    Description

Note: This web page provides data on health facilities only. To file a complaint against a facility, please see: https://www.cdph.ca.gov/Programs/CHCQ/LCP/Pages/FileAComplaint.aspx

The California Department of Public Health (CDPH), Center for Health Care Quality, Licensing and Certification (L&C) Program licenses and certifies more than 30 types of healthcare facilities. The Electronic Licensing Management System (ELMS) is a CDPH data system created to manage state licensing-related data and enforcement actions. This file includes California healthcare facilities that are operational and have a current license issued by the CDPH and/or a current U.S. Department of Health and Human Services’ Centers for Medicare and Medicaid Services (CMS) certification.

To link the CDPH facility IDs with those from other Departments, like HCAI, please reference the "Licensed Facility Cross-Walk" Open Data table at https://data.chhs.ca.gov/dataset/licensed-facility-crosswalk. Facility geographic variables are updated monthly, if latitude/longitude information is missing at any point in time, it should be available when the next time the Open Data facility file is refreshed.

Please note that the file contains the data from ELMS as of the 11th business day of the month. See DATA_DATE variable for the specific date of when the data was extracted.

NHESP updates the data as needed, and the data served here is refreshed nightly by MassGIS.This service contains points for all vernal pools that have been certified by the Natural Heritage and Endangered Species Program (NHESP) according to the Guidelines for the Certification of Vernal Pool Habitat (PDF) (MA Division of Fisheries & Wildlife, 2009).

Vernal pools are small, shallow ponds characterized by lack of fish and by periods of dryness. Vernal pool habitat is extremely important to a variety of wildlife species including some amphibians that breed exclusively in vernal pools, and other organisms such as fairy shrimp, which spend their entire life cycles confined to vernal pool habitat. Many additional wildlife species utilize vernal pools for breeding, feeding and other important functions. Certified vernal pools are protected if they fall under the jurisdiction of the Massachusetts Wetlands Protection Act Regulations (310 CMR 10.00). However, the certification of a pool only establishes that it functions biologically as a vernal pool. Certification does not determine that the pool is within a resource area protected by the Wetlands Protection Act. Certified vernal pools are also afforded protection under the state Water Quality Certification regulations (401 Program), the state Title 5 regulations, and the Forest Cutting Practices Act regulations. Please see layer metadata for more details.Feature service also available.

    Description

This dataset depicts the boundaries of the Bureau of Land Management's (BLM's) National Monuments (NMs) and National Conservation Areas (NCAs) in Arizona as part of the National Landscape Conservation System (NLCS) data standards. The NLCS NM NCA dataset also depicts similar land designations such as Cooperative Management and Protection Areas, Forest Reserves, and Outstanding Natural areas. The NLCS NM NCA dataset is in progress as of April 30, 2019. Only the Gila Box Riparian NCA has completed the process of legal description, map creation, certification, and submittal to Congress. The remaining NMs and NCAs are awaiting the draft of legal descriptions. As legal descriptions are finalized and certified, the NLCS dataset may require updating to ensure that the spatial footprints of the NM and NCA boundary data match their respective legal descriptions. Once the boundaries are confirmed, no further changes to the NM/NCA boundary data should be made. Boundary changes can only be made through an amendment to the legal description and direct notification to Congress. The Antiquities Act of 1906 grants the President authority to designate national monuments in order to protect “objects of historic or scientific interest.” NCAs and similarly designated lands are designated by Congress to conserve, protect, enhance, and manage public lands for the benefit and enjoyment of present and future generations. In June 2000, the Bureau of Land Management (BLM) responded to growing concern over the loss of open space by creating the NLCS. The NLCS brings into a single system some of the BLM's premier designations. By putting these lands into an organized system, the BLM hopes to increase public awareness of the scientific, cultural, educational, ecological, and other values of these NM/NCA boundaries. The Public Law (P.L.) 101-628 established Gila Box Riparian NCA; P.L. 106-538 established the Las Cienegas NCA (also Sonoita Valley Acquisition Planning District), and P.L. 100-696 established San Pedro Riparian NCA. Each of these laws required the BLM to file boundary legal descriptions and maps to Congress for each NCA. As of April 30, 2019, only the Gila Box Riparian National Conservation Area has completed legal description, map depiction, certification, and submission to Congress. All other NM/NCA have gone through review by Arizona field offices. Draft maps have been completed and are awaiting legal descriptions to be drafted with the exception of the Grand Canyon-Parashant NM. All proposed boundaries are included in this dataset. When legal descriptions are finalized and certified, minor updates may be necessary to ensure that the geospatial depiction of the NM/NCA boundary data matches the legal descriptions, after which no further changes to the geospatial NM/NCA boundary data should be made. The standards, format and language for the legal descriptions and boundary maps were developed during regular meetings of the NLCS Coordinator, geospatial specialists and the Cadastral Surveyors regarding Arizona NLCS data. Guidance was provided from Congressional required maps and legal boundary descriptions for the NLCS Designation Manual 6120 (March, 2010). Established through Presidential Proclamation, there is no requirement for BLM to file boundary legal descriptions or maps with Congress for the National Monument. However, the NLCS Coordinator and Cadastral Survey Chief decided that it was prudent to extend the boundary process to NMs.

    Description

A listing of each accidental death associated with drug overdose in Connecticut from 2012 to June 2017. A "Y" value under the different substance columns indicates that particular substance was detected.

Data are derived from an investigation by the Office of the Chief Medical Examiner which includes the toxicity report, death certificate, as well as a scene investigation.

The “Morphine (Not Heroin)” values are related to the differences between how Morphine and Heroin are metabolized and therefor detected in the toxicity results. Heroin metabolizes to 6-MAM which then metabolizes to morphine. 6-MAM is unique to heroin, and has a short half-life (as does heroin itself). Thus, in some heroin deaths, the toxicity results will not indicate whether the morphine is from heroin or prescription morphine. In these cases the Medical Examiner may be able to determine the cause based on the scene investigation (such as finding heroin needles). If they find prescription morphine at the scene it is certified as “Morphine (not heroin).” Therefor, the Cause of Death may indicate Morphine, but the Heroin or Morphine (Not Heroin) may not be indicated.

“Any Opioid” – If the Medical Examiner cannot conclude whether it’s RX Morphine or heroin based morphine in the toxicity results, that column may be checked

    Description

This service identifies the locations of more than 29,000 potential, unverified, vernal pool habitats. Vernal pools are small, shallow ponds characterized by a lack of fish and annual or semi-annual periods of dryness. Vernal pool habitats are extremely important to a variety of wildlife species, including some amphibians that breed exclusively in vernal pools, and other organisms such as fairy shrimp which spend their entire life cycles confined to such locales.Potential vernal pools visible on aerial photographs were interpreted and included in this layer. However, this datalayer does not include every vernal pool in Massachusetts. Many vernal pools have not been identified due to unfavorable conditions in the landscape topography, pool physiography and/or photograph quality. Furthermore, vernal pool habitats occur in a wide variety of landscape settings, including forested swamps, bogs, and other wetlands. Vernal pools within these settings were not typically interpreted, but are nonetheless legitimate and valuable vernal pools. Also, field verification of all potential vernal pools in this study will identify errors such as the inclusion of features that are not actually vernal pools.Potential vernal pools identified in this survey are not to be confused with Certified Vernal Pools. Data pursuant to the official “Guidelines for the Certification of Vernal Pool Habitat” must be collected in the field and presented to the Massachusetts Natural Heritage & Endangered Species Program to obtain official certification for a vernal pool. Potential vernal pools identified in this survey do not receive protection under the Massachusetts Wetlands Protection Act Regulations (310 CMR 10.00), or under any other state or federal wetlands protection laws.More details...Map service also available.
