Military deployment to combat zones puts military personnel to a number of physical and mental challenges that may adversely affect mental health. Until now, few studies have been performed in Europe on mental health utilization after military deployment. We compared the incidence of mental health consultations with the Military Mental Health Service (MMHS) of military deployed to Afghanistan to that of non-deployed military personnel. We assessed utilization of the MMHS by the full cohort of the Netherlands Armed Forces enlisted between 2008 and 2010 through linkage of mental health and human resource information systems. The total population consisted of 50,508 military (18,233 deployed, 32,275 non-deployed), who accounted for 1,906 new consultations with the MMHS. The follow-up was limited to the first 2 years following deployment. We observed higher mental health care utilization in deployed vs. non-deployed military personnel; hazard ratio (HR), adjusted for sex, military branch and time in service, 1.84 [95% CI 1.61–2.11] in the first and 1.28 [1.09–1.49] in the second year after deployment. An increased risk of adjustment disorders (HR 2.59 [2.02–3.32] and 1.74 [1.30–2.32]) and of anxiety disorders (2.22 [1.52–3.25] and 2.28 [1.50–3.45]) including posttraumatic stress disorder (5.15 [2.55–10.40] and 5.28 [2.42–11.50]), but not of mood disorders (1.33 [0.90–1.97] and 1.11 [0.68–1.82]), was observed in deployed personnel in the first- and second-year post-deployment, respectively. Military personnel deployed in a unit with a higher risk of confrontation with potentially traumatic events had a higher HR (2.13 [1.84–2.47] and 1.40 [1.18–1.67]). Though absolute risk was low, in the first and second year following deployment to Afghanistan there was an 80 and 30% higher risk for mental health problems resulting in a consultation with the Dutch MMHS compared to military never deployed to Afghanistan. These observations underscore the need for an adequate mental health infrastructure for those returning from deployment.

BackgroundThere are comparatively few international studies investigating suicide in military veterans and no recent UK–wide studies. This is important because the wider context of being a UK Armed Forces (UKAF) veteran has changed in recent years following a period of intensive operations. We aimed to investigate the rate, timing, and risk factors for suicide in personnel who left the UKAF over a 23–year period.Methods and findingsWe carried out a retrospective cohort study of suicide in personnel who left the regular UKAF between 1996 and 2018 linking national databases of discharged personnel and suicide deaths, using survival analysis to examine the risk of suicide in veterans compared to the general population and conditional logistic regression to investigate factors most strongly associated with suicide after discharge. The 458,058 individuals who left the UKAF accumulated over 5,852,100 person years at risk, with a median length of follow–up of 13 years, were mostly male (91%), and had a median age of 26 years at discharge. 1,086 (0.2%) died by suicide. The overall rate of suicide in veterans was slightly lower than the general population (standardised mortality ratio, SMR [95% confidence interval, CI] 94 [88 to 99]). However, suicide risk was 2 to 3 times higher in male and female veterans aged under 25 years than in the same age groups in the general population (age–specific mortality ratios ranging from 160 to 409). Male veterans aged 35 years and older were at reduced risk of suicide (age–specific mortality ratios 47 to 80). Male sex, Army service, discharge between the ages of 16 and 34 years, being untrained on discharge, and length of service under 10 years were associated with higher suicide risk. Factors associated with reduced risk included being married, a higher rank, and deployment on combat operations. The rate of contact with specialist NHS mental health services (273/1,086, 25%) was lowest in the youngest age groups (10% for 16– to 19–year–olds; 23% for 20– to 24–year–olds). Study limitations include the fact that information on veterans was obtained from administrative databases and the role of pre–service vulnerabilities and other factors that may have influenced later suicide risk could not be explored. In addition, information on contact with support services was only available for veterans in contact with specialist NHS mental health services and not for those in contact with other health and social care services.ConclusionsIn this study, we found suicide risk in personnel leaving the UKAF was not high but there are important differences according to age, with higher risk in young men and women. We found a number of factors which elevated the risk of suicide but deployment was associated with lower risk. The focus should be on improving and maintaining access to mental health care and social support for young service leavers, as well as implementing general suicide prevention measures for all veterans regardless of age.

Data are provided on arms exports and imports, total imports and exports, arms imports and exports as a percentage of the total imports and exports, the gross national product (GNP), and central government expenditures, as well as expenditures on health, education, and the military as a percentage of the GNP, and health and education expenditures as a percentage of military expenditures. Data are also provided on population characteristics, such as the number of teachers, soldiers, and doctors per 1,000 people, the number of teachers and doctors as a percentage of the number of soldiers, and the total population. Other variables provide information on the country's membership in regional alliances such as the North Atlantic Treaty Organization, the former Warsaw Pact, and the Organization of Petroleum Exporting Countries, as well as World Bank membership. Additional variables describe the country's developmental status and the geographic region of the world in which the country is located. In addition, Part 1 data provide information on the total value of arms transferred from major arms supplier countries such as the United States, France, United Kingdom, Italy, Canada, Poland, China, and the Soviet Union, Czechoslovakia, and West Germany. Over the course of the Second World War approximately 127.2 million people were mobilized. The world's population in 1940 was roughly 2.3 billion, meaning that between five and six percent of the world was drafted into the military in some capacity. Approximately one in every 25 people mobilized were women, who generally served in an administrative or medical role, although hundreds of thousands of women did see active combat. Largest armies In absolute numbers, the Soviet Union mobilized the largest number of people at just under 34.5 million, and this included roughly 35 percent of the USSR's male population. By the war's end, more Soviets were mobilized than all European Axis powers combined.
