The Pediatric Participant Use Data File (PUF) is a Health Insurance Portability and Accountability Act (HIPAA)-compliant data file containing cases submitted to the American College of Surgeons National Surgical Quality Improvement Program Pediatric® (ACS NSQIP Pediatric®). The PUF contains patient-level, aggregate data and does not identify hospitals, health care providers, or patients. The ACS NSQIP Pediatric collects data on approximately 120 variables, including preoperative risk factors, intraoperative variables, and 30-day postoperative mortality and morbidity outcomes for patients undergoing major surgical procedures in both the inpatient and outpatient setting. The intended purpose of this file is to provide researchers at participating sites with a data resource they can use to investigate and advance the quality of care delivered to the surgical patient through the analysis of cases captured by ACS NSQIP Pediatric. Additional procedure-specific PUFs are available for appendectomy, spinal fusion, and cerebrospinal fluid shunt.

ABSTRACT Introduction: There is a lack of information about cardiac surgery training and professional practice in Latin American (LATAM) countries. This study is the first comparative analysis of cardiac surgical training and professional practice across LATAM and provides the fundamentals for future academic projects of the Latin American Association of Cardiac and Endovascular Surgery (LACES). Methods: International survey-based comparative analysis of the training and professional practice of cardiac surgeons across LATAM. Trainees (residents/fellows) and staf (graduated) surgeons from LATAM countries were included. Results: A total of 289 respondents (staf surgeons N=221 [76.5%]; residents/fellows N=68 [23.5%]) from 18 different countries participated in the survey. Most surgeons (N=92 [45.3%]) reported being unsatisfied with their salaries. Most respondents (N=181 [62.6%]) stated that it was difficult to obtain a leadership position, and 149 (73.8%) stated that it was difficult to find a job after completing training. Only half of the trainee respondents (N=32 [47.1%]) reported that their program had all resident spots occupied. Only 22.1% (N=15) of residents/fellows were satisfied with their training programs. The majority (N=205 [70.9%]) of respondents would choose cardiac surgery as their specialty again. Most surgeons (N=129 [63.9%]) and residents/fellows (N=52 [76.5%]) indicated that the establishment of a LATAM cardiac surgery board examination would be beneficial. Conclusion: Modernization and standardization of training, as well as greater access to opportunities, may be required in LATAM to increase professional satisfaction of cardiac surgeons and to reduce disparities in the specialty. Such changes may enhance the regional response to the dynamic challenges in the feld.

Does receiving a medical education outside the United States impact a surgeon’s performance? We study this question by matching operations performed by internationally-trained surgeons to those performed by US-trained surgeons in reanalysis of a large health outcomes study. An effective matched design must achieve several goals, including balancing covariate distributions marginally, ensuring units within individual pairs have similar values on key covariates, and using a sufficiently large sample from the raw data. Yet in our study, optimizing some of these goals forces less desirable results on others. We address such tradeoffs from a multi-objective optimization perspective by creating matched designs that are Pareto optimal with respect to two goals. We provide general tools for generating representative subsets of Pareto optimal solution sets and articulate how they can be used to improve decision-making in observational study design. In the motivating surgical outcomes study, formulating a multi-objective version of the problem helps us balance an important variable without sacrificing two other design goals, average closeness of matched pairs on a multivariate distance and size of the final matched sample. BackgroundFrailty is frequently used by clinicians to help determine surgical outcomes. The frailty index, which represents the frequency of frailty indicators present in an individual, is one method for evaluating patient frailty to predict surgical outcomes. However, the frailty index treats all indicators of frailty that are used in the index as equivalent. Our hypothesis is that frailty indicators may be divided into groups of high and low-impact indicators and this separation will improve surgical discharge outcome prediction accuracy.Data and methodsPopulation data for inpatient elective operations was collected from the 2018 American College of Surgeons National Surgical Quality Improvement Program Participant Use Files. Artificial neural network (ANN) models trained using backpropagation are used to evaluate the relative accuracy for predicting surgical outcome of discharge destination using a traditional modified frailty index (mFI) or a new joint mFI that separates high-impact and low-impact indicators into distinct groups as input variables. Predictions are made across nine possible discharge destinations. A leave-one-out method is used to indicate the relative contribution of high and low-impact variables.ResultsExcept for the surgical specialty of cardiac surgery, the ANN model using distinct high and low-impact mFI indexes uniformly outperformed the ANN models using a single traditional mFI. Prediction accuracy improved from 3.4% to 28.1%. The leave-one-out experiment shows that except for the case of otolaryngology operations, the high-impact index indicators provided more support when determining surgical discharge destination outcomes.ConclusionFrailty indicators are not uniformly similar and should be treated differently in clinical outcome prediction systems.
