The population of India is divided into several groups based on social, educational, and financial statuses. The formation of these groups is a result of the historical social structure of the country. Between 2019 and 2021, Other Backward Class (OBC) constituted the largest part of Indian households accounting for about ** percent. On the other hand, Schedule Tribes formed about *** percent of households. How prosperous is India’s caste-based society? India suffers from extreme social and economic inequality. The combined share of Schedule Tribe and Schedule Caste in the affluent population of India was less than ** percent. Contrary to this, economically and socially stronger groups constituted the major part of the affluent population. Hence, indicating a strong relationship between caste and prosperity. India’s thoughts on caste-based reservation The constitution of India provides reservations to the weaker sections of the society for their upliftment and growth. However, the need for reservation has increased with time, making the whole situation even more complicated. People are divided over the existence of a system that provides preference to certain castes or sects. In a survey conducted in 2016 about providing employment reservation to young adults of Schedule Caste and Schedule Tribe, many people expressed opposition. More than ** percent of opposition came from upper Hindu caste. Minimum opposition was observed from the people belonging to Schedule Tribe and Schedule Caste.
The 44th round started from July 1988. The survey period of this round was July 1988 to June 1989. This round has been devoted to mainly three enquiries. First and foremost, there has been an enquiry on the living condition of the tribal population. Of the other two, one is concerned with the housing condition of the general population and the other is a survey on current building construction activity. For the purpose of this enquiry, “tribal population” mean the members of the Scheduled Tribes declared under the Article 342 of the Constitution of India. They are known to be the descendants of the earliest inhabitants of India (hence called “Adivasis”). At present, in most parts of India, they form one of the economically weakest sections of the society. So far there has not been any systematic study of their living conditions covering the whole country. Whatever data are available are derived from the decennial censuses, apart from some micro studies carried out by social anthropologists. In the NSS the tribal population has always been covered as part of the general population. In NSS 32nd and 33rd rounds special surveys had been carried out through an integrated schedule (schedule 16.4) in the North-Eastern region. The survey was conducted in the rural areas of the following States:- 32nd round : Arunachal Pradesh, Assam (N. Cachar and Karbi Anglong districts only), Manipur, Meghalaya and Tripura; 33rd round: In addition to the above States, Mizoram also. Even though this covered many aspects specially related to the life of the people of this region (who are mostly tribals), no such survey has so far been undertaken about the life of the tribals living in the main tribal belt stretching from West Bengal through Bihar, Orissa, Madhya Pradesh to Gujarat and Rajasthan. The scope of the enquiry is to understand the living condition of the tribals living in the main tribal belt stretching from West Bengal through Bihar, Orissa, Madhya Pradesh to Gujarat and Rajasthan.) The object of the enquiry in the this round is to throw light on as many aspects as possible of the tribal population of this country. This relates to aspects of their “level of the living” including demographic and activity particulars, family expenditure etc. as well as to their entrepreneurial activities.
The survey covered the whole of Indian Union except Ladakh and Kargil districts of Jammu and Kashmir state. The rural areas of Nagaland, so far outside NSS coverage up to the 43rd round, have also been brought in this round.

The sample design is stratified two-stage with the census village as the first stage unit in the rural sector and UFS block as the first stage unit in the urban sector. The second stage units are households.
The sample design in the rural sector has been decided with a view to providing good estimates for the tribal enquiry. Except in the north-eastern region, the tribal population is concentrated in some districts within the states having considerable tribal population and even in those districts they are found to be unevenly distributed geographically. Therefore special stratification and selection procedures have been adopted not only to net sufficient number of tribal households in the sample but also to improve the design in general for the tribal enquiry.
While the rural design is oriented towards the tribal enquiry, the urban design is oriented towards the enquiry on construction. As building construction activity is found to be concentrated in some areas in the urban sector, attempts have been made in urban design to demarcate such areas in larger towns as separate strata. Detailed description of the rural and urban sample designs are as follows:

Sampling frame of villages: The list of 1981 census villages constitute the sampling frame for selection of villages in most districts. However in Assam (where '81 census was not done) and a few districts of some other states (where the available lists of villages were not satisfactory), 1971 census village lists have been used as frame.
Stratification :
In Haryana, Jammu & Kashmir, Punjab, Chandigarh, Delhi, Goa, Daman & Diu and Pondicherry where there are practically no tribal population, the strata used in NSS 43rd round were retained. In Meghalaya, Mizoram, Nagaland, Arunachal Pradesh, Sikkim, Dadra and Nagar Haveli and Lakshadweep also the strata of 43rd round were retained because of the high percentage of ST population in these States/U.T.'s. (The strata of 43rd round have been retained in the case of Sikkim as the distribution of tribal population is more or less uniform over all the districts). In the remaining states fresh stratification was carried out as described below.
In these states all districts accounting for the bulk of the state's tribal population were selected for formation of strata with concentration of tribal population. Besides these districts, tribal concentration strata have been demarcated also in some other districts with relatively small tribal population in order to ensure coverage of as many different ethnic groups as possible.
Within each district so identified for formation of tribal concentration strata, the tehsils with relatively high concentration of tribal population, together constituted one stratum. These tehsils were selected in such a way that together they accounted for the bulk (70% or more) of the district tribal population and the proportion of tribal to total population in this stratum was significantly greater than that of the district as a whole. The strata so formed were not always geographically contiguous. These tribal concentration strata are called STRATUM TYPE -1. Further, all the strata of Meghalaya, Mizoram, Nagaland, Arunachal Pradesh, Dadra & Nagar Haveli, Lakshadweep and Sikkim are also considered as stratum type-1. All the remaining strata in the rural sector (in any State/U.T.) are called stratum type -2.

The literacy rate of the total population in the country was about 73 percent in 2011, in comparison to about 66 percent among the scheduled caste population. In India, scheduled caste and scheduled tribe and other backward class are officially recognized by the constitution as groups of disadvantaged indigenous people. They are the primary beneficiaries of reservation policies under the constitution.

In pursuance of the recommendations made by the Governing Council (G.C.) of the National Sample Survey Organization (NSSO) in its 44th meeting held on 16 January 1987 to undertake a comprehensive survey on the socio-economic conditions of the tribal people in the 44th round (July 1988 - June 1989) of NSS, various schedules of enquiries on the subject were drawn up and tested in the field through a try-out survey. The schedules were discussed in details in the meetings of the Working Groups (W.G) set up by the G.C., NSSO for the NSS 44th round. In the light of the experiences gained through the try-out survey, the schedules of enquiry were finalized by the W.G. and subsequently approved by the G.C. of the NSSO in its 45th meeting held on 29 December 1987.
With a view to studying the problems of land alienation faced by the tribal population due to in-migration of non-tribals in the tribal areas and also to assess the differences in the socio-economic standard of living between the tribals and the non-tribals, a schedule of enquiry (schedule type 29.3) was designed and collected information from the non-tribal households residing in the tribal areas.
The Survey measures the disparity between the tribal and non-tribal households in respect of certain key characteristics and also the extent of displacement of the tribal population from the tribal belts due in-migration of non-tribals. This schedule relate to in-migration, economic activity, assets and liabilities along with the details about the extent and manner of acquisition and disposal of land by the non-tribals.

These special sample villages were selected only from the tribal concentration strata (stratum type 1 ) of the 16 States and the U.T. OF Andaman & Nicobar Islands 1) of 16 States and the U.T. of Andaman & Nicobar Islands. There were no special sample villages in the remaining States/U.T.'s. The special samples were called sample type-1 and the general samples, sample type-2.
Schedule type 29.3 was canvassed in the general and special sample villages to tribal strata only. Four households were selected from the frame of non-scheduled tribe households in each of the villages of stratum type 1. In large special sample villages, the distribution of sample households was 2 each from area type 1 and area type 2, Schedule 29.3 was not be canvassed in the urban sector.

However, the major thrust in collecting information on living conditions of tribals will, obviously, be at the household level. There are various aspects of tribal life which need to be studied. From the point of view of analysis it would have, probably, been ideal to collect all the necessary information from the same set of sample households in an integrated manner. But as the factors are numerous, any attempt to accommodate all the items of information in a single schedule of enquiry will make the schedule unwieldy. It was, thus, decided to frame two schedules of enquiries for collecting information from the tribal households - one mainly on socio-economic aspects relating to the level of living of tribals (schedule type - 29.1) and the other on economic and enterprise aspects of the tribal households (schedule type 29.2).

In the special sample villages requiring hamlet group formation, the hamlet group having maximum number of tribal population will be designated as area type 1 and the other hamlet group selected at random from the remaining hamlet groups will be called area type 2. From the area type 1, 4 tribal households will be selected, while only 2 tribal households will be sampled from area type 2 for he schedule - 29.1.

Poverty and empowerment impacts of the Bihar Rural Livelihoods Project: Evidence from a Mixed-Methods Cluster-Randomized Trial Jeevika is a World Bank assisted project focussed (now under the umbrella of the NRLM) on building networks of women's self-help credit and savings groups,and then using them as a base of other "vertical" interventions. This houshold and village survey data was collected over two rounds to conduct an impact evaluation of Phase 2 of the project with random assignment of the project over a two year period. Collaboration: World Bank Social Observatory team with Government of Bihar. Evaluation design, methods and implementation In order to evaluate the impacts of Jeevika, 180 panchayats were randomly selected from within 16 blocks in seven districts where scale-up of the project was planned but had not yet occurred. Some of these blocks were in districts relatively far from Patna, which had not yet been entered by the project (Madhepura, Saharsa, Supaul), while others were within the larger districts within which Jeevika was already operating (Gaya, Nalanda, Madhubani, Muzaffarpur). The project had already entered these districts in Phase 1, but had not yet expanded to all blocks due to (project) capacity constraints. Within each of the study villages, hamlets (tolas) in which the majority of the population belonged to a scheduled caste or scheduled tribe were identified. This was the same procedure as used by Jeevika to identify the target population (of poor women) for mobilization into the project. Tolas were identified through a focus group discussion held in each village, along with the population of target castes (SC/STs) within each. In Bihar, tola boundaries are easily distinguishable. Field teams would enter the tola at a random point, determine the skip pattern based on the population size and target sample size, and select households through a random walk. Survey staff aimed to include 70% SC/ST households, and 30% households from other castes in each village, in order to ensure variation in socio-economic status within the sample. If the households in selected tolas included fewer SC/ST households than this, households from nearby non-SC/ST majority tolas were also included in the sample. Interviews for the quantitative study were conducted using a structured paper survey form. Baseline and follow up surveys included detailed questions on debt, asset holdings, consumption expenditures, livelihood activities, and women’s mobility, role in household decisions, and aspirations. In addition, in each village, a focus group discussion was conducted, through which data were collected on village level attributes such as local sources of credit, interest rates from each source, local wage rates, and the presence of or distance to markets and other institutions and amenities. Respondents were not compensated for their time. If a respondent was unavailable during initial field visit, the supervisor recorded contact details and returned with interviewers at a later date. As long as the survey team was in that district, repeat visits were undertaken, keeping attrition to a minimum. If a household could not be re-surveyed at endline, it was replaced with another household in the same village. Short re-surveys containing a subset of questions from the main survey were conducted by supervisors for 10% of the sample. Staff from the project also conducted occasional visits after the survey was completed in a village to confirm that all modules had been covered by survey staff. Data was entered in duplicate using CSPro and any discrepancies were corrected based on the paper form. Following the baseline survey, panchayats were stratified on the 16 administrative blocks in the sample and the panchayat-level mean of outstanding high cost (monthly interest rate of 4% or higher) debt held by households at baseline. They were then randomly assigned to an early rollout group or a late rollout group using the random number generator within the Stata statistical analysis software package. The baseline survey was administered to 8988 households across 333 villages in 179 panchayats. The target number of households per panchayat was 50, but there was some variation around this in reality. The lowest number of households in a given panchayat was 49 (9 panchayats), and the largest number was 53 households (3 panchayats). To ensure that control panchayats were not entered by the project, Jeevika held a quarterly ""evaluation panchayat"" meeting, which block project managers of the 16 blocks were required to attend. Sample survey data [ssd]
The sample design is stratified two-stage with the census village as the first stage unit in the rural sector and UFS block as the first stage unit in the urban sector. The second stage units are households.
The sample design in the rural sector has been decided with a view to providing good estimates for the tribal enquiry. Except in the north-eastern region, the tribal population is concentrated in some districts within the states having considerable tribal population and even in those districts they are found to be unevenly distributed geographically. Therefore special stratification and selection procedures have been adopted not only to net sufficient number of tribal households in the sample but also to improve the design in general for the tribal enquiry.
While the rural design is oriented towards the tribal enquiry, the urban design is oriented towards the enquiry on construction. As building construction activity is found to be concentrated in some areas in the urban sector, attempts have been made in urban design to demarcate such areas in larger towns as separate strata. Detailed description of the rural and urban sample designs are as follows:
SAMPLE DESIGN : RURAL
Sampling frame of villages: The list of 1981 census villages constitute the sampling frame for selection of villages in most districts. However in Assam (where '81 census was not done) and a few districts of some other states (where the available lists of villages were not satisfactory), 1971 census village lists have been used as frame.
Stratification :
In Haryana, Jammu & Kashmir, Punjab, Chandigarh, Delhi, Goa, Daman & Diu and Pondicherry where there are practically no tribal population, the strata used in NSS 43rd round were retained. In Meghalaya, Mizoram, Nagaland, Arunachal Pradesh, Sikkim, Dadra and Nagar Haveli and Lakshadweep also the strata of 43rd round were retained because of the high percentage of ST population in these States/U.T.'s. (The strata of 43rd round have been retained in the case of Sikkim as the distribution of tribal population is more or less uniform over all the districts). In the remaining states fresh stratification was carried out as described below.
In these states all districts accounting for the bulk of the state's tribal population were selected for formation of strata with concentration of tribal population. Besides these districts, tribal concentration strata have been demarcated also in some other districts with relatively small tribal population in order to ensure coverage of as many different ethnic groups as possible.
Within each district so identified for formation of tribal concentration strata, the tehsils with relatively high concentration of tribal population, together constituted one stratum. These tehsils were selected in such a way that together they accounted for the bulk (70% or more) of the district tribal population and the proportion of tribal to total population in this stratum was significantly greater than that of the district as a whole. The strata so formed were not always geographically contiguous. These tribal concentration strata are called STRATUM TYPE -1. Further, all the strata of Meghalaya, Mizoram, Nagaland, Arunachal Pradesh, Dadra & Nagar Haveli, Lakshadweep and Sikkim are also considered as stratum type-1. All the remaining strata in the rural sector (in any State/U.T.) are called stratum type -2.

The National Family Health Survey 2019-21 (NFHS-5), the fifth in the NFHS series, provides information on population, health, and nutrition for India, each state/union territory (UT), and for 707 districts.
The primary objective of the 2019-21 round of National Family Health Surveys is to provide essential data on health and family welfare, as well as data on emerging issues in these areas, such as levels of fertility, infant and child mortality, maternal and child health, and other health and family welfare indicators by background characteristics at the national and state levels. Similar to NFHS-4, NFHS-5 also provides information on several emerging issues including perinatal mortality, high-risk sexual behaviour, safe injections, tuberculosis, noncommunicable diseases, and the use of emergency contraception.
The information collected through NFHS-5 is intended to assist policymakers and programme managers in setting benchmarks and examining progress over time in India’s health sector. Besides providing evidence on the effectiveness of ongoing programmes, NFHS-5 data will help to identify the need for new programmes in specific health areas.
The clinical, anthropometric, and biochemical (CAB) component of NFHS-5 is designed to provide vital estimates of the prevalence of malnutrition, anaemia, hypertension, high blood glucose levels, and waist and hip circumference, Vitamin D3, HbA1c, and malaria parasites through a series of biomarker tests and measurements.

The survey covered all de jure household members (usual residents), all women aged 15-49, all men age 15-54, and all children aged 0-5 resident in the household.

A uniform sample design, which is representative at the national, state/union territory, and district level, was adopted in each round of the survey. Each district is stratified into urban and rural areas. Each rural stratum is sub-stratified into smaller substrata which are created considering the village population and the percentage of the population belonging to scheduled castes and scheduled tribes (SC/ST). Within each explicit rural sampling stratum, a sample of villages was selected as Primary Sampling Units (PSUs); before the PSU selection, PSUs were sorted according to the literacy rate of women age 6+ years. Within each urban sampling stratum, a sample of Census Enumeration Blocks (CEBs) was selected as PSUs. Before the PSU selection, PSUs were sorted according to the percentage of SC/ST population. In the second stage of selection, a fixed number of 22 households per cluster was selected with an equal probability systematic selection from a newly created list of households in the selected PSUs. The list of households was created as a result of the mapping and household listing operation conducted in each selected PSU before the household selection in the second stage. In all, 30,456 Primary Sampling Units (PSUs) were selected across the country in NFHS-5 drawn from 707 districts as on March 31st 2017, of which fieldwork was completed in 30,198 PSUs.

Computer Assisted Personal Interview [capi]
Four survey schedules/questionnaires: Household, Woman, Man, and Biomarker were canvassed in 18 local languages using Computer Assisted Personal Interviewing (CAPI).
Electronic data collected in the 2019-21 National Family Health Survey were received on a daily basis via the SyncCloud system at the International Institute for Population Sciences, where the data were stored on a password-protected computer. Secondary editing of the data, which required resolution of computer-identified inconsistencies and coding of open-ended questions, was conducted in the field by the Field Agencies and at the Field Agencies central office, and IIPS checked the secondary edits before the dataset was finalized.
Field-check tables were produced by IIPS and the Field Agencies on a regular basis to identify certain types of errors that might have occurred in eliciting information and recording question responses. Information from the field-check tables on the performance of each fieldwork team and individual investigator was promptly shared with the Field Agencies during the fieldwork so that the performance of the teams could be improved, if required.
A total of 664,972 households were selected for the sample, of which 653,144 were occupied. Among the occupied households, 636,699 were successfully interviewed, for a response rate of 98 percent.
In the interviewed households, 747,176 eligible women age 15-49 were identified for individual women’s interviews. Interviews were completed with 724,115 women, for a response rate of 97 percent. In all, there were 111,179 eligible men age 15-54 in households selected for the state module. Interviews were completed with 101,839 men, for a response rate of 92 percent.

The second National Family Health Survey (NFHS-2), conducted in 1998-99, provides information on fertility, mortality, family planning, and important aspects of nutrition, health, and health care. The International Institute for Population Sciences (IIPS) coordinated the survey, which collected information from a nationally representative sample of more than 90,000 ever-married women age 15-49. The NFHS-2 sample covers 99 percent of India's population living in all 26 states. This report is based on the survey data for 25 of the 26 states, however, since data collection in Tripura was delayed due to local problems in the state. IIPS also coordinated the first National Family Health Survey (NFHS-1) in 1992-93. Most of the types of information collected in NFHS-2 were also collected in the earlier survey, making it possible to identify trends over the intervening period of six and one-half years. In addition, the NFHS-2 questionnaire covered a number of new or expanded topics with important policy implications, such as reproductive health, women's autonomy, domestic violence, women's nutrition, anaemia, and salt iodization. The NFHS-2 survey was carried out in two phases. Ten states were surveyed in the first phase which began in November 1998 and the remaining states (except Tripura) were surveyed in the second phase which began in March 1999. The field staff collected information from 91,196 households in these 25 states and interviewed 89,199 eligible women in these households. In addition, the survey collected information on 32,393 children born in the three years preceding the survey. One health investigator on each survey team measured the height and weight of eligible women and children and took blood samples to assess the prevalence of anaemia. SUMMARY OF FINDINGS POPULATION CHARACTERISTICS Three-quarters (73 percent) of the population lives in rural areas. The age distribution is typical of populations that have recently experienced a fertility decline, with relatively low proportions in the younger and older age groups. Thirty-six percent of the population is below age 15, and 5 percent is age 65 and above. The sex ratio is 957 females for every 1,000 males in rural areas but only 928 females for every 1,000 males in urban areas, suggesting that more men than women have migrated to urban areas. The survey provides a variety of demographic and socioeconomic background information. In the country as a whole, 82 percent of household heads are Hindu, 12 percent are Muslim, 3 percent are Christian, and 2 percent are Sikh. Muslims live disproportionately in urban areas, where they comprise 15 percent of household heads. Nineteen percent of household heads belong to scheduled castes, 9 percent belong to scheduled tribes, and 32 percent belong to other backward classes (OBCs). Two-fifths of household heads do not belong to any of these groups. Questions about housing conditions and the standard of living of households indicate some improvements since the time of NFHS-1. Sixty percent of households in India now have electricity and 39 percent have piped drinking water compared with 51 percent and 33 percent, respectively, at the time of NFHS-1. Sixty-four percent of households have no toilet facility compared with 70 percent at the time of NFHS-1. About three-fourths (75 percent) of males and half (51 percent) of females age six and above are literate, an increase of 6-8 percentage points from literacy rates at the time of NFHS-1. The percentage of illiterate males varies from 6-7 percent in Mizoram and Kerala to 37 percent in Bihar and the percentage of illiterate females varies from 11 percent in Mizoram and 15 percent in Kerala to 65 percent in Bihar. Seventy-nine percent of children age 6-14 are attending school, up from 68 percent in NFHS-1. The proportion of children attending school has increased for all ages, particularly for girls, but girls continue to lag behind boys in school attendance. Moreover, the disparity in school attendance by sex grows with increasing age of children. At age 6-10, 85 percent of boys attend school compared with 78 percent of girls. By age 15-17, 58 percent of boys attend school compared with 40 percent of girls. The percentage of girls 6-17 attending school varies from 51 percent in Bihar and 56 percent in Rajasthan to over 90 percent in Himachal Pradesh and Kerala. Women in India tend to marry at an early age. Thirty-four percent of women age 15-19 are already married including 4 percent who are married but gauna has yet to be performed. These proportions are even higher in the rural areas. Older women are more likely than younger women to have married at an early age: 39 percent of women currently age 45-49 married before age 15 compared with 14 percent of women currently age 15-19. Although this indicates that the proportion of women who marry young is declining rapidly, half the women even in the age group 20-24 have married before reaching the legal minimum age of 18 years. On average, women are five years younger than the men they marry. The median age at marriage varies from about 15 years in Madhya Pradesh, Bihar, Uttar Pradesh, Rajasthan, and Andhra Pradesh to 23 years in Goa. As part of an increasing emphasis on gender issues, NFHS-2 asked women about their participation in household decisionmaking. In India, 91 percent of women are involved in decision-making on at least one of four selected topics. A much lower proportion (52 percent), however, are involved in making decisions about their own health care. There are large variations among states in India with regard to women's involvement in household decisionmaking. More than three out of four women are involved in decisions about their own health care in Himachal Pradesh, Meghalaya, and Punjab compared with about two out of five or less in Madhya Pradesh, Orissa, and Rajasthan. Thirty-nine percent of women do work other than housework, and more than two-thirds of these women work for cash. Only 41 percent of women who earn cash can decide independently how to spend the money that they earn. Forty-three percent of working women report that their earnings constitute at least half of total family earnings, including 18 percent who report that the family is entirely dependent on their earnings. Women's work-participation rates vary from 9 percent in Punjab and 13 percent in Haryana to 60-70 percent in Manipur, Nagaland, and Arunachal Pradesh. FERTILITY AND FAMILY PLANNING Fertility continues to decline in India. At current fertility levels, women will have an average of 2.9 children each throughout their childbearing years. The total fertility rate (TFR) is down from 3.4 children per woman at the time of NFHS-1, but is still well above the replacement level of just over two children per woman. There are large variations in fertility among the states in India. Goa and Kerala have attained below replacement level fertility and Karnataka, Himachal Pradesh, Tamil Nadu, and Punjab are at or close to replacement level fertility. By contrast, fertility is 3.3 or more children per woman in Meghalaya, Uttar Pradesh, Rajasthan, Nagaland, Bihar, and Madhya Pradesh. More than one-third to less than half of all births in these latter states are fourth or higher-order births compared with 7-9 percent of births in Kerala, Goa, and Tamil Nadu. Efforts to encourage the trend towards lower fertility might usefully focus on groups within the population that have higher fertility than average. In India, rural women and women from scheduled tribes and scheduled castes have somewhat higher fertility than other women, but fertility is particularly high for illiterate women, poor women, and Muslim women. Another striking feature is the high level of childbearing among young women. More than half of women age 20-49 had their first birth before reaching age 20, and women age 15-19 account for almost one-fifth of total fertility. Studies in India and elsewhere have shown that health and mortality risks increase when women give birth at such young ages?both for the women themselves and for their children. Family planning programmes focusing on women in this age group could make a significant impact on maternal and child health and help to reduce fertility. INFANT AND CHILD MORTALITY NFHS-2 provides estimates of infant and child mortality and examines factors associated with the survival of young children. During the five years preceding the survey, the infant mortality rate was 68 deaths at age 0-11 months per 1,000 live births, substantially lower than 79 per 1,000 in the five years preceding the NFHS-1 survey. The child mortality rate, 29 deaths at age 1-4 years per 1,000 children reaching age one, also declined from the corresponding rate of 33 per 1,000 in NFHS-1. Ninety-five children out of 1,000 born do not live to age five years. Expressed differently, 1 in 15 children die in the first year of life, and 1 in 11 die before reaching age five. Child-survival programmes might usefully focus on specific groups of children with particularly high infant and child mortality rates, such as children who live in rural areas, children whose mothers are illiterate, children belonging to scheduled castes or scheduled tribes, and children from poor households. Infant mortality rates are more than two and one-half times as high for women who did not receive any of the recommended types of maternity related medical care than for mothers who did receive all recommended types of care. HEALTH, HEALTH CARE, AND NUTRITION Promotion of maternal and child health has been one of the most important components of the Family Welfare Programme of the Government of India. One goal is for each pregnant woman to receive at least three antenatal check-ups plus two tetanus toxoid injections and a full course of iron and folic acid supplementation. The National Family Health Survey (NFHS) was carried out as the principal activity of a collaborative project to strengthen the research capabilities of the Population Reasearch Centres (PRCs) in India, initiated by the Ministry of Health and Family Welfare (MOHFW), Government of India, and coordinated by the International Institute for Population Sciences (IIPS), Bombay. Interviews were conducted with a nationally representative sample of 89,777 ever-married women in the age group 13-49, from 24 states and the National Capital Territoty of Delhi. The main objective of the survey was to collect reliable and up-to-date information on fertility, family planning, mortality, and maternal and child health. Data collection was carried out in three phases from April 1992 to September 1993. THe NFHS is one of the most complete surveys of its kind ever conducted in India.
The households covered in the survey included 500,492 residents. The young age structure of the population highlights the momentum of the future population growth of the country; 38 percent of household residents are under age 15, with their reproductive years still in the future. Persons age 60 or older constitute 8 percent of the population. The population sex ratio of the de jure residents is 944 females per 1,000 males, which is slightly higher than sex ratio of 927 observed in the 1991 Census.
The primary objective of the NFHS is to provide national-level and state-level data on fertility, nuptiality, family size preferences, knowledge and practice of family planning, the potentiel demand for contraception, the level of unwanted fertility, utilization of antenatal services, breastfeeding and food supplemation practises, child nutrition and health, immunizations, and infant and child mortality. The NFHS is also designed to explore the demographic and socioeconomic determinants of fertility, family planning, and maternal and child health. This information is intended to assist policymakers, adminitrators and researchers in assessing and evaluating population and family welfare programmes and strategies. The NFHS used uniform questionnaires and uniform methods of sampling, data collection and analysis with the primary objective of providing a source of demographic and health data for interstate comparisons. The data collected in the NFHS are also comparable with those of the Demographic and Health Surveys (DHS) conducted in many other countries.
National
The population covered by the 1992-93 DHS is defined as the universe of all women age 13-49 who were either permanent residents of the households in the NDHS sample or visitors present in the households on the night before the survey were eligible to be interviewed.

SAMPLE DESIGN
The sample design for the NFHS was discussed during a Sample Design Workshop held in Madurai in Octber, 1991. The workshop was attended by representative from the PRCs; the COs; the Office of the Registrar General, India; IIPS and the East-West Center/Macro International. A uniform sample design was adopted in all the NFHS states. The Sample design adopted in each state is a systematic, stratified sample of households, with two stages in rural areas and three stages in urban areas.
SAMPLE SIZE AND ALLOCATION
The sample size for each state was specified in terms of a target number of completed interviews with eligible women. The target sample size was set considering the size of the state, the time and ressources available for the survey and the need for separate estimates for urban and rural areas of the stat. The initial target sample size was 3,000 completed interviews with eligible women for states having a population of 25 million or less in 1991; 4,000 completed interviews for large states with more than 25 million population; 8,000 for Uttar Pradesh, the largest state; and 1,000 each for the six small northeastern states. In States with a substantial number of backward districts, the initial target samples were increased so as to allow separate estimates to be made for groups of backward districts.
The urban and rural samples within states were drawn separetly and , to the extent possible, sample allocation was proportional to the size of the urban-rural populations (to facilitate the selection of a self-weighting sample for each state). In states where the urban population was not sufficiently large to provide a sample of at least 1,000 completed interviews with eligible women, the urban areas were appropriately oversampled (except in the six small northeastern states).
THE RURAL SAMPLE: THE FRAME, STRATIFICATION AND SELECTION
A two-stage stratified sampling was adopted for the rural areas: selection of villages followed by selection of households. Because the 1991 Census data were not available at the time of sample selection in most states, the 1981 Census list of villages served as the sampling frame in all the states with the exception of Assam, Delhi and Punjab. In these three states the 1991 Census data were used as the sampling frame.
Villages were stratified prior to selection on the basis of a number of variables. The firts level of stratification in all the states was geographic, with districts subdivided into regions according to their geophysical characteristics. Within each of these regions, villages were further stratified using some of the following variables : village size, distance from the nearest town, proportion of nonagricultural workers, proportion of the population belonging to scheduled castes/scheduled tribes, and female literacy. However, not all variables were used in every state. Each state was examined individually and two or three variables were selected for stratification, with the aim of creating not more than 12 strata for small states and not more than 15 strata for large states. Females literacy was often used for implicit stratification (i.e., the villages were ordered prior to selection according to the proportion of females who were literate). Primary sampling Units (PSUs) were selected systematically, with probaility proportional to size (PPS). In some cases, adjacent villages with small population sizes were combined into a single PSU for the purpose of sample selection. On average, 30 households were selected for interviewing in each selected PSU.
In every state, all the households in the selected PSUs were listed about two weeks prior to the survey. This listing provided the necessary frame for selecting households at the second sampling stage. The household listing operation consisted of preparing up-to-date notional and layout sketch maps of each selected PSU, assigning numbers to structures, recording addresses (or locations) of these structures, identifying the residential structures, and listing the names of the heads of all the households in the residentiak structures in the selected PSU. Each household listing team consisted of a lister and a mapper. The listing operation was supervised by the senior field staff of the concerned CO and the PRC in each state. Special efforts were made not to miss any household in the selected PSU during the listing operation. In PSUs with fewer than 500 households, a complete household listing was done. In PSUs with 500 or more households, segmentation of the PSU was done on the basis of existing wards in the PSU, and two segments were selected using either systematic sampling or PPS sampling. The household listing in such PSUs was carried out in the selected segments. The households to be interviewed were selected from provided with the original household listing, layout sketch map and the household sample selected for each PSU. All the selected households were approached during the data collection, and no substitution of a household was allowed under any circumstances.
THE RURAL URBAN SAMPLE: THE FRAME, STRATIFICATION AND SELECTION
A three-stage sample design was adopted for the urban areas in each state: selection of cities/towns, followed by urban blocks, and finally households. Cities and towns were selected using the 1991 population figures while urban blocks were selected using the 1991 list of census enumeration blocks in all the states with the exception of the firts phase states. For the first phase states, the list of urban blocks provided by the National Sample Survey Organization (NSSSO) served as the sampling frame.
All cities and towns were subdivided into three strata: (1) self-selecting cities (i.e., cities with a population large enough to be selected with certainty), (2) towns that are district headquaters, and (3) other towns. Within each stratum, the cities/towns were arranged according to the same kind of geographic stratification used in the rural areas. In self-selecting cities, the sample was selected according to a two-stage sample design: selection of the required number of urban blocks, followed by selection of households in each of selected blocks. For district headquarters and other towns, a three stage sample design was used: selection of towns with PPS, followed by selection of two census blocks per selected town, followed by selection of households from each selected block. As in rural areas, a household listing was carried out in the selected blocks, and an average of 20 households per block was selected systematically.

The National Family Health Surveys (NFHS) programme, initiated in the early 1990s, has emerged as a nationally important source of data on population, health, and nutrition for India and its states. The 2005-06 National Family Health Survey (NFHS-3), the third in the series of these national surveys, was preceded by NFHS-1 in 1992-93 and NFHS-2 in 1998-99. Like NFHS-1 and NFHS-2, NFHS-3 was designed to provide estimates of important indicators on family welfare, maternal and child health, and nutrition. In addition, NFHS-3 provides information on several new and emerging issues, including family life education, safe injections, perinatal mortality, adolescent reproductive health, high-risk sexual behaviour, tuberculosis, and malaria. Further, unlike the earlier surveys in which only ever-married women age 15-49 were eligible for individual interviews, NFHS-3 interviewed all women age 15-49 and all men age 15-54. Information on nutritional status, including the prevalence of anaemia, is provided in NFHS3 for women age 15-49, men age 15-54, and young children.
A special feature of NFHS-3 is the inclusion of testing of the adult population for HIV. NFHS-3 is the first nationwide community-based survey in India to provide an estimate of HIV prevalence in the general population. Specifically, NFHS-3 provides estimates of HIV prevalence among women age 15-49 and men age 15-54 for all of India, and separately for Uttar Pradesh and for Andhra Pradesh, Karnataka, Maharashtra, Manipur, and Tamil Nadu, five out of the six states classified by the National AIDS Control Organization (NACO) as high HIV prevalence states. No estimate of HIV prevalence is being provided for Nagaland, the sixth high HIV prevalence state, due to strong local opposition to the collection of blood samples.
NFHS-3 covered all 29 states in India, which comprise more than 99 percent of India's population. NFHS-3 is designed to provide estimates of key indicators for India as a whole and, with the exception of HIV prevalence, for all 29 states by urban-rural residence. Additionally, NFHS-3 provides estimates for the slum and non-slum populations of eight cities, namely Chennai, Delhi, Hyderabad, Indore, Kolkata, Meerut, Mumbai, and Nagpur. NFHS-3 was conducted under the stewardship of the Ministry of Health and Family Welfare (MOHFW), Government of India, and is the result of the collaborative efforts of a large number of organizations. The International Institute for Population Sciences (IIPS), Mumbai, was designated by MOHFW as the nodal agency for the project. Funding for NFHS-3 was provided by the United States Agency for International Development (USAID), DFID, the Bill and Melinda Gates Foundation, UNICEF, UNFPA, and MOHFW. Macro International, USA, provided technical assistance at all stages of the NFHS-3 project. NACO and the National AIDS Research Institute (NARI) provided technical assistance for the HIV component of NFHS-3. Eighteen Research Organizations, including six Population Research Centres, shouldered the responsibility of conducting the survey in the different states of India and producing electronic data files.
The survey used a uniform sample design, questionnaires (translated into 18 Indian languages), field procedures, and procedures for biomarker measurements throughout the country to facilitate comparability across the states and to ensure the highest possible data quality. The contents of the questionnaires were decided through an extensive collaborative process in early 2005. Based on provisional data, two national-level fact sheets and 29 state fact sheets that provide estimates of more than 50 key indicators of population, health, family welfare, and nutrition have already been released. The basic objective of releasing fact sheets within a very short period after the completion of data collection was to provide immediate feedback to planners and programme managers on key process indicators.
The population covered by the 2005 DHS is defined as the universe of all ever-married women age 15-49, NFHS-3 included never married women age 15-49 and both ever-married and never married men age 15-54 as eligible respondents.

SAMPLE SIZE
Since a large number of the key indicators to be estimated from NFHS-3 refer to ever-married women in the reproductive ages of 15-49, the target sample size for each state in NFHS-3 was estimated in terms of the number of ever-married women in the reproductive ages to be interviewed.
The initial target sample size was 4,000 completed interviews with ever-married women in states with a 2001 population of more than 30 million, 3,000 completed interviews with ever-married women in states with a 2001 population between 5 and 30 million, and 1,500 completed interviews with ever-married women in states with a population of less than 5 million. In addition, because of sample-size adjustments required to meet the need for HIV prevalence estimates for the high HIV prevalence states and Uttar Pradesh and for slum and non-slum estimates in eight selected cities, the sample size in some states was higher than that fixed by the above criteria. The target sample was increased for Andhra Pradesh, Karnataka, Maharashtra, Manipur, Nagaland, Tamil Nadu, and Uttar Pradesh to permit the calculation of reliable HIV prevalence estimates for each of these states. The sample size in Andhra Pradesh, Delhi, Maharashtra, Tamil Nadu, Madhya Pradesh, and West Bengal was increased to allow separate estimates for slum and non-slum populations in the cities of Chennai, Delhi, Hyderabad, Indore, Kolkata, Mumbai, Meerut, and Nagpur.
The target sample size for HIV tests was estimated on the basis of the assumed HIV prevalence rate, the design effect of the sample, and the acceptable level of precision. With an assumed level of HIV prevalence of 1.25 percent and a 15 percent relative standard error, the estimated sample size was 6,400 HIV tests each for men and women in each of the high HIV prevalence states. At the national level, the assumed level of HIV prevalence of less than 1 percent (0.92 percent) and less than a 5 percent relative standard error yielded a target of 125,000 HIV tests at the national level.
Blood was collected for HIV testing from all consenting ever-married and never married women age 15-49 and men age 15-54 in all sample households in Andhra Pradesh, Karnataka, Maharashtra, Manipur, Tamil Nadu, and Uttar Pradesh. All women age 15-49 and men age 15-54 in the sample households were eligible for interviewing in all of these states plus Nagaland. In the remaining 22 states, all ever-married and never married women age 15-49 in sample households were eligible to be interviewed. In those 22 states, men age 15-54 were eligible to be interviewed in only a subsample of households. HIV tests for women and men were carried out in only a subsample of the households that were selected for men's interviews in those 22 states. The reason for this sample design is that the required number of HIV tests is determined by the need to calculate HIV prevalence at the national level and for some states, whereas the number of individual interviews is determined by the need to provide state level estimates for attitudinal and behavioural indicators in every state. For statistical reasons, it is not possible to estimate HIV prevalence in every state from NFHS-3 as the number of tests required for estimating HIV prevalence reliably in low HIV prevalence states would have been very large.
SAMPLE DESIGN
The urban and rural samples within each state were drawn separately and, to the extent possible, unless oversampling was required to permit separate estimates for urban slum and non-slum areas, the sample within each state was allocated proportionally to the size of the state's urban and rural populations. A uniform sample design was adopted in all states. In each state, the rural sample was selected in two stages, with the selection of Primary Sampling Units (PSUs), which are villages, with probability proportional to population size (PPS) at the first stage, followed by the random selection of households within each PSU in the second stage. In urban areas, a three-stage procedure was followed. In the first stage, wards were selected with PPS sampling. In the next stage, one census enumeration block (CEB) was randomly selected from each sample ward. In the final stage, households were randomly selected within each selected CEB.
SAMPLE SELECTION IN RURAL AREAS
In rural areas, the 2001 Census list of villages served as the sampling frame. The list was stratified by a number of variables. The first level of stratification was geographic, with districts being subdivided into contiguous regions. Within each of these regions, villages were further stratified using selected variables from the following list: village size, percentage of males working in the nonagricultural sector, percentage of the population belonging to scheduled castes or scheduled tribes, and female literacy. In addition to these variables, an external estimate of HIV prevalence, i.e., 'High', 'Medium' or 'Low', as estimated for all the districts in high HIV prevalence states, was used for stratification in high HIV prevalence states. Objective: To assess the effect of experienced stigma on depth of multidimensional poverty of persons with severe mental illness (PSMI) in Delhi, India, controlling for gender, age and caste. Design: Matching case (hospital)–control (population) study. Setting: University Hospital (cases) and National Capital Region (controls), India. Participants: A case–control study was conducted from November 2011 to June 2012. 647 cases diagnosed with schizophrenia or affective disorders were recruited and 647 individuals of same age, sex and location of residence were matched as controls at a ratio of 1:2:1. Individuals who refused consent or provided incomplete interview were excluded. Main outcome measures: Higher risk of poverty due to stigma among PSMI. Results: 38.5% of PSMI compared with 22.2% of controls were found poor on six dimensions or more. The difference in multidimensional poverty index was 69% between groups with employment and income of the main contributors. Multidimensional poverty was strongly associated with stigma (OR 2.60, 95% CI 1.27 to 5.31), scheduled castes/scheduled tribes/other backward castes (2.39, 1.39 to 4.08), mental illness (2.07, 1.25 to 3.41) and female gender (1.87, 1.36 to 2.58). A significant interaction between stigma, mental illness and gender or caste indicates female PSMI or PSMI from 'lower castes' were more likely to be poor due to stigma than male controls (p<0.001) or controls from other castes (p<0.001). Conclusions: Public stigma and multidimensional poverty linked to SMI are pervasive and intertwined. In particular for low caste and women, it is a strong predictor of poverty. Exclusion from employment linked to negative attitudes and lack of income are the highest contributors to multidimensional poverty, increasing the burden for the family. Mental health professionals need to be aware of and address these issues.
The gigantic task of census taking was completed in two phases. In the first phase, known as House-listing Operations, all building and structures, residential, partly residential or non- residential were identified and listed and the uses to which they were put recorded. Information on houses, household amenities and assets were also collected. In the second phase, known as Population Enumeration, more detailed information on each individual residing in the country, Indian national or otherwise, during the enumeration period was collected. The population of India is divided into several groups based on social, educational, and financial statuses. The formation of these groups is a result of the historical social structure of the country. Between 2019 and 2021, Other Backward Class (OBC) constituted the largest part of Indian households accounting for about ** percent. On the other hand, Schedule Tribes formed about *** percent of households. How prosperous is India’s caste-based society? India suffers from extreme social and economic inequality. The combined share of Schedule Tribe and Schedule Caste in the affluent population of India was less than ** percent. Contrary to this, economically and socially stronger groups constituted the major part of the affluent population. Hence, indicating a strong relationship between caste and prosperity. India’s thoughts on caste-based reservation The constitution of India provides reservations to the weaker sections of the society for their upliftment and growth. However, the need for reservation has increased with time, making the whole situation even more complicated. People are divided over the existence of a system that provides preference to certain castes or sects. In a survey conducted in 2016 about providing employment reservation to young adults of Schedule Caste and Schedule Tribe, many people expressed opposition. More than ** percent of opposition came from upper Hindu caste. Minimum opposition was observed from the people belonging to Schedule Tribe and Schedule Caste.
