This is a release of management information about patients previously identified as clinically extremely vulnerable (CEV) and at high risk of death from coronavirus (COVID-19) who were on the Shielded Patient List (SPL). The purpose behind the public release of SPL data has been to make available anonymous and summarised regional and local data to allow for its use in public health analysis. As the UK government has ended the shielding programme in England, the SPL dashboard will not be updated after 07 October 2021. The final dashboard and release of management information is based on version 75 of the SPL clinical methodology, with the data extracted on 30 September 2021.

This is a release of management information for anonymous summary data for those patients that have been identified on the Shielded Patient List (SPL). Its purpose is to make the summary data available to a wider audience, as open data, to enable a broad base of users to perform analysis from it. The purpose behind releasing this data is to present regional and local data to allow for its use in public health. It will also allow for greater analysis, modelling and planning to be performed using the latest data, to aid in the response to the pandemic. We will update this weekly and we would welcome your feedback to help us develop our open data sets. The data that is published is based on version 74 of the SPL clinical methodology, with the data extracted as at 21 September 2021.

The Chief Medical Officer (CMO) for England, working with the CMOs of the devolved nations and other senior clinicians, commissioned NHS Digital to produce a list of people at “high risk” of complications from COVID-19, who should be shielded for at least 12 weeks. The CMO for Wales commissioned a collaboration of national bodies in Wales (NWIS, DU, NWSSP, PHW) to identify “high risk” people for the Welsh population, based largely on the NHS Digital methodology. This list is referred to as the Shielded Patient List (SPL).
The “high risk” list was defined as a subset of a wider group of people who may be “at risk”. Specific advice applies to these groups, currently this advice is: • “At Risk” – large group normally at risk from the flu - should practice strict social distancing • “At high risk” – a smaller sub-group (circa 70k), defined by CMO – should practice complete social “shielding” NHS Digital have described the methodology that has been used to identify patients who meet the high risk criteria due to their inclusion in one or more of the disease groups.
As there are differences in some of the systems used across the devolved nations, nuanced differences in application and interpretation of CMO guidance, this document describes the Welsh methodology.

Datasets and interview transcripts from a Q-methodology study with 54 individuals with a range of different experiences of, and expertise in relation to, the COVID-19 pandemic. Participants included, for example, seldom-heard and low-income individuals, health practitioners, health and social policy academics and relevant policy makers, key workers, furloughed staff, and individuals directed to shield by the NHS. Participants from England and Scotland rank ordered 60 statements onto a quasi-normal shaped grid according to their point of view in 2021. The dataset includes data from the Q sorts (n=54), socio-demographic survey (n=54) and post-sort qualitative interviews (n=53).
Amongst those hit hardest by COVID-19, and the associated social and economic measures put in place to combat it, will be people in low-income settings, and, within that group, those in hard-to-reach groups continually threatened by financial exclusion. People in such situations already live in precarious financial situations which could be amongst the most vulnerable to measures such as social distancing and self-isolation. A Q study was conducted, with a focus on perceptions of COVID-19 and the societal responses to which study participants have been subjected. Q methodology is an established approach to study subjective opinion and beliefs. The views of seldom-heard individuals were complemented and compared with those of a wide range of other participants, for example, health practitioners, health and social policy academics and relevant policy makers, key workers, furloughed staff, and individuals directed to shield by the NHS. We purposively selected 54 individuals with a range of different experiences of, and expertise in relation to, the COVID-19 pandemic. Participants rank ordered 60 statements onto a quasi-normal shaped grid according to their point of view. These data will be useful in enhancing the acceptability of, adherence to and effective delivery of evidence-based strategies for future prevention and containment.

The Coronavirus disease 2019 (COVID-19) outbreak generated an unprecedented set of emotional challenges for women diagnosed with breast cancer. In the United Kingdom (UK), the pandemic significantly disrupted oncology services as resources were reassigned to care for COVID-19 patients. In addition to service disruptions, many women received a UK Government letter advising them to shield for 12-weeks. We aimed to explore the effect of disruption to scheduled oncology services and the UK Government shielding letter on emotional and cognitive vulnerability. A further aim was to investigate the relationship between COVID-19 related emotional vulnerability (COVID-EMV) and anxiety, depression and perceived cognitive function. Women diagnosed with primary breast cancer (N = 234) completed a series of online questionnaires to assess their cognitive and emotional wellbeing as well as their COVID-EMV. Results indicated that disrupted oncology services had a significant impact on COVID-EMV, anxiety and depression, with those experiencing disruptions expressing higher general emotional vulnerability as well as COVID-EMV. Further, the UK Government letter had a significant effect on perceived cognitive function; those who received the letter reported poorer cognitive function. Regression analyses revealed that after allowing for the effects of sociodemographic and clinical variables, women’s COVID-EMV significantly predicted worse outcomes of anxiety, depression and perceived cognitive function. Our findings indicate that concerns about COVID-19 amongst women affected by breast cancer leads to increased risk of developing affective disorder, such as anxiety and depression symptomatology, among this sample. We advocate the rapid implementation of accessible interventions designed to promote emotional resilience in the breast cancer population.
