    Description

    This dataset contains the locations of Article 28, Article 36, Article 40, and Article 7 health care facilities and programs from the Health Facilities Information System (HFIS). Article 28 facilities are hospitals, nursing homes, diagnostic treatment centers and midwifery birth centers. Article 36 facilities are certified home health care agencies, licensed home care services agencies, and long term home health care programs. Article 40 facilities are hospices. Article 7 are licensed adult care facilities.

    The dataset currently only contains the locations of hospitals and hospital extension clinics. The data for the remaining facility types will be added in the future.

    Note: As of 1/22/25, this dataset is no longer updated. This dataset reports the number of people vaccinated by New York providers with at least one dose and with a complete COVID-19 vaccination series overall since December 14, 2020. Currently, three COVID-19 vaccines have been authorized for emergency use by the FDA and approved by New York State's independent Clinical Advisory Task Force: one that was developed by Pfizer and BioNTech, a second that was developed by Moderna and a third that was developed by Janssen/Johnson & Johnson. New York providers include hospitals, mass vaccination sites operated by the State or local governments, pharmacies, and other providers registered with the state to serve as points of distribution. This dataset is created by the New York State Department of Health from data reported to the New York State Immunization Information System (NYSIIS) and the New York City Citywide Immunization Registry (NYC CIR).

    NYSIIS and CIR are confidential, secure, web-based systems that collect and maintain immunization information in one consolidated record for persons of all ages in NYS governed by Public Health Law 2168. Health care providers are required, by law, to enter all vaccines administered to children up to age 19. Immunizations administered to adults 19 and older may be reported with consent. New York State Department of Health requires all New York State vaccination providers to report all COVID-19 vaccination administration data to NYSIIS and NYC CIR within 24 hours of administration.

    Vaccination data by age is based on address data reported to NYSIIS and NYC CIR by the providers administering vaccines. Age is calculated by subtracting the dob from the date of vaccination. Note that COVID-19 vaccine availability greatly expanded for the different age groups over the period of time this dataset covers. This data does not include vaccine administered through Federal entities or performed outside of New York State to New York residents. NYSIIS and CIR data is used for age group statistics. This dataset is updated weekly.

    Description

    This dataset includes Medicaid Managed Care, Commercial HMO, and Commercial PPO performance data from the Quality Assurance Reporting Requirements (QARR) by member demographic characteristics. QARR is largely based on measures of quality developed and published by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS®). Plans are required to submit quality performance data each year. Demographic information analyzed in this report includes members’ sex, age, race/ethnicity, Medicaid aid category, cash assistance status, behavioral health conditions including serious mental illness (SMI) and substance use disorder (SUD), payer status, and region of residence. Measuring the quality of care, and the ability to measure disparities in care is an important first step to a better understanding of the underlying factors that drive differences in care among certain populations within Medicaid Managed Care, Commercial HMO, and Commercial PPO. These data are published annually for Medicaid Managed Care in the Health Care Disparities in New York State Report and on the NYSDOH web site: https://www.health.ny.gov/health_care/managed_care/reports/

    New York State Community Health Indicator Reports (CHIRS) were developed in 2012, and are updated annually to consolidate and improve data linkages for the health indicators included in the County Health Assessment Indicators (CHAI) for all communities in New York. The CHIRS present data for more than 300 health indicators that are organized by 15 different health topics. Data tables are provided for all 62 New York State counties, 11 regions (including New York City), the State excluding New York City, and New York State.

The primary goal of publishing this dataset is to provide users timely information about local disease spread and COVID-19 case rates by age group. The data will be updated weekly, reflecting tests reported by 12:00 AM three days prior to the date of the update.

    Total positives includes both PCR and antigen positive test results.

This dataset includes information on the number of tests of individuals for COVID-19 infection by zip code performed in New York State beginning March 1, 2020, when the first case of COVID-19 was identified in the state. The primary goal of publishing this dataset is to provide users timely information about local disease spread and reporting of positive cases. The data will be updated weekly, reflecting tests completed by 2:00 pm on the day prior to the date of the update.

    Note: On November 14, 2020, only 14 hours of laboratory data was collected and shared. The 2:00 pm cutoff time was implemented, allowing the NYSDOH to enhance data quality reviews. All other published laboratory data represented 24 hours of data collection. Prior to November 14, 2020 data reflected tests completed by 12:00 am (midnight) the day of the update (i.e., all tests reported by the end of the day on the day before the update).

    As of April 4, 2022, the Department of Health and Human Services (HHS) no longer requires entities conducting COVID testing to report negative or indeterminate antigen test results. This may impact the number and interpretation of total test results reported to the state and also impacts calculation of test percent positivity. Total positives continues to include both PCR and antigen positive test results.

    This dataset contains Potentially Avoidable Antibiotic Prescribing observed and risk-adjusted rates for child Medicaid enrollees by provider county beginning in 2010.

    Potentially Avoidable Antibiotic Prescriptions are antibiotic prescriptions filled for the treatment of acute respiratory infections for which antibiotics are not indicated, contributing to bacterial drug resistance. Index visits for acute respiratory infections and corresponding prescription fills were identified through the use of previously published methods.

    The rates were calculated using Medicaid outpatient claims and encounters, and prescription drug data.

    The observed and risk adjusted rates are presented by provider county (including a statewide total).

Although violation details are collected on inspection reports (i.e., the actual food item, quantity and temperature of food found out of temperature control) as well as corrective actions for critical violations, this data set is limited to the violation number and the corresponding general violation description. This dataset is for reporting purposes only. Any concerns about individual establishments should be referred to the corresponding Local Health Department.

    Description

    The Nursing Homes Profiles quality data provides a consumer-friendly product that allows patients and their families to understand how the New York State Nursing Homes perform within five specific domains of care and overall. The domains (Preventive Care, Quality of Care, Quality of Life, Resident Safety and Resident Status) encompass twenty-four different quality measures. A Domain Rating assesses performance over all the measures within that domain, with 5 stars indicating the highest performance and 1 star the lowest performance. The Overall Rating is a normalized star rating based on the Nursing Homes' performance across the five domains. The normalization of the Overall Rating resets the distribution, with the highest performing Nursing Homes across all the domains having 5 stars and the lowest performing Nursing Homes across the five domains having 1 star. New York’s Nursing Home Domain Rating differs from CMS’ 5-star rating in data reporting period and in methodology.

Description

This dataset contains the locations of Article 28, Article 36, Article 40, and Article 7 health care facilities and programs from the Health Facilities Information System (HFIS). Article 28 facilities are hospitals, nursing homes, diagnostic treatment centers and midwifery birth centers. Article 36 facilities are certified home health care agencies, licensed home care services agencies, and long term home health care programs. Article 40 facilities are hospices. Article 7 are licensed adult care facilities.

The dataset currently only contains the locations of hospitals and hospital extension clinics. The data for the remaining facility types will be added in the future.
