In 2019, India and South Africa were the only BRICS countries with a fertility rate above replacement level (2.1 births per woman). Fertility rates since 2000 show that fertility in China and Russia has either fluctuated or remained fairly steady, as these two countries are at a later stage of the demographic transition than the other three, while Brazil has reached this stage more recently. Fertility rates in India are following a similar trend to Brazil, while South Africa's rate is progressing at a much slower pace. Demographic development is inextricably linked with economic growth; for example, as fertility rates drop, female participation in the workforce increases, as does the average age, which then leads to higher productivity and a more profitable domestic market.

    In 2023, it is estimated that the BRICS countries have a combined population of 3.25 billion people, which is over 40 percent of the world population. The majority of these people live in either China or India, which have a population of more than 1.4 billion people each, while the other three countries have a combined population of just under 420 million. Comparisons Although the BRICS countries are considered the five foremost emerging economies, they are all at various stages of the demographic transition and have different levels of population development. For all of modern history, China has had the world's largest population, but rapidly dropping fertility and birth rates in recent decades mean that its population growth has slowed. In contrast, India's population growth remains much higher, and it is expected to overtake China in the next few years to become the world's most populous country. The fastest growing population in the BRICS bloc, however, is that of South Africa, which is at the earliest stage of demographic development. Russia, is the only BRICS country whose population is currently in decline, and it has been experiencing a consistent natural decline for most of the past three decades. Growing populations = growing opportunities Between 2000 and 2026, the populations of the BRICS countries is expected to grow by 625 million people, and the majority of this will be in India and China. As the economies of these two countries grow, so too do living standards and disposable income; this has resulted in the world's two most populous countries emerging as two of the most profitable markets in the world. China, sometimes called the "world's factory" has seen a rapid growth in its middle class, increased potential of its low-tier market, and its manufacturing sector is now transitioning to the production of more technologically advanced and high-end goods to meet its domestic demand.

    For most of the past two centuries, falling birth rates have been associated with societal progress. During the demographic transition, where pre-industrial societies modernize in terms of fertility and mortality, falling death rates, especially among infants and children, are the first major change. In response, as more children survive into adulthood, women have fewer children as the need to compensate for child mortality declines. This transition has happened at different times across the world and is an ongoing process, with early industrial countries being the first to transition, and Sub-Saharan African countries being the most recent to do so. Additionally, some Asian countries (particularly China through government policy) have gone through their demographic transitions at a much faster pace than those deemed more developed. Today, in countries such as Japan, Italy, and Germany, birth rates have fallen well below death rates; this is no longer considered a positive demographic trend, as it leads to natural population decline, and may create an over-aged population that could place a burden on healthcare systems.

    Description

    The aim of the article is to compare health system outcomes in the BRICS countries, assess the trends of their changes in 2000−2017, and verify whether they are in any way correlated with the economic context. The indicators considered were: nominal and per capita current health expenditure, government health expenditure, gross domestic product (GDP) per capita, GDP growth, unemployment, inflation, and composition of GDP. The study covered five countries of the BRICS group over a period of 18 years. We decided to characterize countries covered with a dataset of selected indicators describing population health status, namely: life expectancy at birth, level of immunization, infant mortality rate, maternal mortality ratio, and tuberculosis case detection rate. We constructed a unified synthetic measure depicting the performance of individual health systems in terms of their outcomes with a single numerical value. Descriptive statistical analysis of quantitative traits consisted of the arithmetic mean (xsr), standard deviation (SD), and, where needed, the median. The normality of the distribution of variables was tested with the Shapiro–Wilk test. Spearman's rho and Kendall tau rank coefficients were used for correlation analysis between measures. The correlation analyses have been supplemented with factor analysis. We found that the best results in terms of health care system performance were recorded in Russia, China, and Brazil. India and South Africa are noticeably worse. However, the entire group performs visibly worse than the developed countries. The health system outcomes appeared to correlate on a statistically significant scale with health expenditures per capita, governments involvement in health expenditures, GDP per capita, and industry share in GDP; however, these correlations are relatively weak, with the highest strength in the case of government's involvement in health expenditures and GDP per capita. Due to weak correlation with economic background, other factors may play a role in determining health system outcomes in BRICS countries. More research should be recommended to find them and determine to what extent and how exactly they affect health system outcomes.

    In 2022, India overtook China as the world's most populous country and now has almost 1.46 billion people. China now has the second-largest population in the world, still with just over 1.4 billion inhabitants, however, its population went into decline in 2023. Global population As of 2025, the world's population stands at almost 8.2 billion people and is expected to reach around 10.3 billion people in the 2080s, when it will then go into decline. Due to improved healthcare, sanitation, and general living conditions, the global population continues to increase; mortality rates (particularly among infants and children) are decreasing and the median age of the world population has steadily increased for decades. As for the average life expectancy in industrial and developing countries, the gap has narrowed significantly since the mid-20th century. Asia is the most populous continent on Earth; 11 of the 20 largest countries are located there. It leads the ranking of the global population by continent by far, reporting four times as many inhabitants as Africa. The Demographic Transition The population explosion over the past two centuries is part of a phenomenon known as the demographic transition. Simply put, this transition results from a drastic reduction in mortality, which then leads to a reduction in fertility, and increase in life expectancy; this interim period where death rates are low and birth rates are high is where this population explosion occurs, and population growth can remain high as the population ages. In today's most-developed countries, the transition generally began with industrialization in the 1800s, and growth has now stabilized as birth and mortality rates have re-balanced. Across less-developed countries, the stage of this transition varies; for example, China is at a later stage than India, which accounts for the change in which country is more populous - understanding the demographic transition can help understand the reason why China's population is now going into decline. The least-developed region is Sub-Saharan Africa, where fertility rates remain close to pre-industrial levels in some countries. For Brazil, China, and India, population growth has gradually fallen over time, as their demographic development progresses. South Africa has had the highest population growth rate since 2010, as its population recovered from the initial impact of the HIV/AIDS pandemic, before it started falling as birth rates fall more in line with death rates.

Of the BRICS countries, life expectancy in Brazil and China has been above the global average during this time, while India's and South Africa's have consistently been below, and Russia's was below until 2017. Life expectancy from birth has risen in all five BRICS countries over these two decades, although there was a drop of almost three years in South Africa between 2000 and 2005, due to the prevalence of HIV/AIDS, and a slight drop of almost half a year in Russia from 2000 to 2003, due to the prevalence of unhealthy lifestyles and alcohol/substance abuse.

Standard of living in India India is one of the so-called BRIC countries, an acronym which stands for Brazil, Russia, India and China, the four states considered the major emerging market countries. They are all in a similar advanced economic state and are expected to advance even further. India is also among the twenty leading countries with the largest gross domestic product / GDP, and the twenty countries with the largest proportion of global gross domestic product / GDP based on Purchasing Power Parity (PPP). Its unemployment rate has been stable over the past few years; India is also among the leading import and export countries worldwide. This alone should put India in a relatively comfortable position economically speaking, however, parts of the population of India are struggling with poverty and health problems. When looking at a comparison of the median age of the population in selected countries – i.e. one half of the population is older and the other half is younger –, it can be seen that the median age of the Indian population is about twenty years less than that of the Germans or Japanese. In fact, the median age in India is significantly lower than the median age of the population of the other emerging BRIC countries – Russia, China and Brazil. Additionally, the total population of India has been steadily increasing. Regarding life expectancy, India is neither among the countries with the highest, nor among those with the lowest life expectancy at birth. The majority of the Indian population is aged between 15 and 64 years, with only about 5 percent being older than 64.

    Throughout the Common Era, Japan's population saw relatively steady growth between each century. Failed invasions and distance from Asia's mainland meant that Japan was unaffected by many pandemics, primarily bubonic plague, therefore its development was not drastically impeded in the same way as areas such as China or Europe. Additionally, religious practices meant that hygiene was prioritized much more in Japan than in other regions, and dietary customs saw lower rates of meat consumption and regular boiling of water in meals or tea; both of these factors contributed to lower rates of infection for many parasitic or water-borne diseases. Fewer international conflicts and domestic stability also saw lower mortality in this regard, and Japan was an considered an outlier by Asian standards, as some shifting trends associated with the demographic transition (such as lower child mortality and fertility) began taking place in the 17th century; much earlier time than anywhere else in the world. Yet the most significant changes came in the 20th century, as Japan's advanced healthcare and sanitation systems saw drastic reductions in mortality. Challenges Japan's isolation meant that, when pandemics did arrive, the population had less protection and viruses could have higher mortality rates; smallpox has been cited as the deadliest of these pandemics, although increased international contact in the late 19th century brought new viruses, and population growth slowed. Earlier isolation also meant that crop failure or food shortages could leave large sections of the population vulnerable, and, as mentioned, the Japanese diet contained relatively little meat, therefore there was a higher reliance on crops and vegetables. It is believed that the shortage of arable land and the acidity of the soil due to volcanic activity meant that agriculture was more challenging in Japan than on the Asian mainland. For most of history, paddy fields were the most efficient source of food production in Japan, but the challenging nature of this form of agriculture and changes in employment trends gradually led to an increased reliance in imported crops. Post-Sakoku Japan Distance from the Asian mainland was not the only reason for Japan's isolation; from 1603 to 1853, under the Tokugawa shogunate, international trade was restricted, migration abroad was forbidden, and most foreign interaction was centered around Nagasaki. American neo-imperialism then forced Japan to open trade with the west, and Japan became an imperial power by the early-1900s. Japanese expansion began with a series of military victories against China and Russia at the turn of the century, and the annexation of Taiwan, Korea, and Manchuria by the 1930s, before things escalated further during its invasion of China and the Second World War. Despite its involvement in so many wars, the majority of conflicts involving Japan were overseas, therefore civilian casualties were much lower than those suffered by other Asian countries during this time. After Japan's defeat in 1945, its imperial ambitions were abandoned, it developed strong economic ties with the West, and had the fastest economic growth of any industrial country in the post-WWII period. Today, Japan is one of the most demographically advanced countries in the world, with the highest life expectancy in most years. However, its population has been in a steady decline for over a decade, and low fertility and an over-aged society are considered some of the biggest challenges to Japanese society today.

    This statistic depicts the age distribution of India from 2013 to 2023. In 2023, about 25.06 percent of the Indian population fell into the 0-14 year category, 68.02 percent into the 15-64 age group and 6.92 percent were over 65 years of age. India’s society is categorized into a hierarchically organized caste system, encompassing certain rights and values for each caste. Gender equality is still non-existant in India, even though most Indians believe that the quality of life is about equal for men and women in their country. India is patriarchal and women still often face forced marriages, domestic violence, dowry killings or rape. As of late, India has come to be considered one of the least safe places for women worldwide. Additionally, infanticide and selective abortion of female fetuses attribute to the inequality of women in India. It is believed that this has led to the fact that the vast majority of Indian children aged 0 to 6 years are male.
