Artificial Intelligence (AI) language models continue to expand in both access and capability. As these models have evolved, the number of academic journals in medicine and healthcare which have explored policies regarding AI-generated text has increased. The implementation of such policies requires accurate AI detection tools. Inaccurate detectors risk unnecessary penalties for human authors and/or may compromise the effective enforcement of guidelines against AI-generated content. Yet, the accuracy of AI text detection tools in identifying human-written versus AI-generated content has been found to vary across published studies. This experimental study used a sample of behavioral health publications and found problematic false positive and false negative rates from both free and paid AI detection tools. The study assessed 100 research articles from 2016–2018 in behavioral health and psychiatry journals and 200 texts produced by AI chatbots (100 by “ChatGPT” and 100 by “Claude”). The free AI detector showed a median of 27.2% for the proportion of academic text identified as AI-generated, while commercial software Originality.AI demonstrated better performance but still had limitations, especially in detecting texts generated by Claude. These error rates raise doubts about relying on AI detectors to enforce strict policies around AI text generation in behavioral health publications.

Objectives: This study aimed to analyse, at national level, the effects of the free healthcare policy for children on the use of health services by children under five in Burkina Faso. We hypothesized that this policy has led to an immediate and sustained increase in the use of health services for these children in the country. Setting: We conducted a controlled interrupted time series. Monthly data at district level, spanning from January 2013 to December 2018 and corresponding to 72 monthly data points (39 before and 33 after), were extracted from the Burkina Faso National Health Information System. The analysed dataset included data from all the 70 health districts of the country. Participants: The study consisted of aggregated data from children under five as the target for the policy with children aged between 5-14 years old as control group. Intervention: The intervention was the introduction of the free healthcare policy for women and children under five years from April 2016. Outcome: The primary outcome was the mean monthly rate of health services visits by children Results: Among the children under five, the rate of visits increased of 57% (IRR= 1.57; 95% CI=1.2 to 2.0) in the month immediately following the launching of the free healthcare policy. An increase in the rate of health facility visits of 1% (IRR=1.01; 95% CI=1.0 to 1.1) per month was also noted during post-intervention. Compared to the control group, we observed an increase of the rate of visits of 2.5% (IRR=1.025; 95%CI= 1.023 to 1.026) per month. Conclusion: Findings suggest that the free healthcare policy increased the use of health facilities for care in Burkina Faso immediately after the implementation of the policy with a small increase in the rate overtime. Strategies to maintain the policy effect over time are necessary. Methods Monthly data were retrospectively extracted from the National Health Information System from January 2013 to December 2018. All the data were extracted in May 07–30, 2021 and updated on May 05 –12, 2022. The dataset included observations on 39 months for the pre-intervention period and 33 months for the post-intervention period for all 70 health districts in the country.

Background: The analysis of clinical free text from patient records for research has potential to contribute to the medical evidence base but access to clinical free text is frequently denied by data custodians who perceive that the privacy risks of data-sharing are too high. Engagement activities with patients and regulators, where views on the sharing of clinical free text data for research have been discussed, have identified that stakeholders would like to understand the potential clinical benefits that could be achieved if access to free text for clinical research were improved. We aimed to systematically review all UK research studies which used clinical free text and report direct or potential benefits to patients, synthesizing possible benefits into an easy to communicate taxonomy for public engagement and policy discussions.Methods: We conducted a systematic search for articles which reported primary research using clinical free text, drawn from UK health record databases, which reported a benefit or potential benefit for patients, actionable in a clinical environment or health service, and not solely methods development or data quality improvement. We screened eligible papers and thematically analyzed information about clinical benefits reported in the paper to create a taxonomy of benefits.Results: We identified 43 papers and derived five themes of benefits: health-care quality or services improvement, observational risk factor-outcome research, drug prescribing safety, case-finding for clinical trials, and development of clinical decision support. Five papers compared study quality with and without free text and found an improvement of accuracy when free text was included in analytical models.Conclusions: Findings will help stakeholders weigh the potential benefits of free text research against perceived risks to patient privacy. The taxonomy can be used to aid public and policy discussions, and identified studies could form a public-facing repository which will help the health-care text analysis research community better communicate the impact of their work.
