This is the current Medical Service Study Area. The Medical Service Study Area (MSSA) polygon layer represents the best fit mapping of all new 2020 California census tract boundaries to the original 2010 census tract boundaries used in the construction of the original 2010 MSSA file. Each of the state's new 9,129 census tracts was assigned to one of the previously established medical service study areas (excluding tracts with no land area), as identified in this data layer. The MSSA Census tract data is aggregated by HCAI, to create this MSSA data layer. This represents the final re-mapping of 2020 Census tracts to the original 2010 MSSA geometries. The 2010 MSSA were based on U.S. Census 2010 data and public meetings held throughout California.Source of update: American Community Survey 5-year 2006-2010 data for poverty. For source tables refer to InfoUSA update procedural documentation. The 2010 MSSA Detail layer was developed to update fields affected by population change. The American Community Survey 5-year 2006-2010 population data pertaining to total, in households, race, ethnicity, age, and poverty was used in the update. The 2010 MSSA Census Tract Detail map layer was developed to support geographic information systems (GIS) applications, representing 2010 census tract geography that is the foundation of 2010 medical service study area (MSSA) boundaries. ***This version is the finalized MSSA reconfiguration boundaries based on the US Census Bureau 2010 Census. In 1976 Garamendi Rural Health Services Act, required the development of a geographic framework for determining which parts of the state were rural and which were urban, and for determining which parts of counties and cities had adequate health care resources and which were "medically underserved". Thus, sub-city and sub-county geographic units called "medical service study areas [MSSAs]" were developed, using combinations of census-defined geographic units, established following General Rules promulgated by a statutory commission. After each subsequent census the MSSAs were revised. In the scheduled revisions that followed the 1990 census, community meetings of stakeholders (including county officials, and representatives of hospitals and community health centers) were held in larger metropolitan areas. The meetings were designed to develop consensus as how to draw the sub-city units so as to best display health care disparities. The importance of involving stakeholders was heightened in 1992 when the United States Department of Health and Human Services' Health and Resources Administration entered a formal agreement to recognize the state-determined MSSAs as "rational service areas" for federal recognition of "health professional shortage areas" and "medically underserved areas". After the 2000 census, two innovations transformed the process, and set the stage for GIS to emerge as a major factor in health care resource planning in California. First, the Office of Statewide Health Planning and Development [OSHPD], which organizes the community stakeholder meetings and provides the staff to administer the MSSAs, entered into an Enterprise GIS contract. Second, OSHPD authorized at least one community meeting to be held in each of the 58 counties, a significant number of which were wholly rural or frontier counties. For populous Los Angeles County, 11 community meetings were held. As a result, health resource data in California are collected and organized by 541 geographic units. The boundaries of these units were established by community healthcare experts, with the objective of maximizing their usefulness for needs assessment purposes. The most dramatic consequence was introducing a data simultaneously displayed in a GIS format. A two-person team, incorporating healthcare policy and GIS expertise, conducted the series of meetings, and supervised the development of the 2000-census configuration of the MSSAs.MSSA Configuration Guidelines (General Rules):- Each MSSA is composed of one or more complete census tracts.- As a general rule, MSSAs are deemed to be "rational service areas [RSAs]" for purposes of designating health professional shortage areas [HPSAs], medically underserved areas [MUAs] or medically underserved populations [MUPs].- MSSAs will not cross county lines.- To the extent practicable, all census-defined places within the MSSA are within 30 minutes travel time to the largest population center within the MSSA, except in those circumstances where meeting this criterion would require splitting a census tract.- To the extent practicable, areas that, standing alone, would meet both the definition of an MSSA and a Rural MSSA, should not be a part of an Urban MSSA.- Any Urban MSSA whose population exceeds 200,000 shall be divided into two or more Urban MSSA Subdivisions.- Urban MSSA Subdivisions should be within a population range of 75,000 to 125,000, but may not be smaller than five square miles in area. If removing any census tract on the perimeter of the Urban MSSA Subdivision would cause the area to fall below five square miles in area, then the population of the Urban MSSA may exceed 125,000. - To the extent practicable, Urban MSSA Subdivisions should reflect recognized community and neighborhood boundaries and take into account such demographic information as income level and ethnicity. Rural Definitions: A rural MSSA is an MSSA adopted by the Commission, which has a population density of less than 250 persons per square mile, and which has no census defined place within the area with a population in excess of 50,000. Only the population that is located within the MSSA is counted in determining the population of the census defined place. A frontier MSSA is a rural MSSA adopted by the Commission which has a population density of less than 11 persons per square mile. Any MSSA which is not a rural or frontier MSSA is an urban MSSA. Each day we assign one of five trends: Emergent, Spreading, Epidemic, Controlled, or End Stage to geographic areas to geographic areas based on the number of new cases, the number of active cases, the total population, and an algorithm (described below) that contextualize the most recent fourteen days with the overall COVID-19 case history. Currently we analyze the countries of the world and the U.S. Counties. The purpose is to give policymakers, citizens, and analysts a fact-based data driven sense for the direction each place is currently going. When a place has the initial cases, they are assigned Emergent, and if that place controls the rate of new cases, they can move directly to Controlled, and even to End Stage in a short time. However, if the reporting or measures to curtail spread are not adequate and significant numbers of new cases continue, they are assigned to Spreading, and in cases where the spread is clearly uncontrolled, Epidemic trend.We analyze the data reported by Johns Hopkins University to produce the trends, and we report the rates of cases, spikes of new cases, the number of days since the last reported case, and number of deaths. We also make adjustments to the assignments based on population so rural areas are not assigned trends based solely on case rates, which can be quite high relative to local populations.Two key factors are not consistently known or available and should be taken into consideration with the assigned trend. First is the amount of resources, e.g., hospital beds, physicians, etc.that are currently available in each area. Second is the number of recoveries, which are often not tested or reported. On the latter, we provide a probable number of active cases based on CDC guidance for the typical duration of mild to severe cases.Reasons for undertaking this work in March of 2020:The popular online maps and dashboards show counts of confirmed cases, deaths, and recoveries by country or administrative sub-region. Comparing the counts of one country to another can only provide a basis for comparison during the initial stages of the outbreak when counts were low and the number of local outbreaks in each country was low. By late March 2020, countries with small populations were being left out of the mainstream news because it was not easy to recognize they had high per capita rates of cases (Switzerland, Luxembourg, Iceland, etc.). Additionally, comparing countries that have had confirmed COVID-19 cases for high numbers of days to countries where the outbreak occurred recently is also a poor basis for comparison.The graphs of confirmed cases and daily increases in cases were fit into a standard size rectangle, though the Y-axis for one country had a maximum value of 50, and for another country 100,000, which potentially misled people interpreting the slope of the curve. Such misleading circumstances affected comparing large population countries to small population counties or countries with low numbers of cases to China which had a large count of cases in the early part of the outbreak. These challenges for interpreting and comparing these graphs represent work each reader must do based on their experience and ability. Thus, we felt it would be a service to attempt to automate the thought process experts would use when visually analyzing these graphs, particularly the most recent tail of the graph, and provide readers with an a resulting synthesis to characterize the state of the pandemic in that country, state, or county.The lack of reliable data for confirmed recoveries and therefore active cases. Merely subtracting deaths from total cases to arrive at this figure progressively loses accuracy after two weeks. The reason is 81% of cases recover after experiencing mild symptoms in 10 to 14 days. Severe cases are 14% and last 15-30 days (based on average days with symptoms of 11 when admitted to hospital plus 12 days median stay, and plus of one week to include a full range of severely affected people who recover). After reviewing epidemiological and pandemic scientific literature, three needs arose. We need to specify which portions of the pandemic lifecycle this map cover. The World Health Organization (WHO) specifies six phases. The source data for this map begins just after the beginning of Phase 5: human to human spread and encompasses Phase 6: pandemic phase. Phase six is only characterized in terms of pre- and post-peak. However, these two phases are after-the-fact analyses and cannot ascertained during the event. Instead, we describe (below) a series of five trends for Phase 6 of the COVID-19 pandemic.Choosing terms to describe the five trends was informed by the scientific literature, particularly the use of epidemic, which signifies uncontrolled spread. The five trends are: Emergent, Spreading, Epidemic, Controlled, and End Stage. Not every locale will experience all five, but all will experience at least three: emergent, controlled, and end stage.This layer presents the current trends for the COVID-19 pandemic by country (or appropriate level). There are five trends:Emergent: Early stages of outbreak. Spreading: Early stages and depending on an administrative area’s capacity, this may represent a manageable rate of spread. Epidemic: Uncontrolled spread. Controlled: Very low levels of new casesEnd Stage: No New cases These trends can be applied at several levels of administration: Local: Ex., City, District or County – a.k.a. Admin level 2State: Ex., State or Province – a.k.a. Admin level 1National: Country – a.k.a. Admin level 0Recommend that at least 100,000 persons be represented by a unit; granted this may not be possible, and then the case rate per 100,000 will become more important.Key Concepts and Basis for Methodology: 10 Total Cases minimum threshold: Empirically, there must be enough cases to constitute an outbreak. Ideally, this would be 5.0 per 100,000, but not every area has a population of 100,000 or more. Ten, or fewer, cases are also relatively less difficult to track and trace to sources. 21 Days of Cases minimum threshold: Empirically based on COVID-19 and would need to be adjusted for any other event. 21 days is also the minimum threshold for analyzing the “tail” of the new cases curve, providing seven cases as the basis for a likely trend (note that 21 days in the tail is preferred). This is the minimum needed to encompass the onset and duration of a normal case (5-7 days plus 10-14 days). Specifically, a median of 5.1 days incubation time, and 11.2 days for 97.5% of cases to incubate. This is also driven by pressure to understand trends and could easily be adjusted to 28 days. Source
United States Department of Agriculture Economic Research Service’s Food Access Research Atlas maps census tracts that are considered to be both low income and low access. The Atlas provides different ways to understand characteristics that can contribute to food deserts, including income level, distance to supermarkets, and vehicle access. The low access and distance measure extracted from the Food Access Research Atlas, and displayed on the Maryland Food System Map, is low income and low access measured at ½ mile and 10 miles. The Food Access Research Atlas defines this measure as being a low-income census tract with at least 500 people or 33 percent of the population living more than ½ mile (urban areas) or more than 10 miles (rural areas) from the nearest supermarket. A low-income census tract is defined as a having either a poverty rate of 42 percent or more, or a median family income less than 80 percent of the State-wide median family income; or a tract in a metropolitan area with a median family income less than 80 percent of the surrounding metropolitan area medium family income. A census tract is urban if its geographic centroid is in an area with more than 2,500 people. All other tracts are rural.

This map uses dot density patterns to indicate which population is larger in each area: urban (green) or rural (blue). Data is from the U.S. Census Bureau’s 2020 Census Demographic and Housing Characteristics. The map's layers contain total population counts by sex, age, and race groups for Nation, State, County, Census Tract, and Block Group in the United States and Puerto Rico.The U.S. Census designates each census block as part of an urban area or as rural. Larger geographies in this map such as block group, tract, county and state can therefore have a mix of urban and rural population. This map illustrates the 100% urban areas with all green dots, and 100% rural areas in dark blue dots. Areas with mixed urban/rural population have a proportional mix of green and blue dots to give a visual indication of where change may be happening. The Census Bureau’s urban areas represent densely developed territory, and encompass residential, commercial, and other non-residential urban land uses. Rural encompasses all population, housing, and territory not included within an urban area.For the 2020 Census, an urban area will comprise a densely settled core of census blocks that meet minimum housing unit density and/or population density requirements. This includes adjacent territory containing non-residential urban land uses. To qualify as an urban area, the territory identified according to criteria must encompass at least 2,000 housing units or have a population of at least 5,000." Data Processing Notes: These 2020 Census boundaries come from the US Census TIGER geodatabases. These are Census boundaries with water and/or coastlines erased for cartographic and mapping purposes. For Census tracts and block groups, the water cutouts are derived from a subset of the 2020 Areal Hydrography boundaries offered by TIGER. Water bodies and rivers which are 50 million square meters or larger (mid to large sized water bodies) are erased from the tract and block group boundaries, as well as additional important features. For state and county boundaries, the water and coastlines are derived from the coastlines of the 2020 500k TIGER Cartographic Boundary Shapefiles. These are erased to more accurately portray the coastlines and Great Lakes. The original AWATER and ALAND fields are unchanged and available as attributes within the data table (units are square meters). The layer contains all US states, Washington D.C., and Puerto Rico. Census tracts with no population that occur in areas of water, such as oceans, are removed from this data service (Census Tracts beginning with 99). Block groups that fall within the same criteria (Block Group denoted as 0 with no area land) have also been removed.Percentages and derived counts, are calculated values (that can be identified by the "_calc_" stub in the field name). Field alias names were created based on the Table Shells file available from the Data Table Guide for the Demographic Profile and Demographic and Housing Characteristics. Not all lines of all tables listed above are included in this layer. Duplicative counts were dropped. For example, P0030001 was dropped, as it is duplicative of P0010001.To protect the privacy and confidentiality of respondents, their data has been protected using differential privacy techniques by the U.S. Census Bureau.
