The NCHS National Post-acute and Long-term Care Study (NPALS) collects data on long-term care every two years for all 50 states and the District of Columbia to monitor the diverse post-acute and long-term care fields. The 2020 survey provided an opportunity to collect COVID-19-related data for residential care communities and adult day services centers, important long-term care settings. These data are not available from other data systems. These data are related to experiences of COVID-19 from January 2020 through mid-July 2021, including the number of COVID-19 cases, hospitalizations, and deaths among users and staff, practices taken to reduce COVID-19 exposure and transmission, and personal protective equipment (PPE) shortages.

The COVID-19 Claims Reimbursement to Health Care Providers and Facilities for Testing, Treatment, and Vaccine Administration for the Uninsured Program provides reimbursements on a rolling basis directly to eligible health care entities for claims that are attributed to the testing, treatment, and or vaccine administration of COVID-19 for uninsured individuals. The program funding information is as follow:

TESTING The American Rescue Plan Act (ARP) which provided $4.8 billion to reimburse providers for testing the uninsured; the Families First Coronavirus Response Act (FFCRA) Relief Fund, which includes funds received from the Public Health and Social Services Emergency Fund, as appropriated in the FFCRCA (P.L. 116-127) and the Paycheck Protection Program and Health Care Enhancement Act (P.L. 116-139) (PPPHCEA), which each appropriated $1 billion to reimburse health care entities for conducting COVID-19 testing for the uninsured.

TREATMENT & VACCINATION The Provider Relief Fund, which includes funds received from the Public Health and Social Services Emergency Fund, as appropriated in the Coronavirus Aid, Relief, and Economic Security (CARES) Act (P.L. 116-136), provided $100 billion in relief funds. The PPPHCEA appropriated an additional $75 billion in relief funds and the Coronavirus Response and Relief Supplemental Appropriations (CRRSA) Act (P.L. 116-260) appropriated another $3 billion. Within the Provider Relief Fund, a portion of the funding from these sources will be used to support healthcare-related expenses attributable to the treatment of uninsured individuals with COVID-19 and vaccination of uninsured individuals. This dataset represents the list of health care entities who have agreed to the Terms and Conditions and received claims reimbursement for COVID-19 testing of uninsured individuals, vaccine administration and treatment for uninsured individuals with a COVID-19 diagnosis.

After over two years of public reporting, the State Profile Report will no longer be produced and distributed after February 2023. The final release was on February 23, 2023. We want to thank everyone who contributed to the design, production, and review of this report and we hope that it provided insight into the data trends throughout the COVID-19 pandemic. The State Profile Report (SPR) is generated by the Data Strategy and Execution Workgroup in the Joint Coordination Cell, in collaboration with the White House. It is managed by an interagency team with representatives from multiple agencies and offices (including the United States Department of Health and Human Services (HHS), the Centers for Disease Control and Prevention, the HHS Assistant Secretary for Preparedness and Response, and the Indian Health Service). The SPR provides easily interpretable information on key indicators for each state, down to the county level.

It is a weekly snapshot in time that:

• Focuses on recent outcomes in the last seven days and changes relative to the month prior
• Provides additional contextual information at the county level for each state, and includes national level information
• Supports rapid visual interpretation of results with color thresholds

The National Hospital Care Survey (NHCS) collects data on patient care in hospital-based settings to describe patterns of health care delivery and utilization in the United States. Settings currently include inpatient and emergency departments (ED). Additionally, the NHCS contributes data that may inform public health emergencies as the survey is designed to capture emerging diseases and viruses that require hospitalizations, including COVID-19 encounters. The 2020 - 2023 NHCS are not yet fully operational so it is important to note that these data are not nationally representative. The data are from 26 hospitals submitting inpatient and 26 hospitals submitting ED Uniform Bill (UB)-04 administrative claims from March 18, 2020-December 26, 2023. Even though the data are not nationally representative, they can provide insight on the impact of COVID-19 on various types of hospitals throughout the country. This information is not available in other hospital reporting systems. The NHCS data from these hospitals can show results by a combination of indicators related to COVID-19, such as length of inpatient stay, in-hospital mortality, comorbidities, and intubation or ventilator use. NHCS data allow for reporting on patient conditions and treatments within the hospital over time.

Provisional counts of deaths by the month the deaths occurred, by age group and HHS region, for select underlying causes of death for 2019-2020. The dataset also includes monthly provisional counts of death for COVID-19, coded to ICD-10 code U07.1 as an underlying or multiple cause of death.

According to the U.S. Department of Health & Human Services (HHS), home health service is health care or supportive care provided in the patient's home by healthcare professionals (often referred to as home health care or formal care). The data presented here is current as of 2012. The data was derived from HHS (http://www.hhs.gov/).

Due to the change in the survey instrument regarding intention to vaccinate, our estimates for “hesitant or unsure” or “hesitant” derived from April 14-26, 2021, are not directly comparable with prior Household Pulse Survey data and should not be used to examine trends in hesitancy. To support state and local communication and outreach efforts, ASPE developed state, county, and sub-state level predictions of hesitancy rates (https://aspe.hhs.gov/pdf-report/vaccine-hesitancy) using the most recently available federal survey data. We estimate hesitancy rates at the state level using the U.S. Census Bureau’s Household Pulse Survey (HPS) (https://www.census.gov/programs-surveys/household-pulse-survey.html) data and utilize the estimated values to predict hesitancy rates at the Public Use Microdata Areas (PUMA) level using the Census Bureau’s 2019 American Community Survey (ACS) 1-year Public Use Microdata Sample (PUMS)(https://www.census.gov/programs-surveys/acs/microdata.html). To create county-level estimates, we used a PUMA-to-county crosswalk from the Missouri Census Data Center(https://mcdc.missouri.edu/applications/geocorr2014.html). PUMAs spanning multiple counties had their estimates apportioned across those counties based on overall 2010 Census populations. The HPS is nationally representative and includes information on U.S. residents’ intentions to receive the COVID-19 vaccine when available, as well as other sociodemographic and geographic (state, region and metropolitan statistical areas) information. The ACS is a nationally representative survey, and it provides key sociodemographic and geographic (state, region, PUMAs, county) information. We utilized data for the survey collection period May 26, 2021 – June 7, 2021, which the HPS refers to as Week 31.. County data on race and Hispanic origin is available for counties with more than 100 COVID-19 deaths. Deaths are cumulative from the week ending January 4, 2020 to the most recent reporting week, and based on county of occurrence. Data is provisional.

During the COVID-19 pandemic, the Chicago Department of Public Health (CDPH) required EMS Region XI (Chicago area) hospitals to report hospital capacity and patient impact metrics related to COVID-19 to CDPH through the statewide EMResource system. This requirement has been lifted as of May 9, 2023, in alignment with the expiration of the national and statewide COVID-19 public health emergency declarations on May 11, 2023. However, all hospitals will still be required by the U.S. Department of Health and Human Services (HHS) to report COVID-19 hospital capacity and utilization metrics into the HHS Protect system through the CDC’s National Healthcare Safety Network until April 30, 2024. Facility-level data from the HHS Protect system can be found at healthdata.gov. Until May 9, 2023, all Chicago (EMS Region XI) hospitals (n=28) were required to report bed and ventilator capacity, availability, and occupancy to the Chicago Department of Public Health (CDPH) daily. A list of reporting hospitals is included below. All data represent hospital status as of 11:59 pm for that calendar day. Counts include Chicago residents and non-residents. ICU bed counts include both adult and pediatric ICU beds. Neonatal ICU beds are not included. Capacity refers to all staffed adult and pediatric ICU beds. Availability refers to all available/vacant adult and pediatric ICU beds. Hospitals began reporting COVID-19 confirmed and suspected (PUI) cases in ICU on 03/19/2020. Hospitals began reporting ICU surge capacity as part of total capacity on 5/18/2020. Acute non-ICU bed counts include burn unit, emergency department, medical/surgery (ward), other, pediatrics (pediatric ward) and psychiatry beds. Burn beds include those approved by the American Burn Association or self-designated. Capacity refers to all staffed acute non-ICU beds. An additional 500 acute/non-ICU beds were added at the McCormick Place Treatment Facility on 4/15/2020. These beds are not included in the total capacity count. The McCormick Place Treatment Facility closed on 05/08/2020. Availability refers to all available/vacant acute non-ICU beds. Hospitals began reporting COVID-19 confirmed and suspected (PUI) cases in acute non-ICU beds on 04/03/2020. Ventilator counts prior to 04/24/2020 include all full-functioning mechanical ventilators, with ventilators with bilevel positive airway pressure (BiPAP), anesthesia machines, and portable/transport ventilators counted as surge. Beginning 04/24/2020, ventilator counts include all full-functioning mechanical ventilators, BiPAP, anesthesia machines and portable/transport ventilators. Ventilators are counted regardless of ability to staff. Hospitals began reporting COVID-19 confirmed and suspected (PUI) cases on ventilators on 03/19/2020. CDPH has access to additional ventilators from the EAMC (Emergency Asset Management Center) cache. These ventilators are included in the total capacity count. Provisional count of deaths involving coronavirus disease 2019 (COVID-19) in the United States by week of death and by hospital referral region (HRR). HRR is determined by county of occurrence. Weekly weighted counts of deaths from all causes and due to COVID-19 are provided by HRR overall and for decedents 65 years and older. The weighted counts by HRRs are based on published methods for aggregating county-level data to HRRs.
