 EAs were selected with probability proportional to EA size. EA size was the number of households residing in the EA. A household listing operation was carried out in all selected EAs, and the resulting lists of households served as a sampling frame for the selection of households in the second stage. In the second stage’s selection, a fixed number of 24 households were selected in every cluster through equal probability systematic sampling, resulting in a total sample size of approximately 13,872 selected households. The household listing was carried out using tablets, and random selection of households was carried out through computer programming. The survey interviewers interviewed only the pre-selected households. To prevent bias, no replacements and no changes of the pre-selected households were allowed in the implementing stages.

Computer Assisted Personal Interview [capi]

The questionnaires, based on The DHS Program’s standard Demographic and Health Survey (DHS-7) questionnaires, were adapted to reflect the population and health issues relevant to Sierra Leone. Comments were solicited from various stakeholders representing government ministries and agencies, nongovernmental organisations, and international donors. The survey protocol was reviewed and approved by the Sierra Leone Ethics and Scientific Review Committee and the ICF Institutional Review Board. All questionnaires were finalised in English, and the 2019 SLDHS used computer-assisted personal interviewing (CAPI) for data collection.
The processing of the 2019 SLDHS data began almost as soon as the fieldwork started. As data collection was completed in each cluster, all electronic data files were transferred via the IFSS to the Stats SL central office in Freetown. These data files were registered and checked for inconsistencies, incompleteness, and outliers. The field teams received alerts on any inconsistencies and errors. Secondary editing, carried out in the central office, involved resolving inconsistencies and coding open-ended questions. The Stats SL data processor coordinated the exercise at the central office. The biomarker paper questionnaires were compared with electronic data files to check for any inconsistencies in data entry. Data entry and editing were carried out using the CSPro Systems software package. Concurrent processing of the data offered a distinct advantage because it maximised the likelihood of the data being error-free and accurate. Timely generation of field check tables allowed for effective monitoring. The secondary editing of the data was completed in mid-October 2019.
A total of 13,793 households were selected for the sample, of which 13,602 were occupied. Of the occupied households, 13,399 were successfully interviewed, yielding a response rate of 99%. In the interviewed households, 16,099 women age 15-49 were identified for individual interviews; interviews were completed with 15,574 women, yielding a response rate of 97%. In the subsample of households selected for the male survey, 7,429 men age 15-59 were identified, and 7,197 were successfully interviewed, yielding a response rate of 97%.

The Liberia Demographic and Health Survey (LDHS) was conducted as part of the worldwide Demographic and Health Surveys (DHS) program, in which surveys are being carried out in countries in Africa, Asia, Latin America, and the Middle East. Liberia was the second country to conduct a DHS and the first country in Africa to do so. THe LDHS was a national-level survey conducted from February to July 1986, covering a sample of 5,239 women aged 15 to 49.
The major objective of the LDHS was to provide data on fertility, family planning and maternal and child health to planners and policymakers in Liberia for use in designing and evaluating programs. Although a fair amount of demographic data was available from censuses and surveys, almost no information existed concerning family planning, health, or the determinants of fertility, and the data that did exist were drawn from small-scale, sub-national studies. Thus, there was a need for data to make informed policy choices for family planning and health projects.
A more specific objective was to provide baseline data for the Southeast Region Primary Health Care Project. In order to effectively plan strategies and to eventually evaluate the progress of the project in meeting its goals, there was need for data to indicate the health situation in the two target counties prior to the implementation of the project. Many of the desired topics, such as immunizations, family planning use, and prenatal care, were already incorporated into the model DHS questionnaire; nevertheless, the LDHS was able to better accommodate the needs of this project by adding several questions and by oversampling women living in Sinoe and Grand Gedeh Counties.
Another important goal of the LDHS was to enhance tile skills of those participating in the project for conducting high-quality surveys in the future. Finally, the contribution of Liberian data to an expanding international dataset was also an objective of the LDHS.
National
Sample survey data
The sample for the Liberia Demographic and Health Survey was based on the sampling frame of about 4,500 censal enumeration areas (EAs) that were created for the 1984 Population Census. It was decided to eliminate very remote EAs prior to selecting the sample. The definition of remoteness used was "any EA in which the largest village was estimated to be more than 3-4 hours' walk from a road." According to the 1984 census, the excluded areas represent less than 3 percent of the total number of households in the country. Since the major analytic objective of the LDHS was to adequately estimate basic demographic and health indicators including fertility, mortality, and contraceptive prevalence for the whole country and the two sub-universes (Since and Grand Gedeh Counties), it was decided to oversample these two counties. Consequently, three explicit sub-universes of EAs were created: (1) Since County, (2) Grand Gedeh County, and (3) the rest of the country.
The design provided a self-weighted sample within each sub-universe, but, because of the oversampling in Sinoe and Grand Gedeh Counties, the sample is not self-weighting at the national level. Eligible respondents for the survey were women aged 15-49 years who were present the night before the interview in any of the households included in the sample selected for the LDHS.
The total sample size was expected to be about 6,000 women aged 15-49 with a target by sub-universe of 1,000 each in Sinoe and Grand Gedeh Counties and 4,000 in the rest of the country. It was decided that a sample of approximately 5,500 households selected through a two-stage procedure would be appropriate to reach those objectives. Sampling was carried out independently in each sub-universe. In the rest of the country sub-universe, counties were arranged for selection in serpentine order from the northwest (Cape Mount County) to the southeast (Maryland County). In the first stage EAs were selected systematically with probability proportional to size (size = number of households in 1984). Twenty-four EAs were selected in each of Sinoe and Grand Gedeh Counties and 108 EAs in the rest of the country.

Face-to-face
The Liberia Demographic and Health Survey (LDHS) utilized two questionnaires: One to list members of the selected households (Household Questionnaire) and the other to record information from all women aged 15-49 who were present in the selected households the night before the interview (Individual Questionnaire).
Both questionnaires were produced in Liberian English and were pretested in September 1985. The Individual Questionnaire was an early version of the DHS model questionnaire. It covered three main topics: (1) fertility, including a birth history and questions concerning desires for future childbearing, (2) family planning knowledge and use, and (3) family health, including prevalence of childhood diseases, immunizations for children under age five, and breasffeeding and weaning practices.
Data from the questionnaires were entered onto microcomputers at the Bureau of Statistics office in Monrovia. The data were then subjected to extensive checks for consistency and accuracy.
Errors detected during this operation were resolved either by referring to the original questionnaire, or, in some cases, by logical inference from other information given in the record. Finally, dates were imputed for the small number of cases where complete dates of important events were not given.
Out of the total of 6,1306 households selected, 14.5 percent were found not to be valid households in the field, either because the dwelling had been vacated or destroyed, or the household could not be located or did not exist. Of the 5,609 households that were found to exist, 90 percent were successfully interviewed. In the households that were interviewed, a total of 5,340 women were identified as being eligible for individual interview (that is, they were aged 15-49 and had spent the night before the interview in the selected household). This represents an average of slightly over one eligible woman per household.
The response rate for eligible women was 98 percent. The main reason for nonresponse was the absence of the woman. Similar data are presented by sample subuniverse.

The standard error can be used to calculate confidence intervals within which one can be reasonably assured the true value of the variable for the whole population falls. For example, for any given statistic calculated from a sample survey, the value of that same statistic as measured in 95 percent of all possible samples of identical size and design will fall within a range of plus or minus two times the standard error of that statistic.
If the sample of women had been selected as a simple random sample, it would have been possible to use straightforward formulas for calculating sampling errors. However, the LDHS sample design depended on stratification, stages, and clusters and consequently, it was necessary to utilize more complex formulas. The computer package CLUSTERS was used to assist in computing the sampling errors with the proper statistical methodology.
Information on the completeness of date reporting is of interest in assessing data quality. With regard to dates of birth of individual women, 42 percent of respondents reported both a month and year of birth, 21 percent gave a year of birth in addition to current age, and 37 percent gave only their ages. With regard to children's dates of birth in the birth history, 85 percent of births had both month and year reported, 12 percent had year and age reported, 1 percent had only age reported, and 2 percent had no date information.
The 2014 Bangladesh Demographic and Health Survey (BDHS) is the seventh DHS undertaken in Bangladesh, following those implemented in 1993-94, 1996-97, 1999-2000, 2004, 2007, and 2011. The main objectives of the 2014 BDHS are to: • Provide information to meet the monitoring and evaluation needs of the health, population, and nutrition sector development program (HPNSDP) • Provide program managers and policy makers involved in the program with the information they need to plan and implement future interventions
The specific objectives of the 2014 BDHS were as follows: • To provide up-to-date data on demographic rates, particularly fertility and infant, and child mortality rates, at the national and divisional level • To measure the level of contraceptive use of currently married women • To provide data on maternal and child health, including antenatal care, assistance at delivery, postnatal care, newborn care, breastfeeding, immunizations, and prevalence and treatment of diarrhea and other diseases among children under age 5 • To assess the nutritional status of children (under age 5) and women by means of anthropometric measurements (weight and height), and to assess infant and child feeding practices • To provide data on knowledge and attitudes of women about sexually transmitted infections and HIV/AIDS • To measure key education indicators, including school attendance ratios • To provide community-level data on accessibility and availability of health and family planning services
National coverage The survey was designed to produce representative results for the country as a whole, for the urban and the rural areas separately, and for each of the seven administrative divisions.
Sample survey data [ssd]
Sample Design The sample for the 2014 BDHS is nationally representative and covers the entire population residing in noninstitutional dwelling units in the country. The survey used a sampling frame from the list of enumeration areas (EAs) of the 2011 Population and Housing Census of the People's Republic of Bangladesh, provided by the Bangladesh Bureau of Statistics (BBS). The primary sampling unit (PSU) for the survey is an EA created to have an average of about 120 households.
Bangladesh is divided into seven administrative divisions: Barisal, Chittagong, Dhaka, Khulna, Rajshahi, Rangpur, and Sylhet. Each division is divided into zilas, and each zila into upazilas. Each urban area in an upazila is divided into wards, which are further subdivided into mohallas. A rural area in an upazila is divided into union parishads (UPs) and, within UPs, into mouzas. These divisions allow the country as a whole to be separated into rural and urban areas.
The survey is based on a two-stage stratified sample of households. In the first stage, 600 EAs were selected with probability proportional to the EA size, with 207 EAs in urban areas and 393 in rural areas. A complete household listing operation was then carried out in all of the selected EAs to provide a sampling frame for the second-stage selection of households. In the second stage of sampling, a systematic sample of 30 households on average was selected per EA to provide statistically reliable estimates of key demographic and health variables for the country as a whole, for urban and rural areas separately, and for each of the seven divisions. With this design, the survey selected 18,000 residential households, which were expected to result in completed interviews with about 18,000 ever-married women.

The 2014 BDHS used three types of questionnaires: a Household Questionnaire, a Woman’s Questionnaire, and a Community Questionnaire. The contents of the Household and Woman’s questionnaires were based on the MEASURE DHS Model Questionnaires. These model questionnaires were adapted for use in Bangladesh during a series of meetings with a Technical Working Group (TWG) that consisted of representatives from NIPORT, Mitra and Associates, International Center for Diarrheal Disease Research, Bangladesh (ICDDR,B), USAID/Bangladesh, and ICF International. Draft questionnaires were then circulated to other interested groups and were reviewed by the 2014 BDHS Technical Review Committee. The questionnaires were developed in English and then translated into and printed in Bangla.
The Household Questionnaire was used to list all the usual members and visitors in the selected households. Some basic information was collected on the characteristics of each person listed, including age, sex, education, current work status, birth registration, and individual possession of mobile phones. The main purpose of the Household Questionnaire was to identify women who were eligible for the individual interview. Information was collected about the dwelling unit, such as the source of water, type of toilet facilities, materials used to construct the floor, roof, and walls, ownership of various consumer goods, and availability of hand washing facilities. In addition, this questionnaire was used to record the height and weight measurements of ever-married women age 15-49 and children under age 6.
The Woman’s Questionnaire was used to collect information from ever-married women age 15-49.
The Community Questionnaire was administered in each selected cluster during the household listing operation and included questions about the existence of development organizations in the community and the availability and accessibility of health services and other facilities. The Community Questionnaire was administered to a group of four to six key informants who were knowledgeable about socioeconomic conditions and the availability of health and family planning services/facilities in the cluster. Key informants included community leaders, teachers, government officials, social workers, religious leaders, traditional healers, and health care providers among others.
The completed 2014 BDHS questionnaires were periodically returned to Dhaka for data processing at Mitra and Associates. The data processing began shortly after fieldwork commenced. Data processing consisted of office editing, coding of open-ended questions, data entry, and editing of inconsistencies found by the computer program. Eight data entry operators and two data entry supervisors processed the data. Data processing commenced on July 24, 2014, and ended on November 20, 2014. The task was carried out using the Census and Survey Processing System (CSPro), a software jointly developed by the U.S. Census Bureau, ICF Macro, and Serpro S.A.
Among a total of 17,989 selected households, 17,565 were found occupied. Interviews were successfully completed in 17,300, or 99 percent of households. A total of 18,245 ever-married women age 15-49 were identified in these households and 17,863 were interviewed, for a response rate of 98 percent. Response rates for households and eligible women are similar to those in the 2011 BDHS. The principal reason for nonresponse among women was their absence from home despite repeated visits to the household. The response rates do not vary notably by urban-rural residence.
The estimates from a sample survey are affected by two types of errors: non-sampling errors and sampling errors. Non-sampling errors are the results of mistakes made in implementing data collection and data processing, such as failure to locate and interview the correct household, misunderstanding of the questions on the part of either the interviewer or the respondent, and data entry errors. Although numerous efforts were made during the implementation of the 2014 Bangladesh DHS (BDHS) to minimize this type of error, non-sampling errors are impossible to avoid and difficult to evaluate statistically.
Sampling errors, on the other hand, can be evaluated statistically. The sample of respondents selected in the 2014 BDHS is only one of many samples that could have been selected from the same population, using the same design and expected size. Each of these samples would yield results that differ somewhat from the results of the actual sample selected. Sampling errors are a measure of the variability between all possible samples. Although the degree of variability is not known exactly, it can be estimated from the survey results.
Sampling error is usually measured in terms of the standard error for a particular statistic (mean, percentage, etc.), which is the square root of the variance. The standard error can be used to calculate confidence intervals within which the true value for the population can reasonably be assumed to fall. For example, for any given statistic calculated from a sample survey, the value of that statistic will fall within a range of plus or minus two times the standard error of that statistic in 95 percent of all possible samples of identical size and design.
If the sample of respondents had been selected as a simple random sample, it would have been possible to use straightforward formulas for calculating sampling errors. However, the 2014 BDHS sample is the result of a multi-stage stratified design, and, consequently, it was necessary to use more complex formulae. The South Africa Demographic and Health Survey 2016 (SADHS 2016) is the third DHS conducted in South Africa and follows surveys carried out in 1998 and 2003. The SADHS 2016 was designed to provide up-to-date information on key indicators needed to track progress in South Africa’s health programmes.
The survey was designed to provide representative estimates for main demographic and health indicators for the country as a whole, for urban and non-urban areas separately, and for each of the nine provinces in South Africa: Western Cape, Eastern Cape, Northern Cape, Free State, KwaZulu-Natal, North West, Gauteng, Mpumalanga, and Limpopo.
Households and individuals
The South African Demographic and Health Survey (SADHS) covered the population living in private households in the country.
Sample survey data
The sample for the SADHS 2016 is a stratified sample selected in two stages from the Master Sampling Frame. Stratification was achieved by separating each province into urban, traditional, and farm areas. In total, 26 sampling strata were created (since there are no traditional areas in Western Cape). Samples were selected independently in each sampling stratum by a two-stage selection. Implicit stratification and proportional allocation were achieved at each of the lower administrative levels within a given sampling stratum by sorting the sampling frame according to administrative units at different levels in each stratum and using probability proportional to size selection at the first stage of sampling.

Five questionnaires were used in the SADHS 2016. Interviewers used tablet computers to record responses during interviews.
Of the total 972 PSUs that were selected, fieldwork was not implemented in three PSUs due to concerns about the safety of the interviewers and the questionnaires for another three PSUs were lost in transit. The data file contains information for a total of 966 PSUs. A total of 12,860 households was selected for the sample and 12,247 were successfully interviewed. The shortfall is primarily due to refusals and to dwellings that were vacant or in which the inhabitants had left for an extended period at the time they were visited by interviewing teams.
Of the 12,638 households occupied 97 percent were successfully interviewed. In these households, 12,327 women were identified as eligible for the individual women's interview (15-49) and interviews were completed with 11,735 or 95 percent of them. In the one half of the households that were selected for inclusion in the adult health survey 14,928 eligible adults age 15 and over were identified of which 13,827 or 93 percent were interviewed. The principal reason for non-response among eligible women and men was the failure to find them at home despite repeated visits to the household. The refusal rate was about 2 percent.
Sampling errors are computed in SAS, using programs developed by ICF. These programs use the Taylor linearization method to estimate variances for survey estimates that are means, proportions, or ratios. The Jackknife repeated replication method is used for variance estimation of more complex statistics such as fertility and mortality rates.
The 2022 Ghana Demographic and Health Survey (2022 GDHS) is the seventh in the series of DHS surveys conducted by the Ghana Statistical Service (GSS) in collaboration with the Ministry of Health/Ghana Health Service (MoH/GHS) and other stakeholders, with funding from the United States Agency for International Development (USAID) and other partners.
The primary objective of the 2022 GDHS is to provide up-to-date estimates of basic demographic and health indicators. Specifically, the GDHS collected information on: - Fertility levels and preferences, contraceptive use, antenatal and delivery care, maternal and child health, childhood mortality, childhood immunisation, breastfeeding and young child feeding practices, women’s dietary diversity, violence against women, gender, nutritional status of adults and children, awareness regarding HIV/AIDS and other sexually transmitted infections, tobacco use, and other indicators relevant for the Sustainable Development Goals - Haemoglobin levels of women and children - Prevalence of malaria parasitaemia (rapid diagnostic testing and thick slides for malaria parasitaemia in the field and microscopy in the lab) among children age 6–59 months - Use of treated mosquito nets - Use of antimalarial drugs for treatment of fever among children under age 5
The information collected through the 2022 GDHS is intended to assist policymakers and programme managers in designing and evaluating programmes and strategies for improving the health of the country’s population.
National coverage
The survey covered all de jure household members (usual residents), all women aged 15-49, men aged 15-59, and all children aged 0-4 resident in the household.

To achieve the objectives of the 2022 GDHS, a stratified representative sample of 18,450 households was selected in 618 clusters, which resulted in 15,014 interviewed women age 15–49 and 7,044 interviewed men age 15–59 (in one of every two households selected).
The sampling frame used for the 2022 GDHS is the updated frame prepared by the GSS based on the 2021 Population and Housing Census.1 The sampling procedure used in the 2022 GDHS was stratified two-stage cluster sampling, designed to yield representative results at the national level, for urban and rural areas, and for each of the country’s 16 regions for most DHS indicators. In the first stage, 618 target clusters were selected from the sampling frame using a probability proportional to size strategy for urban and rural areas in each region. Then the number of targeted clusters were selected with equal probability systematic random sampling of the clusters selected in the first phase for urban and rural areas. In the second stage, after selection of the clusters, a household listing and map updating operation was carried out in all of the selected clusters to develop a list of households for each cluster. This list served as a sampling frame for selection of the household sample. The GSS organized a 5-day training course on listing procedures for listers and mappers with support from ICF. The listers and mappers were organized into 25 teams consisting of one lister and one mapper per team. The teams spent 2 months completing the listing operation. In addition to listing the households, the listers collected the geographical coordinates of each household using GPS dongles provided by ICF and in accordance with the instructions in the DHS listing manual. The household listing was carried out using tablet computers, with software provided by The DHS Program. A fixed number of 30 households in each cluster were randomly selected from the list for interviews.

Face-to-face computer-assisted interviews [capi]
Four questionnaires were used in the 2022 GDHS: the Household Questionnaire, the Woman’s Questionnaire, the Man’s Questionnaire, and the Biomarker Questionnaire. The questionnaires, based on The DHS Program’s model questionnaires, were adapted to reflect the population and health issues relevant to Ghana. In addition, a self-administered Fieldworker Questionnaire collected information about the survey’s fieldworkers.
The GSS organized a questionnaire design workshop with support from ICF and obtained input from government and development partners expected to use the resulting data. The DHS Program optional modules on domestic violence, malaria, and social and behavior change communication were incorporated into the Woman’s Questionnaire. ICF provided technical assistance in adapting the modules to the questionnaires.
DHS staff installed all central office programmes, data structure checks, secondary editing, and field check tables from 17–20 October 2022. Central office training was implemented using the practice data to test the central office system and field check tables. Seven GSS staff members (four male and three female) were trained on the functionality of the central office menu, including accepting clusters from the field, data editing procedures, and producing reports to monitor fieldwork.
From 27 February to 17 March, DHS staff visited the Ghana Statistical Service office in Accra to work with the GSS central office staff on finishing the secondary editing and to clean and finalize all data received from the 618 clusters.
A total of 18,540 households were selected for the GDHS sample, of which 18,065 were found to be occupied. Of the occupied households, 17,933 were successfully interviewed, yielding a response rate of 99%. In the interviewed households, 15,317 women age 15–49 were identified as eligible for individual interviews. Interviews were completed with 15,014 women, yielding a response rate of 98%. In the subsample of households selected for the male survey, 7,263 men age 15–59 were identified as eligible for individual interviews and 7,044 were successfully interviewed.

Data Quality Tables
The principal objective of the SIDHS was to provide current and reliable data on fertility and family planning behaviour, child mortality, adult and maternal mortality, children’s nutritional status, the use of maternal and child health services, and knowledge of HIV and AIDS. Specific survey objectives were to: - collect data at the national level, which will allow the calculation of key demographic rates; - analyse the direct and indirect factors that determine the level and trends of fertility; - measure the level of contraceptive knowledge and practice among women and men by method, urban-rural residence and region; - collect high-quality data on family health, including immunisation coverage among children, prevalence and treatment of diarrhoea and other diseases among children under 5 years, and maternity care indicators, including antenatal visits, assistance at delivery, and postnatal care; - collect data on infant and child mortality; - obtain data on child feeding practices, including breastfeeding, and collect ‘observation’ information to use in assessing the nutritional status of women and children; - collect data on knowledge and attitudes of women and men about sexually transmitted infections and HIV and AIDS, and evaluate patterns of recent behaviour regarding condom use; and - collect data on support to mentally ill people as well as information on the incidence of suicides.
This information is essential for informed policy decisions, planning, monitoring, and evaluating programmes on health in general, and reproductive health in particular, at both the national level as well as in urban and rural areas. A long-term objective of the survey is to strengthen the technical capacity of government organisations to plan, conduct, process, and analyse data from complex national population and health surveys. Moreover, the 2006/2007 SIDHS provides national, rural and urban estimates on population and health that are comparable with data collected in similar surveys in other Pacific DHS pilot countries and other developing countries.
National
Sample survey data [ssd]
Sample design The sample SIDHS was designed to provide reliable estimates of total fertility and infant mortality rates at the national level, with urban and rural breakdown and relatively stable estimates for selected provinces. Although the design considered an urban-rural split, this was not carried out during the sample selection of the enumeration area (EA). The sample was not spread out geographically in proportion to the population; as a result, the SIDHS sample is not self-weighting at the national level and sample weighting factors have been applied to survey records in order to bring them into proportion.
The sample for the survey is a three-stage stratified, nationally representative sample of households. The sampling frame consisted of the estimated number of households in each EA by province and was prepared by SPC from the 1999 population census data and estimated urban and rural population growth rates. Honiara, which is urban in its entirety and selected enumeration areas of Guadalcanal and other provinces, comprise the country’s urban areas. The rest of Guadalcanal and all other provinces are rural. Five domains were identified: Honiara, remaining Guadalcanal, Western, Malaita, and the combined group of smaller provinces (Choiseul, Isabel, Central, Makira/Ullawa, Rennell/Bellona and Temuto). The primary sampling units, comprising 215 EAs, were selected in each province using systematic random sampling with probability proportional to the estimated number of households in the EA. It was not possible to cover several of the selected sample EAs: 5 EAs were damaged or destroyed during the tsunami of 1 April 2007; 5 EAs were refused permission to survey by village elders or the community; and 23 EAs (11 in Honiara, 7 in Western, 4 in Malaita and 1 in Guadalcanal) were not covered due to poor team leadership or poor field monitoring. In Western Province, 7 EAs were not covered due to internal migration of residents after the tsunami. Thus, the survey covered 182 EAs — 60 urban and 122 rural.
Mapping and listing households in each sample EA were undertaken by the interviewers. In each sample point, 20 households were selected by the team supervisor using systematic random sampling. The sample was designed to cover a target sample of 4,300 households with an expected household response rate of 95%. All women aged 15–49 who slept in the sample household on the night prior to the interview date were eligible to be interviewed for the Women’s Questionnaire, and for the anthropometric (height and weight), blood pressure and haemoglobin measurements. Every second household was sub-selected for the male survey. All men aged 15 or over in the sub-selected households were eligible to be interviewed for the Men’s Questionnaire, and for anthropometric and blood pressure measurements. All children aged 0–5 years were eligible for anthropometric measurement, and those aged 6 months to 5 years, for anaemia testing.

Three questionnaires — a household questionnaire, a women's questionnaire and a men's questionnaire —were used in the SIDHS. The contents of these questionnaires were based on model questionnaires developed by the MEASURE DHS program at Macro International.
In consultation with MOH, SINSO and Macro, staff modified the DHS model questionnaires to reflect relevant issues in population, family planning, HIV and AIDS, and other health issues in the Solomon Islands. The questionnaires were translated into Pidgin and back-translated in order to check accuracy.
The household questionnaire was used to list all the usual members and visitors in the selected households. Some basic information was collected on the characteristics of each person listed, including age, sex, education, and relationship to the head of the household. The main purpose of the household questionnaire was to identify women and men who were eligible for the individual interview. The household questionnaire also collected information on characteristics of the household’s dwelling unit, such as source of water, type of toilet facilities, materials used for the floor and roof of the house, ownership of various durable goods, and ownership and use of mosquito nets. In addition, this questionnaire was also used to record height and weight measurements of women aged 15–49, men aged 15 and above, and children under the age of 5 years, as well as consent from women, and children’s parent or guardian to give blood samples for anaemia and blood pressure testing among women and men.
The women's questionnaire was used to collect information from all women aged 15–49 on: • Background characteristics (education, residential history, media exposure, etc.) • Reproductive history and child mortality • Knowledge and use of family planning methods • Fertility preferences • Antenatal and delivery care • Breastfeeding and infant feeding practices • Vaccinations and childhood illnesses • Marriage and sexual activity • Woman’s work and husband’s background characteristics • Infant and child feeding practices • Awareness and behaviour about AIDS and other sexually transmitted infections.
The men’s questionnaire collected similar information contained in the women's questionnaire, but was shorter because it did not contain questions on reproductive history, contraceptive calendar, and maternal and child health and nutrition.
Both informal and formal pre-tests of the questionnaires were undertaken. In July 2006, an informal pre-test was done through self-administration of the individual women’s and men’s questionnaires, respectively, by six female and four male SINSO staff members.
A more formal three-week pre-test was undertaken for the interviewers from 21 August to 2 September 2006, inclusive of Saturdays. The pre-test training for the nurses/health technicians started a few days later, and went from 25 August to 2 September. Twelve pre-test interviewers (seven males and five females) were expected to become team supervisors and field editors during the main enumeration. Four nurses/health technicians were trained for accuracy and reliability of the various measurements. Most pre-test interviewers had experience as interviewers in the 2006 Household Income and Expenditures Survey. Recruitment was done through radio advertisement and recommendation of SINSO staff.
Pre-test training for the interviewers consisted of classroom lectures, demonstration interviews, front-of-class interviews, mock interviews, quizzes and tests, and three days of field practice. Instructional materials included the household questionnaire, the women’s questionnaire, the men’s questionnaire, four field control forms, and various PowerPoint presentations. A whiteboard, an electronic projector and a laptop computer were also used during the pre-test training. The pre-test resulted in revising the translation of some questions and skip instructions.
The computer processing of SIDHS data began a few weeks after the fieldwork began. The Macro Inc data processing consultant held a training from 30 October 30 to 10 November 2006. A data processing specialist from SPC, the data processing head from SINSO, and two data processing staff from the Republic of the Marshall Islands attended. The sampling frame used for the 2017-18 JPFHS is based on Jordan's Population and Housing Census (JPHC) frame for 2015. The current survey is designed to produce results representative of the country as a whole, of urban and rural areas separately, of three regions, of 12 administrative governorates, and of three national groups: Jordanians, Syrians, and a group combined from various other nationalities.
The sample for the 2017-18 JPFHS is a stratified sample selected in two stages from the 2015 census frame. Stratification was achieved by separating each governorate into urban and rural areas. Each of the Syrian camps in the governorates of Zarqa and Mafraq formed its own sampling stratum. In total, 26 sampling strata were constructed. Samples were selected independently in each sampling stratum, through a two-stage selection process, according to the sample allocation. Before the sample selection, the sampling frame was sorted by district and sub-district within each sampling stratum. By using a probability-proportional-to-size selection for the first stage of selection, an implicit stratification and proportional allocation were achieved at each of the lower administrative levels.
In the first stage, 970 clusters were selected with probability proportional to cluster size, with the cluster size being the number of residential households enumerated in the 2015 JPHC. The sample allocation took into account the precision consideration at the governorate level and at the level of each of the three special domains. After selection of PSUs and clusters, a household listing operation was carried out in all selected clusters. The resulting household lists served as the sampling frame for selecting households in the second stage. A fixed number of 20 households per cluster were selected with an equal probability systematic selection from the newly created household listing.

Four questionnaires were used for the 2017-18 JPFHS: the Household Questionnaire, the Woman’s Questionnaire, the Man’s Questionnaire, and the Biomarker Questionnaire. These questionnaires, based on The DHS Program’s standard Demographic and Health Survey questionnaires, were adapted to reflect population and health issues relevant to Jordan. After all questionnaires were finalised in English, they were translated into Arabic.
All electronic data files for the 2017-18 JPFHS were transferred via IFSS to the DOS central office in Amman, where they were stored on a password-protected computer. The data processing operation included secondary editing, which required resolution of computer-identified inconsistencies and coding of open-ended questions. Data editing was accomplished using CSPro software. During the duration of fieldwork, tables were generated to check various data quality parameters, and specific feedback was given to the teams to improve performance. Secondary editing and data processing were initiated in October 2017 and completed in February 2018.
A total of 19,384 households were selected for the sample, of which 19,136 were found to be occupied at the time of the fieldwork. Of the occupied households, 18,802 were successfully interviewed, yielding a response rate of 98%.
In the interviewed households, 14,870 women were identified as eligible for an individual interview; interviews were completed with 14,689 women, yielding a response rate of 99%. A total of 6,640 eligible men were identified in the sampled households and 6,429 were successfully interviewed, yielding a response rate of 97%. Response rates for both women and men were similar across urban and rural areas.
The estimates from a sample survey are affected by two types of errors: nonsampling errors and sampling errors. Nonsampling errors are the results of mistakes made in implementing data collection and data processing, such as failure to locate and interview the correct household, misunderstanding of the questions on the part of either the interviewer or the respondent, and data entry errors. Although numerous efforts were made during the implementation of the 2017-18 Jordan Population and Family Health Survey (JPFHS) to minimise this type of error, nonsampling errors are impossible to avoid and difficult to evaluate statistically.
Sampling errors, on the other hand, can be evaluated statistically. The sample of respondents selected in the 2017-18 JPFHS is only one of many samples that could have been selected from the same population, using the same design and sample size. Each of these samples would yield results that differ somewhat from the results of the actual sample selected. Sampling errors are a measure of the variability among all possible samples. Although the degree of variability is not known exactly, it can be estimated from the survey results.
Sampling error is usually measured in terms of the standard error for a particular statistic (mean, percentage, etc.), which is the square root of the variance. The standard error can be used to calculate confidence intervals within which the true value for the population can reasonably be assumed to fall. For example, for any given statistic calculated from a sample survey, the value of that statistic will fall within a range of plus or minus two times the standard error of that statistic in 95% of all possible samples of identical size and design.
If the sample of respondents had been selected by simple random sampling, it would have been possible to use straightforward formulas for calculating sampling errors. However, the 2017-18 JPFHS sample was the result of a multi-stage stratified design, and, consequently, it was necessary to use more complex formulas. Sampling errors are computed using SAS programmes developed by ICF International. These programmes use the Taylor linearisation method to estimate variances for survey estimates that are means, proportions, or ratios. The Jackknife repeated replication method is used for variance estimation of more complex statistics such as fertility and mortality rates.
The Taylor linearisation method treats any percentage or average as a ratio estimate, r = y/x, where y represents the total sample value for variable y, and x represents the total number of cases in the group or subgroup under consideration.

The 2003-2004 South African Demographic and Health Survey is the second national health survey to be conducted by the Department of Health, following the first in 1998. Compared with the first survey, the new survey has more extensive questions around sexual behaviour and for the first time included such questions to a sample of men. Anthropometric measurements were taken on children under five years, and the adult health module has been enhanced with questions relating to physical activity and micro-nutrient intake, important risk factors associated with chronic diseases. The 2003-2004 SADHS has introduced a chapter reporting on the health, health service utilisation and living conditions of South Africa's older population (60 years or older) and how they have changed since 1998. This has been introduced because this component of the population is growing at a much higher rate than the other age groups. The chapter on adolescent health in 1998 focussed on health risk-taking behaviours of people aged 15-19 years. The chapter has been extended in the 2003-2004 SADHS to include indicators of sexual behaviour of youth aged 15-24 years.
A total of 10 214 households were targeted for inclusion in the survey and 7 756 were interviewed, reflecting an 85 percent response rate. The survey used a household schedule to capture basic information about all the members of the household, comprehensive questionnaires to all women aged 15-49, as well as anthropometric measurements of all children five years and younger. In every second household, interviews of all men 15-59 were conducted and in the alternate households, interviews and measurements of all adults 15 years and older were done including heights, weights, waist circumference, blood pressure and peak pulmonary flow. The overall response rate was 75 percent for women, 67 percent for men, 71 percent for adults, and 84 percent for children. This is slightly lower than the overall response rate for the 1998 SADHS, but varied substantially between provinces with a particularly low response rate in the Western Cape.
OBJECTIVES
In 1995 the National Health Information System of South Africa (NHIS/SA) committee identified the need for improved health information for planning services and monitoring programmes. The first South African Demographic and Health Survey (SADHS) was planned and implemented in 1998. At the time of the survey it was agreed that the survey had to be conducted every five years to enable the Department of Health to monitor trends in health services.
STUDY LIMITATIONS AND RECOMMENDATIONS
Comparison of the socio-demographic characteristics of the sample with the 2001 Population Census shows an over-representation of urban areas and the African population group, and an under-representation of whites and Indian females. It also highlights many anomalies in the ages of the sample respondents, indicating problems in the quality of the data of the 2003 survey. Careful analysis has therefore been required to distinguish the findings that can be considered more robust and can be used for decision making. This has involved considering the internal consistency in the data, and the extent to which the results are consistent with other studies.
Some of the key demographic and adult health indicators show signs of data quality problems. In particular, the prevalence of hypertension, and the related indicators of quality of care are clearly problematic and difficult to interpret. In addition, the fertility levels and the child mortality estimates are not consistent with other data sources. The data problems appear to arise from poor fieldwork, suggesting that there was inadequate training, supervision and quality control during the implementation of the survey. It is imperative that the next SADHS is implemented with stronger quality control mechanisms in place. Moreover, consideration should be given to the frequency of future surveys. It is possible that the SADHS has become overloaded - with a complex implementation required in the field. Thus it may be appropriate to consider a more frequent survey with a rotation of modules as has been suggested by the WHO.
The SADHS sample was designed to be a nationally representative sample.
Households and individuals
The population covered by the 2003-2004 SADHS is defined as the universe of all women age 15-49, all men 15-59 in South Africa.
Sample survey data
The SADHS sample was designed to be a nationally representative probability sample of approximately 10000 households. The country was stratified into the nine provinces and each province was further stratified into urban and non-urban areas.
The sampling frame for the SADHS was provided by Statistics South Africa (Stats SA) based on the enumeration areas (EAs) list of approximately 86000 EAs created during the 2001 census. Since the Indian population constitutes a very small fraction of the South African population, the Census 2001 EAs were stratified into Indian and non-Indian. An EA was classified as Indian if the proportion of persons who classified themselves as Indian during Census 2001 enumeration in that EA was 80 percent or more, otherwise it was classified as Non-Indian. Within the Indian stratum, EAs were sorted descending by the proportion of persons classified as Indian. It should be noted that some provinces and non-urban areas have a very small proportion of the Indian population hence the Indian stratum could not be further stratified by province or urban/non-urban. A sample of 1000 households was allocated to the stratum. Probability proportional to size (PPS) systematic sampling was used to sample EAs and the proportion of Indian persons in an EA was the measure of size. The non-Indian stratum was stratified explicitly by province and within province by the four geo types, i.e. urban formal, urban informal, rural formal and tribal. Each province was allocated a sample of 1000 households and within province the sample was proportionally allocated to the secondary strata, i.e. geo type. For both the Indian and Non-Indian strata the sample take of households within an EA was sixteen households. The number of visited households in an EA as recorded in the Census 2001, 09 Books was used as the measure of size (MOS) in the Non-Indian stratum.
The second stage of selection involved the systematic sampling of households/stands from the selected EAs. Funds were insufficient to allow implementation of a household listing operation in selected EAs. Fortunately, most of the country is covered by aerial photographs, which Statistics SA has used to create EA-specific photos. Using these photos, ASRC identified the global positioning system (GPS) coordinates of all the stands located within the boundaries of the selected EAs and selected 16 in each EA, for a total of 10080 selected. The GPS coordinates provided a means of uniquely identifying the selected stand. As a result of the differing sample proportions, the SADHS sample is not self-weighting at the national level and weighting factors have been applied to the data in this report.
A total of 630 Primary Sampling Units (PSUs) were selected for the 2003-2004 SADHS (368 in urban areas and 262 in non-urban areas). This resulted in a total of 10214 households being selected throughout the country1. Every second household was selected for the adult health survey. In this second household, in addition to interviewing all women aged 15-49, all adults aged 15 and over were eligible to be interviewed with the adult health questionnaire. In every alternate household selected for the survey, not interviewed with the adult health questionnaire, all men aged 15-59 years were also eligible to be interviewed. It was expected that the sample would yield interviews with approximately 10000 households, 12500 women aged 15-49, 5000 adults and 5000 men.

The survey utilised five questionnaires: a Household Questionnaire, a Women's Questionnaire, a Men's Questionnaire, an Adult Health Questionnaire and an Additional Children Questionnaire. The contents of the first three questionnaires were based on the DHS Model Questionnaires. These model questionnaires were adapted for use in South Africa during a series of meetings with a Project Team that consisted of representatives from the National Department of Health, the Medical Research Council, the Human Sciences Research Council, Statistics South Africa, National Department of Social Development and ORCMacro. Draft questionnaires were circulated to other interested groups, e.g. such as academic institutions. The Additional Children and Men's Questionnaires were developed to address information needs identified by stakeholders, e.g. information on children who were not staying with their biological mothers. All questionnaires were developed in English and then translated in all 11 official languages in South Africa (English, Afrikaans, isiXhosa, isiZulu, Sesotho, Setswana, Sepedi, SiSwati, Tshivenda, Xitsonga and isiNdebele).
a) The Household Questionnaire was used to list all the usual members and visitors in the selected households. Basic information was collected on the characteristics of each person listed, including age, sex, education and relationship to the head of the household. The 2002 Vietnam Demographic and Health Survey (VNDHS 2002) is a nationally representative sample survey of 5,665 ever-married women age 15-49 selected from 205 sample points (clusters) throughout Vietnam. It provides information on levels of fertility, family planning knowledge and use, infant and child mortality, and indicators of maternal and child health. The survey included a Community/ Health Facility Questionnaire that was implemented in each of the sample clusters.
The survey was designed to measure change in reproductive health indicators over the five years since the VNDHS 1997, especially in the 18 provinces that were targeted in the Population and Family Health Project of the Committee for Population, Family and Children. Consequently, all provinces were separated into “project” and “nonproject” groups to permit separate estimates for each. Data collection for the survey took place from 1 October to 21 December 2002.
The Vietnam Demographic and Health Survey 2002 (VNDHS 2002) was the third DHS in Vietnam, with prior surveys implemented in 1988 and 1997. The VNDHS 2002 was carried out in the framework of the activities of the Population and Family Health Project of the Committee for Population, Family and Children (previously the National Committee for Population and Family Planning).
The main objectives of the VNDHS 2002 were to collect up-to-date information on family planning, childhood mortality, and health issues such as breastfeeding practices, pregnancy care, vaccination of children, treatment of common childhood illnesses, and HIV/AIDS, as well as utilization of health and family planning services. The primary objectives of the survey were to estimate changes in family planning use in comparison with the results of the VNDHS 1997, especially on issues in the scope of the project of the Committee for Population, Family and Children.
VNDHS 2002 data confirm the pattern of rapidly declining fertility that was observed in the VNDHS 1997. It also shows a sharp decline in child mortality, as well as a modest increase in contraceptive use. Differences between project and non-project provinces are generally small.
The 2002 Vietnam Demographic and Health Survey (VNDHS 2002) is a nationally representative sample survey. The VNDHS 1997 was designed to provide separate estimates for the whole country, urban and rural areas, for 18 project provinces and the remaining nonproject provinces as well. Project provinces refer to 18 focus provinces targeted for the strengthening of their primary health care systems by the Government's Population and Family Health Project to be implemented over a period of seven years, from 1996 to 2002 (At the outset of this project there were 15 focus provinces, which became 18 by the creation of 3 new provinces from the initial set of 15). These provinces were selected according to criteria based on relatively low health and family planning status, no substantial family planning donor presence, and regional spread. These criteria resulted in the selection of the country's poorer provinces. Nine of these provinces have significant proportions of ethnic minorities among their population.
The population covered by the 2002 VNDHS is defined as the universe of all women age 15-49 in Vietnam.
Sample survey data
The sample for the VNDHS 2002 was based on that used in the VNDHS 1997, which in turn was a subsample of the 1996 Multi-Round Demographic Survey (MRS), a semi-annual survey of about 243,000 households undertaken regularly by GSO. The MRS sample consisted of 1,590 sample areas known as enumeration areas (EAs) spread throughout the 53 provinces/cities of Vietnam, with 30 EAs in each province. On average, an EA comprises about 150 households. For the VNDHS 1997, a subsample of 205 EAs was selected, with 26 households in each urban EA and 39 households for each rural EA. A total of 7,150 households was selected for the survey. The VNDHS 1997 was designed to provide separate estimates for the whole country, urban and rural areas, for 18 project provinces and the remaining nonproject provinces as well. Because the main objective of the VNDHS 2002 was to measure change in reproductive health indicators over the five years since the VNDHS 1997, the sample design for the VNDHS 2002 was as similar as possible to that of the VNDHS 1997.
Although it would have been ideal to have returned to the same households or at least the same sample points as were selected for the VNDHS 1997, several factors made this undesirable. Revisiting the same households would have held the sample artificially rigid over time and would not allow for newly formed households. This would have conflicted with the other major survey objective, which was to provide up-to-date, representative data for the whole of Vietnam. Revisiting the same sample points that were covered in 1997 was complicated by the fact that the country had conducted a population census in 1999, which allowed for a more representative sample frame.
In order to balance the two main objectives of measuring change and providing representative data, it was decided to select enumeration areas from the 1999 Population Census, but to cover the same communes that were sampled in the VNDHS 1997 and attempt to obtain a sample point as close as possible to that selected in 1997. Consequently, the VNDHS 2002 sample also consisted of 205 sample points and reflects the oversampling in the 20 provinces that fall in the World Bank-supported Population and Family Health Project. The sample was designed to produce about 7,000 completed household interviews and 5,600 completed interviews with ever-married women age 15-49.

As in the VNDHS 1997, three types of questionnaires were used in the 2002 survey: the Household Questionnaire, the Individual Woman's Questionnaire, and the Community/Health Facility Questionnaire. The first two questionnaires were based on the DHS Model A Questionnaire, with additions and modifications made during an ORC Macro staff visit in July 2002. The questionnaires were pretested in two clusters in Hanoi (one in a rural area and another in an urban area). After the pretest and consultation with ORC Macro, the drafts were revised for use in the main survey.
a) The Household Questionnaire was used to enumerate all usual members and visitors in selected households and to collect information on age, sex, education, marital status, and relationship to the head of household. The main purpose of the Household Questionnaire was to identify persons who were eligible for individual interview (i.e. ever-married women age 15-49). In addition, the Household Questionnaire collected information on characteristics of the household such as water source, type of toilet facilities, material used for the floor and roof, and ownership of various durable goods.
b) The Individual Questionnaire was used to collect information on ever-married women aged 15-49 in surveyed households. These women were interviewed on the following topics:
- Respondent's background characteristics (education, residential history, etc.);
- Reproductive history;
- Contraceptive knowledge and use;
- Antenatal and delivery care;
- Infant feeding practices;
- Child immunization;
- Fertility preferences and attitudes about family planning;
- Husband's background characteristics;
- Women's work information; and
- Knowledge of AIDS.
c) The Community/Health Facility Questionnaire was used to collect information on all communes in which the interviewed women lived and on services offered at the nearest health stations. The Community/Health Facility Questionnaire consisted of four sections. The first two sections collected information from community informants on some characteristics such as the major economic activities of residents, distance from people's residence to civic services and the location of the nearest sources of health care. The last two sections involved visiting the nearest commune health centers and intercommune health centers, if these centers were located within 30 kilometers from the surveyed cluster. For each visited health center, information was collected on the type of health services offered and the number of days services were offered per week; the number of assigned staff and their training; medical equipment and medicines available at the time of the visit.
The first stage of data editing was implemented by the field editors soon after each interview. Field editors and team leaders checked the completeness and consistency of all items in the questionnaires. The completed questionnaires were sent to the GSO headquarters in Hanoi by post for data processing. The editing staff of the GSO first checked the questionnaires for completeness. The data were then entered into microcomputers and edited using a software program specially developed for the DHS program, the Census and Survey Processing System, or CSPro. Data were verified on a 100 percent basis, i.e., the data were entered separately twice and the two results were compared and corrected. The data processing and editing staff of the GSO were trained and supervised for two weeks by a data processing specialist from ORC Macro. Office editing and processing activities were initiated immediately after the beginning of the fieldwork and were completed in late December 2002.
The results of the household and individual
The 2017 Philippines National Demographic and Health Survey (NDHS 2017) is a nationwide survey with a nationally representative sample of approximately 30,832 housing units. The primary objective of the survey is to provide up-to-date estimates of basic demographic and health indicators. Specifically, the NDHS 2017 collected information on marriage, fertility levels, fertility preferences, awareness and use of family planning methods, breastfeeding, maternal and child health, child mortality, awareness and behavior regarding HIV/AIDS, women’s empowerment, domestic violence, and other health-related issues such as smoking.
The information collected through the NDHS 2017 is intended to assist policymakers and program managers in the Department of Health (DOH) and other organizations in designing and evaluating programs and strategies for improving the health of the country’s population.
National coverage
The survey covered all de jure household members (usual residents) and all women age 15-49 years resident in the sample household.
Sample survey data [ssd]
The sampling scheme provides data representative of the country as a whole, for urban and rural areas separately, and for each of the country’s administrative regions. The sample selection methodology for the NDHS 2017 is based on a two-stage stratified sample design using the Master Sample Frame (MSF), designed and compiled by the PSA. The MSF is constructed based on the results of the 2010 Census of Population and Housing and updated based on the 2015 Census of Population. The first stage involved a systematic selection of 1,250 primary sampling units (PSUs) distributed by province or HUC. A PSU can be a barangay, a portion of a large barangay, or two or more adjacent small barangays.
In the second stage, an equal take of either 20 or 26 sample housing units were selected from each sampled PSU using systematic random sampling. In situations where a housing unit contained one to three households, all households were interviewed. In the rare situation where a housing unit contained more than three households, no more than three households were interviewed. The survey interviewers were instructed to interview only the pre-selected housing units. No replacements and no changes of the preselected housing units were allowed in the implementing stage in order to prevent bias. Survey weights were calculated, added to the data file, and applied so that weighted results are representative estimates of indicators at the regional and national levels.
All women age 15-49 who were either permanent residents of the selected households or visitors who stayed in the households the night before the survey were eligible to be interviewed. Among women eligible for an individual interview, one woman per household was selected for a module on domestic violence.

Two questionnaires were used for the NDHS 2017: the Household Questionnaire and the Woman’s Questionnaire. Both questionnaires, based on The DHS Program’s standard Demographic and Health Survey (DHS-7) questionnaires, were adapted to reflect the population and health issues relevant to the Philippines. Input was solicited from various stakeholders representing government agencies, universities, and international agencies.
The processing of the NDHS 2017 data began almost as soon as fieldwork started. As data collection was completed in each PSU, all electronic data files were transferred via an Internet file streaming system (IFSS) to the PSA central office in Quezon City. These data files were registered and checked for inconsistencies, incompleteness, and outliers. The field teams were alerted to any inconsistencies and errors while still in the PSU. Secondary editing involved resolving inconsistencies and the coding of openended questions; the former was carried out in the central office by a senior data processor, while the latter was taken on by regional coordinators and central office staff during a 5-day workshop following the completion of the fieldwork. Data editing was carried out using the CSPro software package. The concurrent processing of the data offered a distinct advantage, because it maximized the likelihood of the data being error-free and accurate. Timely generation of field check tables allowed for more effective monitoring. The secondary editing of the data was completed by November 2017. The final cleaning of the data set was carried out by data processing specialists from The DHS Program by the end of December 2017.
A total of 31,791 households were selected for the sample, of which 27,855 were occupied. Of the occupied households, 27,496 were successfully interviewed, yielding a response rate of 99%. In the interviewed households, 25,690 women age 15-49 were identified for individual interviews; interviews were completed with 25,074 women, yielding a response rate of 98%.
The household response rate is slightly lower in urban areas than in rural areas (98% and 99%, respectively); however, there is no difference by urban-rural residence in response rates among women (98% for each).

The 2019-20 Gambia Demographic and Health Survey (2019-20 GDHS) is a nationwide survey with a nationally representative sample of residential households. The survey was implemented by The Gambia Bureau of Statistics (GBoS) in collaboration with the Ministry of Health (MoH).
The primary objective of the 2019-20 GDHS is to provide up-to-date estimates of basic demographic and health indicators. Specifically, the 2019-20 GDHS: ▪ collected data on fertility levels and preferences; contraceptive use; maternal and child health; infant, child, and neonatal mortality levels; maternal mortality; gender; nutrition; awareness about HIV/AIDS; self-reported sexually transmitted infections (STIs); and other health issues relevant to the achievement of the Sustainable Development Goals (SDGs) ▪ obtained information on the availability of, access to, and use of mosquito nets as part of the National Malaria Control Programme ▪ gathered information on other health issues such as injections, tobacco use, hypertension, diabetes, and health insurance ▪ collected data on women’s empowerment, domestic violence, fistula, and female genital mutilation/cutting ▪ tested household salt for the presence of iodine ▪ obtained data on child feeding practices, including breastfeeding, and conducted anthropometric measurements to assess the nutritional status of children under age 5 and women age 15-49 ▪ conducted anaemia testing of women age 15-49 and children age 6-59 months ▪ conducted malaria testing of children age 6-59 months

The sampling frame used for the 2019-20 GDHS was based on an updated version of the 2013 Gambia Population and Housing Census (2013 GPHC) conducted by GBoS. The census counts were updated in 2015-16 based on district-level projected counts from the 2015-16 Integrated Household Survey (IHS). Administratively, The Gambia is divided into eight Local Government Areas (LGAs). Each LGA is subdivided into districts and each district is subdivided into settlements. A settlement, a group of small settlements, or a part of a large settlement can form an enumeration area (EA). These units allow the country to be easily separated into small geographical area units, each with an urban or rural designation. There are 48 districts, 120 wards, and 4,098 EAs in The Gambia; the EAs have an average size of 68 households.
The sample for the 2019-20 GDHS was a stratified sample selected in two stages. In the first stage, EAs were selected with a probability proportional to their size within each sampling stratum. A total of 281 EAs were selected.
In the second stage, the households were systematically sampled. A household listing operation was undertaken in all of the selected clusters. The resulting lists of households served as the sampling frame from which a fixed number of 25 households were systematically selected per cluster, resulting in a total sample size of 7,025 selected households. Results from this sample are representative at the national, urban, and rural levels and at the LGA levels.

Computer Assisted Personal Interview [capi]
Five questionnaires were used for the 2019-20 GDHS: the Household Questionnaire, the Woman’s Questionnaire, the Man’s Questionnaire, the Biomarker Questionnaire, and the Fieldworker Questionnaire. These questionnaires, based on The DHS Program’s standard questionnaires, were adapted to reflect the population and health issues relevant to The Gambia. Suggestions were solicited from various stakeholders representing government ministries, departments, and agencies; nongovernmental organisations; and international donors. All questionnaires were written in English, and interviewers translated the questions into the appropriate local language to carry out the interview.
All electronic data files were transferred via the Internet File Streaming System (IFSS) to the GBoS central office. The IFSS automatically encrypts the data and sends the data to a server, and the server in turn downloads the data to the data processing supervisor’s password-protected computer in the central office. The data processing operation included secondary editing, which required resolution of computeridentified inconsistencies and coding of open-ended questions. The data were processed by two IT specialists and three secondary editors who took part in the main fieldwork training; they were supervised remotely by staff from The DHS Program. Data editing was accomplished using CSPro software. During the fieldwork, field-check tables were generated to check various data quality parameters, and specific feedback was given to the teams to improve performance. Secondary editing and data processing were initiated in November 2019 and completed in May 2020.
All 6,985 households in the selected housing units were eligible for the survey, of which 6,736 were occupied. Of the occupied households, 6,549 were successfully interviewed, yielding a response rate of 97%. Among the households successfully interviewed, 1,948 interviews were completed in 2019 and 4,601 in 2020.
In the interviewed households, 12,481 women age 15-49 were identified for individual interviews; interviews were completed with 11,865 women, yielding a response rate of 95%, a 4 percentage point increase from the 2013 GDHS. Among men, 5,337 were eligible for individual interviews, and 4,636 completed an interview; this yielded a response rate of 87%, a 5 percentage point increase from the previous survey.
The estimates from a sample survey are affected by two types of errors: nonsampling errors and sampling errors. Nonsampling errors are the results of mistakes made in implementing data collection and data processing, such as failure to locate and interview the correct household, misunderstanding of the questions on the part of either the interviewer or the respondent, and data entry errors. Although numerous efforts were made during the implementation of the 2019-20 Gambia Demographic and Health Survey (GDHS) to minimise this type of error, nonsampling errors are impossible to avoid and difficult to evaluate statistically.
Sampling errors, on the other hand, can be evaluated statistically. The sample of respondents selected in the 2019-20 GDHS is only one of many samples that could have been selected from the same population, using the same design and expected size. Each of these samples would yield results that differ somewhat from the results of the actual sample selected. Sampling errors are a measure of the variability among all possible samples. Although the degree of variability is not known exactly, it can be estimated from the survey results.
Sampling error is usually measured in terms of the standard error for a particular statistic (mean, percentage, etc.), which is the square root of the variance. The standard error can be used to calculate confidence intervals within which the true value for the population can reasonably be assumed to fall. For example, for any given statistic calculated from a sample survey, the value of that statistic will fall within a range of plus or minus two times the standard error of that statistic in 95% of all possible samples of identical size and design.
If the sample of respondents had been selected as a simple random sample, it would have been possible to use straightforward formulas for calculating sampling errors. However, the 2019-20 GDHS sample is the result of a multi-stage stratified design, and, consequently, it was necessary to use more complex formulas. Sampling errors are computed in SAS, using programs developed by ICF. These programs use the Taylor linearisation method to estimate variances for survey estimates that are means, proportions, or ratios. The Jackknife repeated replication method is used for variance estimation of more complex statistics such as fertility and mortality rates.
