Across the U.S., state and local public health officials have found themselves at the center of a political storm as they combat the worst pandemic in a century. Amid a fractured federal response, the usually invisible army of workers charged with preventing the spread of infectious disease has become a public punching bag.
In the midst of the coronavirus pandemic, at least 303 state or local public health leaders in 41 states have resigned, retired or been fired since April 1, 2020, according to an ongoing investigation by The Associated Press and KHN.
According to experts, that is the largest exodus of public health leaders in American history.
Many left due to political blowback or pandemic pressure, as they became the target of groups that have coalesced around a common goal — fighting and even threatening officials over mask orders and well-established public health activities like quarantines and contact tracing. Some left to take higher profile positions, or due to health concerns. Others were fired for poor performance. Dozens retired. An untold number of lower level staffers have also left.
The result is a further erosion of the nation’s already fragile public health infrastructure, which KHN and the AP documented beginning in 2020 in the Underfunded and Under Threat project.

KHN and AP counted how many state and local public health leaders have left their jobs between April 1, 2020 and Sept. 12, 2021.
The government tasks public health workers with improving the health of the general population, through their work to encourage healthy living and prevent infectious disease. To that end, public health officials do everything from inspecting water and food safety to testing the nation’s babies for metabolic diseases and contact tracing cases of syphilis.
Many parts of the country have a health officer and a health director/administrator by statute. The analysis counted both of those positions if they existed. For state-level departments, the count tracks people in the top and second-highest-ranking job.
The analysis includes exits of top department officials regardless of reason, because no matter the reason, each left a vacancy at the top of a health agency during the pandemic. Reasons for departures include political pressure, health concerns and poor performance. Others left to take higher profile positions or to retire. Some departments had multiple top officials exit over the course of the pandemic; each is included in the analysis.
Reporters compiled the exit list by reaching out to public health associations and experts in every state and interviewing hundreds of public health employees. They also received information from the National Association of City and County Health Officials, and combed news reports and records.
Public health departments can be found at multiple levels of government. Each state has a department that handles these tasks, but most states also have local departments that either operate under local or state control. The population served by each local health department is calculated using the U.S. Census Bureau 2019 Population Estimates based on each department’s jurisdiction.
KHN and the AP have worked since the spring on a series of stories documenting the funding, staffing and problems around public health. A previous data distribution detailed a decade's worth of cuts to state and local spending and staffing on public health. That data can be found here.

TROPIS is the acronym for the Tree Growth and Permanent Plot Information System sponsored by CIFOR to promote more effective use of existing data and knowledge about tree growth.
TROPIS is concerned primarily with information about permanent plots and tree
growth in both planted and natural forests throughout the world. It has five
components:
- a network of people willing to share permanent plot data and tree
growth information;
- an index to people and institutions with permanent plots;
- a database management system to promote more efficient data management;
- a method to find comparable sites elsewhere, so that observations
can be supplemented or contrasted with other data; and
- an inference system to allow growth estimates to be made in the
absence of empirical data.
- TROPIS is about people and information. The core of TROPIS is an
index to people and their plots maintained in a relational
database. The database is designed to fulfil two primary needs:
- to provide for efficient cross-checking, error-checking and
updating; and to facilitate searches for plots matching a wide range
of specified criteria, including (but not limited to) location, forest
type, taxa, plot area, measurement history.
The database is essentially hierarchical: the key element of the
database is the informant. Each informant may contribute information
on many plot series, each of which has consistent objectives. In turn,
each series may comprise many plots, each of which may have a
different location or different size. Each plot may contain many
species. A series may be a thinning or spacing experiment, some
species or provenance trials, a continuous forest inventory system, or
any other aggregation of plots convenient to the informant. Plots need
not be current. Abandoned plots may be included provided that the
location is known and the plot data remain accessible. In addition to
details of the informant, we try to record details of additional
contact people associated with plots, to maintain continuity when
people transfer or retire. Thus the relational structure may appear
complex, but ensures data integrity.
At present, searches are possible only via mail, fax or email requests
to the TROPIS co-ordinator at CIFOR. Self-service on-line searching
will also be available in 1997. Clients may search for plots with
specified taxa, locations, silvicultural treatment, or other specified
criteria and combinations. TROPIS currently contains references to
over 10,000 plots with over 2,000 species contributed by 100
individuals world-wide.
This database will help CIFOR as well as other users to make more
efficient use of existing information, and to develop appropriate and
effective techniques and policies for sustainable forest management
world-wide.

This round of Euro-Barometer surveys queried respondents on standard Euro-Barometer measures such as public awareness of and attitudes toward the Common Market and the European Community (EC), and also focused on consumer goods, Social Security, health care and health care benefits, the elderly, retirement, and alcohol and drug use. Questions concerning consumer goods asked whether respondents read product information before purchasing, what additional product information they would like to see, what three things other than price were most important in deciding whether to purchase an item, and whether it was necessary to have the same type of product information available for all members of the European Community (EC). Respondents' attitudes and opinions on Social Security were probed with questions that asked whether they agreed that Social Security properly protects the unemployed, the elderly, the sick or disabled, those with work-related injuries or illness, and the poor. Respondents were also asked whether policies on pensions, minimum income, and unemployment should be decided by national governments or by the EC, and whether foreigners should have the same Social Security benefits as citizens. The general health of respondents and their health care benefits were assessed through questions that asked whether they had a long-standing illness, disability, or infirmity, whether they had cut down their activity due to illness or injury, and whether they had taken medicine or talked to a doctor within the last 30 days. Respondents were also queried about which conditions they would see a doctor for and what type of examinations they had had in the past three years. Respondents were asked to rate what they paid for various medical services, the general quality of their health care, and the nature and availability of health insurance. The main problems facing the elderly and the role the elderly play in society were also topics of investigation in this survey. Questions elicited respondents' views toward possible changes in pension terms, whether retirement should occur at a fixed age, what types of discrimination affect the elderly who are working, whether the government should introduce laws to try to stop age discrimination, whether a minimum level of income should be provided to the elderly, and whether the elderly needing personal care should go into residential/nursing homes or should have social services help them remain in their homes for as long as possible. Respondents were also asked whether they provided long-term care to anyone either living with them or not living with them, who was in the best position to decide which services are most important for the elderly, what the best method of financing long-term care for the elderly was, and whether the EC was doing enough with regard to the elderly. Questions on retirement dealt with what ages respondents retired/planned to retire, whether the retired felt their pensions to be adequate, whether working people looked forward to retiring, whether pensions should be extended to widows and dependent children, whether pensions should be reduced for those who work for earned income beyond retirement, and whether pensions should be provided through government taxation, employer/employee contributions, or private contracts between workers and pension companies. Queries about alcohol and drug usage probed the use of beer, wine, spirits, and other forms of alcohol, age at which the respondent began drinking, familiarity with major forms of drugs, age at which drugs were first offered, how difficult it was to get drugs, and the means available for getting drugs. Additional questions focused on how the respondent viewed the drug problem, the top priority in eliminating the drug problem, diminishing the effects of drug use, whether drug use leads to AIDS, prostitution, health problems, social problems, violence, suicide, personality breakdowns, and problems with the law, and the major reasons for alcohol and drug use. Demographic and other background information was gathered on life satisfaction, number of people residing in the home, size of locality, home ownership, trade union membership, region of residence, and occupation of the head of household, as well as the respondent's age, sex, marital status, education, occupation, work sector, religiosity, subjective social class, use of media, left-right political self-placement, and opinion leadership.
