    Description

    We present here a new dataset of per capita disposable income for 42 European countries (and more than 120,000 administrative units at the subnational level), over the 2010-2020 period (with few additional years for some countries). This dataset was created by harmonizing disparate income data (net earnings, gross income, disposable income, etc.) gathered from national statistical institutes across Europe. Disposable income was converted to constant 2015 EU27 PPP€ to adjust for the costs of living and inflation across countries and to allow comparability over time. Total population and a measure of income inequality (Gini index) are also provided for subnational administrative units. Users can download the aggregated dataset covering the whole years (Disposable_Inc_DB.gpkg) or yearly files.

    Background: This study aims to compare the cost-effectiveness of treatment sequences with available biologics, including adalimumab (ADA), biosimilar infliximab (bsIFX), originator infliximab (IFX) and vedolizumab (VEDO) for luminal Crohn’s disease in nine European countries. Methods: A Markov-model was constructed to simulate five-year medical costs and quality-adjusted life years (QALYs). Data on clinical efficacy were obtained from randomised controlled trials. Country-specific unit costs, discount rates and a third-party payer perspective were applied. Results: The bsIFX versus conventional therapy resulted in the most favourable incremental cost-utility ratios (ICURs) ranging from €34,580 (Hungary) to €77,062/QALY (Sweden). Compared to bsIFX, the bsIFX-ADA sequence was more cost-effective than the bsIFX-VEDO sequence with ICURs varying between €70,277 (France) and €162,069/QALY (Germany). The ICURs of the bsIFX-ADA-VEDO sequence versus the bsIFX-ADA strategy were between €206,266 (The Netherlands) and €363,232/QALY (Spain). Conclusion: We are the first to compare cost-effectiveness of multiple biological sequences for luminal Crohn’s disease. Based on our findings, bsIFX can be recommended as a first-line treatment in patients unresponsive to conventional treatments. While biological sequences only slightly differ in their associated health gains, their costs vary greatly. The bsIFX-ADA-VEDO seems to be the most cost-effective sequence of the available biologics across Europe.

    BackgroundPatient reported outcomes and costs of illness are useful to capture some of the multiple effects of a disease and its treatments. Our aim was to assess quality of life (QoL) and costs of Parkinson's disease (PD) in Hungary, and to analyze their associations.MethodsA cross-sectional questionnaire survey was conducted in one neurology university clinic. Clinical characteristics, PD related resource utilizations and productivity loss in the past 12 months were recorded; the Hoehn&Yahr (HY) scale, PDQ-39 and EQ-5D questionnaires were applied. Cost calculation was performed from the societal perspective.Results110 patients (34.5% female) were involved with mean age of 63.3 (SD = 11.3) and disease duration of 8.2 (SD = 5.8) years. PDQ-39 summary score was 48.1 (SD = 13.4). The average EQ-5D score was 0.59 (SD = 0.28), and was significantly lower than the population norm in age-groups 45–74. The correlation was significant between EQ-5D and PDQ-39 (−0.47, p = 0.000), the HY scale and EQ-5D (−0.3416, p = 0.0008) and PDQ-39 (0.3419, p = 0.0006) scores. The total mean cost was €6030.2 (SD = 6163.0)/patient/year (direct medical 35.7%, direct non-medical 29.4%, indirect cost 34.9%). A one year increase in disease duration and 0.1 decrease of the EQ-5D utility score increase the yearly costs by 8 to 10%, and 7.8%, respectively. The effect of the PDQ-39 score on total cost was not significant.ConclusionsDisease severity and public health importance of PD are clearly demonstrated by the magnitude of QoL loss. PD-related costs are substantial, but are much lower in Hungary than in Western European countries. Disease duration and EQ-5D score are significant proxy of costs.

    The PECUNIA Reference Unit Costs Compendium (PECUNIA RUC Compendium) is a Microsoft Excel®-based multi-sectoral, multi-country database of unit costs comparable across countries and sectors and developed using standardized and scientifically validated methods (so called Reference Unit Costs (RUCs)) by the scientific partners of the PECUNIA Consortium between 2019 and 2021 based on the preliminary conceptual and methodological work conducted in 2018. The RUCs were developed using standardized and validated PECUNIA RUC Templates to introduce a higher degree of methodological harmonization and transparency in unit costs within and across sectors and countries. They are consistent with the harmonised PECUNIA Costing Concept (PECUNIA Care Atom, PECUNIA Service Lists, PECUNIA Coding System) and therefore with other PECUNIA Costing Tools (PECUNIA RUC Templates, PECUNIA RUM Instrument) to achieve comparability by referring to content rather than linguistic equivalence of services and other resource items. In the PECUNIA RUC Compendium, each listed RUC is complemented by multiple meta-data and a summary indicator of the overall level of certainty based on the robustness of the costing approach used and the outcome of the external validation process for fully transparency.

    Version 1.0 of the PECUNIA RUC Compendium contains RUC estimates (in Euro, for year 2019) for a core set of service and resource use items commonly occurring in health economic evaluations with a societal perspective

    for multiple sectors: health and social care (HSC), education (ED), (criminal) justice (CJ), employment and productivity (EP), and patient, family and informal care (PFI);

    for multiple European countries (Austria, Germany, Hungary, The Netherlands, England, and Spain).

    It is a living document planned to be regularly updated and expanded both in terms of the covered service and resource use items and the number of countries over time.
