    Description

    After May 3, 2024, this dataset and webpage will no longer be updated because hospitals are no longer required to report data on COVID-19 hospital admissions, and hospital capacity and occupancy data, to HHS through CDC’s National Healthcare Safety Network. Data voluntarily reported to NHSN after May 1, 2024, will be available starting May 10, 2024, at COVID Data Tracker Hospitalizations.

    The following dataset provides state-aggregated data for hospital utilization. These are derived from reports with facility-level granularity across two main sources: (1) HHS TeleTracking, and (2) reporting provided directly to HHS Protect by state/territorial health departments on behalf of their healthcare facilities.

    The file will be updated regularly and provides the latest values reported by each facility within the last four days for all time. This allows for a more comprehensive picture of the hospital utilization within a state by ensuring a hospital is represented, even if they miss a single day of reporting.

    No statistical analysis is applied to account for non-response and/or to account for missing data.

    The below table displays one value for each field (i.e., column). Sometimes, reports for a given facility will be provided to more than one reporting source: HHS TeleTracking, NHSN, and HHS Protect. When this occurs, to ensure that there are not duplicate reports, prioritization is applied to the numbers for each facility.

    On June 26, 2023 the field "reporting_cutoff_start" was replaced by the field "date".

    This dataset contains data for the Healthcare Payments Data (HPD) Services report. The term "Services" refers to individual procedures reported on the service lines of healthcare claims in California, categorized using the Restructured Berenson-Eggers Type of Services (BETOS) Classification System (RBCS) from the Centers for Medicare & Medicaid Services (CMS). The data in the report includes three main metrics: Total services, the total member count, and the service rate per 1,000 members. Total services represents the total number of services received by members during the reporting year. The member count reports the total number of unique individuals who received at least one service during the reporting year. The service rate per 1,000 members is calculated by dividing the total number of services during the reporting year by the total sum of monthly member enrollments (provided in the data) and multiplying the result by 12,000. The metrics can be grouped by year, age, sex (assigned at birth), county of residence (including an option for Los Angeles Service Planning Areas, or SPAs), Covered California Region, and payer.

    Users can choose to view the data at two different levels. The most aggregate level groups the data by the eight main RBCS categories: Anesthesia, Durable Medical Equipment (DME), Evaluation and Management (E&M), Imaging, Procedure, Test, Treatment and Other. The second level breaks the eight aggregate RBCS categories into more specific subcategories. Data files are provided for each choice.

    This dataset includes Medicaid Managed Care, Commercial HMO, and Commercial PPO performance data from the Quality Assurance Reporting Requirements (QARR) by member demographic characteristics. QARR is largely based on measures of quality developed and published by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS®). Plans are required to submit quality performance data each year. Demographic information analyzed in this report includes members’ sex, age, race/ethnicity, Medicaid aid category, cash assistance status, behavioral health conditions including serious mental illness (SMI) and substance use disorder (SUD), payer status, and region of residence. Measuring the quality of care, and the ability to measure disparities in care is an important first step to a better understanding of the underlying factors that drive differences in care among certain populations within Medicaid Managed Care, Commercial HMO, and Commercial PPO.

    Description

    The Centers for Medicare & Medicaid Services (CMS) EHR Incentive Program provides incentive payments for eligible hospitals to adopt and meaningfully use certified health IT. Among the requirements to receive an incentive payment, participating hospitals must report on public health measures. These measures include the electronic reporting of data regarding: immunizations, emergency department visits (syndromic surveillance), reportable infectious disease laboratory results, and electronic patient data to specialized registries, like cancert. As of 2015, stage 2 of the EHR Incentive Program requires hospitals to report on three public health measures, when applicable, and modified stage 2 of the program requires hospitals to report on two of the three measures. This dataset includes the percentage of hospitals who reported on these measures in program years, 2013, 2014 and 2015.

    After over two years of public reporting, the State Profile Report will no longer be produced and distributed after February 2023. The final release was on February 23, 2023. We want to thank everyone who contributed to the design, production, and review of this report and we hope that it provided insight into the data trends throughout the COVID-19 pandemic. Data about COVID-19 will continue to be updated at CDC’s COVID Data Tracker.

    The State Profile Report (SPR) is generated by the Data Strategy and Execution Workgroup in the Joint Coordination Cell, in collaboration with the White House. It is managed by an interagency team with representatives from multiple agencies and offices (including the United States Department of Health and Human Services (HHS), the Centers for Disease Control and Prevention, the HHS Assistant Secretary for Preparedness and Response, and the Indian Health Service). The SPR provides easily interpretable information on key indicators for each state, down to the county level.

    It is a weekly snapshot in time that:

    • Focuses on recent outcomes in the last seven days and changes relative to the month prior
    • Provides additional contextual information at the county level for each state, and includes national level information
    • Supports rapid visual interpretation of results with color thresholds

    In 2020, the Washington State Legislature enacted Engrossed Substitute Senate Bill (ESSB) 6404 (Chapter 316, Laws of 2020, codified at RCW 48.43.0161), which requires that health carriers with at least one percent of the market share in Washington State annually report certain aggregated and de-identified data related to prior authorization to the Office of the Insurance Commissioner (OIC). Prior authorization is a utilization review tool used by carriers to review the medical necessity of requested health care services for specific health plan enrollees. Carriers choose the services that are subject to prior authorization review. The reported data includes prior authorization information for the following categories of health services: • Inpatient medical/surgical • Outpatient medical/surgical • Inpatient mental health and substance use disorder • Outpatient mental health and substance use disorder • Diabetes supplies and equipment • Durable medical equipment The carriers must report the following information for the prior plan year (PY) for their individual and group health plans for each category of services: • The 10 codes with the highest number of prior authorization requests and the percent of approved requests. • The 10 codes with the highest percentage of approved prior authorization requests and the total number of requests. • The 10 codes with the highest percentage of prior authorization requests that were initially denied and then approved on appeal and the total number of such requests. Carriers also must include the average response time in hours for prior authorization requests and the number of requests for each covered service in the lists above for: • Expedited decisions. • Standard decisions. • Extenuating-circumstances decisions. Engrossed Second Substitute House Bill 1357 added additional prescription drug prior authorization reporting requirements for health carriers beginning in reporting year 2024. Carriers were provided the opportunity to submit voluntary prescription drug prior authorization data for the 2023 reporting period.
