Does receiving a medical education outside the United States impact a surgeon’s performance? We study this question by matching operations performed by internationally-trained surgeons to those performed by US-trained surgeons in reanalysis of a large health outcomes study. An effective matched design must achieve several goals, including balancing covariate distributions marginally, ensuring units within individual pairs have similar values on key covariates, and using a sufficiently large sample from the raw data. Yet in our study, optimizing some of these goals forces less desirable results on others. We address such tradeoffs from a multi-objective optimization perspective by creating matched designs that are Pareto optimal with respect to two goals. We provide general tools for generating representative subsets of Pareto optimal solution sets and articulate how they can be used to improve decision-making in observational study design. In the motivating surgical outcomes study, formulating a multi-objective version of the problem helps us balance an important variable without sacrificing two other design goals, average closeness of matched pairs on a multivariate distance and size of the final matched sample. Keywords: matching; Pareto optimality; fine balance; network flow optimization; multi-objective optimization.

ABSTRACT Introduction: There is a lack of information about cardiac surgery training and professional practice in Latin American (LATAM) countries. This study is the first comparative analysis of cardiac surgical training and professional practice across LATAM and provides the fundamentals for future academic projects of the Latin American Association of Cardiac and Endovascular Surgery (LACES). Methods: International survey-based comparative analysis of the training and professional practice of cardiac surgeons across LATAM. Trainees (residents/fellows) and staf (graduated) surgeons from LATAM countries were included. Results: A total of 289 respondents (staf surgeons N=221 [76.5%]; residents/fellows N=68 [23.5%]) from 18 different countries participated in the survey. Most surgeons (N=92 [45.3%]) reported being unsatisfied with their salaries. Most respondents (N=181 [62.6%]) stated that it was difficult to obtain a leadership position, and 149 (73.8%) stated that it was difficult to find a job after completing training. Only half of the trainee respondents (N=32 [47.1%]) reported that their program had all resident spots occupied. Only 22.1% (N=15) of residents/fellows were satisfied with their training programs. The majority (N=205 [70.9%]) of respondents would choose cardiac surgery as their specialty again. Most surgeons (N=129 [63.9%]) and residents/fellows (N=52 [76.5%]) indicated that the establishment of a LATAM cardiac surgery board examination would be beneficial. Conclusion: Modernization and standardization of training, as well as greater access to opportunities, may be required in LATAM to increase professional satisfaction of cardiac surgeons and to reduce disparities in the specialty. Such changes may enhance the regional response to the dynamic challenges in the feld.
In 2024, nearly 7.9 million Botulinum toxin, commonly known as Botox, injections were performed by plastic surgeons globally, making it the most popular non-invasive cosmetic treatment. Hyaluronic acid injections and hair removal rounded out the top three most sought-after minimally invasive procedures, highlighting the diverse range of options available to those seeking aesthetic enhancements. Global distribution of cosmetic procedures The United States remains at the forefront of the cosmetic procedure industry, accounting for 13 percent of all plastic surgeons worldwide in 2024. While Brazil leads in surgical procedures, the U.S. maintains its top position for total cosmetic procedures due to the high volume of nonsurgical treatments. Countries like Japan and Italy also show a preference for nonsurgical "touch-ups" over surgical treatments. Rising popularity of medical tourism Cosmetic surgery tourism has become increasingly common, with some countries becoming hotspots for foreign patients. Colombian surgeons, for instance, reported that an average of 36 percent of its cosmetic surgery patients were from abroad in 2023, primarily from the United States, Venezuela, and Ecuador. This trend extends to other regions as well, with Turkey seeing the largest share of foreign patients traveling from Germany for procedures. The willingness of individuals to travel internationally for cosmetic treatments demonstrates the varying accessibility, quality, safety, and costs of procedures across different countries.

BackgroundFrailty is frequently used by clinicians to help determine surgical outcomes. The frailty index, which represents the frequency of frailty indicators present in an individual, is one method for evaluating patient frailty to predict surgical outcomes. However, the frailty index treats all indicators of frailty that are used in the index as equivalent. Our hypothesis is that frailty indicators may be divided into groups of high and low-impact indicators and this separation will improve surgical discharge outcome prediction accuracy.Data and methodsPopulation data for inpatient elective operations was collected from the 2018 American College of Surgeons National Surgical Quality Improvement Program Participant Use Files. Artificial neural network (ANN) models trained using backpropagation are used to evaluate the relative accuracy for predicting surgical outcome of discharge destination using a traditional modified frailty index (mFI) or a new joint mFI that separates high-impact and low-impact indicators into distinct groups as input variables. Predictions are made across nine possible discharge destinations. A leave-one-out method is used to indicate the relative contribution of high and low-impact variables.ResultsExcept for the surgical specialty of cardiac surgery, the ANN model using distinct high and low-impact mFI indexes uniformly outperformed the ANN models using a single traditional mFI. Prediction accuracy improved from 3.4% to 28.1%. The leave-one-out experiment shows that except for the case of otolaryngology operations, the high-impact index indicators provided more support when determining surgical discharge destination outcomes.ConclusionFrailty indicators are not uniformly similar and should be treated differently in clinical outcome prediction systems.

BackgroundThousands of physicians attend scientific conferences each year. While recent data indicate that variation in staffing during such meetings impacts survival of non-surgical patients, the association between treatment during conferences and outcomes of a surgical population remain unknown. The purpose of this study was to examine mortality resulting from traumatic injuries and the influence of hospital admission during national surgery meetings.Study designRetrospective analysis of in-hospital mortality using data from the Trauma Quality Improvement Program (2010–2011). Identified patients admitted during four annual meetings and compared their mortality with that of patients admitted during non-conference periods. Analysis included 155 hospitals with 12,256 patients admitted on 42 conference days and 82,399 patients admitted on 270 non-conference days. Multivariate analysis performed separately for hospitals with different levels of trauma center verification by state and American College of Surgeons (ACS) criteria.ResultsPatient characteristics were similar between meeting and non-meeting dates. At ACS level I and level II trauma centers during conference versus non-conference dates, adjusted mortality was not significantly different. However, adjusted mortality increased significantly for patients admitted to trauma centers that lacked ACS trauma verification during conferences versus non-conference days (OR 1.2, p = 0.008), particularly for patients with penetrating injuries, whose mortality rose from 11.6% to 15.9% (p = 0.006).ConclusionsTrauma mortality increased during surgery conferences compared to non-conference dates for patients admitted to hospitals that lacked ACS trauma level verification. The mortality difference at those hospitals was greatest for patients who presented with penetrating injuries.
