    Description

    Health systems in Canada struggle to generate and use knowledge to improve equity-centred quadruple aim measures, resulting in care that is misaligned with local contexts. Learning Health Systems (LHS) offer a solution by aligning real-time evidence, informatics, patient-provider partnerships, and institutional strategies to support continuous improvements in care. Despite their potential, LHS initiatives in Canada remain siloed and lack harmonized leadership, knowledge exchange, and capacity building. To address these gaps, the Learning Health Hub was established to foster collaboration, disseminate best practices, and enhance collective capacity for LHS in Canada. In June 2024, the Learning Health Hub hosted its inaugural virtual symposium. This event brought together partners, researchers, health professionals, system operators, and policymakers from across Canada interested in LHS work. The symposium aimed to map assets and build momentum for larger-scale impact in LHS. Participants engaged in generative activities to define challenges and co-create solutions, resulting in the identification of key learning priorities. Three learning themes were identified: Patient, Caregiver, and Community Partnership; Enabling Environments; and Benchmarking and Evaluation. By advancing these themes, the Learning Health Hub aims to drive meaningful, sustainable change and improve healthcare quality and outcomes.

    The Canadian Community Health Survey (CCHS) is a cross-sectional survey that collects information related to health status, health care utilization and health determinants for the Canadian population. The CCHS operates on a two-year collection cycle. The first year of the survey cycle “.1” is a large sample, general population health survey, designed to provide reliable estimates at the health region level. The second year of the survey cycle “.2” is a smaller survey designed to provide provincial level results on specific focused health topics. This Microdata File contains data collected in Cycle 1.2 Mental Health and Well-Being. This cycle mainly measures aspects linked to the mental health of Canadians. The primary objectives of the CCHS Mental Health and Well-being are to: Provide timely, reliable, cross-sectional estimates of mental health determinants, mental health status and mental health system utilization across Canada Determine prevalence rates of selected mental disorders to assess the impact of burden of illness Juxtapose access and utilization of mental health services with respect to perceived needs Assess the disabilities associated with mental health problems to individuals and society Information was collected between May 2002 and December 2002, for the ten provinces. Cycle 1.2 collects responses from persons aged 15 or older, living in private occupied dwellings. Excluded from the sampling frame are individuals living on Indian Reserves and on Crown Lands, Health Care institution residents, full-time members of the Canadian Armed Forces, and residents of certain remote regions.

    Description

    BackgroundIn Canada, cannabis legalization altered the way that the public can access cannabis for medical purposes. However, Canadians still struggle with finding healthcare professionals (HCPs) who are involved in medical cannabis counselling and authorization. This raises questions about the barriers that are causing this breakdown in care. Our study explored the perceptions of primary care providers regarding cannabis in their practice.MethodsSemi-structured interviews were conducted by Zoom with HCPs in Newfoundland and Labrador (NL) to discuss their experiences with medical and non-medical cannabis in practice. Family physicians and nurse practitioners who were practicing in primary care in NL were included. The interview guide and coding template were developed using the Theoretical Domains Framework (TDF). A thematic analysis across the TDF was then conducted.ResultsTwelve participants with diverse demographic backgrounds and experience levels were interviewed. Five main themes emerged including, knowledge acquisition, internal influences, patient influences, external HCP influences, and systemic influences. The TDF domain resulting in the greatest representation of codes was environmental context and resources.InterpretationThe findings suggested that HCPs have significant knowledge gaps in authorizing medical cannabis, which limited their practice competence and confidence in this area. Referring patients to cannabis clinics, while enforcing harm-reduction strategies, was an interim option for patients to access cannabis for medical purposes. However, developing practice guidelines and educational resources were suggested as prominent facilitators to promote medical cannabis authorization within the healthcare system.

The CCHS has the following objectives: - Support health surveillance programs by providing health data at the national, provincial and intra-provincial levels; - Provide a single data source for health research on small populations and rare characteristics; - Timely release of information easily accessible to a diverse community of users; - Create a flexible survey instrument that includes a rapid response option to address emerging issues related to the health of the population. The CCHS produces an annual microdata file and a file combining two years of data. The CCHS collection years with both consistent design and consistent population representation can also be combined by users to examine populations or rare characteristics. The primary use of the CCHS data is for health surveillance and population health research. Federal and provincial departments of health and human resources, social service agencies, and other types of government agencies use the information collected from respondents to monitor, plan, implement and evaluate programs to improve the health of Canadians. Researchers from various fields use the information to conduct research to improve health. Non-profit health organizations and the media use the CCHS results to raise awareness about health, an issue of concern to all Canadians. The survey began collecting data in 2001 and was repeated every two years until 2005. Starting in 2007, data for the Canadian Community Health Survey (CCHS) were collected annually instead of every two years. While a sample of approximately 130,000 respondents were interviewed during the reference periods of 2001, 2003 and 2005, the sample size was changed to 65,000 respondents each year starting in 2007. The CCHS has undergone two major redesigns. The first in 2015 reviewed the sampling methodology, adopted a new sample frame, modernized and updated its health content, and reviewed the target population. The second in 2022, further reviewed and updated the content of the survey as well as transitioned it to an online electronic questionnaire (EQ) that was available for direct self reporting by selected respondents. Consultations were held with federal, provincial and territorial share partners, health region authorities and academics for both redesigns. As a result of these redesigns and major changes to collection and sampling approaches as well as content updates, caution should be taken when comparing data from previous cycles to data released for the 2015 and 2021 and for data released 2022 and onwards.

    This survey provides data on the lifestyle and health of Canadians, complementing existing administrative data bases. Major areas of coverage include risk factors, health status, and consequences. Risks to future health of three main types are identified and probed in the CHS. They are lifestyle, bio-medical and environmental. Lifestyle, in particular, lends itself to measurement in a household survey. Risks have an impact on future health, but, since the time element is missing from a cross-sectional survey, the CHS was restricted to examining the distribution of known hazards. Health status - only partially the outcome of earlier risk exposure - has positive and negative dimensions, which were measured for both the physical and emotional facets of health. Because morbidity statistics are most commonly derived from health records which report a single episode rather than entire histories, it is impossible to assess the personal burden of ill health, especially for those problems which do not reach the health care system. The CHS rectifies the situation by identifying health problems and their consequences for each individual in the sample, and using these to evaluate the impact of ill health.

    In 1991, the National Task Force on Health Information cited a number of issues and problems with the health information system. To respond to these issues, the Canadian Institute for Health Information (CIHI), Statistics Canada and Health Canada joined forces to create a Health Information Roadmap. From this mandate, the Canadian Community Health Survey (CCHS) was conceived. The CCHS is a cross-sectional survey that collects information related to health status, health care utilization and health determinants for the Canadian population. The survey is offered in both official languages. It relies upon a large sample of respondents and is designed to provide reliable estimates at the health region level every 2 years. The CCHS has the following objectives: * Support health surveillance programs by providing health data at the national, provincial and intra-provincial levels; * Provide a single data source for health research on small populations and rare characteristics; * Timely release of information easily accessible to a diverse community of users; * Create a flexible survey instrument that includes a rapid response option to address emerging issues related to the health of the population. The CCHS produces an annual microdata file and a file combining two years of data. The CCHS collection years can also be combined by users to examine populations or rare characteristics. The primary use of the CCHS data is for health surveillance and population health research. Federal and provincial departments of health and human resources, social service agencies, and other types of government agencies use the information collected from respondents to monitor, plan, implement and evaluate programs to improve the health of Canadians. Researchers from various fields use the information to conduct research to improve health. Non-profit health organizations and the media use the CCHS results to raise awareness about health, an issue of concern to all Canadians. The survey began collecting data in 2001 and was repeated every two years until 2005. Starting in 2007, data for the Canadian Community Health Survey (CCHS) were collected annually instead of every two years. While a sample of approximately 130,000 respondents were interviewed during the reference periods of 2001, 2003 and 2005, the sample size was changed to 65,000 respondents each year starting in 2007. In 2012, CCHS began work on a major redesign project that was completed and implemented for the 2015 cycle. The objectives of the redesign were to review the sampling methodology, adopt a new sample frame, modernize the content and review the target population. Consultations were held with federal, provincial and territorial share partners, health region authorities and academics. As a result of the redesign, the 2015 CCHS has a new collection strategy, is drawing the sample from two different frames and has undergone major content revisions. With all these factors taken together, caution should be taken when comparing data from previous cycles to data released for the 2015 cycle onwards.

    Background: Stroke is a devastating disease, but it is treatable with alteplase or tissue plasminogen activator (tPA). The effectiveness of tPA is highly time-dependent, meaning rapid treatment is critical. Fast treatment with tPA has been reported in many urban hospitals, but hospitals in rural locations struggle to reduce treatment times. This qualitative study examines current thrombolysis processes in one urban and two rural hospitals in Nova Scotia, Canada, by mapping and comparing the treatment process in these settings for acute ischemic stroke (AIS) patients, and by analyzing the healthcare professionals views on various treatment topics.Methods: Structured interviews were conducted with healthcare professionals involved in stroke treatment across the three sites. The interviews focused on the various activities in the thrombolysis treatment at each site. Additionally, participants were asked about the following 10 topics: comfort treating acute ischemic stroke patients; perceptions about tPA; appropriate tPA treatment window; stroke patient priority; tPA availability; patient consent; urban-rural treatment differences; efficiency of their treatment process; treatment delays; and suggested process improvements. Results were analyzed using the Framework Method, as well as through the development of process maps.Results: Twenty three healthcare professionals were interviewed at 2 rural hospitals and 1 urban hospital. Acute ischemic stroke patients are triaged as the highest or urgent priority at each included site. Physicians are more hesitant to treat with tPA in rural settings. A total of 11 urban-rural treatment differences were noted by the rural sites. Additionally, 11 patient-related and 29 system treatment delays were described. A process map was developed for each site, representing the arrival by ambulance and by private vehicle pathways.Conclusions: Guidelines and clear protocols are critical in reducing treatment times and ensuring consistent access to treatment. The majority of treatment delays encountered are system delays, which can be appropriately planned for to reduce delays within the care pathway. There is a general consensus that there is an urban-rural treatment gap for acute ischemic stroke patients in Nova Scotia, and that continuing education is key in rural hospitals to improve Emergency Department (ED) physician comfort with treating patients with tPA.
