    This is the fourth round of the physician survey component of the Community Tracking Study (CTS). The first round was conducted in 1996-1997 (ICPSR 2597), the second round in 1998-1999 (ICPSR 3267), and the third in 2000-2001 (ICPSR 3820). Sponsored by the Robert Wood Johnson Foundation, the CTS is a large-scale investigation of changes in the American health care system and their effects on people. As in the previous rounds, physicians were sampled in the 60 CTS sites: 51 metropolitan and 9 nonmetropolitan areas that were randomly selected to form the core of the CTS and to be representative of the nation as a whole. However, the fourth round lacks an independent supplemental national sample of physicians, which augmented the CTS site sample in the previous rounds. Information collected by the survey includes net income from the practice of medicine, year of birth, sex, race, Hispanic origin, year of graduation from medical school, specialty, board certification status, compensation model, patient mix (e.g., race/Hispanic origin of patients and percent with chronic conditions), career satisfaction, practice type, size, and ownership, percent of practice revenue from Medicare, Medicaid, or managed care, acceptance of new Medicaid and Medicare patients and, if applicable, reasons for not accepting them, use of information technology for care management, number of patient visits and hours worked in medically related activities during the last complete week of work, and the number of hours spent providing charity care in the last month. In addition, the survey elicited views on a number of issues such as patient-physician interactions, competition among practices, the influence of financial incentives on the quantity of services provided to patients, trends in the amount and quality of nursing support, one's ability to provide quality care and obtain needed services for patients, and the importance of various factors that may limit the quality of care. Part 3, the Site and County Crosswalk Data File, identifies the counties that constitute each CTS site. Part 4, Physician Survey Summary File, contains site-level estimates and standard errors for selected physician characteristics, e.g., the average age of physicians, the average percentage of patients with a formulary, and the percentage of physicians who said medical errors in hospitals are a minor problem.

    Description

    BackgroundPhysicians are expected to serve as role models for healthy lifestyles, but long work hours reduce time for healthy behaviors. A hospital-based physical activity intervention could improve physician health and increase counseling about exercise.MethodsWe conducted a two-phase intervention among 104 medical residents at a large hospital in Boston, Massachusetts. Phase 1 was a 6-week randomized controlled trial comparing daily steps of residents assigned to an activity monitor displaying feedback about steps and energy consumed (intervention) or to a blinded monitor (control). Phase 2 immediately followed and was a 6-week non-randomized team steps competition in which all participants wore monitors with feedback. Phase 1 outcomes were: 1) median steps/day and 2) proportion of days activity monitor worn. The Phase 2 outcome was mean steps/day on days monitor worn (≥500 steps/day). Physiologic measurements were collected at baseline and study end. Median steps/day were compared using Wilcoxon rank-sum tests. Mean steps were compared using repeated measures regression analyses.ResultsIn Phase 1, intervention and control groups had similar activity (6369 vs. 6063 steps/day, p = 0.16) and compliance with wearing the monitor (77% vs. 77% of days, p = 0.73).     BackgroundIn the last 2 weeks of January 2021, Portugal was the worst country in the world in incidence of infections and deaths due to COVID-19. As a result, the pressure on the healthcare system increased exponentially, exceeding its capacities and leaving hospitals in near collapse. This scenario caused multiple constraints, particularly for hospital medical staff. Previous studies conducted at different moments during the pandemic reported that COVID-19 has had significant negative impacts on healthcare workers’ psychological health, including stress, anxiety, depression, burnout, post-traumatic stress symptoms, and sleep disturbances. However, there are many uncertainties regarding the professional quality of life of hospital nurses and physicians. To address gaps in previous research on secondary traumatic stress, we focused on healthcare workers working in hospitals affected by a major traumatic event: the third wave of COVID-19.ObjectivesThe aim of the present study was to identify the contribution of personal and work-related contextual variables (gender, age, parental status, occupation, years of experience, working with patients affected by COVID-19) on professional quality of life of healthcare workers.MethodsCross-sectional study with a web-based questionnaire given to physicians and nurses working in a hospital setting. A total of 853 healthcare professionals (276 physicians and 586 nurses; median age 37 years old) participated in the survey assessing professional quality of life compassion satisfaction, secondary traumatic stress, and burnout. Factors of professional quality of life were assessed using regression analysis.ResultsMost of the participants showed moderate (80%; n = 684) or high (18%; n = 155) levels of compassion satisfaction, whereas the majority of them experienced moderate levels of burnout (72%; n = 613) and secondary traumatic stress (69%; n = 592). The analyzed variables demonstrated no differences between professionals who were directly or not involved in the care of COVID-19 patients. Parental status was found to be a significant factor in compassion satisfaction. Female gender was significantly associated with more susceptibility to secondary traumatization. Factors that may potentially contribute to burnout include years of professional experience and the number of work hours per week.ConclusionThe COVID-19 pandemic has created a new challenge for the healthcare system. Burnout and secondary traumatic stress can lead to medical errors and impact standards of patient care, particularly compromising compassionate care. It is therefore recommended that hospitals develop psychoeducational initiatives to support professionals in dealing with barriers to compassion.

According to this survey, most U.S. physicians work on average 50 to 59 hours per week in 2021, a significantly higher number of hours than the traditional American workweek of 40. Moreover, the distribution of doctors per number of hours worked weekly reveals that a third of physicians work over 60 hours a week, of which 7.7 percent work 80 hours or more.
