    Description

    This database contains tobacco consumption data from 1970-2015 collected through a systematic search coupled with consultation with country and subject-matter experts. Data quality appraisal was conducted by at least two research team members in duplicate, with greater weight given to official government sources. All data was standardized into units of cigarettes consumed and a detailed accounting of data quality and sourcing was prepared. Data was found for 82 of 214 countries for which searches for national cigarette consumption data were conducted, representing over 95% of global cigarette consumption and 85% of the world’s population. Cigarette consumption fell in most countries over the past three decades but trends in country specific consumption were highly variable. For example, China consumed 2.5 million metric tonnes (MMT) of cigarettes in 2013, more than Russia (0.36 MMT), the United States (0.28 MMT), Indonesia (0.28 MMT), Japan (0.20 MMT), and the next 35 highest consuming countries combined. The US and Japan achieved reductions of more than 0.1 MMT from a decade earlier, whereas Russian consumption plateaued, and Chinese and Indonesian consumption increased by 0.75 MMT and 0.1 MMT, respectively. These data generally concord with modelled country level data from the Institute for Health Metrics and Evaluation and have the additional advantage of not smoothing year-over-year discontinuities that are necessary for robust quasi-experimental impact evaluations. Before this study, publicly available data on cigarette consumption have been limited—either inappropriate for quasi-experimental impact evaluations (modelled data), held privately by companies (proprietary data), or widely dispersed across many national statistical agencies and research organisations (disaggregated data). This new dataset confirms that cigarette consumption has decreased in most countries over the past three decades, but that secular country specific consumption trends are highly variable. The findings underscore the need for more robust processes in data reporting, ideally built into international legal instruments or other mandated processes. To monitor the impact of the WHO Framework Convention on Tobacco Control and other tobacco control interventions, data on national tobacco production, trade, and sales should be routinely collected and openly reported.     Abstract

    The Global Youth Tobacco Survey (GYTS) is a school-based survey designed to enhance the capacity of countries to monitor tobacco use among youth and to guide the implementation and evaluation of tobacco prevention and control programmes. The information generated from the GYTS can be used to stimulate the development of tobacco control programmes and can serve as a means to assess progress in meeting programme goals. In addition, GYTS data can be used to monitor seven Articles in the WHO FCTC.

    Methodology

    In December 1998, TFI convened a meeting in Geneva with the Centers for Disease Control and Prevention (CDC), the United Nations Children’s Fund (UNICEF), the World Bank and representatives from countries in each of the six WHO regions to discuss the need for standardized mechanisms to collect youth tobacco use information on a global basis. The outcome of this meeting was the development by WHO and CDC of a Global Tobacco Surveillance System, which uses the Global Youth Tobacco Survey (GYTS) as its data collection mechanism.

    The GYTS uses a standard methodology for constructing the sampling frame, selecting schools and classes, preparing questionnaires, following consistent field procedures, and using consistent data management procedures for data processing and analysis.

    GYTS is composed of 56 "core" questions designed to gather data on the following seven domains. The questionnaire also allows countries to insert their own country-specific questions.

    • Knowledge and attitudes of young people towards cigarette smoking
    • Prevalence of cigarette smoking and other tobacco use among young people
    • Role of the media and advertising in young people’s use of cigarettes
    • Access to cigarettes

    Description

    BackgroundWaterpipe (WP) use is rapidly increasing among young people worldwide due to the widespread misperception that it is safer than cigarette smoking. Health warning labels (HWLs) can effectively communicate tobacco-related health risks but have yet to be developed for WP. This study aimed to optimize and adapt a set of 16 pictorial WP-specific HWLs, developed by an international Delphi study, to the Tunisian context. HWLs were grouped into four themes: WP health risks, WP harm to others, WP-specific harms, and WP harm compared to cigarettes.MethodsUsing a mixed method approach, we conducted ten focus groups combined with a survey among young WP users and nonusers (N = 63; age 18–34 years). In the survey, participants rated the HWLs on several communication outcomes (e.g., reaction, harm perception, effectiveness) and were then instructed to view all HWLs in each theme and rank them in the order of overall perceived effectiveness, from the most to the least effective. Afterward, participants provided in-depth feedback on HWLs and avenues for improvement. Mean effectiveness rating scores and percentages of participants’ top-ranked HWLs were calculated. Discussions were audio-taped, transcribed verbatim, and analyzed thematically.ResultsThe top-ranked HWLs were those showing oral cancers, orally transmitted diseases, and a sick child. Focus group discussion illustrated that these selections were based on participants’ reactions to the direct impact of WP on a person’s physical appearance and evoking guilt over children’s exposure to WP smoke. Suggestions for improvement highlighted the need to use the local dialect and more affirmative statements (e.g., avoiding "may" or "can").ConclusionsThis study is the first in North Africa to attempt to advance HWLs policy as the World Health Organization recommended. The results of this study can be used as a basis for implementing WP-specific health messages in the Eastern Mediterranean Region.
