    Description

    The Centers for Medicare & Medicaid Services (CMS) EHR Incentive Program provides incentive payments for eligible hospitals to adopt and meaningfully use certified health IT. Among the requirements to receive an incentive payment, participating hospitals must report on public health measures. These measures include the electronic reporting of data regarding: immunizations, emergency department visits (syndromic surveillance), reportable infectious disease laboratory results, and electronic patient data to specialized registries, like cancert. As of 2015, stage 2 of the EHR Incentive Program requires hospitals to report on three public health measures, when applicable, and modified stage 2 of the program requires hospitals to report on two of the three measures. This dataset includes the percentage of hospitals who reported on these measures in program years, 2013, 2014 and 2015.

    Description

    The 2020 ASTHO Profile Survey is a survey conducted by the Association of State and Territorial Health Officials (ASTHO) to gather information on state, territorial, and freely associated state public health agencies (S/THAs) and their activities, structure, and resources. The survey aims to define the scope of state and territorial public health services, identify variations in practice among public health agencies, and contribute to the development of best practices in governmental public health. The instrument was significantly shortened for this data collection and separated into three separate surveys. The surveys were administered in December 2020 and completed by state and territorial health agency staff at each S/THA including senior deputies, chief financial officers, and human resource directors. The survey closed in March 2021; 80% of states and DC responded to at least one survey, of which 40% of states and DC responded to all three surveys; 38% of territories responded to at least one survey, of which 11% responded to all three surveys. Changes may be made to the dataset after it is archived. Please contact profile@astho.org to request the most updated datasets.     Description

    The first Social Drivers of Health (SDoH) dataset contains percentages of preventable hospitalizations (i.e., discharges) by Race/Ethnicity, preferred language spoken, expected payer, percent of employment, percent of home ownership, percent of park access and percent of access to basic kitchen facilities by the stated year. Preventable hospitalizations rates were created by dividing the number of patients who are 18 years and older and were admitted to a hospital for at least one of the preventable hospitalization diagnoses (see list below) by the total number of hospitalizations. List of preventable hospitalization diagnoses: diabetes with short-term complications, diabetes with long-term complications, uncontrolled diabetes without complications, diabetes with lower-extremity amputation, chronic obstructive pulmonary disease, asthma, hypertension, heart failure, angina without a cardiac procedure, dehydration, bacterial pneumonia, or urinary tract infection were counted as a preventable hospitalization. These conditions correspond with the conditions used in the Agency for Healthcare Research and Quality’s (AHRQ), Prevention Quality Indicator - Overall Composite Measure (PQI #90). The SDoH "overtime" dataset contains percentages of preventable hospitalizations (i.e., discharges) by Race/Ethnicity, preferred language spoken and expected payer overtime in the stated year range.
