    The COVID Tracking Project was a volunteer organization launched from The Atlantic and dedicated to collecting and publishing the data required to understand the COVID-19 outbreak in the United States. Our dataset was in use by national and local news organizations across the United States and by research projects and agencies worldwide.

    On August 12, 2020, we launched the Long-Term-Care COVID Tracker with weekly data back to May 28, 2020, but the work of compiling the dataset began much earlier. In mid-April 2020, a team within The COVID Tracking Project started collecting long-term care data from every state that reported it. The aim of our work on long-term-care (LTC) data was to ensure that the pandemic's impact on residents and workers in a broad range of LTC facilities was entered into the historical record.

    Every Thursday evening until March 4, 2021, this dedicated team of volunteers gathered COVID-19 case and death data of long-term-care facility residents and staff from state and territory websites, recent press conferences and releases, and directly from state health department officials. We published this data every Friday as cumulative, current outbreak, and facility-level datasets. These datasets are available for download on our Download the Data page and, for each state and jurisdiction, on the Data by State pages.

    These datasets aim to document facilities for seniors like nursing homes, assisted living facilities, and residential care homes. The data excludes, when possible, facilities not specifically for the elderly, like chemical dependency residential treatment centers, behavioral health residential facilities, and intermediate care facilities for individuals with intellectual disabilities.

    Nursing Homes A nursing home is commonly referred to as a skilled nursing facility, long term care (LTC) facility, or rest home, and may have a different standardized name throughout the United States, but is most commonly referred to as a nursing home. A nursing home traditionally offers 24-hour (skilled) nursing to the elderly or to disabled patients having a variety of medical conditions who require personal care services above that of an assisted living but do not require hospitalization. The personal care services provided may or may not include, but are not limited to: skilled nursing, long term inpatient care, room and board, meals, laundry, and assistance with: dressing, grooming, getting in and out of bed, medications, bathing, and toileting. For purposes of this dataset, an assisted living facility is defined as a facility where the elderly, who are not related to the operator, reside and receive care, treatment, or services. Although not at the level of a nursing home, the services are above the level of an independent living community. They may include several hours per week of supportive care, personal care, or nursing care per resident. Generally, an assisted living facility offers help in daily living (laundry, cooking, cleaning, etc.) and personal assistance (bathing, eating, clothing, etc.). Many assisted living facilities offer assistance with medication and a lesser level of nursing care than what is offered at a nursing home. Assisted living facilities may be regulated by size restrictions depending on which type of assisted living facility it is considered to be in the state in which it exists. For example, Adult Family Homes in Wisconsin have between 3-4 elderly residents while Community Based Residential Facilities have 5 or more. Almost every state has different terminology to describe their version of the assisted living facility system. The structures in which assisted living facilities exist are varied as well. Depending on the type, an assisted living facility may operate out of a personal residence or a nursing home style structure, and it may be set up as apartment style living or as a campus setting in a continuing care retirement community. Multiple assisted living facilities may exist at one location or may be co-located with nursing homes and/or other similar health care facilities. If a facility is licensed by a state and holds multiple licenses, it is represented once in this dataset for each license, even if the licenses are for the same location. This dataset does not include retirement communities, adult daycare facilities, or rehabilitation facilities. Nursing Homes that are operated by and co-located with a hospital are also excluded because the locations are included in the hospital dataset. Records with "-DOD" appended to the end of the [NAME] value are located on a military base, as defined by the Defense Installation Spatial Data Infrastructure (DISDI) military installations and military range boundaries. "#" and "*" characters were automatically removed from standard fields populated by TechniGraphics. Double spaces were replaced by single spaces in these same fields. Text fields in this dataset have been set to all upper case to facilitate consistent database engine search results. All diacritics (e.g., the German umlaut or the Spanish tilde) have been replaced with their closest equivalent English character to facilitate use with database systems that may not support diacritics. The currentness of this dataset is indicated by the [CONTDATE] field. Based on this field, the oldest record dates from 09/29/2009 and the newest record dates from 01/08/2010.

    Description

    These data are part of NACJD's Fast Track Release and are distributed as they were received from the data depositor. The files have been zipped by NACJD for release, but not checked or processed except for the removal of direct identifiers. Users should refer to the accompanying readme file for a brief description of the files available with this collection and consult the investigator(s) if further information is needed.

    The purpose of the study was to:

    • Examine resident abuse from staff report by direct care workers in a nationally representative sample of assisted living settings;
    • Expand the scope of our understanding of resident abuse by including medication abuse in assisted living settings; and
    • Use the data collected to examine: a. rates of abuse; b. the association of external organizational, and internal factors with resident abuse; c. direct care worker characteristics associated with abuse; d. resident characteristics associated with abuse; e. leadership characteristics associated with abuse; f. the inter-correlations of different types of abuse; and, g. resident-to-resident abuse.

    For the purposes of this research, an assisted living facility is defined as a long term care setting that typically provides residents for activities of daily living and instrumental activities of daily living. Using state listings from websites, researchers identified approximately 21,000 of assisting living facilities. A random sample of approximately seven percent (n=1,500) of these eligible assisted living facilities were selected from all 50 states. Administrators of assisted living facilities were asked if they were willing to complete the questionnaire (n=1,376). Administrators were also asked if they would be willing to distribute the questionnaire to direct care workers (n=12,555). Researchers requested that they distribute the survey to all full-time and part-time direct care workers on all shifts. Follow up reminder mailings and emails were used for the administrators.

    The California Department of Public Health (CDPH), Center for Health Care Quality, Licensing and Certification (L&C) Program licenses more than 30 types of healthcare facilities. The Electronic Licensing Management System (ELMS) is a California Department of Public Health data system created to manage state licensing-related data. This file lists the bed types and bed type capacities that are associated with California healthcare facilities that are operational and have a current license issued by the CDPH and/or a current U.S. Department of Health and Human Services’ Centers for Medicare and Medicaid Services (CMS) certification. Article 28 facilities are hospitals, nursing homes, and diagnostic treatment centers. Article 36 facilities are certified home health care agencies and long term home health care programs. Article 40 facilities are hospices. The dataset currently only contains the locations of hospitals and hospital extension clinics. The data for the remaining facility types will be added in the future.     The Federal Pharmacy Partnership for Long-Term Care (LTC) Program was a partnership between CDC and CVS, Walgreens, and Managed Health Care Associates, Inc. The program offered on-site COVID-19 vaccination services for residents of nursing homes and assisted living facilities. The Federal Pharmacy Partnership for LTC Program was in effect after vaccines became available to April 23, 2021. This is the historical archived data related to the LTC Program and represents data that was shown on COVID Data Tracker through September 30, 2021.     The American Community Survey (ACS) is an ongoing survey that provides vital information on a yearly basis about our nation and its people by contacting over 3.5 million households across the country. The resulting data provides incredibly detailed demographic information across the US aggregated at various geographic levels which helps determine how more than $675 billion in federal and state funding are distributed each year. Businesses use ACS data to inform strategic decision-making. ACS data can be used as a component of market research, provide information about concentrations of potential employees with a specific education or occupation, and which communities could be good places to build offices or facilities.     This dataset denotes HUD subsidized Multifamily Housing properties excluding insured hospitals with active loans. HUD’s Multifamily Housing property portfolio consist primarily of rental housing properties with five or more dwelling units such as apartments or town houses, but can also include nursing homes, hospitals, elderly housing, mobile home parks, retirement service centers, and occasionally vacant land. HUD provides subsidies and grants to property owners and developers in an effort to promote the development and preservation of affordable rental units for low-income populations, and those with special needs such as the elderly, and disabled.     Description

    This feature class/shapefile contains locations of Hospitals for 50 US states, Washington D.C., US territories of Puerto Rico, Guam, American Samoa, Northern Mariana Islands, Palau, and Virgin Islands. The dataset only includes hospital facilities based on data acquired from various state departments or federal sources which has been referenced in the SOURCE field. Hospital facilities which do not occur in these sources will be not present in the database. The source data was available in a variety of formats (pdfs, tables, webpages, etc.) which was cleaned and geocoded and then converted into a spatial database. The database does not contain nursing homes or health centers. Hospitals have been categorized into children, chronic disease, critical access, general acute care, long term care, military, psychiatric, rehabilitation, special, and women based on the range of the available values from the various sources after removing similarities.

    Centers for Medicare & Medicaid Services - Nursing HomesThis feature layer, utilizing data from the Centers for Medicare & Medicaid Services (CMS), displays the locations of nursing homes in the U.S. Nursing homes provide a type of residential care. They are a place of residence for people who require constant nursing care and have significant deficiencies with activities of daily living. Per CMS, "Nursing homes, which include Skilled Nursing Facilities (SNFs) and Nursing Facilities (NFs), are required to be in compliance with Federal requirements to receive payment under the Medicare or Medicaid programs. The Secretary of the United States Department of Health & Human Services has delegated to the CMS and the State Medicaid Agency the authority to impose enforcement remedies against a nursing home that does not meet Federal requirements."     Multifamily Portfolio datasets (section 8 contracts) - The information has been compiled from multiple data sources within FHA or its contractors. HUD oversees more than 22,000 privately owned multifamily properties, and more than 1.4 million assisted housing units. These homes were originally financed with FHA-insured or Direct Loans and many are supported with Section 8 or other rental assistance contracts. Our existing stock of affordable rental housing is a critical resource for seniors and families who otherwise would not have access to safe, decent places to call home.

    The FHA insured Multifamily Housing portfolio consists primarily of rental housing properties with five or more dwelling units such as apartments or town houses, but can also be nursing homes, hospitals, elderly housing, mobile home parks, retirement service centers, and occasionally vacant land. Please note that this dataset overlaps the Multifamily Properties Assisted layer. The Multifamily property locations represent the approximate location of the property. Location data for HUD-related properties and facilities are derived from HUD's enterprise geocoding service. While not all addresses are able to be geocoded and mapped to 100% accuracy, we are continuously working to improve address data quality and enhance coverage. Please consider this issue when using any datasets provided by HUD. When using this data, take note of the field titled “LVL2KX” which indicates the overall accuracy of the geocoded address using the following return codes: ‘R’ - Interpolated rooftop (high degree of accuracy, symbolized as green) ‘4’ - ZIP+4 centroid (high degree of accuracy, symbolized as green) ‘B’ - Block group centroid (medium degree of accuracy, symbolized as yellow) ‘T’ - Census tract centroid (low degree of accuracy, symbolized as red) ‘2’ - ZIP+2 centroid (low degree of accuracy, symbolized as red) ‘Z’ - ZIP5 centroid (low degree of accuracy, symbolized as red) ‘5’ - ZIP5 centroid (same as above, low degree of accuracy, symbolized as red) Null - Could not be geocoded (does not appear on the map) For the purposes of displaying the location of an address on a map only use addresses and their associated lat/long coordinates where the LVL2KX field is coded ‘R’ or ‘4’. These codes ensure that the address is displayed on the correct street segment and in the correct census block. The remaining LVL2KX codes provide a cascading indication of the most granular level geography for which an address can be confirmed. For example, if an address cannot be accurately interpolated to a rooftop (‘R’), or ZIP+4 centroid (‘4’), then the address will be mapped to the centroid of the next nearest confirmed geography: block group, tract, and so on. When performing any point-in polygon analysis it is important to note that points mapped to the centroids of larger geographies will be less likely to map accurately to the smaller geographies of the same area. For instance, a point coded as ‘5’ in the correct ZIP Code will be less likely to map to the correct block group or census tract for that address. In an effort to protect Personally Identifiable Information (PII), the characteristics for each building are suppressed with a -4 value when the “Number_Reported” is equal to, or less than 10. Please note that there are two commercial spaces (not coded under field “comments”)recorded in our property directory which are not part of the portfolio for Facility Planning.
