    Background The pathophysiology behind spina bifida and other neural tube defects (NTDs) is unclear. Folic acid is one variable, but other factors remain. Studies suggest that substances active at the GABA receptor may produce NTDs. To test this hypothesis pregnant rats were exposed to either the GABA a agonist muscimol (1, 2 or 4 mg/kg), the GABA a antagonist bicuculline (.5, 1, or 2 mg/kg), the GABA b agonist baclofen (15, 30, 60 mg/kg), or the GABA b antagonist hydroxysaclofen (1, 3, or 5 mg/kg) during neural tube formation. Normal saline was used as a control and valproic acid (600 mg/kg) as a positive control. The embryos were analyzed for the presence of a spina bifida like NTD. Results After drug administration the pregnancies were allowed to proceed to the 21st day of gestation. Then embryos were removed and skeletons staining and cleared. Vertebral arch closure was measured. Results indicate that the GABAa receptor agonist muscimol, the GABAa receptor antagonist bicuculline, and the GABAb agonist baclofen produced NTDs characterized by widening of the vertebral arch. Oppositely the GABAb antagonist hydroxysaclofen produced narrowing of the vertebral arches. Conclusions The findings indicate that GABA a or b ligands are capable of altering neural formation. GABA may play a greater than appreciated role in neural tube formation and may be important in NTDs. The narrowing of the vertebral arch produced by the GABA b antagonist hydroxysalcofen suggests that GABA b receptor may play an undefined role in neural tube closure that differs from the GABA a receptor.

    Original survey questions and anonymised participants' replies data set.The survey (available in Microsoft Word document) is composed of 86 multiple choice questions, where multiple responses were possible. Questions are grouped in six main categories:General information: demographic data, bowel symptoms, how general health condition is perceived as impacting on bowel symptoms.Bowel management history: data on onset of bowel symptoms, bowel management methods and health care professional support.Reaching good bowel care: data on healthcare seeking behaviour and access to specialist health care professionals.Bowel management over time: data on bowel symptom changes over time, bowel management challenges and factors influencing bowel symptoms.Reaching transanal irrigation: data on TAI usage to explore routine, challenges and benefits.Perception of treatment explored: what people would like to achieve in their bowel care and if this changes according to life situation.Partecipant answers (available in Microsoft Excel documents) are transferred in spreadsheet and reviewed by the authors. Descriptive analysis only was possible. We described how people with Multiple Sclerosis, Spinal Cord Injury and Spina bifida expressed their preference, we do not intend to add a qualitative or quantitative description.The analysis focused on the most generalisable data on the lived experience of participants with Neurogenic Bowel Dysfunction.

    Description

    Spinal dysraphism describes a spectrum of conditions resulting from aberrant neural tube closure during the first trimester of fetal development. Consequent neurological deficits may lead to a range of musculoskeletal abnormalities requiring orthopedic intervention. This study sought to evaluate the impact of spinal dysraphism on patients and their families. Free text questionnaires/interviews were collected from parents. Quantitative questionnaires were collected from children using an emoji response system. These assessed contentment in activities of daily living. Qualitative data were analyzed via thematic analysis. Descriptive statistics were applied to quantitative data. In total 32 responses (13 patients and 19 carers) were collected and analyzed. The median age of patients was 10 years (IQR = 3), with a sex distribution of 7 boys and 6 girls. Overall, 3 key themes were identified: (i) Independence, (ii) Mental Health, (iii) Family Impact, each with corresponding subthemes. Quantitative data corroborated these themes well, highlighting impacts on activities of daily living in the majority of domains assessed. Spinal dysraphism has a multifactorial impact on patient and family quality of life spanning the societal, mental, and physical domains. These findings will be included in ongoing work to create a set of core outcomes for use in the clinical and research settings.

    Description

    The Open Spina Bifida Aperta (OSBA) atlas is a set of neuroimaging templates for newborns with open spina bifida (spina bifida aperta, SBA). The OSBA atlas was constructed using structural T2-weighted and diffusion tensor MRI of 28 newborns with SBA. The corrected gestational age at MRI was 38.1 ± 1.1 weeks (mean ± SD). The OSBA atlas consists of T2-weighted and fractional anisotropy templates, along with 9 tissue prior maps and region of interest (ROI) delineations. The OSBA atlas offers a standardized reference space for spatial normalization and anatomical ROI definition.
