    The National Survey of Children’s Health (NSCH) is sponsored by the Maternal and Child Health Bureau of the Health Resources and Services Administration, an Agency in the U.S. Department of Health and Human Services.The NSCH examines the physical and emotional health of children ages 0-17 years of age. Special emphasis is placed on factors related to the well-being of children. These factors include access to - and quality of - health care, family interactions, parental health, neighborhood characteristics, as well as school and after-school experiences.The NSCH is also designed to assess the prevalence and impact of special health care needs among children in the US and explores the extent to which children with special health care needs (CSHCN) have medical homes, adequate health insurance, access to needed services, and adequate care coordination. Other topics may include functional difficulties, transition services, shared decision-making, and satisfaction with care. Information is collected from parents or caregivers who know about the child's health.

    The National Survey of Children's Health, funded by the Maternal and Child Health Bureau (MCHB), is a module of the State and Local Area Integrated Telephone Survey (SLAITS) that is conducted by the National Center for Health Statistics (NCHS) at the Centers for Disease Control and Prevention (CDC). The survey was conducted to assess how well each state, and the nation as a whole, met MCHB's strategic plan goals and national performance measures. These goals include providing national leadership for maternal and child health, promoting an environment that supports maternal and child health, eliminating health barriers and disparities, improving the health infrastructure and systems of care, assuring quality care, working with states and communities to plan and implement policies and programs to improve the social, emotional, and physical environment, and acquiring the best available evidence to develop and promote guidelines and practices to assure a social, emotional, and physical environment that supports the health and well-being of women and children. The National Survey of Children's Health (NSCH) was designed to produce national- and state-specific prevalence estimates for a variety of physical, emotional, and behavioral health indicators and measures of children's experiences with the health care system. Respondents were asked an extensive battery of questions about the family, including parental health, stress and coping behaviors, family activities, and parental concerns about their children, as well as their perceptions of the child's neighborhood. Demographic information includes race, gender, family income, and education level.

Approximate sample size is 95,000 over two rounds of data collection. Data were suppressed for cell sizes less than 30 persons, or where the relative standard error more than 30% of the mean.     This follow-up to the NATIONAL MATERNAL AND INFANT HEALTH SURVEY, 1988 (NMIHS) (ICPSR 9730) consists of three components that provide information on early childhood morbidity and health. The Live Birth Survey (Part 1) obtained data on national health issues affecting children, such as child development, effects of low birth weight, childhood injury, child care, pediatric care, health insurance coverage, child safety, and acute and chronic childhood illnesses. For the Medical Provider Survey (Part 12), respondents to the Live Birth Survey were asked to provide the names of all medical providers and hospitals where their children were diagnosed, treated, and/or admitted. Each health care provider was asked to supply information on its organization, the child's health status and history, and each visit or hospitalization. The Fetal and Infant Death Survey (Part 21) interviewed women who were identified through the 1988 NMIHS as having lost a fetus or an infant during the study period. These respondents were reinterviewed to gather information about their health and about any pregnancies since their loss in 1988. The 1991 follow-up data can be merged with data from the 1988 NMIHS, which was designed to explore factors that cause negative pregnancy outcomes.

    Abstract

    The Tanzania Demographic and Health Survey (TDHS) is part of the worldwide Demographic and Health Surveys (DHS) programme, which is designed to collect data on fertility, family planning, and maternal and child health.

    The primary objective of the 1999 TRCHS was to collect data at the national level (with breakdowns by urban-rural and Mainland-Zanzibar residence wherever warranted) on fertility levels and preferences, family planning use, maternal and child health, breastfeeding practices, nutritional status of young children, childhood mortality levels, knowledge and behaviour regarding HIV/AIDS, and the availability of specific health services within the community.1 Related objectives were to produce these results in a timely manner and to ensure that the data were disseminated to a wide audience of potential users in governmental and nongovernmental organisations within and outside Tanzania. The ultimate intent is to use the information to evaluate current programmes and to design new strategies for improving health and family planning services for the people of Tanzania.

    The TRCHS used a three-stage sample design. Overall, 176 census enumeration areas were selected (146 on the Mainland and 30 in Zanzibar) with probability proportional to size on an approximately self-weighting basis on the Mainland, but with oversampling of urban areas and Zanzibar. To reduce costs and maximise the ability to identify trends over time, these enumeration areas were selected from the 357 sample points that were used in the 1996 TDHS, which in turn were selected from the 1988 census frame of enumeration in a two-stage process (first wards/branches and then enumeration areas within wards/branches). Before the data collection, fieldwork teams visited the selected enumeration areas to list all the households. From these lists, households were selected to be interviewed. The sample was designed to provide estimates for the whole country, for urban and rural areas separately, and for Zanzibar and, in some cases, Unguja and Pemba separately. The health facilities component of the TRCHS involved visiting hospitals, health centres, and pharmacies located in areas around the households interviewed. In this way, the data from the two components can be linked and a richer dataset produced.

    Mode of data collection

    Research instrument

    The household survey component of the TRCHS involved three questionnaires: 1) a Household Questionnaire, 2) a Women’s Questionnaire for all individual women age 15-49 in the selected households, and 3) a Men’s Questionnaire for all men age 15-59.

    The health facilities survey involved six questionnaires: 1) a Community Questionnaire administered to men and women in each selected enumeration area; 2) a Facility Questionnaire; 3) a Facility Inventory; 4) a Service Provider Questionnaire; 5) a Pharmacy Inventory Questionnaire; and 6) a questionnaire for the District Medical Officers.

    All these instruments were based on model questionnaires developed for the MEASURE programme, as well as on the questionnaires used in the 1991-92 TDHS, the 1994 TKAP, and the 1996 TDHS. These model questionnaires were adapted for use in Tanzania during meetings with representatives from the Ministry of Health, the University of Dar es Salaam, the Tanzania Food and Nutrition Centre, USAID/Tanzania, UNICEF/Tanzania, UNFPA/Tanzania, and other potential data users. The questionnaires and manual were developed in English and then translated into and printed in Kiswahili.

    The Household Questionnaire was used to list all the usual members and visitors in the selected households. Some basic information was collected on the characteristics of each person listed, including his/her age, sex, education, and relationship to the head of the household. The main purpose of the Household Questionnaire was to identify women and men who were eligible for individual interview and children under five who were to be weighed and measured. Information was also collected about the dwelling itself, such as the source of water, type of toilet facilities, materials used to construct the house, ownership of various consumer goods, and use of iodised salt. Finally, the Household Questionnaire was used to collect some rudimentary information about the extent of child labour.

    The Women’s Questionnaire was used to collect information from women age 15-49. These women were asked questions on the following topics: · Background characteristics (age, education, religion, type of employment) · Birth history · Knowledge and use of family planning methods · Antenatal, delivery, and postnatal care · Breastfeeding and weaning practices · Vaccinations, birth registration, and health of children under age five · Marriage and recent sexual activity · Fertility preferences · Knowledge and behaviour concerning HIV/AIDS.

    The Men’s Questionnaire covered most of these same issues, except that it omitted the sections on the detailed reproductive history, maternal health, and child health. The final versions of the English questionnaires are provided in Appendix E.

    Before the questionnaires could be finalised, a pretest was done in July 1999 in Kibaha District to assess the viability of the questions, the flow and logical sequence of the skip pattern, and the field organisation. Modifications to the questionnaires, including wording and translations, were made based on lessons drawn from the exercise.

    Response rate

    In all, 3,826 households were selected for the sample, out of which 3,677 were occupied. Of the households found, 3,615 were interviewed, representing a response rate of 98 percent. The shortfall is primarily due to dwellings that were vacant or in which the inhabitants were not at home despite of several callbacks.

    In the interviewed households, a total of 4,118 eligible women (i.e., women age 15-49) were identified for the individual interview, and 4,029 women were actually interviewed, yielding a response rate of 98 percent. A total of 3,792 eligible men (i.e., men age 15-59), were identified for the individual interview, of whom 3,542 were interviewed, representing a response rate of 93 percent. The principal reason for nonresponse among both eligible men and women was the failure to find them at home despite repeated visits to the household. The lower response rate among men than women was due to the more frequent and longer absences of men.

    The response rates are lower in urban areas due to longer absence of respondents from their homes. One-member households are more common in urban areas and are more difficult to interview because they keep their houses locked most of the time. In urban settings, neighbours often do not know the whereabouts of such people.

    Sampling error estimates

    The estimates from a sample survey are affected by two types of errors: (1) non-sampling errors, and (2) sampling errors. Non-sampling errors are the results of mistakes made in implementing data collection and data processing, such as failure to locate and interview the correct household, misunderstanding of the questions on the part of either the interviewer or the respondent, and data entry errors. Although numerous efforts were made during the implementation of the TRCHS to minimise this type of error, nonsampling errors are impossible to avoid and difficult to evaluate statistically.

    Sampling errors, on the other hand, can be evaluated statistically. The sample of respondents selected in the TRCHS is only one of many samples that could have been selected from the same population, using the same design and expected size. Each of these samples would yield results that differ somewhat from the results of the actual sample selected. Sampling errors are a measure of the variability between all possible samples. Although the degree of variability is not known exactly, it can be estimated from the survey results.

    A sampling error is usually measured in terms of the standard error for a particular statistic (mean, percentage, etc.), which is the square root of the variance. The standard error can be used to calculate confidence intervals within which the true value for the population can reasonably be assumed to fall. For example, for any given statistic calculated from a sample survey, the value of that statistic will fall within a range of plus or minus two times the standard error of that statistic in 95 percent of all possible samples of identical size and design.

    If the sample of respondents had been selected as a simple random sample, it would have been possible to use straightforward formulas for calculating sampling errors. However, the TRCHS sample is the result of a two-stage stratified design, and, consequently, it was necessary to use more complex formulae. The computer software used to calculate sampling errors for the TRCHS is the ISSA Sampling Error Module (SAMPERR). This module used the Taylor linearisation method of variance estimation for survey estimates that are means or proportions. The Jackknife repeated replication method is used for variance estimation of more complex statistics such as fertility and mortality rate

    Abstract

    The National Family Health Survey 2019-21 (NFHS-5), the fifth in the NFHS series, provides information on population, health, and nutrition for India, each state/union territory (UT), and for 707 districts.

    The primary objective of the 2019-21 round of National Family Health Surveys is to provide essential data on health and family welfare, as well as data on emerging issues in these areas, such as levels of fertility, infant and child mortality, maternal and child health, and other health and family welfare indicators by background characteristics at the national and state levels. Similar to NFHS-4, NFHS-5 also provides information on several emerging issues including perinatal mortality, high-risk sexual behaviour, safe injections, tuberculosis, noncommunicable diseases, and the use of emergency contraception.

    The information collected through NFHS-5 is intended to assist policymakers and programme managers in setting benchmarks and examining progress over time in India’s health sector. Besides providing evidence on the effectiveness of ongoing programmes, NFHS-5 data will help to identify the need for new programmes in specific health areas.

    The clinical, anthropometric, and biochemical (CAB) component of NFHS-5 is designed to provide vital estimates of the prevalence of malnutrition, anaemia, hypertension, high blood glucose levels, and waist and hip circumference, Vitamin D3, HbA1c, and malaria parasites through a series of biomarker tests and measurements.

    Sampling procedure

    A uniform sample design, which is representative at the national, state/union territory, and district level, was adopted in each round of the survey. Each district is stratified into urban and rural areas. Each rural stratum is sub-stratified into smaller substrata which are created considering the village population and the percentage of the population belonging to scheduled castes and scheduled tribes (SC/ST). Within each explicit rural sampling stratum, a sample of villages was selected as Primary Sampling Units (PSUs); before the PSU selection, PSUs were sorted according to the literacy rate of women age 6+ years. Within each urban sampling stratum, a sample of Census Enumeration Blocks (CEBs) was selected as PSUs. Before the PSU selection, PSUs were sorted according to the percentage of SC/ST population. In the second stage of selection, a fixed number of 22 households per cluster was selected with an equal probability systematic selection from a newly created list of households in the selected PSUs. The list of households was created as a result of the mapping and household listing operation conducted in each selected PSU before the household selection in the second stage. In all, 30,456 Primary Sampling Units (PSUs) were selected across the country in NFHS-5 drawn from 707 districts as on March 31st 2017, of which fieldwork was completed in 30,198 PSUs.

    Four survey schedules/questionnaires: Household, Woman, Man, and Biomarker were canvassed in 18 local languages using Computer Assisted Personal Interviewing (CAPI).

    Cleaning operations

    Electronic data collected in the 2019-21 National Family Health Survey were received on a daily basis via the SyncCloud system at the International Institute for Population Sciences, where the data were stored on a password-protected computer. Secondary editing of the data, which required resolution of computer-identified inconsistencies and coding of open-ended questions, was conducted in the field by the Field Agencies and at the Field Agencies central office, and IIPS checked the secondary edits before the dataset was finalized.

    Field-check tables were produced by IIPS and the Field Agencies on a regular basis to identify certain types of errors that might have occurred in eliciting information and recording question responses. Information from the field-check tables on the performance of each fieldwork team and individual investigator was promptly shared with the Field Agencies during the fieldwork so that the performance of the teams could be improved, if required.

    Response rate

    A total of 664,972 households were selected for the sample, of which 653,144 were occupied. Among the occupied households, 636,699 were successfully interviewed, for a response rate of 98 percent.

    In the interviewed households, 747,176 eligible women age 15-49 were identified for individual women’s interviews. Interviews were completed with 724,115 women, for a response rate of 97 percent. In all, there were 111,179 eligible men age 15-54 in households selected for the state module. Interviews were completed with 101,839 men, for a response rate of 92 percent.

    Child Dental Health Survey 2013, England, Wales and Northern Ireland The 2013 Children's Dental Health (CDH) Survey, commissioned by the Health and Social Care Information Centre, is the fifth in a series of national children's dental health surveys that have been carried out every ten years since 1973. The 2013 survey provides statistical estimates on the dental health of 5, 8, 12 and 15 year old children in England, Wales and Northern Ireland, using data collected during dental examinations conducted in schools on a random sample of children by NHS dentists and nurses. The survey measures changes in oral health since the last survey in 2003, and provides information on the distribution and severity of oral diseases and conditions in 2013. The survey oversampled schools with high rates of free school meal eligibility to enable comparison of children from lower income families* (children eligible for free school meals in 2013) with other children of the same age, in terms of their oral health, and related perceptions and behaviours*. The 2013 survey dental examination was extended so that tooth decay (dental caries) could be measured across a range of detection thresholds. This reflects the way in which the detection and management of tooth decay has evolved towards more preventive approaches to care, rather than just providing treatment for disease. This survey provides estimates for dental decay across the continuum of caries, including both restorative and preventive care needs*. Complementary information on the children's experiences, perceptions and behaviours relevant to their oral health was collected from parents and 12 and 15 year old children using self-completion questionnaires.     The goal of this study was to test specific hypotheses illustrating the relationships among serious victimization experiences, the mental health effects of victimization, substance abuse/use, and delinquent behavior in adolescents. The study assessed familial and nonfamilial types of violence. It was designed as a telephone survey of American youth aged 12-17 living in United States households and residing with a parent or guardian. One parent or guardian in each household was interviewed briefly to establish rapport, secure permission to interview the targeted adolescent, and to ensure the collection of comparative data to examine potential nonresponse bias from households without adolescent participation. All interviews with both parents and adolescents were conducted using Computer-Assisted Telephone Interviewing (CATI) technology. From the surveys of parents and adolescents, the principal investigators created one data file by attaching the data from the parents to the records of their respective adolescents. Adolescents were asked whether violence and drug abuse were problems in their schools and communities and what types of violence they had personally witnessed. They were also asked about other stressful events in their lives, such as the loss of a family member, divorce, unemployment, moving to a new home or school, serious illness or injury, and natural disaster. Questions regarding history of sexual assault, physical assault, and harsh physical discipline elicited a description of the event and perpetrator, extent of injuries, age at abuse, whether alcohol or drugs were involved, and who was informed of the incident. Information was also gathered on the delinquent behavior of respondents and their friends, including destruction of property, assault, theft, sexual assault, and gang activity. Other questions covered history of personal and family substance use and mental health indicators, such as major depression, post-traumatic stress disorders, weight changes, sleeping disorders, and problems concentrating. Demographic information was gathered from the adolescents on age, race, gender, number of people living in household, and grade in school. Parents were asked whether they were concerned about violent crime, affordable child care, drug abuse, educational quality, gangs, and the safety of their children at school. In addition, they were questioned about their own victimization experiences and whether they discussed personal safety issues with their children. Parents also supplied demographic information on gender, marital status, number of children, employment status, education, race, and income.

    The 1992 Malawi Demographic and Health Survey (MDHS) was a nationally representative sample survey designed to provide information on levels and trends in fertility, early childhood mortality and morbidity, family planning knowledge and use, and maternal and child health. The survey was implemented by the National Statistical Office during September to November 1992. In 5323 households, 4849 women age 15-49 years and 1151 men age 20-54 years were interviewed. The Malawi Demographic and Health Survey (MDHS) was a national sample survey of women and men of reproductive age designed to provide, among other things, information on fertility, family planning, child survival, and health of mothers and children. Specifically, the main objectives of the survey were to: Collect up-to-date information on fertility, infant and child mortality, and family planning Collect information on health-related matters, including breastleeding, antenatal and maternity services, vaccinations, and childhood diseases and treatment Assess the nutritional status of mothers and children Collect information on knowledge and attitudes regarding AIDS Collect information suitable for the estimation of mortality related to pregnancy and childbearing Assess the availability of health and family planning services. MAIN FINDINGS The findings indicate that fertility in Malawi has been declining over the last decade; at current levels a woman will give birth to an average of 6.7 children during her lifetime. Fertility in rural areas is 6.9 children per woman compared to 5.5 children in urban areas. Fertility is higher in the Central Region (7.4 children per woman) than in the Northem Region (6.7) or Southern Region (6.2). Over the last decade, the average age at which a woman first gives birth has risen slightly over the last decade from 18.3 to 18.9 years. Still, over one third of women currently under 20 years of age have either already given birlh to at least one child or are currently pregnant. Although 58 percent of currently married women would like to have another child, only 19 percent want one within the next two years. Thirty-seven percent would prefer to walt two or more years. Nearly one quarter of married women want no more children than they already have. Thus, a majority of women (61 percent) want either to delay their next birth or end childbearing altogether. This represents the proportion of women who are potentially in need of family planning. Women reported an average ideal family size of 5.7 children (i.e., wanted fertility), one child less than the actual fertility level measured in the surveyfurther evidence of the need for family planning methods. Knowledge of contraceptive methods is high among all age groups and socioeconomic strata of women and men. Most women and men also know of a source to obtain a contraceptive method, although this varies by the type of method. The contraceptive pill is the most commonly cited method known by women; men are most familiar with condoms. Despite widespread knowledge of family planning, current use of contraception remains quite low. Only 7 percent of currently married women were using a modem method and another 6 percent were using a traditional method of family planning at the time of the survey. This does, however, represent an increase in the contraceptive prevalence rate (modem methods) from about 1 percent estimated from data collected in the 1984 Family Formation Survey. The modem methods most commonly used by women are the pill (2.2 percent), female sterilisation (1.7 percent), condoms (1.7 percent), and injections (1.5 percent). Men reported higher rates of contraceptive use (13 percent use of modem methods) than women. However, when comparing method-specific use rates, nearly all of the difference in use between men and women is explained by much higher condom use among men. Early childhood mortality remains high in Malawi; the under-five mortality rate currently stands at 234 deaths per 1000 live births. The infant mortality rate was estimated at 134 per 10130 live births. This means that nearly one in seven children dies before his first birthday, and nearly one in four children does not reach his fifth birthday. The probability of child death is linked to several factors, most strikingly, low levels of maternal education and short intervals between births. Children of uneducated women are twice as likely to die in the first five years of life as children of women with a secondary education. Similarly, the probablity of under-five mortality for children with a previous birth interval of less than 2 years is two times greater than for children with a birth interval of 4 or more years. Children living in rural areas have a higher rate ofunder-fwe mortality than urban children, and children in the Central Region have higher mortality than their counterparts in the Northem and Southem Regions. Data were collected that allow estimation ofmatemalmortality. It is estimated that for every 100,000 live births, 620 women die due to causes related to pregnancy and childbearing. The height and weight of children under five years old and their mothers were collected in the survey. The results show that nearly one half of children under age five are stunted, i.e., too short for their age; about half of these are severely stunted. By age 3, two-thirds of children are stunted. As with childhood mortality, chronic undernutrition is more common in rural areas and among children of uneducated women. The duration of breastfeeding is relatively long in Malawi (median length, 21 months), but supplemental liquids and foods are introduced at an early age. By age 2-3 months, 76 percent of children are already receiving supplements. Mothers were asked to report on recent episodes of illness among their young children. The results indicate that children age 6-23 months are the most vulnerable to fever, acute respiratory infection (ARI), and diarrhea. Over half of the children in this age group were reported to have had a fever, about 40 percent had a bout with diarrhea, and 20 percent had symptoms indicating ARI in the two-week period before the survey. Less than half of recently sick children had been taken to a health facility for treatment. Sixty-three percent of children with diarrhea were given rehydration therapy, using either prepackaged rehydration salts or a home-based preparation. However, one quarter of children with diarrhea received less fluid than normal during the illness, and for 17 percent of children still being breastfed, breastfeeding of the sick child was reduced. Use of basic, preventive maternal and child health services is generally high. For 90 percent of recent births, mothers had received antenatal care from a trained medical person, most commonly a nurse or trained midwife. For 86 percent of births, mothers had received at least one dose of tetanus toxoid during pregnancy. Over half of recent births were delivered in a health facility. Child vaccination coverage is high; 82 percent of children age 12-23 months had received the full complement of recommended vaccines, 67 percent by exact age 12 months. BCG coverage and first dose coverage for DPT and polio vaccine were 97 percent. However, 9 percent of children age 12-23 months who received the first doses of DPT and polio vaccine failed to eventually receive the recommended third doses. Information was collected on knowledge and attitudes regarding AIDS. General knowledge of AIDS is nearly universal in Malawi; 98 percent of men and 95 percent of women said they had heard of AIDS. Further, the vast majority of men and women know that the disease is transmitted through sexual intercourse. Men tended to know more different ways of disease transmission than women, and were more likely to mention condom use as a means to prevent spread of AIDS. Women, especially those living in rural areas, are more likely to hold misconceptions about modes of disease transmission. Thirty percent of rural women believe that AIDS can not be prevented.

    The Yemen Demographic and Maternal and Child Health Survey (YDMCHS) is the first national survey conducted in Yemen since unification of the country. It was designed to collect data on households, ever-married women of reproductive age, and children under age five. The subjects covered in the household survey were: characteristics of households, housing and living conditions, school enrollment, labor force participation, general mortality, disability, fertility, and child survival. The areas covered in the survey of women of reproductive age were: demographic and socioeconomic characteristics, marriage and reproductive history, fertility regulation and preferences, antenatal care, breastfeeding, and child care. For children under five in the survey, the topics included diarrheal and other morbidity, nutritional supplementation, accidents, vaccination, and nutritional status. The survey was carried out as a part of the DHS program and also the PAPCHILD program. The DHS program is assisting governments and private agencies in the implementation of household surveys in developing countries; PAPCHILD has similar goals for developing countries in the Arab League. The main objectives of the DHS project are to: (a) provide decision makers with a data base and analyses useful for informed policy choices, (b) expand the international population and health data base, (c) advance survey methodology, and (d) develop skills and resources necessary to conduct high quality demographic and health surveys in the participating countries. The YDMCHS was specifically aimed at furnishing information on basic population and household characteristics, maternal and child health, fertility, family planning, and infant and child mortality in Yemen. The survey also presents information on breastfeeding practices and the nutritional status of children under age five. The survey will provide policymakers and planners with important information for use in formulating programs and policies regarding maternal and child health, child mortality, and reproductive behavior.

    The Children’s Dental Health (CDH) Survey series is a set of national surveys of children’s dental health that have been carried out every 10 years since 1973. Since its inception, the survey has provided important information to underpin the development and monitoring of dental health care for children.

    The 1973 survey established baseline information on the state of the dental health of children in England and Wales. The survey coverage was then extended in 1983 to include Scotland and Northern Ireland. The 2013 survey covers England, Wales and Northern Ireland. Description

The National Survey of Children’s Health (NSCH) is sponsored by the Maternal and Child Health Bureau of the Health Resources and Services Administration, an Agency in the U.S. Department of Health and Human Services.The NSCH examines the physical and emotional health of children ages 0-17 years of age. Special emphasis is placed on factors related to the well-being of children. These factors include access to - and quality of - health care, family interactions, parental health, neighborhood characteristics, as well as school and after-school experiences.The NSCH is also designed to assess the prevalence and impact of special health care needs among children in the US and explores the extent to which children with special health care needs (CSHCN) have medical homes, adequate health insurance, access to needed services, and adequate care coordination. Other topics may include functional difficulties, transition services, shared decision-making, and satisfaction with care. Information is collected from parents or caregivers who know about the child's health.
