    Context

    Available research shows that social connections are important for our well-being. Having support from family and friends is important for our happiness and health, and is also instrumental to our ability to share information, learn from others, and seize economic opportunities.

    In this dataset we can explore data on loneliness and social connections across countries and over time, and review available evidence on how and why social connections and loneliness affect our health and emotional welfare, as well as our material well-being.

    Despite the fact that there is a clear link between social connections and well-being, more research is needed to understand causal mechanisms, effect sizes and changes over time.

    Researches show here, oversimplified narratives that compare loneliness with smoking, or that claim we are living a ‘loneliness epidemic’, are wrong and unhelpful.

    Dr. Vivek Murthy, former Surgeon General of the United States, recently wrote: “Loneliness and weak social connections are associated with a reduction in lifespan similar to that caused by smoking 15 cigarettes a day”.

    This ‘15 cigarettes a day’ figure has been reproduced and reported in the news many times, under headlines such as “Loneliness is as lethal as smoking 15 cigarettes per day”.

    It is indeed quite a shocking comparison since around 7 million deaths globally are attributed to smoking every year, and back-of-the-envelope calculations published in medical journals say one cigarette reduces your lifespan by 11 minutes.

    Here we can dig deeper to try to understand what the data and research tell us about the link between social relations and health. In a nutshell, the reading of the evidence is as follows:

    • There is a huge amount of evidence showing individuals who report feelings of loneliness are more likely to have health problems later in their life.
    • There is credible theory and explanation of biological mechanisms, whereby isolation can set off unconscious surveillance for social threat, producing cognitive biases, reducing sleep and affecting hormones.
    • It’s very likely there is a causal link, but there is no credible experimental evidence that would allow us to have a precise estimate of the magnitude of the causal effect that loneliness has on key metrics of health, such as life expectancy.
    • The fact that we struggle to pin down the magnitude of the effect of loneliness on health doesn’t mean we should dismiss the available evidence. But it does show that more research is needed.

    Observational studies: A first look at the data

    Measuring loneliness

    Psychologists and social neuroscientists often refer to loneliness as painful isolation. The emphasis on painful is there to make a clear distinction between solitude – the state of being alone – and subjective loneliness, which is the distressing feeling that comes from unmet expectations of the types of interpersonal relationships we wish to have.

    Researchers use several kinds of data to measure solitude and loneliness. The most common source of data are surveys where people are asked about different aspects of their lives, including whether they live alone, how much time they spend with other people in a given window of time (e.g. ‘last week’) or specific context (e.g. ‘at social events, clubs or places of worship’); and whether they experience feelings of loneliness (e.g. ‘I have no-one with whom I can discuss important matters with’). Researchers sometimes study these survey responses separately, but often they also aggregate them in a composite index.

    Surveys confirm that people respond differently to questions about subjective loneliness and physical social isolation, which suggests people do understand these as two distinct issues.

    The fact that we see such high levels of loneliness, with substantial divergence across countries, explains why this is an important and active research area. Indeed, there are literally hundreds of papers that have used survey data to explore the link between loneliness, solitude, and health. Below is an overview of what these studies find.

    Background This bibliometric analysis examines the top 50 most-cited articles on COVID-19 complications, offering insights into the multifaceted impact of the virus. Since its emergence in Wuhan in December 2019, COVID-19 has evolved into a global health crisis, with over 770 million confirmed cases and 6.9 million deaths as of September 2023. Initially recognized as a respiratory illness causing pneumonia and ARDS, its diverse complications extend to cardiovascular, gastrointestinal, renal, hematological, neurological, endocrinological, ophthalmological, hepatobiliary, and dermatological systems. Methods Identifying the top 50 articles from a pool of 5940 in Scopus, the analysis spans November 2019 to July 2021, employing terms related to COVID-19 and complications. Rigorous review criteria excluded non-relevant studies, basic science research, and animal models. The authors independently reviewed articles, considering factors like title, citations, publication year, journal, impact factor, authors, study details, and patient demographics. Results The focus is primarily on 2020 publications (96%), with all articles being open-access. Leading journals include The Lancet, NEJM, and JAMA, with prominent contributions from Internal Medicine (46.9%) and Pulmonary Medicine (14.5%). China played a major role (34.9%), followed by France and Belgium. Clinical features were the primary study topic (68%), often utilizing retrospective designs (24%). Among 22,477 patients analyzed, 54.8% were male, with the most common age group being 26–65 years (63.2%). Complications affected 13.9% of patients, with a recovery rate of 57.8%. Conclusion Analyzing these top-cited articles offers clinicians and researchers a comprehensive, timely understanding of influential COVID-19 literature. This approach uncovers attributes contributing to high citations and provides authors with valuable insights for crafting impactful research. As a strategic tool, this analysis facilitates staying updated and making meaningful contributions to the dynamic field of COVID-19 research. Methods A bibliometric analysis of the most cited articles about COVID-19 complications was conducted in July 2021 using all journals indexed in Elsevier’s Scopus and Thomas Reuter’s Web of Science from November 1, 2019 to July 1, 2021. All journals were selected for inclusion regardless of country of origin, language, medical speciality, or electronic availability of articles or abstracts. A total of 5940 articles were accessed, of which the top 50 most cited articles about COVID-19 and Complications of COVID-19 were selected through Scopus. Each article was reviewed for its appropriateness for inclusion. The articles were independently reviewed by three researchers (JRP, MAM and TS) (Table 1). Differences in opinion with regard to article inclusion were resolved by consensus. The inclusion criteria specified articles that were focused on COVID-19 and Complications of COVID-19. Articles were excluded if they did not relate to COVID-19 and or complications of COVID-19, Basic Science Research and studies using animal models or phantoms. Review articles, Viewpoints, Guidelines, Perspectives and Meta-analysis were also excluded from the top 50 most-cited articles (Table 1). The top 50 most-cited articles were compiled in a single database and the relevant data was extracted. The database included: Article Title, Scopus Citations, Year of Publication, Journal, Journal Impact Factor, Authors, Number of Authors, Department Affiliation, Number of Institutions, Country of Origin, Study Topic, Study Design, Sample Size, Open Access, Non-Original Articles, Patient/Participants Age, Gender, Symptoms, Signs, Co-morbidities, Complications, Imaging Modalities Used and outcome.

    Description

    My HealtheVet (www.myhealth.va.gov) is a Personal Health Record portal designed to improve the delivery of health care services to Veterans, to promote health and wellness, and to engage Veterans as more active participants in their health care. The My HealtheVet portal enables Veterans to create and maintain a web-based PHR that provides access to patient health education information and resources, a comprehensive personal health journal, and electronic services such as online VA prescription refill requests and Secure Messaging. Veterans can visit the My HealtheVet website and self-register to create an account, although registration is not required to view the professionally-sponsored health education resources, including topics of special interest to the Veteran population. Once registered, Veterans can create a customized PHR that is accessible from any computer with Internet access.

Descriptions of important findings that result from basic, translational, or clinical research are appropriate for submission. Clinical research can include, but is not limited to, interventional trials, genetics studies, epidemiology, and health services research. The JID places a high priority on publication of new insights into basic cutaneous biology, disease pathogenesis and treatment. Reports that describe a new methodology, technique, or tool in combination with mechanistic insights into the problem that is being investigated are encouraged.
