We are interested in what models exist to provide guidance on how researchers engage patients in research. We are conducting an interpretive synthesis of the literature to understand what is being said about patient engagement and what types of frameworks, guidelines, tools, and models exist to guide and support patient engagement. Research Questions: What frameworks, models, tools, and guidelines exist to guide researchers in engaging patients in research? What are the assumptions around patient engagement underlying these frameworks? Specifically, what are the assumptions around the value of patient engagement, who is considered a patient, the role of patients on a research team, and the outcomes of patient engagement? In formulating our research questions, we draw on work by Dixon-Woods et al. (2012) in their development of a rapid narrative review and synthesis of the literature. The authors treated the research question as a compass, not an anchor, such that the questions are open to being refined as the review proceeds. In orienting to the research questions in this manner, the authors reviewed the literature in an iterative, intuitive manner in combination with more formal systematic search techniques, along with a focus on selective, judicious sampling and the integration of various literatures. We adopt a similar approach in this review. While we first began with a broad interest in existing frameworks to guide patient engagement, throughout the process of data collection and analysis, we continued to refine our research questions and integrate related literatures as relevant.

Introduction: Current health care delivery relies on complex, computer-generated risk models constructed from insurance claims and medical record data. However, these models produce inaccurate predictions of risk levels for individual patients, do not explicitly guide care, and undermine health management investments in many patients at lesser risk. Therefore, this study prospectively validates a concise patient-reported risk assessment that addresses these inadequacies of computer-generated risk models. Methods: Five measures with well-documented impacts on the use of health services are summed to create a "What Matters Index." These measures are: 1) insufficient confidence to self-manage health problems, 2) pain, 3) bothersome emotions, 4) polypharmacy, and 5) adverse medication effects. We compare the sensitivity and predictive values of this index with two representative risk models in a population of 8619 Medicaid recipients. Results: The patient-reported "What Matters Index" and the conventional risk models are found to exhibit similar sensitivities and predictive values for subsequent hospital or emergency room use. Furthermore, the "What Matters Index" is also reliable: akin to its performance during development, for patients with index scores of 1, 2, and ≥3, the odds ratios (with 95% confidence intervals) for subsequent hospitalization within 1 year, relative to patients with a score of 0, are 1.3 (1.1-1.6), 2.0 (1.6-2.4), and 3.4 (2.9-4.0), respectively; for emergency room use, the corresponding odds ratios are 1.3 (1.1-1.4), 1.9 (1.6-2.1), and 2.9 (2.6-3.3). Similar findings were replicated among smaller populations of 1061 mostly older patients from nine private practices and 4428 Medicaid patients without chronic conditions. Summary: In contrast to complex computer-generated risk models, the brief patient-reported "What Matters Index" immediately and unambiguously identifies fundamental, remediable needs for each patient and more sensibly directs the delivery of services to patient categories based on their risk for subsequent costly care.

Dataset from Barello S, Caruso R, Palamenghi L, Nania T, Dellafiore F, Bonetti L, Silenzi A, Marotta C, Graffigna G. Factors associated with emotional exhaustion in healthcare professionals involved in the COVID-19 pandemic: an application of the job demands-resources model. Int Arch Occup Environ Health. 2021 Nov;94(8):1751-1761. doi: 10.1007/s00420-021-01669-z. Epub 2021 Mar 3. PMID: 33660030; PMCID: PMC7928172.
Abstract
Purpose: The purpose of the present cross-sectional study is to investigate the role of perceived COVID-19-related organizational demands and threats in predicting emotional exhaustion, and the role of organizational support in reducing the negative influence of perceived COVID-19 work-related stressors on burnout. Moreover, the present study aims to add to the understanding of the role of personal resources in the Job Demands-Resources model (JD-R) by examining whether personal resources-such as the professionals' orientation towards patient engagement-may also strengthen the impact of job resources and mitigate the impact of job demands.
Methods: This cross-sectional study involved 532 healthcare professionals working during the COVID-19 pandemic in Italy. It adopted the Job-Demands-Resource Model to study the determinants of professional's burnout. An integrative model describing how increasing job demands experienced by this specific population are related to burnout and in particular to emotional exhaustion symptoms was developed.
Results: The results of the logistic regression models provided strong support for the proposed model, as both Job Demands and Resources are significant predictors (OR = 2.359 and 0.563 respectively, with p < 0.001). Moreover, healthcare professionals' orientation towards patient engagement appears as a significant moderator of this relationship, as it reduces Demands' effect (OR = 1.188) and increases Resources' effect (OR = 0.501).
Conclusions: These findings integrate previous findings on the JD-R Model and suggest the relevance of personal resources and of relational factors in affecting professionals' experience of burnout.
