    As of April 2024, 55 percent of the LGBT population lived in the 50 states and the District of Columbia where private health insurance service providers weren't allowed to deny coverage for transgender-related health care services, such as sex reassignment surgery.     Description

    Supplementary file 1: Data for serum/plasma laboratory tests with sex-specific reference intervals performed on patients with sexual orientation/gender identity (SOGI) field differences for legal sex/sex assigned at birth (SAAB)/gender identity (GI) in the electronic medical record (EMR) at the University of Iowa Hospitals and Clinics from January 1, 2021 to December 31, 2021. The data elements include: deidentified patient number, age (years) at time of laboratory testing, location type (outpatient, inpatient, or emergency department) at time of specimen collection, legal sex in EMR, SAAB in EMR, GI in EMR, presence of SOGI mismatch (yes/no), change of legal sex in EMR, gender-affirming gonadectomy (yes/no), GI from chart review, gender-affirming hormone and route of administration, values for tests for 17 laboratory tests, and how the values of these 17 laboratory tests were relative to age-matched reference intervals for cisgender men and women. Assays details and reference intervals are summarized in Supplemental file 3. Chart review for SOGI fields previously reported [1]. There are a total of 1,166 unique patients (all 18.0 years or older) and 7,530 laboratory tests.

    Supplementary file 2: Data for plasma creatinine performed for patients 18.0 years or older taking gender-affirming estradiol or testosterone at the University of Iowa Hospitals and Clinics from January 1, 2021 to December 31, 2021. The data elements include: deidentified patient number, age (years) at time of laboratory testing, location type at time of specimen collection, legal sex in EMR, SAAB in EMR, GI in EMR, GI from chart review, gender-affirming hormone and route of administration, self-declared race in EMR (African-American or not), plasma creatinine (mg/dL), estimated glomerular filtration rate (eGFR) calculation using female sex and the 2021 CKD-EPI equation without race refit, eGFR using male as sex, and chronic kidney disease (CKD) stage using either female or male as sex. eGFR equation and CKD stages are from references 2 and 3, respectively. There are a total of 620 unique patients and 1,469 plasma creatinine values.

    Description

    IntroductionThe Centers for Disease Control and Prevention (CDC) and World Health Organization (WHO) created separate growth charts for girls and boys because growth patterns and rates differ between sexes. However, scenarios exist in which this dichotomizing “girls versus boys” approach may not be ideal, including the care of non-binary youth or transgender youth undergoing transitions consistent with their gender identity. There is therefore a need for growth charts that age smooth differences in pubertal timing between sexes to determine how youth are growing as “children” versus “girls or boys” (e.g., age- and sex-neutral, compared to age- and sex-specific, growth charts).MethodsEmploying similar statistical techniques and datasets used to create the CDC 2000 growth charts, we developed age-adjusted, sex non-specific growth charts for height, weight, and body mass index (BMI), and z-score calculators for these parameters. Specifically, these were created using anthropometric data from five US cross-sectional studies including National Health Examination Surveys II-III and National Health and Nutrition Examination Surveys I-III. To illustrate contemporary clinical practice, we overlaid our charts on CDC 2000 girls and boys growth charts.Results39,119 youth 2-20 years old (49.5% female; 66.7% non-Hispanic White; 21.7% non-Hispanic Black) were included in the development of our growth charts, reference ranges, and z-score calculators. Respective curves were largely superimposable through around 10 years of age after which, coinciding with pubertal onset timing, differences became more apparent.DiscussionWe conclude that age-adjusted, sex non-specific growth charts may be used in clinical situations such as transgender youth in which standard “girls versus boys” growth charts are not ideal. Until longitudinal auxological data are available in these populations, our growth charts may help to assess a transgender youth’s growth trajectory and weight classification, and expectations surrounding these.

    In 2023, 7.6 percent of adults surveyed in the United States stated they identify as lesbian, gay, bisexual, or transgender (LGBT). This is an increase from 2012, when 3.5 percent of surveyed adults identified as LGBT. These four categories are among the most common sexual orientations and gender identities outside cis-heterosexuals.

    Differences across ages and genders 

    Zooming in, it is possible to see how the share of people identifying as LGBT changed considerably according to different demographics. Only two percent of Baby Boomers, indeed, reported being part of the LGBT community. On the other hand, among Generation Z individuals, this share reached almost 16 percent. Moreover, surveys suggest that women were more likely to identify as LGBT than men.

    An ever-understanding society 

    The fact that an increasing amount of people feel free and safe to openly identify as LGBT might be a consequence of the improved acceptance of the different sexual orientations and genders by the American people at large. While in 2001 more than half of U.S. citizens considered homosexual relationships to be morally wrong, 20 years later this percentage dropped to 30 percent. Support for same-sex marriage has also been consistently increasing, even among traditionally conservative political groups. Despite this, it is important to remember that every year hundreds of people are still victims of hate crimes in the United States simply because of their gender or sexual orientation.

    Description

    IntroductionPuberty suppression using gonadotropin-releasing hormone agonists (GnRHa) is a reversible medical intervention that halts endogenous puberty, allowing transgender and gender-diverse (TGD) adolescents to avoid the development of secondary sexual characteristics that may cause psychological distress. This pause in pubertal progression provides time to explore gender identity or facilitates alignment with affirmed gender in those with an established identity. While widely used, long-term evidence on the efficacy and safety of GnRHa in this population remains limited. This systematic review aims to synthesize current data on the benefits and potential risks of GnRHa in TGD adolescents.MethodsWe conducted a comprehensive literature search across PubMed, EMBASE, Cochrane Library, and other databases, covering studies published from February 2011 to February 2024. Eligible studies included adolescents under 18 with gender dysphoria or incongruence treated with GnRHa, reporting outcomes related to efficacy or side effects. Fifty-one studies met inclusion criteria, and data on physical health, mental health, bone density, fertility, and adverse events were extracted and assessed using the GRADE approach.ResultsOf the 51 studies, 22 were rated as moderate to high-quality evidence. GnRHa effectively suppressed puberty and secondary sex characteristics. Effects on growth and body composition varied; bone mineral density declined during treatment, particularly in AMAB individuals. Mental health improved significantly, including reduced depression, anxiety, and suicidality—especially when GnRHa was followed by gender-affirming hormone therapy (GAHT). Quality of life improved over time, while body dissatisfaction often persisted during suppression and improved after GAHT or surgery. No moderate- or high-quality evidence was found on fertility, sexual function, or cancer risk.ConclusionGnRHa is effective in halting puberty and improving mental health in TGD adolescents. However, key clinical and ethical considerations—such as bone health monitoring, fertility counseling, psychological support, and informed decision-making—must guide treatment. Long-term safety remains uncertain, particularly regarding skeletal health, reproductive outcomes and cancer risk.     Non-binary

    Someone who is non-binary does not identify with the binary categories of man and woman. In these results the category includes people who identified with the specific term "non-binary" or variants thereon. However, those who used other terms to describe an identity that was neither specifically man nor woman have been classed in "All other gender identities".

    Sex

    This is the sex recorded by the person completing the census. The options were "Female" and "Male".

    Trans

    Transgender people, often referred to as the Hijras in the Indian subcontinent, are an officially recognized third gender in the country and consider themselves neither male nor female. As per the latest census in 2011, India recorded over 487 thousand people who identified as the third gender, most of whom came from the northern state of Uttar Pradesh.

    Socioeconomics of Hijras 

    Hijras are associated by several terms, depending on culture and linguistics. In various Indian languages, Hijras are called Napunsakudu (Telugu), Thiru Nangai (Tamil), Khwaja Sara (Urdu), Khusra (Punjabi) and Mangalamukhi (Kannada) among others. The third gender is inherent to Indian mythology.

    Socio-economically, Hijras live in well-defined communities, led by a Guru or teacher. They consist of generations of the third gender who were rejected by or fled from their birth-families. Apart from sex-work, performing at auspicious social gatherings or plain extortion was how most survived. This was due to the lack of education and employment due to their low status in their society’s hierarchy. This also meant that they underwent brutal violence with little or no opportunity for justice.

     Indian law and the third gender  

    When the country’s Supreme Court re-criminalized homosexuality in 2013, there was a sharp increase in violence against the third gender by the Indian Police Service, far from investigating reports of sexual assault. Later in 2014, with the support of activists and NGOs lobbying for human rights, the same highest court ruled the recognition of the third gender community, equal to the socially and economically backward class. This meant that they now had access to proportional representation in education and jobs.

Description

As of April 2024, 55 percent of the LGBT population lived in the 50 states and the District of Columbia where private health insurance service providers weren't allowed to deny coverage for transgender-related health care services, such as sex reassignment surgery. However, only 40 percent lived in states where health insurance protections included sexual orientation and gender identity.
