    Description

The Texas Department of Insurance, Division of Workers’ Compensation (DWC) maintains a database of pharmacy medical billing services (SV4). It contains charges, payments, and prescriptions billed on a DWC Form-066, Statement of Pharmacy Services by pharmacies with dates of service for the last five years. For datasets going back to 2010, see pharmacy medical billing services (SV4) detail information – historical. The detail section contains information to identify insurance carriers, injured employees, employers, place of service, and diagnostic information. The bill details are individual line items that are grouped in the header section of a single bill. The bill selection date and bill ID must be used to group individual line items into a single bill. Find more information in our pharmacy medical billing services (SV4) detail data dictionary. See pharmacy medical billing services (SV4) header information for the corresponding header records related this dataset. Go to our page on DWC medical state reporting public use data file (PUDF) to learn more about using this information.

The Quarterly Prescription Drug Plan Formulary, Pharmacy Network, and Pricing Information files contain formulary, pharmacy network, and pricing data for Medicare Prescription Drug Plans and Medicare Advantage (MA) Prescription Drug Plans (with the exception of employer and Program of All-Inclusive Care for the Elderly plans). Notice: CMS has identified an issue that resulted in a 15% coinsurance for plans with Defined Standard benefits to be listed rather than a 25% coinsurance in the Beneficiary Cost File under certain scenarios. This issue affected the 2023 Q4 to 2024 Q3 data. CMS will re-post the corrected data in batches between now and May 2025. These non-identifiable files are available on a quarterly basis and are comprised of the following tables: Plan Information - Information such as plan name, contract ID, plan ID, service area, and plan type. Geographic Locator - MA and Prescription Drug Plans region codes and county codes. Basic Drugs Formulary - Formulary details for each plan including National Drug Codes (NDCs), cost share tier level, and indicators for step therapy, quantity limits, and prior authorization. Excluded Drugs Formulary - Enhanced alternative plans may elect to provide a supplemental benefit and cover excluded drugs. File includes formulary details for excluded drugs that are covered by the plan (for enhanced alternative plans only). Beneficiary Cost - Plan level cost sharing details for preferred, non-preferred, and mail order network pharmacies. Pharmacy Network - National Provider Identifier (NPI) numbers for each network pharmacy including preferred, retail, and mail order indicators. Pricing - Plan level average monthly costs for formulary Part D drugs (note: this table is only available in the quarterly files). Indication Based Coverage Formulary File - Includes drugs covered based on FDA-approved indication for each plan. Insulin Beneficiary Cost File - Plan level cost sharing details for insulin at preferred, non-preferred and mail order network pharmacies. These are large files and can take time to download. Please read the “Agreement for Use” in the Resources section below. This document contains important information regarding timeframes for obtaining data as well as data accuracy and integrity. The Monthly Prescription Drug Plan Formulary and Pharmacy Network Information is also available to access for the monthly level information. Please note: The Part D benefit year information for plans become available in October of the year prior. For example, year 2024 data is available in the fourth quarter file of 2023. Year 2024 data continues to be available in the Q1-Q3 2024 files, then in the fourth quarter of 2024 year 2025 data becomes available. Estimated release dates for upcoming 2025 quarterly data (files reflect data for the quarter that ended the month before the file was released): 4/23/25 7/30/25 Files older than contract year 2019 can be purchased.

    NHS Business Services Authority
    Description

The pharmacy opening and closures data set shows the numbers of pharmacies in England that are included on a Pharmaceutical List held by NHS England on the last calendar day of each month, and so can provide NHS prescriptions and pharmaceutical services. These are split into three groups by the number of premises in each pharmacy group: Small (1-5), Medium (6-99) and Large (100+) pharmacy groups. This data set also separately includes figures for pharmacies subject to the 100 hours condition and distance selling pharmacies. Pharmacies which have changed ownership will be counted as a closure with a new pharmacy opening on the next day, however this doesn’t affect the net change figures. ‘Opened’ means pharmacies that have joined a Pharmaceutical List and started providing NHS services between the first and last calendar day of the month; and ‘closed’ means pharmacies that have been removed from a list and have ceased providing NHS services between first and last calendar day of the month. Data is as accurate as the data held by the NHSBSA data warehouse at the time of publishing each month. Total figures may not agree month to month due to openings and closures resulting in pharmacies changing group size or where backdated amendments are made. This dataset differs from the General Pharmaceutical Services (GPhS) official statistics publication as the two publications count the number of pharmacies included in a Pharmaceutical List in two different ways. The GPhS publication presents active pharmacies as those which submitted prescriptions to the NHSBSA for processing across a whole year. That methodology used for the GPhS publication differs from the methodology used for this dataset which is set out above, so we would not expect the two to be comparable. We publish the related dataset Consolidated Pharmaceutical List on a quarterly basis.

    Description

The differential pattern and characteristics of completeness in adverse event (AE) reports generated by hospitals/clinics, pharmacies, consumer and pharmaceutical companies remain unknown. Thus, we identified the characteristics of complete AE reports, compared with those of incomplete AE reports, using a completeness score. We used Korea Institute of Drug Safety and Risk Management-Korea Adverse Event Reporting System Database (KIDS-KD) between January 1, 2016 and December 31, 2016. The completeness score was determined out of a total of 100 points, based on the presence of information on temporal relationships, age and sex of patients, AE progress, name of reported medication, reporting group by profession, causality assessment, and informational text. AE reports were organized into four groups based on affiliation: hospitals/clinics, pharmacies, consumers, and pharmaceutical companies. Affiliations that had median completeness scores greater than 80 points were classified as ‘well-documented’ and these reports were further analyzed by logistic regression to estimate the adjusted odds ratios and 95% confidence intervals. We examined 228,848 individual reports and 735,745 drug-AE combinations. The median values of the completeness scores were the highest for hospitals/clinics (95 points), followed by those for consumers (85), pharmacies (75), and manufacturers (72). Reports with causality assessment of ‘certain’, ‘probable’, or ‘possible’ were more likely to be ‘well-documented’ than reports that had causality assessments of ‘unlikely’. Serious reports of AEs were positively associated with ‘well-documented’ reports and negatively associated with hospitals/clinics.
