Note: This web page provides data on health facilities only. To file a complaint against a facility, please see: https://www.cdph.ca.gov/Programs/CHCQ/LCP/Pages/FileAComplaint.aspx

The California Department of Public Health (CDPH), Center for Health Care Quality, Licensing and Certification (L&C) Program licenses and certifies more than 30 types of healthcare facilities. The Electronic Licensing Management System (ELMS) is a CDPH data system created to manage state licensing-related data and enforcement actions. This file includes California healthcare facilities that are operational and have a current license issued by the CDPH and/or a current U.S. Department of Health and Human Services’ Centers for Medicare and Medicaid Services (CMS) certification.

To link the CDPH facility IDs with those from other Departments, like HCAI, please reference the "Licensed Facility Cross-Walk" Open Data table at https://data.chhs.ca.gov/dataset/licensed-facility-crosswalk. Facility geographic variables are updated monthly, if latitude/longitude information is missing at any point in time, it should be available when the next time the Open Data facility file is refreshed.

Please note that the file contains the data from ELMS as of the 11th business day of the month. See DATA_DATE variable for the specific date of when the data was extracted. To access the CDPH Facilities map, follow this link: https://go.cdii.ca.gov/cdph-facilities

    Description

In this course, you will explore a variety of open-source technologies for working with geosptial data, performing spatial analysis, and undertaking general data science. The first component of the class focuses on the use of QGIS and associated technologies (GDAL, PROJ, GRASS, SAGA, and Orfeo Toolbox). The second component of the class introduces Python and associated open-source libraries and modules (NumPy, Pandas, Matplotlib, Seaborn, GeoPandas, Rasterio, WhiteboxTools, and Scikit-Learn) used by geospatial scientists and data scientists. We also provide an introduction to Structured Query Language (SQL) for performing table and spatial queries. This course is designed for individuals that have a background in GIS, such as working in the ArcGIS environment, but no prior experience using open-source software and/or coding. You will be asked to work through a series of lecture modules and videos broken into several topic areas, as outlined below. Fourteen assignments and the required data have been provided as hands-on opportunites to work with data and the discussed technologies and methods. If you have any questions or suggestions, feel free to contact us. We hope to continue to update and improve this course. This course was produced by West Virginia View (http://www.wvview.org/) with support from AmericaView (https://americaview.org/). This material is based upon work supported by the U.S. Geological Survey under Grant/Cooperative Agreement No. G18AP00077. The views and conclusions contained in this document are those of the authors and should not be interpreted as representing the opinions or policies of the U.S. Geological Survey. Mention of trade names or commercial products does not constitute their endorsement by the U.S. Geological Survey. After completing this course you will be able to: apply QGIS to visualize, query, and analyze vector and raster spatial data. use available resources to further expand your knowledge of open-source technologies. describe and use a variety of open data formats. code in Python at an intermediate-level. read, summarize, visualize, and analyze data using open Python libraries. create spatial predictive models using Python and associated libraries. use SQL to perform table and spatial queries at an intermediate-level.

DEP, Water Resource Management - Certification & Restoration Program currently licenses water and wastewater treatment plant operators and water distribution system operators throughout Florida. These are individuals that are licensed to operate DEP permitted water treatment facilities (DW), wastewater treatment facilities (WW) and water distribution systems (DS). In all, there are 12 different levels of licensure (DS1, DS2, DS3, DWA, DWB, DWC, DWD, WWA, WWB, WWC and WWD) and two licensure statuses ("Active" and "Inactive"). It is our intent to create a visual representation of the location of each of these individual licenses in Florida by using the addresses that are stored in the program's Oracle database. This layer only includes those licenses which are "Active".

A community's permit file must have an official record that shows new buildings and substantial improvements in all identified Special Flood Hazard Areas (SFHAs)are properly elevated. This elevation information is needed to show compliance with the floodplain management ordinance. FEMA encourages communities to use the Elevation Certificate developed by FEMA to fulfill this requirement since it also can be used by the property owner to obtain flood insurance. Communities participating in the Community Rating System (CRS) are required to use the FEMA Online Elevation Certificate, FEMA Form FF-206-FY-22-152 (formerly 086-0-33).

The areas in this data set illustrate the eligibility of streams in the area for a nationwide 401 permit. Three categories are identified, eligible, ineligible, and possibily eligible, with additional field screening required. Categories of Eligibility: Eligible for Coverage under Nationwide Permits (NWP): Areas with no shading. Other terms and conditions may apply. Ineligible for Coverage under NWPs: Projects affecting high quality streams and undesignated streams draining directly to high quality streams, as represented in the map, must undergo individual 401 review process. While quality of undesignated streams within these designated catchments is unknown, we do know that impacts to these upstream waters could affect the known high quality receiving stream. As such, an individual WQC would be required, including a complete antidegradation review allowing for public comment. Possibly Eligible for Coverage under NWP - Additional Field Screening Procedures Required:Projects affecting undesignated streams within those HUC12 watersheds that do not directly but eventually drain into high quality waters, might be eligible for coverage under Ohio certification of NWPs depending on the results of an abbreviated field screening assessment. To reproduce the colors of the online map layer use these values and colors: Ineligible: Value = 1, color (R:197 G:0 B:255); Possibly Eligible: Value = 2, color (R:255 G: 255 B:115); Eligible: Value = 3, color: no color, transparent. Color values are RGB (Red, Green, Blue).
