BackgroundIdentifying individuals at high risk of atherosclerotic cardiovascular disease (ASCVD) and implementing targeted prevention strategies might be the key to reducing the heavy disease burden in China. This study aimed to evaluate the prevalence, awareness, treatment, and risk factor control among individuals with high 10-year ASCVD risk in Guangzhou, China.MethodsThis study included 15,165 adults (aged 18 years and older) from 138 urban and rural communities in the 2018 survey of China Chronic Disease and Risk Factors Surveillance in Guangzhou. 10-year ASCVD risk was estimated using the risk assessment models recommended in the Chinese Guideline for the Prevention of Cardiovascular Disease 2017. The prevalence, awareness, treatment, and risk factor control of high ASCVD risk (defined as 10-year risk ≥10%) were examined.ResultsAmong the study population, the weighted proportion of men was 51.9%, and the mean age was 41.27 ± 0.52 years. The overall standardized prevalence of high 10-year ASCVD risk was 13.8% (95% CI, 12.4%–15.3%). The awareness rates for hypertension, diabetes, and hyperlipidemia were 48.0% (95% CI, 42.8%–53.4%), 48.3% (95% CI, 43.0%–53.7%), and 17.9% (95% CI, 14.4%–22.1%) among those with corresponding risk factors. The proportions of drug use in prevention were relatively low in primary prevention, with the rates of using BP-lowering, glucose-lowering, lipid-lowering, and aspirin being 37.7% (95% CI, 32.8%–42.8%), 41.4% (95% CI, 35.8%–47.3%), 6.7% (95% CI, 4.5%–10.0%), and 1.0% (95% CI, 0.6%–1.8%), respectively. As for risk factor control, only 29.3% (95% CI, 25.7%–33.2%), 16.8% (95% CI, 15.0%–18.6%), and 36.0% (95% CI, 31.1%–41.2%) of individuals with high ASCVD risk had ideal levels of blood pressure, LDL-C, and body weight.ConclusionThe estimated prevalence of 10-year high ASCVD risk was high in Guangzhou, while the rates of treatment and risk factor control in primary prevention were still far from optimal, especially for lipid management. These findings suggested that substantial improvement in ASCVD prevention is needed in this population.

BackgroundThe association of prediabetes and diabetes on health-related quality of life (HRQoL) remains inconclusive in current epidemiological research. In this investigation, we administered 5-level EuroQoL-5 dimension version (EQ-5D-5L) to systematically assess HRQoL across glycemic strata (diabetes, prediabetes, and normal glycemic levels) in Guangzhou, and to offer baseline data that can be easily compared to other regions in China or across countries.MethodThis investigation utilized baseline cross-sectional data extracted from a three-year prospective cohort study conducted at the Health Management Center of Guangzhou 11th People’s Hospital. Propensity score matching was implemented at a 1:1:4 ratio to balance participants across diabetes, prediabetes, and normal glycemic group. HRQoL outcomes, operationalized through EQ-Index and EQ visual analog scale (EQ-VAS) measurements, were compared across groups using one-way ANOVA or Wilcoxon rank-sum tests. Multivariate linear regression was constructed to adjust for potential confounders, followed by subgroup analyses stratified by sex, age categories, body mass index (BMI) classifications, and hypertension comorbidity status.ResultsA total of 18,605 participants were included in the study. After propensity score matching, 533 participants allocated to the prediabetes group, 533 to the diabetes group, and 2064 to the normal glycemic group. Intergroup comparisons demonstrated significantly lower EQ-VAS scores in the diabetes group (79.11) compared to both prediabetes (80.67) and normal glycemic group (81.65). Similarly, the diabetes group exhibited the lowest EQ-Index scores (0.968) relative to prediabetes (0.972) and normal glycemic group (0.972). Multivariate linear regression adjusted with sex, age, BMI, etc. revealed a 2.139-point reduction in EQ-VAS scores for the diabetes group versus normal glycemic group (95% CI: -3.748, -0.530; P=0.009). Subgroup analyses identified particularly compromised HRQoL in diabetes and prediabetes populations among female participants, individuals with obesity, and those aged ≥60 years.DiscussionPrediabetes and diabetes mellitus are associated with diminished HRQoL compared to normal glycemic levels, with a more pronounced negative associations observed among female populations, older adults, and individuals with obesity. These findings emphasize the clinical necessity for implementing targeted interventions to optimize HRQoL outcomes in these high-risk subgroups, which aligns with the fundamental objectives of contemporary diabetes management frameworks.Clinical trial registrationClinicalTrials.gov ID: NCT05315895.

Klebsiella aerogenes is a common infectious bacterium that poses a threat to human health. Nevertheless, there are limited data on the population structure, genetic diversity, and pathogenicity of K. aerogenes, especially among men who have sex with men (MSM). The present study aimed to clarify the sequence types (STs), clonal complexes (CCs), resistance genes, and virulence factors of popular strains. Multilocus sequence typing was used to describe the population structure of K. aerogenes. The Virulence Factor Database and Comprehensive Antibiotic Resistance Database were used to assess the virulence and resistance profiles. In this study, next-generation sequencing was performed on nasal swabs specimens collected in an HIV Voluntary Counseling Testing outpatient department in Guangzhou, China, from April to August 2019. The identification results showed that a total of 258 K. aerogenes isolates were collected from 911 participants. We found that the isolates were most resistant to furantoin (89.53%, 231/258) and ampicillin (89.15%, 230/258), followed by imipenem (24.81%, 64/258) and cefotaxime (18.22%, 47/258). The most common STs in carbapenem-resistant K. aerogenes were ST4, ST93, and ST14. The population has at least 14 CCs, including several novel ones identified in this study (CC11-CC16). The main mechanism of drug resistance genes was antibiotic efflux. Based on the presence of the iron carrier production genes irp and ybt, we identified two clusters according to virulence profiles. In cluster A, CC3 and CC4 carry the clb operator encoding the toxin. Increased monitoring is needed for the three main ST type strains carried by MSM. The main clone group CC4 has a large number of toxin genes, and it spreads among MSM. Caution is needed to prevent further spread of this clone group in this population. In sum, our results may provide a foundation for the development of new therapeutic and surveillance strategies for treating MSM.
