Master Beneficiary Summary File (MBSF) A beneficiary-level data from CMS/ResDAC is known as the Master Beneficiary Summary File (MBSF). The raw MBSF data contains basic demographic information of individuals, an indicator of death, and some details on their medicare enrollment. Mortality Denominator We refer to the mortality denominator as the clean subset of MBSF health data that is representative of the older population. The subset consists of the US elder population; notably, individuals under 64 who qualified because of a disability are excluded.

The Medicare Health Outcomes Survey (HOS) is the first patient reported health outcomes measure for the Medicare population in managed care settings. It is a continuous study that measures the physical and mental health and well-being of Medicare beneficiaries over a two-year period. Starting in 1998, a baseline survey was administered to a new cohort of respondents each year in the spring and a follow-up survey was conducted of those same respondents two years later. Cohorts 1-5 include the Baseline Data, the Follow-Up Data, and the Analytic Data file, which contains the merged Baseline and Follow-Up files along with supplemental variables. Beginning with Cohort 6, the Follow-Up Data were included only in the Analytic file. The HOS consists of the SF-36 Health Survey, which yields physical and mental health summary measures, as well as questions on topics such as chronic medical conditions, activities of daily living (ADLs), depression, smoking, physical health symptoms, weight and height, and additional questions corresponding to HEDIS (Health Care Employer Data and Information Set) measures such as urinary incontinence in older adults, osteoporosis testing in older women, and fall risk management. In 2006, CMS implemented the Medicare HOS 2.0 for Medicare Advantage Organizations (MAOs) which evaluates physical and mental health status using the Veteran RAND 12-Item Health Survey (VR-12). The revised instrument contains questions that gather information for case-mix and risk-adjustment variables, and collects information on respondents' physical functioning, bodily pain, social functioning, mental health, vitality, general health, and how respondents' physical and emotional health affects their lives. The survey includes case-mix adjustment variables which may be used to adjust the survey response data for beneficiary characteristics that are known to be related to systematic biases in the way people respond to survey questions. All states (including the District of Columbia) provide data to the Centers for Medicare & Medicaid Services (CMS) on a range of Medicaid and Children’s Health Insurance Program (CHIP) eligibility and enrollment metrics. These data reflect state-reported information on Medicaid and CHIP eligibility renewals initiated and scheduled for completion during the reporting period. In addition to reporting the outcomes of renewals at the end of each reporting period, states also provide an update on renewals that were reported pending as of the end of a reporting period. For more information on these data, see Sections II and III of the Eligibility Processing Data Report specifications. Notes: Georgia reported data for individuals who continue to be eligible following a change in circumstances and were granted a new 12-month eligibility period during the reporting period, along with data on individuals due for renewal in the month. North Carolina reports renewal outcomes for only initiated renewals scheduled for completion in the report month, and as such, the data do not reflect renewals that should have been completed in the reporting period that the state was unable to initiate by the end of the report month.

This data set accompanies the Profile of the California Medicare Population chartbook, published by the Office of Medicare Innovation and Integration in February 2022, and available at (https://www.dhcs.ca.gov/services/Documents/OMII-Medicare-Databook-February-18-2022.pdf). The three data files in this data set were analyzed from federal administrative data (the Medicare Master Beneficiary Summary File) for beneficiary characteristics as of March 2021. These datasets include: Medicare enrollment, Medicare Advantage enrollment (and its converse fee-for-service Medicare enrollment), dual Medi-Cal eligibility and enrollment (and its converse Medicare-only enrollment), by county. Medicare Savings Program enrollees were considered Medicare-only and not dually enrolled in Medi-Cal. Abstract (en): This data collection is the second in a series of data releases from the Medicare Current Beneficiary Survey (MCBS) relating to beneficiary access to medical care. The MCBS is a continuous, multipurpose survey of a representative sample of the Medicare population, both aged and disabled. Sample persons are interviewed three times a year over several years to form a continuous profile of their health care experience. Interviews are conducted regardless of whether the sample person resides at home or in a long-term care facility, using the questionnaire version appropriate to the setting. The MCBS also collects a variety of information about demographic characteristics (date of birth, sex, race, education, military service, and marital status), health status and functioning, access to care, sources of and satisfaction with care, insurance coverage, financial resources, and family supports. The 1992 interview data were collected during September through December of 1992, the fourth round of data collection. The 1992 data are designed to stand alone for cross-sectional analysis, or they can be used for longitudinal analysis. Weights are provided for both cross-sectional and longitudinal analysis. Medicare beneficiaries. Respondents were sampled from the Medicare enrollment file to be representative of the Medicare population as a whole and by age group: under 45, 45-64, 65-69, 70-74, 75-79, 80-84, and 85 and over. Because of interest in their special health care needs, the oldest old (85 and over) and the disabled (64 and under) were oversampled to permit detailed analysis of these subpopulations. The sample was drawn from 107 primary sampling units (PSUs). The 1992 Round 4 data include interviews for 10,388 persons who were interviewed in 1991 and for 1,995 new people added to the survey during the current round. The 1992 supplementary sample included newly enrolled beneficiaries, as well as previously enrolled beneficiaries who were included to improve coverage or to maintain the desired sample size. 2006-01-12 All files were removed from dataset 25 and flagged as study-level files, so that they will accompany all downloads.1997-04-22 Part 7 (Health Status and Functioning Record File) and Part 11 (Health Insurance Record File) have been resupplied by the principal investigator to correct several variables. In addition, the billing records data (Parts 25-30) were withdrawn from distribution by the principal investigator. This dataset is a de-identified summary table of vision and eye health data indicators from Medicare claims, stratified by all available combinations of age group, race/ethnicity, sex, and state. Medicare claims for VEHSS includes beneficiaries who were fully enrolled in Medicare Part B Fee-for-Service (FFS) for the duration of the year. Medicare claims provide a convenience sample that includes approximately 30 million individuals annually, which represents nearly 89% of the US population aged 65 and older and 3.3% of the US population younger than 65, including persons disabled due to blindness. Medicare data for VEHSS include Service Utilization and Medical Diagnoses indicators. Data were suppressed for de-identification to ensure protection of patient privacy. Data will be updated as it becomes available. Detailed information on VEHSS Medicare analyses can be found on the VEHSS Medicare webpage (cdc.gov/visionhealth/vehss/data/claims/medicare.html). The MarketScan health claims database is a compilation of nearly 110 million patient records with information from more than 100 private insurance carriers and large self-insuring companies. Public forms of insurance (i.e., Medicare and Medicaid) are not included, nor are small (< 100 employees) or medium (1000 employees). We excluded the relatively few (n=6735) individuals over 65 years of age because Medicare is the primary insurance of U.S. adults over 65. The EQI was constructed for 2000-2005 for all US counties and is composed of five domains (air, water, built, land, and sociodemographic), each composed of variables to represent the environmental quality of that domain. Domain-specific EQIs were developed using principal components analysis (PCA) to reduce these variables within each domain while the overall EQI was constructed from a second PCA from these individual domains (L. C. Messer et al., 2014). To account for differences in environment across rural and urban counties, the overall and domain-specific EQIs were stratified by rural urban continuum codes (RUCCs) (U.S. Department of Agriculture, 2015). This dataset is not publicly accessible because: EPA cannot release personally identifiable information regarding living individuals, according to the Privacy Act and the Freedom of Information Act (FOIA). This dataset contains information about human research subjects. Because there is potential to identify individual participants and disclose personal information, either alone or in combination with other datasets, individual level data are not appropriate to post for public access. The MBSF mortality denominator can be used to study mortality rates of the elder population in the US. Access to CMS data is restricted. Processed datasets cannot be shared. Contact the authors if you've purchased CMS data through RESDAC and would like to use our data processing pipelines to clean CMS raw data and generate the MBSF mortality denominator. Medicare Overview In the United States, when individuals reach the age of 65, they become eligible for the Medicare federal health insurance program. Medicare also covers individuals under 65 if they have certain disabilities or End-Stage Renal Disease (ESRD) or Amyotrophic Lateral Sclerosis (ALS). About CMS The Center for Medicare & Medicaid Services (CMS) is the federal agency responsible for managing the Medicare and Medicaid data. CMS makes predefined datasets available for purchase to researchers after they enter a Data usage agreement through Research Data Assistance Center (ResDAC). Master Beneficiary Summary File (MBSF) A beneficiary-level data from CMS/ResDAC is known as the Master Beneficiary Summary File (MBSF). The raw MBSF data contains basic demographic information of individuals, an indicator of death, and some details on their medicare enrollment. Mortality Denominator We refer to the mortality denominator as the clean subset of MBSF health data that is representative of the older population. The subset consists of the US elder population; notably, individuals under 64 who qualified because of a disability are excluded.
